Definitions
- the present invention relates to a method for inserting an intraocular lens (IOL) and a set for recording resp. Insert an intraocular lens.
- IOL intraocular lens
- lens carriers or cartridges have been developed for inserting an intraocular lens, into which a lens can be loaded and then ejected from the lens carrier by means of an injector.
- Examples of such lens carriers or cartridges and injectors are known from the American patents US 6,267,768, US 5,810,833, US 6,283,975 and US 6,248,111.
- the aforementioned documents are hereby incorporated by reference into the present application.
- the injector is an operating tool with a sleeve and a plunger which is received in the sleeve so as to be axially movable.
- a recess for example a slot, is provided in the jacket of the sleeve, into which a lens carrier can be loaded.
- the lens carrier has a mostly cylindrical loading channel, on which an injector nozzle tapering axially towards the tip (distal end of the Lens wearer). The lens holder is held in the injector so that the plunger is flush with the charging channel. When advancing, the plunger penetrates the loading channel and pushes the lens out of the injector nozzle.
- US 6,248,111 proposes to coat the inner wall of the injector nozzle with a surface structure, e.g. Trenches or the like, so that the lens comes into contact with only a part of the wall when passing through the injection channel. In this way, the friction between the inner wall and the lens and thus the ejection force should be reduced.
- US 6,283,975 proposes an injection system with improved lubrication properties.
- a water-soluble lubricant e.g. a surfactant, physically bound to the plasma-treated inner wall of the injector nozzle.
- the lubricant is bound differently than by a covalent chemical bond.
- the lubricant present on the inner wall is intended to reduce the force for applying the lens.
- the injector plunger which is usually made of metal, act directly on the lens, but to insert a plastic buffer between the plunger and the lens.
- the softer plastic buffer compared to the injector plunger reduces the risk of damage to the lens when it comes into direct contact with it.
- the intraocular lenses that are mostly used today are made of a hydrophilic material, usually based on acrylic. Such lenses are supplied sterile packed by the manufacturer in a liquid bath. Storage in a liquid is necessary to protect the lens from drying out. During the operation, the lens must be removed from the packaging in the sterile area and inserted into the cartridge and then this cartridge must be loaded into the injector.
- the lenses represent very small and resilient structures, there is a risk when the cartridge is loaded that the lens is dropped or jumps off when folded, thereby losing its sterility.
- the lenses are very sensitive structures that can easily be damaged when folded and reloaded into a cartridge.
- the risk of damage is particularly great for the so-called haptic, which surrounds the optical part of the lens. It is also possible for the optical part to be damaged, for example, by the tweezers when it is inserted into the cartridge. This risk is particularly great because, as a rule, the lens does not just have to be inserted into the cartridge, but also has to be folded when it is inserted.
- the need to populate the lens carrier or cartridge during surgery and the resulting mishandling are responsible for most complications during or after the injection.
- the aim is in particular to propose a method with which the risk of damage to the lens during implantation is significantly reduced.
- a tool set is to be proposed with which the implantation of a lens through an injector nozzle opening of less than 2 mm is possible.
- the object is achieved by a method in which, for ejecting the intraocular lens from the lens carrier, the cartridge generates an overpressure from the proximal opening and displaces the lens together with at least part of the lubricant from the charging channel into the injector nozzle and finally pushes it out becomes.
- the method according to the invention has the advantage that the lens can be ejected from the cartridge without direct or at least with a greatly reduced contact with a plunger.
- the lubricant also has the function of a hydraulic medium.
- the hydraulic fluid transfers the from a displacement element, e.g. a piston, exerted pressure on the lens. Accordingly, the procedure is very gentle and damage to the lens during implantation is avoided.
- Another advantage is that this method allows a lens to be pressed through openings less than about 1.5 mm in diameter without being damaged.
- overpressure may be from a source of overpressure, e.g. Pump or pressure vessel, or by a displacement element, e.g. a piston, generated mechanically.
- a source of overpressure e.g. Pump or pressure vessel
- a displacement element e.g. a piston
- an at least liquid-tight connection can be produced between the piston skirt and the inner wall of the charging channel by means of an elastic and compressible material, so that when the piston is moved, the pressure is transferred to the lens via the existing lubricant.
- known injectors can be used to carry out the method.
- the lens carrier with the lens can be loaded into an injector having a movable plunger and a displacement element serving as a piston can be inserted or inserted into the proximal opening of the loading channel.
- a displacement element e.g. Piston
- an overpressure is generated which displaces the lens floating in the lubricant into the injector nozzle and finally ejects it.
- known injectors can be used is another advantage of the method. So much lubricant is advantageously used that the lens is received in a lubricant plug.
- the lubricant plug ensures low leakage resistance, since it ensures all-round and even lubrication. It is particularly advantageous that the lens and the lubricant are simultaneously displaced by the novel injection method.
- the displacement element can be placed on the plunger of an injector and then, after the loaded cartridge has been inserted into the injector, to insert it through the opening of the charging channel.
- the displacement element can already be inserted into the loading channel behind the lens when the lens carrier is loaded.
- the lens is expediently arranged at a distance from the displacement element and embedded in lubricant, so that a buffer zone formed of lubricant and possibly little air is present between the lens and the displacement element.
- the present invention also relates to a set, in particular a lens carrier set, for accommodating a lens with a lens carrier with a an opening-having charging channel for a lens and an injector nozzle which preferably adjoins the charging channel axially, and means for generating an overpressure which act through the opening.
- the overpressure means can be connected to the opening of the charging channel or can be inserted into it.
- the set according to the invention has the advantage that a lens can be gently implanted, in particular without direct or at least with a greatly reduced contact with an injector plunger.
- Sources of overpressure such as pumps or pressure vessels, can be used as overpressure as well as a piston that can be inserted sealingly into the charging channel.
- the displacement element can also be partially inserted into the injector nozzle. Silicon or soft elastic or foamed plastic have proven themselves as materials.
- the displacement element respectively. the material from which the displacement element is made has a degree of hardness between 10 and 80 shore, preferably between 20 and 65 shore, and very particularly preferably between 25 and 40 shore.
- the displacement element is therefore preferably flexible and can adapt to the inside diameter of the charging channel and the injector nozzle while reducing the outside diameter and increasing the length.
- means are provided at the proximal end of the displacement element in order to detachably attach the displacement element to an injector plunger.
- These means can be realized by appropriate design of the tappet end and the displacement element.
- Displacement element and plunger can for example be form-fitting, e.g. interact with each other by means of plug connection.
- An expedient embodiment provides that the displacement element designed as a piston has a recess at the proximal (rear) end, e.g. Blind hole, in which the plunger can engage.
- the piston can be inserted into the loading channel when the lens carrier is loaded. Accordingly, the plunger and piston can collide on the face.
- the loading channel of the lens carrier is preferably cylindrical and consists of two half-shells that can be connected to one another. These can be unilaterally a film hinge.
- rectangular wings are preferably formed on the half-shells. When folded, the wings lie flat on top of each other.
- the charging channel is at least liquid-tightly closed by the wings in the circumferential direction.
- a snap lock is advantageously formed on the half-shells.
- An expedient embodiment provides that the lens carrier and the displacement element are stored in a sterile package. The sterile package can be opened immediately before the implantation and the lens can be loaded with the lubricant into the lens carrier. The set is usually intended for single use.
- the present invention also relates to an implantation set with a lens holder or cartridge, characterized by an injector with means for receiving a lens holder or cartridge and a movable plunger that can be inserted into the loading channel.
- the length of the plunger and of the displacement element is preferably dimensioned such that the displacement element is not ejected from the injector nozzle when the plunger is fully retracted.
- This set has the advantage that the plunger can be pushed in completely without fear that the displacement element could be ejected with the lens.
- An exemplary embodiment provides a set, which is characterized in that the lens is arranged in the lens holder in a tension-free state and that the lens holder together with the lens and optionally a holder for the lens holder are stored together in sterile packaging until they are used.
- the lens is inserted into the lens capsule bag during the operation by making the smallest possible incision in the folded state, where it then returns to its original shape. It is not possible to leave the lens in a folded state for a long time before using it, otherwise it will not return to its original shape after insertion. For this reason, the lens is stored on the lens carrier in a stress-free state until it is used.
- the lens holder is made of a relatively expensive, biocompatible material, because when it is stored together with the lens, especially when stored in a liquid, it must not release any harmful substances that could then be absorbed by the lens. But since the lens holder is relatively small Has dimensions, the material price for the lens holder is not important. What is essential for the material of the lens carrier is primarily its behavior during storage, while price, strength and sliding properties play a subordinate role.
- the lens does not have to be manipulated. It is therefore not necessary to insert the lens into the cartridge with any tools. Rather, the lens carrier together with the lens arranged therein need only be removed from the sterile packaging and inserted into the injector.
- the lens carrier therefore expediently has means which bring about the folding of the lens in a predetermined direction. It is possible to provide each half-shell with a longitudinal groove as a simple means for folding the lens in a predetermined direction, in such a way that the longitudinal groove comes to lie close to the side of the half-shell opposite the film hinge in order to grasp the edge of the lens.
- the longitudinal grooves are therefore arranged outside the middle of the half shells. It has been shown that the lens is always folded in the intended direction with great certainty.
- the half-shells are expediently arranged in the unfolded state in such a way that the greatest internal width between the half-shells corresponds to the lens diameter of preferably 5 to 7 mm.
- This has the advantage that when the lens holder is removed with the lens, the lens cannot fall out of the lens holder.
- the half-shells can thus be arranged at an angle of 60 to 150 degrees, preferably 90 degrees to one another.
- the invention also relates to a set which has an injector, in particular a single-use injector, with an injector body and a plunger with a plug. It proves expedient for the injection of a lens to provide such a set. There is then a guarantee that an improper injector will not be mistakenly attempted to inject the lens.
- the disposable injector advantageously consists of an injector housing consisting of two parts, in which a nozzle part, which has a channel, is inserted is which narrows conically towards the nozzle.
- the nozzle part can consist of a material with low sliding friction and have the correct dimensions for the lens included in the set. Such an injector can be manufactured inexpensively.
- the injector housing advantageously has an inlet to a space which is located in front of the opening into which the lens holder is inserted before the operation. Through this inlet, the entire space and the remaining cavity in the lens carrier can be filled with viscoelastic liquid.
- the packaging e.g. a bottle, the lens holder with the tension-free lens held in it by a holder. With this holder, the lens holder can be easily removed from the packaging.
- Fastening means for the lens support can be formed on the holder and the lens support can contain residual means, e.g. have a cam formed on a tab for cooperation with the fastening means.
- An opening in the holder which is dimensioned in such a way that the lens in the lens holder is kept stress-free, advantageously serves as the fastening means.
- a recess is expediently present in at least one side wall of the opening, into which a cam of the lens carrier can engage.
- a securing element can be provided which can be pushed between the lobes of the lens carrier.
- a stop on the bottom of the opening of the holder and a stop on the securing element can be provided.
- the holder is advantageous as a closure, e.g. formed as a stopper, or as part of the closure of the packaging.

- FIG. 17 shows the two housing halves of the injector from FIG. 16,
- FIG. 19 shows a nozzle part with a larger diameter than in FIG. 16.
- the lens holder 11 shown in FIGS. 1 to 3 has a cylindrical charging channel 13 with a proximal inlet opening 14 and an injector nozzle 15 which is aligned with the charging channel 13.
- the injector nozzle 15 is tapered towards the tip. It has a beveled tip 18 with a distal outlet opening 17. The beveled tip 18 allows the injector nozzle to be introduced into the eye through a small incision of 2 to 3 mm in width.
- the lens holder 11 can be folded apart or. foldable.
- the loading channel 13 consists of two cylindrical half-shells 19a, 19b which are held together on one side by means of a film hinge 22.
- rectangular wings 21a, 21b are molded onto the half-shells 19a, 19b.
- lens carriers 11 Preferred embodiments of lens carriers 11 are described below with reference to FIGS. 12 to 13.
- a displacement means designed as a piston 23 (FIG. 4, FIG. 4a) is provided.
- the diameter of the piston 23 corresponds approximately to the diameter and preferably the inner contour of the charging channel 13.
- the piston 23 is largely able to seal the charging channel 13 to the rear (proximal end). It has a blind hole on the rear end face 25 27, in which the plunger 33 of a known injector can engage (FIGS. 4 and 5).
- An injector 29 which is frequently used for the implantation, has an elongated sleeve 31, in which a plunger is received in an axially displaceable manner.
- a plunger 33 is used, which has a smaller diameter than the inner diameter of the charging channel 13.
- a slot-like recess 35 is provided, into which a lens holder 11 can be inserted. If the lens carrier 11 is inserted into the injector 29, the charging channel 13 is aligned with the plunger 33.
- An injector (FIG. 8) can also be used in which the plunger 33 has a screw thread 37 which has a corresponding internal thread Sleeve 31 interacts.
- the injectors can also be designed in such a way that the lens carrier 11 can be inserted from the front.
- the lens holder is used as follows: A lens holder 11 to be loaded is removed from the sterile packaging and opened.
- the charging channel 13 is then at least partially, but preferably completely, filled with a viscous lubricant 39 (FIG. 2).
- An intraocular lens (IOL) 41 is then placed in the loading channel 13 filled with lubricant 39 such that lubricant 39 is located both in front of and behind the lens (FIG. 3).
- “Front” means closer to the distal opening 17 and "behind” closer to the proximal entry opening 14. Then the lens holder 11 is folded up. When folded, the lens 41 folds.
- a resilient displacement element for example a piston 23, is then placed on the plunger 33 of the injector 29, and the lens carrier 11 is loaded into the slot-like recess 35 of the injector 29.
- the plunger 33 is pushed forward into the sleeve 31.
- the piston 23 applied to the plunger 33 penetrates into the charging channel 13. Since the piston 23 largely seals the charging channel 13 on one side, when the piston 23 is advanced further, the lubricant 39 together with the lens 41 accommodated therein is first displaced into the injector nozzle 15 and then ejected from the latter.
- the compressible and deformable piston 23 has the property that it largely adapts to the channel diameter.
- the piston can preferably at least partially into the tapered injector nozzle, which is a smaller diameter at the front has to be inserted as the loading channel.
- the piston stretches accordingly in length while maintaining volume, as a rule by at least a factor of 1.5.
- the charging channel has a diameter between 2.5 and 3 mm and the injector nozzle has a diameter of less than 2 mm.
- a holder 43 is provided for placing the piston 23 on the tappet 33 (FIG. 9).
- the holder 43 has a handle 45 for gripping with the fingers and a receptacle 47 for receiving a piston 23 made of an elastomeric material, e.g. Silicone, the hardness of which is between 10 to 80 shore, preferably between 20 to 65, and very particularly preferably between 25 and 40 shore.
- a shield 49 is arranged between the handle 45 and the receptacle.
- the receptacle 47 consists of a blind hole or a through hole.
- the receptacle 47 is provided with flat side walls in cross section, so that a piston 23 arranged in the receptacle touches the side walls of the receptacle 47 in three or four limited, flat areas.
- the holder 43 is hollow cylindrical.
- the handle 45 is provided with ribs for better grip.
- the shield 49 is trimmed such that it lies on a flat surface on a line transverse to a hollow cylinder axis of the handle 45. This prevents the holder from rolling away on an inclined plane.
- the holder 43 with the piston 23 present in the receptacle 47 is taken in one hand and the plunger 33 in the other hand.
- the blind hole 27 on the piston 23 is provided at the end of the piston 23 which faces away from the handle 45. Now the front end of the plunger is inserted into the blind hole 27 on the piston 23. Now the plunger with the piston 23 attached can be separated from the holder 43. The plunger 33 equipped with the piston 23 can now be inserted into the charging channel 13.
- the 10 shows a cartridge 10 for use with an injector.
- the set for the cartridge 10 contains a lens carrier 11 and a lens 41 arranged therein in a tension-free state according to FIG. 11 and the cartridge part 16.
- the lens carrier is advantageously designed as described later with reference to FIGS. 13 and 14.
- the lens carrier 11 consists of a biocompatible and autoclavable material. Lens carrier 11 and lens 41 are stored together in sterile packaging until use. Consists the lens made of hydrophilic material, the common storage takes place in a liquid.
- the lens carrier 11 can be folded together with the lens 41.
- the lens carrier 11 is formed by two half-shells 19a, 19b which are connected to one another by means of a film hinge 22.
- the shape of the lens carrier 11 is such that it has a preferably hollow cylindrical shape after folding (FIG. 1).
- the half-shells 19a, 19b are arranged at an angle of approximately 90 degrees. However, this angle can vary in a range from about 60 to 150 degrees.
- a wing 21a, 21b is formed on each half-shell, which facilitates the folding of the lens carrier 11.
- the cartridge part 16 (FIG. 10) consists of a material with good sliding properties for the lens, has a cylindrical shape and is provided with an injector nozzle 15.
- a receiving opening 20 extends coaxially to the channel 24 of the injector nozzle 15. This channel 24 narrows slightly conically towards the nozzle 17.
- the receiving opening 20 has a larger diameter than the outer diameter of the lens carrier after the folding.
- Ribs 24, 26, 28 extend inward to receive the lens carrier 11, a groove being provided for receiving the wings 21a, 21b of the lens carrier 11.
- the set can also be the displacement element 23 (Fig. 4) made of elastomeric material, e.g. Silicone.
- the lens 41 is placed in the lens holder 11. It is practically de-energized.
- the lens carrier 11 with the lens 41 is then packaged sterile.
- the sterile packaging is usually made of polycarbonate or glass and is filled with a storage liquid in the case of hydrophilic lenses. In this sterile packaging, the lens 41 can be sterilized together with the lens carrier 11 in an autoclave at 140 degrees.
- the lens carrier 11 with the lens 41 is removed from the sterile package.
- the lens carrier can be carefully gripped and folded on the wings 21a, 21b using a suitable tool.
- the folded lens carrier 11 is then inserted into the receiving opening 20 of the cartridge part 16.
- the cartridge 10 is then ready to be inserted into the injector.
- a groove 30, a rib or some other suitable measure ensures the correct angular position of the cartridge 10 and thus also the lens 41 in the injector.
- an embodiment of the lens carrier 11 provides a longitudinal groove 32 on the inside of each half-shell 19a, 19b in the vicinity of the wing, which groove detects the lens edge of the lens 41.
- the arrangement of the longitudinal grooves 32 outside the center of the half-shells means that the lens 41 always folds against the hinge joint 22.
- a snap device 34 causes the lens carrier 11 to remain in the folded position after the lens 41 has been folded.
- the lens holder 11 according to FIGS. 12 and 13 has no injector nozzle and is intended for use with a separate injector nozzle (80, 80 '), as can be seen from FIGS. 16 and 19.
- FIG. 14, shows a lens carrier 11 which is of the same design as that of FIGS. 12 and 13, but has an injector nozzle 15 like that of FIGS. 1 to 3.
- a holder device is shown, which in the packaging, e.g. a bottle which the lens holder 11 holds with the lens 41 mounted tension-free therein.
- This holding device essentially consists of the holder 57 and the securing element 91.
- a biocompatible plastic is used as the material for the securing element.
- the holder 57 can be used as a plug for closing a package, e.g. Bottle, be trained. Silicone or another flexible, biocompatible material can be used as the material for the stopper. However, it is also possible to use the holder 57 as part of a closure, e.g. a screw cap.
- a biocompatible plastic can be used as the material.
- the opening 93 serves as a fastening means for the lens holder 11.
- the opening 93 there is a recess 95 in at least one side wall 94, into which a cam 96 of the lens holder 11 can snap.
- the lens holder 11 is otherwise of the same design as that described above with reference to FIGS. 12 and 13.
- the opening 93 is dimensioned such that the lens 41 in the lens holder 11 is kept free of tension. Depending on the size of the lens 41, the opening 93 is therefore dimensioned accordingly. However, the same lens holder 11 can be used for different lens sizes.
- the lens carrier 11 is first inserted into the holder 57 and then the lens 41 is inserted into the longitudinal grooves 32 using tweezers, whereupon the securing element 91 between the wings 21a, 21b the lens holder is pushed.
- the head 92 of the securing element 91 is dimensioned in accordance with the opening 93 and thus in accordance with the resulting opening angle between the wings 21a, 21b of the lens carrier 11. After inserting the securing element 91, static friction prevents the securing element 91 from falling out.
- the securing element 91 also causes the lens 41 to be axially positioned between the stop 97 at the bottom of the opening 93 and the stop 98 of the securing element.
- the holder 57 can now be turned through 180 degrees and inserted into a container.
- the lens carrier 11 In order to be able to remove the lens 41 from the holder 57 during the operation, the lens carrier 11 must be grasped on the wings 21a, 21b and pressed together. The lens 41 is folded and remains securely in this position after the snap device 34 has snapped in. Even if the user should lose the lens holder 11 when removing it from the holder 57 or when inserting it into the injector, the lens remains protected in the folded state. If the slipped lens carrier 11 has thus remained in the sterile area, the operation can be continued without problems and without disadvantageous consequences.
- FIG. 16 shows a preferred exemplary embodiment 73 of an injector.
- the housing 74 for example made of plastic, consists of the housing halves 75, 77 (FIG. 17).
- a housing half, e.g. 75, has pins 76 which can snap into recesses 78 in the other housing half 77.
- the injector 73 has a plunger 33 with a displacement element made of the elastomeric material previously described.
- a nozzle part 80 is inserted in the housing 74. In this there is a channel 81 which narrows conically towards the nozzle 83. Depending on the type and size of the lens to be injected, the nozzle 83 has a larger or smaller diameter.
- So Figure 19 shows a Nozzle part 80 'with a larger diameter of the nozzle 83 than in FIG. 16.
- the injector housing 74 has a lateral opening 85 into which the lens holder 11 can be inserted.
- an inlet channel 89 for the viscoelastic liquid, which fills the space 90 and the cavity in the lens carrier 11, is provided transversely to the longitudinal channel 87.
- the lens carrier 11 becomes sterile with the lens 41
- the lens 41 is conveyed through the channel of the nozzle part 80 without direct or at least greatly reduced contact, in which the viscoelastic liquid serves not only as a lubricant but also as a hydraulic medium.
- an overpressure is generated in order to eject a lens 41, which is floating in a lubricant 39, from the injector nozzle.
- the compressible and deformable piston 23 can adapt to the forwardly narrowing nozzle channel.
- the lens 41 is folded further on its way and has a very small diameter at the end of its way. The incision can therefore be kept very small.
- the set contains a lens holder 11 and a lens 41.
- the lens 41 is in a tension-free state in the lens holder 11.
- Lens 41 and lens holder 11 are preferably carried by a holder 57 and are packaged sterile until use in a package, in the case of one hydrophilic lens in a liquid that protects the lens from drying out.
- the lens carrier 11 together with the lens 41 stored therein is removed from the pack, folded and inserted into the injector. A viscoelastic liquid is then poured in through the channel 89.
- the lens 41 can now in the capsular bag treating eye are injected. There is no risk of the lens being injured when folding and injecting.
