Claims (14)
- 肝内の門脈体循環シャントを生成するための装置であって、当該装置は、
患者の頸静脈を下って肝静脈内に挿入される伸長体を有するカテーテルと、
前記伸長体の遠位部分上の血管内撮像デバイスと、
前記伸長体の遠位部分上の出口開口部と、
前記カテーテル内のルーメン内に配置され、前記出口開口部から押出されて前記伸長体の側面から遠ざかるように伸長して門脈に入る組織貫通デバイスと、
前記組織貫通デバイス上に配置され、圧力の測定値を提供するセンサとを含む、装置。 - 前記組織貫通デバイスは、前記伸長体の側面から少なくとも1cmの距離だけ遠ざかるように伸長する針を含む、請求項1に記載の装置。
- 前記針は、前記針が前記出口開口部から出て行く際に湾曲形状をとる形状記憶材料を含む、請求項2に記載の装置。
- 前記針はガイドワイヤを受けるための大きさがあるルーメンを含む、請求項2に記載の装置。
- 前記血管内撮像デバイスが血管内超音波トランスデューサを含む、請求項1に記載の装置。
- 前記針は、前記カテーテルの本体の側面から遠ざかるように前記出口開口部から組織を通って伸長して門脈に入る寸法に作られている、請求項2に記載の装置。
- 前記針は、肝静脈と門脈との間の組織を貫通し、それによって門脈体循環シャントを生成する鋭利な先端を含む、請求項2に記載の装置。
- 前記血管内超音波トランスデューサは、肝静脈内にあるときに門脈の画像を作成する、請求項5に記載の装置。

前記カテーテル上又は前記針のルーメンによって受け入れられるガイドワイヤ上に配置されたセンサであって、圧力及び/又は流量の測定値を提供するセンサと、
前記測定値を受信し、前記門脈体循環シャントの両側で当該門脈体循環シャントによって接続される2つの別々の位置で提供された前記測定値から確認された血流予備量に基づいて前記門脈体循環シャントの開存性を決定するプロセッサとを含む、システム。
