Claims (25)
- 長尺状部材に結合されたハンドルアセンブリと、
前記長尺状部材の遠位端に結合されたドッキング部材であって、前記ドッキング部材は、心外膜アンカーデバイスに着脱可能に結合されるように構成されており、前記心外膜アンカーデバイスは、心臓の心室壁の外側の位置にて、心臓内に植え込まれた人工心臓弁から延びるテザーを固定するように構成されている、ドッキング部材と、
前記ハンドルアセンブリに結合された力センサデバイスであって、前記力センサデバイスは、前記力センサデバイスに作用する力を測定するように構成されており、前記力は、前記長尺状部材及び前記ハンドルアセンブリ内に延びる前記テザーの張力に関係する、力センサデバイスと、を備える装置。 - 前記力センサデバイスの近位側の前記ハンドルアセンブリに結合されたテザー固定デバイスであって、前記テザー固定デバイスは、前記長尺状部材及び前記ハンドルアセンブリ内に延びる前記テザーを、前記ハンドルアセンブリに対する固定位置に固定するように構成されたテザー固定デバイスをさらに備える、請求項1に記載の装置。

前記テザー上の前記マーカーが当該装置の近位端の近位側に配置されているときの、前記透明セグメント上の前記表示に対する前記インジケータの位置が、心臓に接する前記心外膜アンカーデバイスの表面と心臓の弁輪との間の距離に相当する、請求項1に記載の装置。 - 心外膜アンカーデバイス内にテザーを挿入するステップであって、前記テザーが心臓内に植え込まれた人工心臓弁から延び、前記テザーが前記心臓の外部に延びる、ステップと、
前記テザーの近位端が位置決めデバイスの近位端の外側に延びるように前記位置決めデバイス内に前記テザーを挿入するステップと、
前記位置決めデバイスの遠位端に配置されたドッキング部材に前記心外膜アンカーデバイスを結合するステップと、
前記テザーにかかる所望の張力が得られるまで前記テザーを近位側に引くステップと、
前記心臓の外壁の所望の位置に前記心外膜アンカーデバイスを位置決めするステップと、
前記位置決めするステップの後、前記テザーを前記心外膜アンカーデバイスに固定するステップと、
前記心外膜アンカーデバイスを前記ドッキング部材から解放するステップと、を含む方法。 - 前記テザーを前記心外膜アンカーデバイスに固定する前記ステップの前に、前記位置決めデバイスの近位端に配置されたテザー固定デバイスに前記テザーの近位端部分を固定するステップと、
前記テザー固定デバイスを固定位置に保持しつつ、前記心外膜アンカーデバイスを遠位側に動かすことによって、前記テザーにかかる張力を増加させるステップと、
前記テザーを前記テザー固定デバイスから解放するステップと、をさらに含む、請求項9に記載の方法。 - 前記テザーを前記心外膜アンカーデバイスに固定する前記ステップの前に、前記位置決めデバイスの近位端に配置されたテザー固定デバイスに前記テザーの近位端部分を固定するステップと、
前記テザーが前記テザー固定デバイスに固定された状態で、前記心外膜アンカーデバイスに対して近位側に前記テザー固定デバイスを動かすことによって、前記テザーにかかる張力を増加させるステップと、

前記ロッド部材に結合された引張部材であって、テザーが前記テザー固定デバイスに固定されると、前記引張部材が前記ロッド部材に対して遠位側に動き、前記長尺状部材及び前記ハンドルアセンブリ内に延びる前記テザーにかかる張力を調整するように構成されている、引張部材と、をさらに備える、請求項17に記載の装置。 - 前記ハンドルアセンブリに結合されたロッド部材と、
前記ロッド部材に結合された引張部材であって、前記引張部材が回転すると、前記ロッド部材が近位側に動き、前記長尺状部材及び前記ハンドルアセンブリ内に延びるテザーにかかる張力を調整するように構成されている、引張部材と、をさらに備える、請求項17に記載の装置。 - 前記ハンドルアセンブリに結合されたロッド部材をさらに備え、前記ロッド部材が前記ロッド部材の遠位端に配置されたインジケータを有し、
前記ハンドルアセンブリが、表示を有する透明セグメントを含み、前記ロッド部材の前記遠位端に配置された前記インジケータを、前記透明セグメントを通して見ることができ、マーカーが上に配置された前記長尺状部材及び前記ハンドルアセンブリ内にテザーが延びており、
前記テザー上の前記マーカーが当該装置の近位端の近位側に配置されているときの、前記透明セグメント上の前記表示に対する前記インジケータの位置が、心臓に接する前記心外膜アンカーデバイスの表面と心臓の弁輪との間の距離に相当する、請求項17に記載の装置。 - 長尺状部材に結合されたハンドルを含むテザー解放ツールであって、前記長尺状部材が、成形された遠位側先端部を含み、前記成形された遠位側先端部が、心臓内に植え込まれた人工心臓弁から外壁前記心臓へと延びるテザーを固定するように構成された心外膜アンカーデバイスの開口部の形状に合致する形状を有する、テザー解放ツールを備え、
前記長尺状部材の前記遠位側先端部が、前記心外膜アンカーデバイスの前記対応する開口部内に受け入れられ、前記心外膜アンカーデバイスに固定されたテザーにかかる張力を調整することができるように、前記心外膜アンカーデバイスのテザーピン留め機構をロック解除するために使用されるように構成されている、装置。 - 前記長尺状部材が、アクセスポートと流体連通している遠位開口部を規定し、前記テザーを前記心外膜アンカーデバイスに対して近位側に引っ張り、前記テザーにかかる張力を調整することができるように、前記長尺状部材の前記遠位開口部が、前記ロック解除されたテザーの一部を前記長尺状部材の前記遠位開口部内に受け入れ、前記アクセスポートを通じて出すように構成されている、請求項21に記載の装置。
- 心外膜アンカーデバイスによって規定される開口部にテザー解放ツールの遠位側先端部を挿入するステップであって、前記テザー解放ツールが、長尺状部材に結合されたハンドルを含み、前記長尺状部材が前記遠位側先端部を含み、前記遠位側先端部が、前記心外膜アンカーデバイスの前記開口部の形状に合致する形状を有する、ステップと、
挿入する前記ステップの後、前記テザー解放ツールを回転させて前記心外膜アンカーデバイスのロック機構をロック解除し、前記心外膜アンカーデバイスに結合されたテザーを解放するステップと、
前記長尺状部材の前記遠位側先端部を前記心外膜アンカーデバイスの前記開口部から取り外すステップと、
前記テザーを前記心外膜アンカーデバイスに対して近位側に引き、前記テザーにかかる張力を調整するステップと、を含む方法。 - 前記テザーにかかる所望の張力が得られると、前記長尺状部材の前記遠位側先端部を前記心外膜アンカーデバイスの前記開口部に挿入するステップと、
前記テザー解放ツールを回転させて前記心外膜アンカーデバイスの前記ロック機構をロックし、前記テザーを前記心外膜アンカーデバイスに固定するステップと、をさらに含む、請求項23に記載の方法。 - 前記テザーを引く前記ステップの前に、前記テザーの近位端部分を前記長尺状部材内に規定される遠位開口部内に挿入し、前記長尺状部材によって規定されるアクセスポートを通して出すステップをさらに含み、前記アクセスポートが前記遠位開口部と流体連通している、請求項23に記載の方法。
