Description
本願は、2015年4月29日に出願された仮出願第62/154,595号(代理人管理番号48428−704.101)からの優先権を主張するものであり、該仮出願の全開示は、参照により本明細書中に援用される。
(1.発明の分野)
本発明は、エネルギーを組織に印加するための医療器具およびシステムに関し、より具体的には、内視鏡下手技において組織を切断および抽出するために適合される電気外科手術用デバイスに関する。
本願の開示は、出願第13/857,068号のものに類似する。関連特許および刊行物は、米国第5,622,647号、米国第5,672,174号、および米国第7,824,398号を含む。
本発明は、例えば、以下を提供する。
(項目1)
電気外科手術用デバイスであって、
軸およびその遠位部分内の窓を有する、伸長シャフトと、
前記窓の両側間の弧内で往復して回転振動するように構成される、ワイヤ状電極と、
前記ワイヤ状電極に動作可能に接続され、前記ワイヤ状電極を回転振動させ、前記窓内に受容された組織を切断する、モータと、

窓を有する作業端および前記窓の両側間の弧内で往復して振動するように適合されるモータ駆動式電極を伴う、伸長シャフトを提供するステップと、
前記作業端を前記組織に対して位置付け、前記組織を前記窓と界面接触させるステップと、
前記モータおよびRF源を作動させ、前記窓と界面接触する組織を前記振動する電極を用いて切断するステップであって、前記切断された組織は、前記伸長シャフト内の内部に進入する、ステップと、
を含む、方法。

前記伸長シャフト内の内部通路と連通する負圧源を作動させ、切断された組織を前記伸長シャフト内の内部通路を通して前記作業端から抽出するステップをさらに含む、項目29に記載の方法。
(項目31)
前記組織を液体中に浸漬させるステップをさらに含む、項目29に記載の方法。
(項目32)
流体を流体源から前記作業端に前記伸長シャフト内の流動チャネルを通して前記組織に送達するステップをさらに含む、項目31に記載の方法。

前記組織は、前記組織を切除する間、ガス環境内に存在する、項目29に記載の方法。
Claims (14)
- 電気外科手術用デバイスであって、
伸長シャフトであって、前記伸長シャフトは、軸を有し、かつ、前記伸長シャフトの遠位部分において窓を有する、伸長シャフトと、
前記窓の両側間の弧内で往復して回転振動するように構成されたワイヤ状の電極であって、前記ワイヤ状の電極は、各回転弧の終了時、剪断運動において前記窓の両側を越えて掃引するように構成されている、ワイヤ状の電極と、
前記窓内に受容された組織を切断するために前記ワイヤ状の電極を回転振動させるように前記ワイヤ状の電極に動作可能に接続されたモータと
を備える、電気外科手術用デバイス。 - 前記遠位部分は、セラミック材料を含む、請求項1に記載の電気外科手術用デバイス。
- 前記弧は、60o〜210oに及ぶ、請求項1または請求項2に記載の電気外科手術用デバイス。
- 前記弧は、90o〜180oに及ぶ、請求項3に記載の電気外科手術用デバイス。
- 前記電極は、1サイクル/秒〜50サイクル/秒のレートにおける往復サイクルで回転振動される、請求項1〜4のいずれか一項に記載の電気外科手術用デバイス。
- 前記デバイスは、電極振動レートの調節を可能にするように構成されている、請求項1〜5のいずれか一項に記載の電気外科手術用デバイス。
- 前記デバイスは、1つの方向では第1のレートで、別の方向では第2のレートで、回転振動するように構成されている、請求項1〜6のいずれか一項に記載の電気外科手術用デバイス。
- 前記電極に動作可能に結合されるように構成されたRF源と、前記伸長シャフト内の内部通路と連通するように構成された負圧源と、前記RF源および前記負圧源を制御するように構成されたコントローラとをさらに備える、請求項1〜7のいずれか一項に記載の電気外科手術用デバイス。
- 前記コントローラは、(1)前記RF源および前記負圧源のうちの少なくとも1つからのフィードバック信号に応答したモータ動作パラメータと、(2)前記モータ動作パラメータおよび前記負圧源のうちの少なくとも1つからのフィードバック信号に応答したRFパラメータと、(3)前記モータ動作パラメータおよび前記RF源のうちの少なくとも1つからのフィードバック信号に応答した負圧パラメータとのうちの少なくとも1つを調節するようにさらに構成されており、前記コントローラは、前記窓に対して選択された位置において回転を停止するように構成されており、前記コントローラは、前記電極が停止位置にあるとき、凝固RFエネルギーを組織に送達するように構成されている、請求項8に記載の電気外科手術用デバイス。
- 前記ワイヤ状の電極を前記モータに動作可能に結合する回転可能な駆動シャフトをさらに備え、前記シャフトは、回転抵抗を吸収する衝撃吸収機構を含む、請求項1〜9のいずれか一項に記載の電気外科手術用デバイス。
- 前記伸長シャフトは、引張ワイヤによって関節運動されるように構成されている、請求項1〜10のいずれか一項に記載の電気外科手術用デバイス。
- 前記ワイヤ状の電極は、前記窓に対して軸方向に移動されるように構成されている、請求項1に記載の電気外科手術用デバイス。
- 前記ワイヤ状の電極は、フック形状を有する、請求項1に記載の電気外科手術用デバイス。
- 前記伸長シャフトは、チャネルを有し、前記チャネルは、流体を前記シャフトの遠位部分に送達するために流体に除去可能に接続されるように構成されている、請求項1に記載の電気外科手術用デバイス。
