Claims (1)
- 【特許請求の範囲】 1.a) 外側顆面と内側顆面からなる遠位および後部関節連結面部と内方向 非関節連結面とを有する大腿骨部品と、 b) 使用中、大腿骨部品の関節連結面を収容する凹状関節連結面を有する脛 骨部品とを備え、 大腿骨部品の内方向非関節連結面と外側顆部の関節連結面との距離が、大腿骨 部品の関節連結面の少なくとも一部で、内方向非関節連結面と内側顆部の関節連 結面との距離と異なり、機構軸についての大腿骨回転の角度が膝の動作の正常な 範囲で動作するとき変化する人工膝関節。 2.前記後部関節連結面部が、患者の大腿骨の額面から異なる距離に位置する 後部外側顆面部と後部内側顆面部とからなり、外側顆面が、内側顆面より額面か らより遠い位置にある請求項1記載の人工膝関節。 3.前記遠位内側顆面部が、遠位外側顆面より横平面から遠い位置にあり、後 部外側顆部と後部内側顆部の厚さが同じであり、使用中機構軸について大腿骨の 回転の角度が患者の膝のある範囲の動きを受けるとき変化する請求項1記載の人 工膝関節。 4.前記非関節連結面が、少なくとも遠位面と後部面を含む複数の交わる面か らなる請求項1記載の人工膝関節。 5.前記後部非関節連結面は、単一平面で形成される請求項4記載の人工膝関 節。 6.前記非関節連結面が、平らな交わる平面からなる請求項 1記載の人工膝関節。 7.前部関節連結面をさらに備えてなる請求項1記載の人工膝関節。 8.前部非関節連結面をさらに備え、この前部非関節連結面が、実質的に、平 らであり、前記遠位非関節連結面と少なくとも90°の角度を形成する請求項7 記載の人工膝関節。 9.前記後部非関節連結面が、実質的に上、平らであり、前記遠位非関節連結 面と少なくとも90°の角度を形成する請求項4記載の人工膝関節。 10.前記機構軸についての大腿骨の回転の角度が、遠位(0°の屈曲)から 約90°の屈曲までの範囲の動きを受けるとき徐々に変化する請求項1記載の人 工膝関節。 11.前記機構軸についての大腿骨の回転の角度が、遠位(0°の屈曲)から 約130°の屈曲までの範囲の動きを受けるとき徐々に変化する請求項1記載の 人工膝関節。 12.前記機構軸についての大腿骨の回転の角度が、約−20°の屈曲から約 90°の屈曲までの範囲の動きを受けるとき徐々に変化する請求項1記載の人工 膝関節。 13.前記機構軸についての大腿骨の回転の角度が、約−20°の屈曲から約 140°の屈曲までの範囲の動きを受けるとき徐々に変化する請求項1記載の人 工膝関節。 14.前記機構軸についての大腿骨回転の角度が、約20°の屈曲から約80 °の屈曲までの範囲の動きを受けるとき徐々に変化する請求項1記載の人工膝関 節。 15.前記機構軸についての大腿骨回転の角度が、約60°の屈曲の範囲の動 きを受けるとき徐々に変化する請求項1記載の人工膝関節。 16.前記機構軸についての大腿骨の回転の角度が、約80°の屈曲の範囲の 動きを受けるとき徐々に変化する請求項1記載の人工膝関節。 17.前記機構軸についての大腿骨の回転の角度が、約90°の屈曲の範囲の 動きを受けるとき徐々に変化する請求項1記載の人工膝関節。 18.前記大腿骨部品が、個々の後部顆部において後部非関節連結面から後部 関節連結面までそれぞれ測定された異なる厚さを有する内側後部顆部と外側後部 顆部からなる請求項2記載の人工膝関節。 19.前記脛骨部品は、内側より外側においてより厚い請求項3記載の人工膝 関節。 20.a) 内方向非関節連結面と前部関節連結面と遠位および後部外側およ び内側顆関節連結面を含む外方向関節連結面とを有する大腿骨部品と、 b) 大腿骨部品の遠位顆間の中心に定義される大腿骨部品の中心線と、 c) 使用中に大腿骨部品の前部関節連結面と関節連結する関節連結面を有す る膝蓋骨部品または自然の膝蓋骨とを備え、 d) 大腿骨部品が膝の標準的な関節動作中に前記膝蓋骨部品がたどる膝蓋骨 溝を前部関節連結面内に有し、 e) 前記溝の縦軸が、大腿骨部品の前部関節連結面の上位部位における前記 中心線に関して外側に配置され、前部関節連結面の遠位部位において内側方向に 曲がる人工膝関節。 21.前記溝の軸が、前部関節連結面の上位部位における前記中心線に対し、 実質的に、平行である請求項20記載の人工膝関節。 22.前記溝の軸が、大腿骨部品の遠位部位において前記中心線に実質的に位 置する請求項20記載の人工膝関節。 23.中心線の外側から内側方向への前記溝の縦軸の位置の変化が、漸進的で ある請求項20または21記載の人工膝関節。 24.前記膝蓋骨溝が少なくとも長さの一部にわたり曲げられた請求項20記 載の人工膝関節。 25.前記大腿骨部品が、膝蓋骨または膝蓋骨部品と関節連結するための前部 関節連結面中に位置する膝蓋骨溝をさらに備え、前記溝が前部関節連結面の上位 部位において大腿骨部品の縦中心線の外側に位置する、大腿骨部品の遠位部位の 内側方向に曲がる縦軸を有する請求項1記載の人工膝関節。 26.前記溝の軸が、前部関節連結面の上位部位において前記中心線に対し、 実質的に、平行である請求項25記載の人工膝関節。 27.前記溝の軸が、大腿骨部品の遠位部位において実質的に前記中心線に位 置する請求項25記載の人工膝関節。 28.中心線の外側から内側方向への前記溝の縦軸の位置の変化が漸進的であ る請求項25または26記載の人工膝関節。 29.前記膝蓋骨溝は少なくとも長さの一部にわたり曲げられた請求項25記 載の人工膝関節。 30.a) 内方向非関節連結面とを有する大腿骨部品と、 b) 使用中、大腿骨部品の関節連結表面を収容する凹状関節連結面を有する 脛骨部品とを備え、 c) 大腿骨部品が前部、遠位及び後部の関節連結面部を有し、前部近位部が 前部面の前にある弧の中心を有することによって、大腿骨部品が前部近位部位に おいて外側から見て凹状である人工膝関節。 31.前記大腿骨部品が、前部、遠位及び後部の関節連結面部を備え、前部近 位部は前部面の前にある弧の中心を有することによって、大腿骨部品が前部近位 部位において外側から見て凹状である請求項1または20記載の人工膝関節。 32.a)患者の後部顆の部分を取り除く後部切断と遠位切断を含む複数の切 断で患者の遠位大腿骨を切除する処置と、 b)患者の切除された遠位大腿骨に、処置“a”で行われた後部と遠位の切断 に密着する後部と遠位の非関節連結部を少なくとも有する人工大腿骨関節を取り 付ける処置と、 c)人工脛骨関節を収容する患者の脛骨を切除する処理と、 d)処置“d”で切除された脛骨に人工脛骨関節を取り付ける処置と、 e)患者の機構軸における大腿骨の回転の角度が、患者の膝の標準的な範囲の 動きを受けた時徐々に変化するように、大腿骨と脛骨の切断を互いに関連させか つ、人工脛骨関節と人工大 腿骨関節に関連させて形成する処置とを備えてなる患者の遠位大腿骨に人工大腿 骨関節をインプラントする方法。 33.処置“b”において人工関節が、前部または後部非関節連結から異なる 距離に位置する関節連結面を備えた内側および外側後部顆部とを有する請求項3 2の方法。 34.前記遠位大腿骨が、少なくとも遠位、前部及び後部切断で切除される請 求項32の方法。 35.処置“b”において人工関節が、遠位切断から異なる距離に位置する関 節連結面を備えた内側および外側遠位顆部を有する請求項32の方法。
