[발명의 상세한 설명]
본원 발명은 치아 또는 뼈 이식체에 관한 것이다. 더 특이적으로는, 재흡수성이 다른 3가지 층을 갖는 치아 또는 뼈 이식체에 관한 것이다.
[발명의 배경]
조직내에 합성물질을 이식하는 문제가 항상 연구자들의 관심을 끌어오긴 하였으나, 18세기말에 와서야 비로소 첫번째로 시도되었다. 그러나, 당시의 연구도 만족스러운 결과를 가져오지는 못하였다. 대신, 1950년대초 이래로 티탄 또는 탄탈금속 편평부를 뼈, 예를 들면 턱뼈내에 집어넣는데 각각 성공을 거두었다.
수술의 어려움 및 감염 위험성으로 인하여 거대 골막하 구조물 및 거대 골내막 편평부는 날로 실용성을 잃고 있다. 이들은 단일 나사(Branemark, Straumann, Bioceram; 참고 1 Albrektson 등 ; special reprint. 현재 사용되고 있는 치아 이식물의 장기효능(The long term efficacy of currently used dental implants.) A review and proposed criteria of success. Int. J. Oral and Max. Fac. Implants 1, 1986) 및 단일 치근과 유사한 거대 원추형 실린더가 사용되어 왔다. 오늘날에 있어서는 구조물을 치조뼈속에 집어넣는 방법이 통상적으로 실시되고 있다. 따라서 뼈와 이식체를 기계적으로 접촉시키는 것이 가능하게 되었다. 이식체는 또한 치육을 뚫고 육안으로 관찰가능한 돌출부를 남긴다. 상기 돌출부는 유상의치용 지지체로서 작용한다. 이때, 세균침입을 막기위해 돌출부의 표면에 상피를 밀착시킬 필요가 있다. 그와같이 하기 위해서는 해당 부위를 항상 청결하게 유지시킬 것이 요구된다.
상술한 방법에서는 이따금 감염된 조직층으로 인하여 이식체와 뼈의 부착이 방해받는다. 이와같은 문제는 2단계 수술 기술, 즉 제1단계에서 이식체를 뼈속에 통째로(나사로) 틀어박고(ad modum Branemark, 참고 1.) 제2단계에서는 상기 제1단계 수술을 행하고 3-6개월 경과후, 돌출부를 이식체상에 올려놓는다. 결과는 만족스러웠다.
상술한 방법을 변형시킨 여러 방법들이 발표된 바 있다(IK-이식, IMZ-이식, 코어-벤트-이식). 사용된 재료는 백금 또는 플라스마 분무(plasma spraying)(TiO2)를 이용한 Ti와 Co-Cr 합금이다. 제조업자들은 수술후 곧바로 중량 가능한 것으로 선전하나, 치유기간으로서 3-6개월이 요구된다. 주된 결점은 방법이 어렵고 복잡하다는 것이다.
보다 간단하기는 하지만 효용성이 적은 방법으로서 스트라우만(Straumann) 방법을 들 수 있으며, 그에 따르면 단 한 번의 수술로 공동이 뚫린 실린더를 치육을 거쳐 뼈속에 접어넣게 된다. 재료는 플라즈마 분무에 의해 TiO2가 피복된 Co-Cr 합금이다. 상기 실린더는 그의 비효용성으로 인해 1986년 시판이 중단되었다.
현재는 세번째 유형의 이식체로서 발치후 확장된 치근 공동내에 곧바로 집어넣는 다결정성 Al2O2-세라믹, 프리알리트(Frialit)-이식체가 사용되고 있다. 형태면에 있어서, 상기 이식체는 끝으로 갈수록 뚜렷한 단계들로 좁아진다. 세라믹의 부서지기 쉬운 특성으로 인하여, 상기 이식체는 앞니와 같이 교합압력을 적게 받는 단일 치아를 지탱시켜 주는 데에만 사용될 수 있다.
일본 바이오세람 Al-크리스탈-사파이어 이식체는 교세란(Kyoceran's) 이식체로서 프리알리트 이식체보다 더 작다. 이들 이식체는 다른 강도 특성들면에서는 높은 값을 가지나 연성이 낮다. 또한 재료가 부서지기 쉬운 특성을 지니므로 어닐링 공정을 조심스럽게 행하여야 한다. 수득된 임상결과는 제조업자들이 선전하는 결과들과 일치하지는 않는다. 바이오세람과 뼈의 접착이 만족스럽지 못한 것으로 나타난다. 그 이유는 아마도 표면의 낮은 마찰과 불활성 및 작은 표면적에 기인된 것으로 추론된다.
장기간 치유 공정을 거친다 하더라도 강력하게 부가시키는데 충분치 못하다.
거대하고 간단한 부가 메카니즘에서 요구되는 뼈의 최소치수는 깊이 10㎜ 및 넓이 5.5㎜(스트라우만)이다. 발치후, 치조뼈가 크게 수축되어 상기 이식체를 집어 넣기에 충분할 정도의 공간이 남지 않는다.
티탄 및 Al2O3둘다 얇은 구조물로서는 필요한 기계적 강도를 견딜 수가 없으므로, 상기 이식체들은 지나치게 큰 형태를 취하게 된다. 또한 Ti의 제조가 어렵다. 다른 한편으로, 작은 이식체(Branemark)는 복잡하고 값비싸며, 가장 작은 비금속(Bioceram) 이식체는 부서지기 쉽다.
사용되는 재료의 종류들을 다음과 같이 분류할 수 있다.
1. 생체 비적합성 금속:
Co-Cr-합금, 스테인레스 스틸, Ni-합금, 유형 Ⅳ의 귀금속(Indumed, Holland).
2. 생체조직과 반응하지는 않되 뼈에 밀착시킬 수 있는 불활성 금속:
Al2O3-세라믹, Ta-및 Ti-금속, 이들의 합금(TiV4A16), 다른 형태의 탄소, 테플론
3. 뼈에 재빨리 및 원활하게 부가되는, 즉 뼈의 표면과의 반응성을 지니며, 뼈의 성장을 유도하는 생체활성 물질:
히드록실 아파타이트, CaP-글래스 및 CaP-글래스세라믹 바이오글래스, 트리칼슘포스페이트, 즉 TCP(재흡수, 즉 조직내에 용해됨).
조성 및 제조방법이 이들 재료의 재흡수에 영향을 미친다. 즉 완전히 재흡수되는 경우로부터 재흡수가 일어나지 않는 상황에 이르기까지 다양하게 달라진다.
이하의 표에는 사용하는 재료 및 그의 특성들을 나타낸다.
* 인장강도/파쇄 스트레스
+++ 뼈 및 상피와의 결합이 매우 양호하고 나아가 뼈의 성장이 유도되는 경우
++ 뼈와의 결합이 매우 양호

현재, 사용되고 있는 생체활성 물질 및 그 상황은 다음과 같다.
1. 금속과 히드록실 아파타이트(HA)와의 접합이 문제점이다. 기계적 접합이 시도된 바 있으나, HA의 부서지기 쉬운 특성이 문제가 되며, 만족스러운 결과가 수득되지 않는다. 또한, 플라즈마 분무기술도 시도된바 있으나, 결정구조가 보존되지 않고 표면만이 용이하게 재흡수된 TCP로 개질되므로 접합부가 내구성을 지니지 못한다(World High Tec. Congress, Milano 1986). 산호를 주성분으로 하는 HA도 또한 합성된 것보다 조금도 더 우수하지 않다. 그러나, HA는 값이 저렴한 재료이고, 상기 HA상에 조직의 콜라겐 섬유가 부가되어 매우 급속히 무기질화된다(쟈르코(Jarcho)등의 각종 연구 ; 참조 2 de Putter, de Lange, de Groot: 치밀한 히드록실 아파타이트의 퍼뮤코잘 치아이식(Permucosal dental implants of dense hydroxyl apatite). 치조뼈내의 고정(Fixation in alveolar bone) ; 경구 및 악골안면 복구시의 조직 동화에 관한 국제회의(Int. Congress on tissue integration in oral and maxillofacial reconstruction) 초록, 브뤼셀 1985).
2. Cap-글래스 및 유리 세라믹, 즉 소위 바이오-글래스(bio-glass)가 양호한 뼈와의 결합을 형성하는 것으로 보고된 바 있다(헨케(Hencke)등, 1971; 참고 3. 그로스 에트 스트롱(Gross et Strung) : 골질 이식층과 각종 유리 및 유리 세라믹의 계면(The interface of various glasses and glass ceramics with a bony implantation bed). J. Biomed. Mat. Res. 251-271; 19, 1985). 상기 반응은 SiO2-풍부층의 생성 및 Ca와 P의 침전을 기초로 한다. 이어서 Ca와 P는 표면에 부가된 콜라겐 섬유 부근에서 HA로 결정화된다. 이상적인 방법은 다음과 같은 요건들을 충족시킨다:
1. 이식체는 작고 강하며 생체와의 적합성을 지녀야 한다.
－따라서, 탄성 Ti 및 알레르기 유발성이 강한 Ni-및 Cr-합금은 사용할 수가 없다.
－나머지 다른 것들은 고가의 귀금속(Au-,Pt-,Pd-합금) 또는 딱딱한 Ti-합금이다.
2. 이식체가 뼈 및 상피와 결합할 수 있는 표면을 가져야 하며, 뼈의 성장을 강화시키는 효능을 지녀야 한다.
위와 같은 표면들로서는:
－HA=히드록실 아파타이트
－TCP=트리칼슘 포스페이트
－바이오글래스 또는 유리세라믹이 있다.
현재는 충분히 작은 크기 및 높은 강도를 얻기 위한 목적으로 금속 핵심부를 갖는 이식체가 사용되고 있다. 상기 핵심부는 자체의 금속 산화물 함량에 기인되어 재흡수가 되지 않는 바이오글래스(CaP-글래스)로 피복되어 있다. 또한, 뼈 또는 생체물질에 재흡수될 수 있는 CaP-글래스로 구성된 외부층을 갖고 있다(참고. 미합중국 특허 제4497629호 오지노(Ogino)등). 따라서, 피상적으로는 상술한 요건 1과 2가 충족되나 ; 외부층이 재흡수됨에 따라 내부층이 조직과 만족스러운 결합을 형성하지 못한다.
[발명의 요약]
히드록실 아파타이트(HA)를 사용하는 것으로 의치부가가 개선되었다. 본 발명에 따르면, HA는 재흡수될 수 있는 외부층내에(재흡수성 CaP-글래스+HA) 또한 바람직하게는 재흡수될 수 없는 내부층내에도(비-재흡수성 CaP-글래스+HA) 사용된다. 또한, 중간층을 이용함으로써 상기 2가지 층들의 접합이 실질적으로 강화되며, 중간층은 조절가능한 재흡수율을 갖는 CaP-글래스를 갖고, 바람직하게는 HA를 함유한다. 본 발명은 원칙적으로 이하에 첨부하는 특허청구 범위에 기술하는 바에 의해 특징지워진다.
보다 구체적으로 설명하면, 첫번째 비-재흡수성 층은 10-100중량％의 CaP-글래스 및 0-90중량％의 HA를 함유하고 ; 천천히 재흡수되는 중간층은 10-100중량％의 CaP-글래스 및 0-90중량％의 HA를 함유하며 ; 또한 두번째 재흡수성층은 10-99중량％의 CaP-글래스 및 1-90중량％의 HA를 함유한다.
첫번째 층은 금속 산화물(들)을 첨가함으로써 비-재흡수성을 갖도록 할 수 있다. 상기 금속 산화물로서는 티탄, 알루미늄 및 지르코늄의 산화물이 적당하다. 첫번째 층은 또한 CaP-글래스 CaP, SiO2및/또는 Na2O간의 비율들을 변화시킴으로써 비-재흡수성을 갖도록 할 수도 있다.
이하에서, 치아, 즉 뼈속 이식체로서 적합한 본 발명의 용도에 관하여 상세히 기술하고자 한다.
[바람직한 구체예의 설명]
도면에 나타낸, 본 발명에 따른 이식체는 : 열팽창계수 10－15×10-6/K의 딱딱하고, 표면 산화된 금속 핵심부(1)(금속과 세라믹의 합금)을 갖고, 상기 핵심부는 CaP-글래스와 HA-세라믹의 혼합물(2)로서 보다 작은 열팽창계수 9－14×10-6/K를 갖는 강하고 치밀하며 부드럽고 재흡수가 되지 않는 청동 바이오글래스로 피복되어 있으며, 상기(2)층은 CaP-글래스와 HA의 혼합물(3)로 피복되고, 이때 (3)층은 소결되어 치밀하거나 또는 다공성을 지닌 재흡수가 되지 않는 층으로 된 후 부드러운 CaP-글래스-HA 혼합물층(4)으로 피복되어 재흡수될 수 있게 되며 뼈의 계면에 대하여 뼈의 성장을 활성화시키고, 뼈에 대하여 과도한 압력을 유발함이 없이 곧바로 뼈와 이식체가 기계적으로 부착되도록 한다.
외부층(4)은 또한 필요에 따라 유약 처리될 수도 있다. 따라서, 본 발명의 핵심은 외부층(4)은 재흡수하고 금속상의 내부층(2)은 재흡수하지 못하는 층 구조이다. 따라서, 층(2)이 재흡수 억제제(예, 금속 산화물)를 함유하므로 금속 표면에서는 재흡수가 전혀 일어나지 않는다. 응용 발명으로서 기술한 바에 따르면 2와 4사이에 반-재흡수성 중간층이 존재한다. 또한, 모든 CaP-글래스층 2, 3 및 4에 HA가 부가되어 존재한다. 층들(및 HA)과 금속 및 층들 상호간의 접합은 유리에 기인되어 강력하고 안정하다. 특히, 따라서 HA도 또한 금속과 접합된다는 것에 주의하여야 한다. 그러나, 대부분의 경우에 있어서, HA를 금속표면상의 층(2)에 부가하지 않는 경우, 또는 CaP-글래스-HA 혼합물층(3)을 제거(배제)한 경우에 있어서만 만족스러운 안정한 결과를 수득하는 것이 가능하다.
강력한 구조가 필요치 않은 경우에는 금속 핵심부를 배제시킨다. 이는, 예를 들면 이식체를 뼈강화용으로 사용하는 경우에 있어서 가능하다.
유리/HA 혼합물의 재흡수율은 자체의 조성을 제어함으로써 조절할 수 있고, 더불어 부가물(예, 금속 산화물)들도 순수한 CaP-글래스의 재흡수율과 동일하게 조절할 수 있다. 본 발명에 따른 CaP-글래스/HA 혼합물 표면은 신속하게(1-8시간내에) 세포에 부가되며, 이식체가 무기물을 함유하는 것에 기인하여 무기질화 또한 재빨리 일어난다. 상기 뼈와의 결합은, 이식체가 뼈의 성장을 유도하는 효능을 가지는 것에 기인하여 약한 뼈 구조에서도 또한 강력하게 일어난다. 따라서, 본 발명의 이식체 구조는 종래보다 더 작게 만들 수 있다. 또한, 필요에 따라 금속 핵심부가, 자체에 부가된 세라믹 구조를 지탱시켜 준다. 표면에 유약처리를 행할 경우, 뼈와 이식체 사이에 존재하는 헐거운 연결 조직과 상피의 분리를 방지할 수 있다. 나아가, 상이한 구조들로 본 발명을 실시할 수 있다. 이것들을 이하의 표에 나타냈다 : 층 2, 3 및 4에 있어서 다른 적당한 조성을 가질 수도 있다 :
－금속 표면이 횡행구(transverse groove)를 가질 수 있다.
－이식체의 형태가 원뿔형, 원통형 또는 세믹형을 취할 수 있고, 아울러 다양한 크기를 가질 수 있다 :
두께 예. 2.5-7.0㎜
길이 예. 7.0-700㎚
－치유 기간동안 이식체 상부에 세라믹 캡을 위치시킬 수 있다.
－골막 하부의 뼈속에 이식체를 통째로 매립시키거나, 또는 부분적으로 치육외부에 남길 수 있다.
본 발명에 따른 골내 이식체의 기타 다른 실시품은 다음과 같다 :
B1. 골막과 뼈 사이에 위치시킨 골막하 이식체 제3도에 있어서 ; (1)층은 금속이고, (2)층은 불활성 바이오글래스이며, (3)층은 HA/바이오글래스 혼합물이고, (4)층은 재흡수될 수 있는 HA/바이오글래스 혼합물이다.
B2. 접합 보철상의 층들은 상술한 것과 동일하고, 나아가 접합 표면은 불활성의 치밀한 유리 세라믹 또는 금속이다.
제4도에 나타낸 구조에서 층(1), (2), (3) 및 (4)는 전술한 바와 같다.
C. 뼈의 공동을 채우는 골내 과립.
제5도에서 층(2), (3) 및 (4)는 전술한 바와 같다. 충전시, 제5도에 나타낸 바와 같이 직경 1-5㎜를 갖는, 다음과 같은 큰 과립들이 사용되었다.
(a) 과립의 핵심부는 천천히 재흡수되고, 표면은 재빨리 재흡수된다.
(b) 과립의 일부는 재흡수되지 않고(2), 일부는 천천히 재흡수되며(3), 일부는 재빨리 재흡수된다(4).
(c) 과립의 일부는 천천히 재흡수되고(3), 나머지 부분들은 재빨리 재흡수(2)되거나 또는 재흡수되지 않는다(4).
(d) 과립의 일부가 천천히(3) 또는 재빨리(4) 재흡수된다.
D. 골막하 및 치근막 과립 충전은 상술한 바와 같으나, 과립들이 작고 천천히 재흡수된다. 직경은 0.1∼1㎜ 사이이고, 층들은 갖지 않는다.
C 및 D는 공간에 따라 목적하는 형태를 제공하는 경화성 플라스틱 매트릭스일 수도 있다.
E. 제6도는 뼈 치환용 과립형 바이오 세라믹블록의 다양한 예들을 나타낸다. 이와 같은 세라믹 블록은 제6(a)도에 나타낸 바와 같이 부분적으로 또는 전체가 천천히 재흡수되거나(3) 또는 재빨리 재흡수되는 CaP-글래스/HA 혼합물 표면층을 가지므로 뼈와 결합을 형성할 수 있다. 또한, 제6(b)도에 나타낸 바와 같이 블록에 금속 부분들을 미리 부가시킬 수도 있다. 블록은 이들 천천히 재흡수되는 것들(3)과 재흡수되지 않는(2) CaP-글래스/HA 혼합물 물질의 경납층으로 구성된다. 또한, 뼈와의 결합을 강화시키고자 하는 경우에 있어서는 외부 표면층이 재빨리 재흡수되는(4) 물질로 구성되도록 할 수도 있다.

Claims (10)

  1. (a) 바이오글래스, 즉 CaP-글래스를 함유하는, 재흡수되지 않는 첫번째 층(2) ; (b) CaP-글래스와 히드록실 아파타이트의 혼합물인, 뼈 또는 생체조직내로 재흡수될 수 있는 두번째 층(4) ; 및 (c) CaP-글래스를 함유하고, 재흡수율이 두번째 층(4)보다 더 느린 재흡수성을 갖는 상기 첫번째와 두번째 층 사이에 배치된 중간층(3)으로 구성되며 ; 상기 히드록실 아파타이트가 두번째 층(4)내에 포함됨으로써 이식체와 조직과의 접합 안정성이 확보되는 치아 또는 다른 부위의 뼈 조직 치환용 이식체.
  2. 제1항에 있어서, 강도가 요구되는 경우, 상기 첫번째 층(2)이 부착되는 금속 핵심부(1)를 추가로 포함하는 이식체.
  3. 제1항 또는 제2항에 있어서, 상기 첫번째 층이 금속 산화물(들)의 첨가에 의해 재흡수되지 않도록 만들어진 이식체.
  4. 제1항에 있어서, 중간층(3)이 CaP-글래스와 히드록실 아파타이트의 혼합물인 이식체.
  5. 제1항에 있어서, 첫번째 층(2)이 CaP-글래스와 히드록실 아파타이트의 혼합물인 이식체.
  6. 제1항에 있어서, 금속 핵심부(1) ; 상기 금속 핵심부에 부착된, CaP-글래스와 히드록실 아파타이트의 혼합물로 이루어진 재흡수되지 않는 내부층(2) ; CaP-글래스와 히드록실 아파타이트의 혼합물로 이루어진, 재흡수될 수 있는 외부층(4) ; 및 상기 내부 및 외부층 사이에 배치된, CaP-글래스와 히드록실 아파타이트의 혼합물로 이루어진, 재흡수율이 외부층(4)보다 더 느린 재흡수성을 갖는 중간층(3)으로 구성된 치아 치환용 이식체.
  7. 제1항에 있어서, 이식체가 접합부에 사용되어 접합부의 뼈 성분이 내부에 존재할 경우 이식체는 CaP-글래스와 히드록실 아파타이트의 혼합물로 이루어진, 재흡수될 수 있는 내부층(4) ; CaP-글래스를 함유하는, 재흡수되지 않는 외부층(2) ; 및 CaP-글래스와 히드록실 아파타이트의 혼합물로 이루어진, 재흡수율이 내부층(4)보다 더 느린 재흡수성을 갖는 중간층(3)으로 구성되고, 접합부의 뼈 성분이 외부에 존재하는 경우에는 그 반대인 이식체.
  8. CaP-글래스를 함유하는 재흡수되지 않는 내부층(2)을 갖는 과립 ; CaP-글래스와 히드록실 아파타이트의 혼합물로 이루어진 재흡수될 수 있는 외부층(4) ; 및 CaP-글래스와 히드록실 아파타이트의 혼합물로 이루어진, 재흡수율이 외부층(4)보다 더 느린 재흡수성을 갖는 중간층(3)으로 구성되고 ; 상기 과립들이 뼈와 또는 그들의 재흡수된 뼈를 거쳐서 과립 상호간에 부착되어 있는, 뼈 공동내에 사용될 수 있는 과립 충전제.
  9. 일부 또는 전체가 CaP-글래스와 히드록실 아파타이트의 천천히(3) 또는 재빨리(4) 재흡수될 수 있는 혼합물로 만들어진 뼈 치환용 블록.
  10. 제9항에 있어서, 재흡수되지 않는 CaP-글래스와 히드록실 아파타이트의 혼합물(2)에 블록에 부가된 금속 부분(1)을 추가로 포함하는 블록.
