Abstract

Description
【技术领域】
本发明涉及医学和医疗技术领域,即骨植入物。这些是替换或补充骨骼组织或连接骨骼部分的植入物,通常它们完全或部分固定在骨骼组织中。
【背景技术】
最常用作骨植入物的无机材料植入物可分为两大类-金属植入物和由氧化物材料制成的植入物,特别是氧化铝,氧化锆,氧化硅。
金属植入物在发现钛的骨质集成后被广泛使用(Branemark,1969年)。纯钛及其合金(钛与V,Al,Nb,Zr)仍然最常用作植入物的基础。为了提高生物兼容性,钛植入物的表面经常被修改。喷砂、蚀刻和氧化增加了钛植入物表面的微粗糙度和亲水性,提高了骨积分化的速度。
由基于氧化物的材料制成的植入物的优点是耐化学性。多晶,特别是陶瓷,以及单晶和玻璃氧化物材料被用于制造植入物。已知由包含多晶氧化铝的陶瓷制成的植入物(Hemke,1975)。也已知单晶氧化铝植入物(Hirabayashi,1978)。单晶氧化铝的压缩和拉伸强度以及耐化学性非常高。
由ZrO2基氧化物陶瓷制成的植入物强度高,美观性高;它们的应用频率仅次于钛植入物。以伊特朗定硅基材料具有高化学和生物稳定性、高强度、相对较低的杨模量(Yilmaz,2007年)。
自从以SiO2-P2O5-CaO-Na2O组成的45S5生物玻璃以来,基于氧化硅的骨植入物是已知的(Hench,1971)。生物玻璃可以粉末形式用作填充骨缺损的植骨填充剂。骨整合的生物玻璃随后被活骨组织取代。最初假设骨整合是由来自45S5生物玻璃的P2O5和CaO诱导的,该生物玻璃的生物玻璃的组成与骨骼组织的羟基磷灰石组成近似。然而,进一步发现无P2O5和无CaO的硅酸盐玻璃也确实具有骨诱导特性(Hench,1979)。
硅酸盐材料的强度增强和减少吸收也以二硅酸盐锂玻璃陶瓷(Barrett,1980年)和阿帕提特狼星玻璃陶瓷(Yoshida,1985年)制成的人造牙齿而闻名。这种组合物的硅酸盐生物陶瓷与周围组织具有极佳的兼容性,强度高,远远高于生物玻璃。
与钛类似,含硅植入物的生物兼容性和骨整合能力取决于表面结构。通过将样品置于SBF溶液(SBF-模拟体液)中,进行了确定氧化硅材料各个表面上羟基磷灰石晶体形成的实验(Li,1992)。SBF测试通常用于评估骨整合的可能性,并作为材料生物兼容性的指标。已经发现,羟基磷灰石层在SBF中形成在具有发达的粗糙表面的硅胶表面上,但是对于具有光滑表面的二氧化硅玻璃却不是这种情况。
【发明内容】
在本发明中,骨植入物完全由或部分由硅酸盐岩石或天然或人工矿物骨料制成,其密集缠绕纤维或精细晶体结构称为"玉石"。
影响其应用成功的骨植入物最重要的特性是机械强度、耐化学性、生物兼容性、低电化学活性、杨的模量、集成到骨骼中的能力(骨合一)。
钛植入物具有出色的强度,骨整合能力,既定的医疗规程和长期使用。金属植入物(包括钛植入物)的缺点是金属的电化学活性,在使用钛及其合金植入物时会引起不同的并发症(Pozhitkov,2015年)。
用羟基磷灰石或生物玻璃涂覆钛植入物的表面可以改善植入物的骨塑性,减少电化学活性,但由于应用表面层的再吸收,这种效果是短暂的,而且较差。
氧化铝非常耐用,但具有生物惰性,在生理pH下具有高的杨氏模量和正的ζ电势。随着植入物的长期使用,随着时间的流逝,这可能导致骨整合和骨吸收方面的困难。
氧化锆陶瓷在水性环境中可能的老化可能导致长期使用过程中氧化锆植入物稳定性的不可靠预测。
硅酸盐生物玻璃具有高电感和导电性,但强度低,易碎,容易在骨骼中长时间停留而产生再吸收。这些特性使得很难将它们用作不可再移植的骨植入物。二硅酸盐和阿帕提特-沃拉氏石硅酸盐生物陶瓷的强度比双眼镜高,但长期使用的预后未知。
这些数据显示了在医学中使用硅酸盐材料制成的骨植入物的前景,但是许多此类材料制成的植入物的性质限制了其应用范围。
在本发明中,为了扩大植入骨组织的医疗设备的范围,并获得能够分离的高强度植入物,骨植入物由硅酸盐矿物骨料和具有密集缠绕纤维或细晶体结构的岩石制成,俗称"玉器"。
作为翡翠,除其他外,已知以下矿物聚集体和岩石:翡翠玉Na(Al,Fe)Si2O6,软玉翡翠Ca2(Mg,Fe)5Si8O22(OH)2,硬硅钙石(硬硅钙石玉)Ca6(Si6O17(OH)2),维苏石(加利福尼亚玉)Ca10(Mg,Fe)2Al4((OH)4(SiO4)5(Si2O7)2),方钠石(新玉)(Mg,Fe)3(OH)4Si2O5,沸石(阿拉斯加玉)NaCa2(Si3O8(OH)),水硬性(Transvaal玉)Ca3Al2(SiO4)2(OH)4。
与许多矿物、矿物集料和岩石一样,由于异构现象,玉石的成分可能略有不同,具体取决于地层条件和沉积位置,即成分的变异性,同时保持晶体结构和材料特性。玉石是巨大而坚硬的矿物骨料和硅酸盐岩石,发生在自然界中,或是具有相似结构和成分的合成材料。出乎意料的是,这些材料的植入物具有融入骨组织的最佳特性。
这些矿物聚集体和岩石具有抗开裂性,具有致密坚韧的结构,结合了相对较低的弹性模量即可提供高强度。除翡翠外,其他翡翠均为氢硅酸盐。它们是半透明的,薄层半透明的,通常会获得特征性的粗糙亚光表面。自古以来,玉器在石材切割行业就受到高度评价,许多工具(刀,斧头)和艺术品(浮雕,小雕像,珠宝)都是用玉器制成的。这些矿物骨料的机械性能可以与钛和钢竞争,并且抗压强度优于许多合金。然而,玉器的性质与整合入骨组织的可能性有关,并未引起研究人员的注意。
玉石的组成包括氧化硅,这使得它们在生物兼容性和骨积分整合能力方面很有希望。这些硅酸盐的纤维结构提供弹性和微粗糙度,需要与骨组织形成牢固的结合。玉石作为介电不参与电化学反应,是亲水性的,在生理条件下具有负泽塔潜力。考虑玉石的特性,让我们希望根据本发明对由玉器制成的高质量骨植入物进行改造。
在评估玉石作为骨植入材料的应用可能性时,已通过实验证实了其高强度。在测试中使用了没有裂纹,内含物和其他缺陷的抛光玉石样品。可以确定玉石具有高的抗压强度(大于100MPa),弯曲强度(大于50MPa),高的冲击强度,高的断裂韧性(大于2MPa/m1/2)。玉的杨氏模量相对较低,可与钛的弹性模量(100-250GPa)相媲美。
对水溶液中材料特性的试验表明,玉石是亲水的,润湿角小于90度。在玉石中,泽塔电位是负的,对于能够进行骨质整合的材料来说,这是典型的。
这些结果表明使用玉石生产骨植入物的前景。然而,这些硅酸盐与骨组织的相互作用以前没有被研究过。以下结果表明,由玉石制成的植入物具有出乎意料的良好生物兼容性和骨整合能力。
为了对生物兼容性进行初步评估,根据该程序将玉石置于SBF溶液中(Li,1992年)。在实验开始的1-4周后,在玉石的表面上形成了羟基磷灰石晶体,从而证实了生物兼容性的观点以及根据本发明的玉植入物骨整合的可能性。
为了在体外测试玉器植入物的生物兼容性,研究了它们与小鼠成骨细胞的相互作用。已发现玉器植入物对细胞没有毒性作用,成骨细胞附着在玉器表面并增殖。
将玉植入物植入骨组织中以验证骨整合。将由玉石制成的圆柱形植入物放置在豚鼠胫骨的一个孔中。在植入物的末端有用于测量扭矩的小平面。经过6周的实验后,测量了植入物破裂所需的扭矩,发现植入植入物后的扭矩超过将植入物接种到骨组织中之后的扭矩(大于5N/cm)。这些数据证实了根据本发明的植入物骨整合的能力。
根据本发明,玉器制成的骨植入物具有机械性能高、亲水性高、生物兼容性高、具有骨集成性、低电化学活性、高审美特性等特点。玉器植入物的另一个优点是其介电特性。低电导率可减少使用电信号的MRI、CT和其他医疗程序的问题。玉器植入物固有的特性显示了它们在手术、骨科、牙科中使用的承诺。
【具体实施方式】
根据本发明,一个示范性的、非详尽的玉器植入物清单包括用于更换骨组织的植入物、颗粒状体积填充物,以填充骨组织缺陷的成分、骨螺钉、骨融合植入物、固定植入物或植入物针脚、颅重建植入物、骨科植入物、透皮性骨质集成植入物、牙科植入物。
对于矫形植入物,玉体的高强度特别重要,这使得其可用于内风的关节和重型部分的植入物。骨科植入物组包括髋关节植入物、膝关节植入物、肩关节植入物、肘关节植入物、关节植入物、踝关节植入物、下牙关节植入物、骨关节植入物、手指关节植入物,或一般、关节植入物、脊柱植入物,包括椎骨植入物和椎间盘植入物,径向骨骼头部植入物,拇指植入物,骨切除术(高骨切除术)植入物。特别首选的骨科植入物是关节植入物,特别是髋关节植入物和膝关节植入物。髋关节的植入可以是头部假肢和骨干假体(内假体干或皮质的茎)和科蒂利酮腔假肢。膝关节植入物可以包括膝关节的头骨和股骨成分。
