Abstract
Description
de l'extrémité proximale du cubitus (olécrane). from the proximal end of the ulna (olecranon).
Currently existing devices require for the most part their ablation in the medium term because of the gene caused by the material (protruding under the skin). This ablation is done in the operating room under anesthesia, which involves a second intervention (with an increase in the morbidity inherent in any intervention) and a new hospitalization.
They are either devices consisting of 2 pins which, introduced on both sides of the fracture focus, hold them thanks to a steel wire passed at 8 around the pins (guy wire), or screwed plates which constitute a gene because of their subcutaneous thickness, ie intra-medullary nails not making it possible to ensure stability of the fracture site such that rapid reeducation can be undertaken.
The device according to the invention overcomes these disadvantages. It comprises in fact according to a first feature a nail traversed by at least two screws for anchoring this nail in the bone. The nail can be full or hollow. If the nail is full, it has tunnels allowing the screws to cross it. If the nail is hollow it has orifices allowing the screws to cross it. The upper end of this nail is bevelled and follows the natural curvature of the bone at this point of the anatomy thus avoiding its externalization. The proximal diameter of the nail is greater than its most distal diameter. An orifice or tunnel is present at the top of the nail and follows an oblique descending path towards the coronoid process. This oblique descending path is intended to receive a screw in a passage from the proximal portion to the distal portion which allows the bone anchoring of the nail and the compression of the fracture focus. This nail also has at least one orifice or tunnel located at the distal portion of the nail, perpendicular to the axis thereof, and intended to receive a screw which allows the bone anchoring of the nail, avoids its mobilization towards the part. proximal to the olecranon and prevents rotation of the fracture site. The entire device thus makes it possible to undertake early rehabilitation in immediate postoperative thanks to a stable synthesis and material buried in the bone.
According to the anatomical variations of the patients to operate and the particular modes of realization: The diameter and the length of the nail can vary The nail can be full or hollow The diameter and the length of the screws can vary The material used for the manufacture of the nail or the The angulation of the proximal tunnel with the axis of the nail may vary. The number and position of the distal tunnels may vary. The accompanying drawings illustrate the invention: FIG. 1 represents in sagittal section the device of the invention FIG. 2 shows a front section of the device of the invention. Referring to these drawings, the device comprises a nail (1) which can be solid or hollow. If the nail is hollow, it has at least 4 holes; an orifice at the top (2), another at its middle part (2a) and at least two others at its distal part (3 and 3a). If the nail is full, it has at least two tunnels; a tunnel starting from the top of the nail in a direction oblique to the axis of the nail (2), and at least one tunnel located at the distal portion of the nail and perpendicular to the axis thereof (3). These tunnels are intended to each receive a screw for anchoring in the bone. The screw passing through the proximal tunnel (4) makes it possible to compress the fracture site (6 and 7). The screw (s) distal (5) can block rotations and prevent the rise of the nail.
The proximal portion of the nail having at its apex an orifice (2) is of a diameter greater than the diameter of its distal portion. This difference in diameter allows a better stability of the nail in the proximal portion of the ulna, wider at the olecranon. This proximal part of the nail takes a beveled shape so that it adapts to the anatomy of the olecranon.
The proximal screw (4) has a thread on its distal portion only. This thread is intended to ensure its grip in the bone. The proximal and distal portion of this screw is smooth and has no threading; it is the part in contact with the nail. The top of the screw comprises a head, of a diameter greater than the diameter of the body of the screw and the tunnel (2) through which the screw passes which stops the progression of the screw in its tunnel and to obtain a compression effect at the fracture site.
The distal screw (s) (5) has (ent) a continuous thread so that the catch can be done on the two cortices of the bone. It also has a head with a diameter greater than the diameter of the body of the screw (s) and the tunnel (3) that borrows (ent) this (these) screws.
The osteosynthesis device described above is associated with an ancillary device to facilitate the installation of said material and in particular to achieve the screwing by automatic aiming.
The device according to the invention is particularly intended for the treatment of fractures of the olecranon.
