Abstract
Description
本出願は、2007年7月27日出願の米国仮出願第60/962,169号の利益と、2008年5月16日出願の米国特許出願第12/122,456号に対する優先権とを主張し、それらを参照により組み込む。
図1を参照して、システム20に関してさらなる態様について記載する。システム20は、医学的診断および/または医学的治療のため、身体Bの内部の画像を提供するように構成される。システム20は、それぞれプローブデバイス60に操作可能に連結された、超音波イメージングシステム40およびRF治療システム50を備える制御ステーション、ならびに、適切な操作者入力デバイス(例えば、キーボードおよびマウス、または標準的な種類の他のポインティングデバイス)と操作者表示デバイス(例えば、CRT、LCD、プラズマスクリーン、またはOLEDモニタ)とを備える。
イメージングシステム40は、デバイス60の遠位先端70に収容された超音波トランスデューサアレイ82と関連付けられた信号およびデータを生成し処理するように構成される。トランスデューサアレイ82は、好ましくは、質の高い3D視覚化情報を作成することができる多次元イメージングアレイであり、アレイ82は、好ましくは、圧電被加工物をダイシングして、本明細書に記載されるような適切な数の素子にすることによって構築される。しかし、システム20は、当該分野において知られている多数の異なるイメージングアレイ、例えば、Eberleらの米国特許第5,857,974号および同第6,962,567号、Shelgaskowらの米国特許第6,994,674号、および/またはSewardらの米国特許第7,156,812号に記載されているものを用いて有用に実現されてもよい。
近位側に延びる外側シース62を部分的に切り欠いて示した遠位先端70を概略的に示す図2を参照すると、RF治療サブシステム50は、電流を発生させ、結果として得られる電流をデバイス60の遠位先端70上にある1つまたは複数の治療電極72、74に送るように設計される。電極72、74は、遠位先端70の前方端部の外部に蒸着された薄い金属層であり、遠位先端70は、円錐形(図3〜5)または滑らかに湾曲した(図6〜10)形状を有してもよい。RF電極は、フレックス回路99上の導電性トレースに、またフレックストレースを通してカテーテルケーブル97に電気的に接続される。
治療電極72、74は前方に面しており、それによって、例えば、部分的または完全に閉塞した動脈を通る通路を作るのに有用であるように、デバイス60が通り抜けるように動作することが可能になる。電極72、74はまた、好ましくは、デバイス60の長手方向軸線100の周りで間隔を空けた関係で遠位先端70上に配列され、各電極が独立して動作できるようにして構成される。これによって、デバイス60の遠位先端70の長手方向軸線100に対して対称的または非対称的に治療を適用することが可能になる。対称的な切除は、真直ぐな通り抜け(すなわち、デバイス60の長手方向軸線の方向での通り抜け)を達成する傾向があるが、非対称的な切除は、作動されている電極の方向での通り抜けに結び付く。
本明細書に記載する治療カテーテルは、多数の従来技術に従って構築された多数の異なるイメージングアレイを用いて有用に実現することができる。しかし、アレイが非常に小さく、非常に質の高い3Dイメージング情報が提供されるように高周波で動作することが好ましい。現在の治療カテーテルに、または小型の高周波アレイを介する超音波イメージングが有益である他のあらゆる用途に使用される小型の高周波イメージングアレイについて、図11に関連して記載する。
次に、発明の実施形態のある特定の特徴を示す具体的な実施例について参照する。ただし、これらの実施例は例示のために提供されるものであり、本発明の範囲をそれによって何ら限定するものではないことを理解すべきである。

次に、金またはチタンの介在層を追加して、同じアレイ素子をモデル化した。結果をそれぞれ、図13A〜13Cおよび図14A〜Cにプロットしている。モデル化した金層の厚さは1μmであり、モデル化したチタン層の厚さは1.5μmであった。モデル化した結果は、1.5μmのチタン層(図14A〜14C)は、アレイの動作性に著しく影響を及ぼさず、本明細書に開示するデバイスに対する有効な材料選択を示すことを実証している。1.0μmの金層(図13A〜13C)は、チタンに対して33%薄いものの、アレイの性質により悪影響を及ぼしている。後者の例では、挿入損失は10dB劣っており、パルス・リングダウン(pulse ringdown)は約0.3マイクロ秒劣っている。それにも関わらず、特に周波数フィルタリングなどの補正技術が実現される場合、金は現実的な選択肢であると思われる。音響的特性が劣っているにも関わらず、金および本開示で言及した他の金属は、容認できるものと予想され、また、処理がより容易である、長時間の動作にわたって耐酸化性である、また生体適合性であるなど、金属の材料特性に関する利点を有することができる。
Claims (35)
- 患者の脈管構造に挿入されるように適合され、近位部分が前記患者の外にある状態で、前記患者の脈管構造内部にあるように動作可能な遠位部分を規定する細長いカテーテル本体と、
前記患者の脈管構造内のプラークの制御された切除を行うための、前記遠位部分上の複数の遠位側に面する電極と、
前記電極の近位側で前記カテーテル本体に位置付けられ、前記電極を通して超音波パルスを送受信して、前記電極によって切除すべきプラークのリアルタイムのイメージング情報を提供するように構成された、遠位側に面する超音波イメージングトランスデューサのアレイとを備える、患者の脈管構造において画像誘導治療を提供するための腔内カテーテル。 - 前記トランスデューサのアレイが15MHzを超える特性動作周波数を有する、請求項1に記載のカテーテル。
