Description
本発明は、概して、組織を通る通路を作り出すことに関し、より詳細には、骨の内部に湾曲路を作ることに関する。

近年、椎体形成術及び椎骨形成術において用いられる手術手技を通じて、低侵襲性手段によって椎体にアクセスする技法が開発されてきている。椎弓根を通って椎体の側部/前部の内部へ向かって脊椎の椎骨分節にアクセスすることは、治療装置(例えば骨セメント送達装置及び/又はRFプローブ)を椎骨の内部に配置する主要な方法であるが、プローブを椎骨の後正中線部内に配置することは困難である。さらに、直線状のアクセス路では脊椎のS1分節の後正中線部にアクセスすることは困難である。プローブは好ましくは、S1椎体の後部及び腰椎分節内の同じ標的領域に誘導することが可能である必要がある。加えて、頚椎及び胸椎内の脊椎分節も標的にすることが可能であることが意図される。
図1及び図2は、骨を通って経路を形成するシステムすなわちキット10を含む、本発明の第1の実施形態を示す。本システムは針トロカール20(器具セットの本体)を含む。トロカール20は、その近位端32にあるハンドル24と、トロカール20の遠位端22まで貫通する中央内腔36とを有する細長いシャフト28を含む。中央内腔36は概して、システム10における他の器具が患者内の治療領域まで摺動可能に導入されることを可能にするサイズになっている。システム10は、軟組織及び皮質シェルを貫通して初期経路を作って海綿骨へのアクセスを可能にするのに針トロカール20とともに用いられる鋭利な先端付き針84をその遠位端に有する直線状スタイレット80と、骨/組織の内部に湾曲経路を作る/維持するのに用いられる湾曲カニューレ50とを更に含む。湾曲部を真っ直ぐに矯正するとともに、湾曲カニューレ50を針トロカール20に装填するのに矯正用スタイレット40が用いられる。骨/組織の内部に湾曲経路を作るのに湾曲スタイレット60が湾曲カニューレ50と併用され、湾曲カニューレ50によって作られた湾曲経路の端を越えて(RFプローブ100のような)治療装置のための作業用チャネルを作るのに経路形成用スタイレット90が用いられる。
図8〜図18Bは、本発明による、骨の内部に湾曲経路を作り出すシステム201を示す。図8は、患者の体内での展開の準備が整った状態のシステム201の斜視図を示す。システム201は、細長い送達チューブ204に連結されている近位端ハウジング202を有する誘導針/トロカール210を含む。遠位端先端208は、椎体のような骨塊の少なくとも一部に入り、該骨塊の少なくとも一部を通って送達するのを容易にするために、鋭利な、かつ/又はベベル面先端を有する。

前記湾曲経路は、前記椎体の前記海綿骨領域の少なくとも一部を通って作り出される、湾曲可能なカニューレと、
前記中央通路を通して前記湾曲経路を越えた位置まで送達されるように構成されている治療プローブと、
を含む、脊椎治療システム。
前記プルコードは、前記湾曲可能なカニューレの前記遠位端に引張力を加えて前記湾曲可能なカニューレを付勢して湾曲した態様にするように構成されている、実施形態24によるシステム。
前記トロカールの前記近位端はハウジングを備え、

前記駆動ナットは、前記トロカール、前記湾曲可能なカニューレ又は前記プローブのうちの1つ又は複数を、骨を通して前進させるための衝撃力を加えるのに適した硬化近位面を含み、
前記駆動ナットは、前記プローブの前記近位端を収容するように構成されているねじ切り遠位凹部を含み、
前記プローブは、前記駆動ナットに対する前記プローブの制御された移動を可能にするように前記遠位凹部と嵌合するねじ山を含む、実施形態29によるシステム。
前記交換可能なキャップは治療エネルギーを供給するよう前記プローブへのアクセスを提供するように構成されている、実施形態30によるシステム。
Claims (30)
- 骨の内部に経路を形成するシステムであって、
近位端と、遠位端と、中央チャネルとを有するトロカールであって、
前記中央チャネルは、該トロカールの中心軸に沿って設けられ、前記近位端から前記遠位端に向かって延びており、
該トロカールは、該トロカールの前記遠位端にあるか又は該遠位端の近くにある半径方向開口部を含み、該半径方向開口部は前記中央チャネルと連通する、トロカールと、
前記中央チャネル内に受け入れられるサイズになっているとともに、前記近位端から前記半径方向開口部に向けて送達される湾曲可能なカニューレであって、
該湾曲可能なカニューレは、前記半径方向開口部から横方向外向きに、前記トロカールから離れるように延びる湾曲経路内に延びるように構成されている湾曲可能な遠位端を含み、
該湾曲可能なカニューレは、中央通路であって、プローブが該中央通路を通って前記湾曲経路を越えた位置まで送達されることを可能にするように構成されている直径を有する中央通路を含む、湾曲可能なカニューレと、
を含み、
前記湾曲可能なカニューレは近位本体を備える近位端を含み、
前記トロカールの前記近位端はハウジングを備え、
前記ハウジングは、前記湾曲可能なカニューレの前記近位本体の往復動を可能にするように構成されている近位凹部を有し、
前記近位凹部は前記中央チャネルと連通する、
システム。 - 前記湾曲可能なカニューレの近位本体は、前記トロカールの前記ハウジング内での移動に対して解放可能に拘束されるように構成される、請求項1に記載のシステム。
- 前記カニューレの前記中央チャネル内に適合するサイズになっているプローブを更に含む、
請求項2に記載のシステム。 - 前記プローブは、前記カニューレの前記近位本体内での移動に対して解放可能に拘束されるように構成されている近位端を含む、請求項3に記載のシステム。
- 前記湾曲可能なカニューレに連結されている駆動ナットを更に含み、
前記駆動ナットは、前記トロカール、前記湾曲可能なカニューレ又は前記プローブのうちの1つ又は複数を、骨を通して前進させるための衝撃力を加えるのに適した硬化近位面を含む、請求項4に記載のシステム。 - 前記駆動ナットは、前記プローブの前記近位端を収容するように構成されているねじ切り遠位凹部を含む、請求項5に記載のシステム。
- 前記駆動ナットの前記近位面は交換可能なキャップを備え、
前記交換可能なキャップは、治療エネルギーを供給するよう前記プローブへのアクセスを提供するように構成されている、請求項5又は6に記載のシステム。 - 前記トロカールは、骨に穿孔して骨を通る直線経路を作り出すように構成されている鋭利な遠位先端を更に有する、請求項1から7のいずれかに記載のシステム。
- 前記湾曲可能なカニューレは、骨に穿孔して、前記トロカールによって作り出された直線経路から延びる湾曲経路を作り出すように構成されている鋭利な遠位先端を有する、請求項1から8のいずれかに記載のシステム。
- 前記湾曲可能なカニューレの前記遠位端は、真っ直ぐな態様で前記トロカールを通して送達されるとともに、前記中心軸に対して或る角度で前記半径方向開口部から外向きに湾曲した態様で展開されるように変形可能である、請求項1から9のいずれかに記載のシステム。
- 前記湾曲可能なカニューレの前記遠位先端に連結されているプルコードであって、前記トロカールの前記近位端まで延びるプルコードを更に含み、
前記プルコードは、前記湾曲可能なカニューレの前記遠位端に引張力を加えて前記湾曲可能なカニューレを付勢して湾曲した態様にするように構成されている、請求項10に記載のシステム。 - 前記トロカールの前記中央チャネルは、前記半径方向開口部につながる斜面で終端し、前記斜面は前記半径方向開口部から外方向への前記湾曲可能なカニューレの展開を容易にする、請求項1から11のいずれかに記載のシステム。
- 前記トロカールに対して前記湾曲可能なカニューレの動きを制限するように構成されている前記湾曲可能なカニューレの前記近位本体の周りに螺合する止めナットをさらに備える、請求項1から12のいずれかに記載のシステム。
- 骨の内部に経路を形成するシステムであって、
近位端と、遠位端と、中央チャネルとを有するトロカールであって、
前記中央チャネルは、該トロカールの中心軸に沿って設けられ、前記近位端から前記遠位端に向かって延びており、

該湾曲可能なカニューレは、中央通路であって、プローブが該中央通路を通って前記湾曲経路を越えた位置まで送達されることを可能にするように構成されている直径を有する中央通路を含む、湾曲可能なカニューレと、

前記湾曲可能なカニューレの前記遠位端は、真っ直ぐな態様で前記トロカールを通して送達されるとともに、前記中心軸に対して或る角度で前記半径方向開口部から外向きに湾曲した態様で展開されるように変形可能であり、
前記湾曲可能なカニューレの前記遠位先端に連結されているプルコードであって、前記トロカールの前記近位端まで延びるプルコードを更に含み、
前記プルコードは、前記湾曲可能なカニューレの前記遠位端に引張力を加えて前記湾曲可能なカニューレを付勢して湾曲した態様にするように構成されている、
システム。 - 前記湾曲可能なカニューレの前記遠位先端に加えられる前記引張力は、前記湾曲可能なカニューレを所望の経路に沿って導くように前記トロカールの前記近位端から制御することができる、請求項14に記載のシステム。
- 前記湾曲可能なカニューレの遠位端は、関節式に連結して湾曲形状になるように構成されている複数の結合リンクを備える、請求項14又は15に記載のシステム。
- 前記複数の結合リンクの遠位リンクは、べベル面前縁を備える、
請求項14から16のいずれかに記載のシステム。 - 前記プルコードは、前記湾曲可能なカニューレの近位本体から前記遠位リンクの孔まで延びている、請求項17に記載のシステム。
- 前記トロカールの前記中央チャネルは、前記半径方向開口部につながる斜面で終端し、前記斜面は前記半径方向開口部から外方向への前記湾曲可能なカニューレの展開を容易にする、請求項14から18のいずれかに記載のシステム。
- 前記トロカールは、骨に穿孔して骨を通る直線経路を作り出すように構成されている鋭利な遠位先端を更に有する、請求項14から19のいずれかに記載のシステム。
- 前記カニューレの前記中央チャネル内に適合するサイズになっているプローブを更に含み、
前記プローブは、前記カニューレの前記近位本体内での移動に対して解放可能に拘束されるように構成されている近位端を含む、請求項14から20のいずれかに記載のシステム。 - 前記湾曲可能なカニューレに連結されている駆動ナットを更に含み、
前記駆動ナットは、前記トロカール、前記湾曲可能なカニューレ又は前記プローブのうちの1つ又は複数を、骨を通して前進させるための衝撃力を加えるのに適した硬化近位面を含む、請求項21に記載のシステム。 - 前記駆動ナットは、前記プローブの前記近位端を収容するように構成されているねじ切り遠位凹部を含み、
前記プローブは、前記駆動ナットに対する前記プローブの制御された移動を可能にするように前記遠位凹部と嵌合するねじ山を含む、請求項22に記載のシステム。 - 前記駆動ナットの前記近位面は交換可能なキャップを備え、

- 前記湾曲可能なカニューレの遠位端は、関節式に連結して湾曲形状になるように構成されている複数の結合リンクを備える、請求項26に記載のシステム。
