Abstract
【解決手段】血管の閉塞を実現するのに有用な装置、方法及びシステムが記載されている。例えば、静脈逆流に起因する静脈瘤状態を治療するとき、大伏在静脈を閉塞し且つ除去するため、経皮的方法が使用される。特定の実施の形態は、1つ又は複数の脈管閉塞装置をスルーアンドスルー状態に残すスルーアンドスルー経皮的方法を介して、1つ又はより多くの脈管閉塞装置を配置するステップを備えている。
Description
別の実施の形態において、本発明は、人間の伏在静脈を閉塞する方法を提供する。該方法は、導入針にて伏在静脈にアクセスするステップを含む。穿刺ワイヤーを伏在静脈内に通す。この穿刺ワイヤーは、アクセス開口部から隔てた位置にて伏在静脈から出るために使用される。シースをアクセス開口部から出口開口部まで通し、また、細長い閉塞装置をシースから伏在静脈内に送り出す。
バイオトロピック(biotropic)な性質を有するコラーゲン細胞外基質材料(ECMs)により適宜な生物学的に再編可能な材料を提供することができる。これらは、凍結乾燥又はその他の乾燥し又は水和化状態にて又は追加的に或いは、代替的にゲル又はその他の流動可能な(選択的に硬化可能な)状態にて血管に送り出すことができる。例えば、適宜なコラーゲン材料は、肝臓基底膜を含む粘膜下層、腎皮膜、皮膚コラーゲン、硬膜、心膜、漿膜、腹膜又は基底膜層、のようなECMsを含む。これら目的の適宜な粘膜下層材料は、例えば、小腸粘膜下層組織を含む腸粘膜下層組織、胃粘膜下層、膀胱粘膜下層及び子宮粘膜下層を含む。
Claims (71)
- 人間の下肢の静脈不全を治療する方法において、
下肢の伏在静脈に対し少なくとも1つの経皮的アクセス部を提供するステップと、
送り出し装置を経皮的アクセス部を通して伏在静脈内に挿入するステップと、
閉塞装置を送り出し装置により伏在静脈まで送り出すステップとを備える、人間の下肢の静脈不全を治療する方法 - 請求項1に記載の方法において、伏在静脈は大伏在静脈である、方法。
- 請求項1に記載の方法において、伏在静脈は小伏在静脈である、方法。
- 請求項1ないし3の何れか1つの項に記載の方法において、閉塞装置は再吸収可能である、方法。
- 請求項1ないし4の何れか1つの項に記載の方法において、閉塞装置はコラーゲン状である、方法。
- 請求項1ないし5の何れか1つの項に記載の方法において、閉塞装置は細胞外基質材料を備える、方法。
- 請求項6に記載の方法において、細胞外基質材料は、粘膜下層、漿膜、心膜、硬膜、腹膜、又は皮膚コラーゲンを備える、方法。
- 請求項5に記載の方法において、閉塞装置は組み換えたコラーゲンを備える、方法。
- 請求項1ないし8の何れか1つの項に記載の方法において、閉塞装置は少なくとも約10cmの長さを有する、方法。
- 請求項1ないし9の何れか1つの項に記載の方法において、閉塞装置は、少なくとも1つの穿通枝静脈と伏在静脈との接続箇所を横断し得るよう送り出される、方法。
- 請求項1ないし10の何れか1つの項に記載の方法において、閉塞装置は伏在静脈の第一及び第二の外科的開口部の間を伸びる、方法。
- 請求項1ないし11の何れか1つの項に記載の方法において、閉塞装置は少なくとも約20cmの長さを有する、方法。
- 請求項1ないし12の何れか1つの項に記載の方法において、閉塞装置は粘膜下層を備える、方法。
- 請求項1ないし13の何れか1つの項に記載の方法において、閉塞装置は、第一の圧縮状態に形態設定可能であり且つ、送り出されたとき、第二の膨張状態に膨張可能である細長い本体を備える、方法。
- 血管を閉塞する方法において、
血管に第一の開口部及び第二の開口部を提供するステップと、
第一の開口部と第二の開口部との間を伸びる血管内に閉塞装置を配置するステップとを備える、血管を閉塞する方法。 - 請求項15に記載の方法において、血管は静脈である、方法。
- 請求項16に記載の方法において、静脈は人間の下肢にある、方法。
- 請求項17に記載の方法において、静脈は伏在静脈である、方法。
- 請求項18に記載の方法において、伏在静脈は大伏在静脈である、方法。
- 請求項18に記載の方法において、伏在静脈は小伏在静脈である、方法。
- 請求項18に記載の方法において、伏在静脈は大伏在静脈及び小伏在静脈の双方を含む、方法。
- 請求項15ないし21の何れか1つの項に記載の方法において、第一及び第二の開口部は経皮的に形成される、方法。
- 請求項22に記載の方法において、第一の開口部は経皮的入口により形成され、第二の開口部は、血管を通って第一の開口部から該第一の開口部から隔てられた位置まで進めた細長い穿刺装置により形成される、方法。
- 請求項15ないし23の何れか1つの項に記載の方法において、閉塞装置は再吸収可能な材料を備える、方法。
- 請求項24に記載の方法において、閉塞装置はコラーゲン状材料を備える、方法。
- 請求項15ないし25の何れか1つの項に記載の方法において、閉塞装置は細胞外基質材料を備える、方法。
- 請求項26に記載の方法において、細胞外基質材料は、粘膜下層、漿膜、心膜、硬膜、腹膜、又は皮膚コラーゲンを備える、方法。
- 請求項25に記載の方法において、閉塞装置は組み換えたコラーゲンを備える、方法。
- 請求項15ないし28の何れか1つの項に記載の方法において、閉塞装置は少なくとも約10cmの長さを有する、方法。
- 人間の血管を閉塞するのに有用な閉塞装置において、少なくとも約10cmの長さを有し、血管を閉塞させ得るよう血管内に経皮的に送り出し得る形態とされた閉塞器本体を備える、閉塞装置。
- 請求項30に記載の閉塞装置において、閉塞器本体は少なくとも約20cmの長さを有する、閉塞装置。
- 請求項30に記載の閉塞装置において、閉塞器本体は少なくとも約30cmの長さを有する、閉塞装置。
- 請求項30に記載の閉塞装置において、閉塞器本体は少なくとも約40cmの長さを有する、閉塞装置。
- 請求項30ないし33の何れか1つの項に記載の閉塞装置において、閉塞器本体は、細長い中心本体と、該細長い中心本体から伸びる少なくとも1つの血栓形成要素とを備える、閉塞装置。
- 請求項30ないし34の何れか1つの項に記載の閉塞装置において、閉塞器本体は第一の圧縮状態にて形態設定され、また、送り出されたとき、第二の膨張状態に膨張可能である、閉塞装置。
- 請求項35に記載の閉塞装置において、前記第一の圧縮状態にある閉塞器本体は、少なくとも1つのロール巻きした部分又は折り重ねた部分を有する、閉塞装置。
- 請求項36に記載の閉塞装置において、前記第一の圧縮状態にある閉塞器本体は、複数の折り重ねた部を有する、閉塞装置。
- 請求項30ないし37の何れか1つの項に記載の閉塞装置において、閉塞器本体は再吸収可能な材料を備える、閉塞装置。
- 請求項30ないし38の何れか1つの項に記載の閉塞装置において、閉塞器本体はコラーゲン状材料を備える、閉塞装置。
- 請求項39に記載の閉塞装置において、閉塞器本体は細胞外基質材料を備える、閉塞装置。
- 請求項40に記載の閉塞装置において、細胞外基質材料は、粘膜下層、漿膜、心膜、硬膜、腹膜、又は皮膚コラーゲンを備える、閉塞装置。

請求項30ないし42の何れか1つの項に記載の閉塞装置と、
血管内に経皮的に進む少なくとも1つの追加的な装置とを備える、医療用キット。 - 請求項43に記載の医療用キットにおいて、少なくとも1つの追加的な装置は、閉塞装置を血管内に送り出す送り出し装置である、医療用キット。
- 請求項44に記載の医療用キットにおいて、送り出し装置は管腔を備え、閉塞装置は該管腔内に受容可能であり且つ、該管腔から送り出し可能である、医療用キット。
- 脈管の閉塞装置を送り出すのに有用な医療用キットにおいて、
血管を通って進み且つ、血管からの経皮的出口を提供する細長い穿刺装置と、
細長い穿刺装置を受容する管腔を有するカニューレ付き案内装置と、
カニューレ付き案内装置の上を進むことのできるシースと、
シースを通って進むことができ且つ、細長い脈管の閉塞装置と係合し、閉塞装置をシースを通して押し出し得る形態とされた細長い装置とを備える、医療用キット。 - 血管内にて脈管の閉塞装置を送り出すのに有用な医療用キットにおいて、
血管を通って進み且つ血管からの経皮的出口を提供する細長い穿刺装置と、
細長い穿刺装置を受容する管腔を有するカニューレ付き案内装置と、
カニューレ付き案内装置の上を進むことのできる第一のシースと、
第一のシースを通って進むことができ且つ、脈管の閉塞装置を保持し得る形態とされた第二のシースとを備える、医療用キット。 - 請求項47に記載の医療用キットにおいて、第二のシースは、予装填された脈管の閉塞装置を保持する、医療用キット。
- 請求項47又は48に記載の医療用キットにおいて、脈管の閉塞装置は、人間の大伏在静脈を閉塞し得る形態とされる、医療用キット。
- 請求項48に記載の医療用キットにおいて、予装填された脈管の閉塞装置は、請求項30ないし42の何れか1つの項に記載の閉塞装置である、医療用キット。
- 請求項47に記載の医療用キットにおいて、閉塞装置を更に備える、医療用キット。
- 請求項51に記載の医療用キットにおいて、閉塞装置は、請求項30ないし42の何れか1つの項に記載の閉塞装置である、医療用キット。
- 脈管の閉塞装置を送り出す医療組立体において、
請求項30ないし42の何れか1つの項に記載の脈管の閉塞装置と、
送り出し装置とを備え、
前記脈管の閉塞装置は前記送り出し装置と係合する、医療組立体。 - 請求項53に記載の医療組立体において、送り出し装置は管腔を備え、脈管の閉塞装置は、該管腔内に受容されることによって送り出し装置と係合する、医療組立体。
- 請求項53に記載の医療組立体において、脈管の閉塞装置は、送り出し装置の上方に受容されることによって送り出し装置と係合する、医療組立体。
- 請求項53に記載の医療組立体において、脈管の閉塞装置は、送り出し装置の一端に又は該一端に隣接して取り付けることによって送り出し装置と係合する、医療組立体。
- 人間の伏在静脈逆流を治療する方法において、
静脈逆流を閉塞し且つ阻止し得るよう閉塞装置を大伏在静脈内に経皮的に送り出すステップを備える、方法。 - 請求項57に記載の方法において、閉塞装置は、請求項30ないし42の何れかに記載の閉塞装置である、方法。
- 請求項57又は58に記載の方法において、閉塞装置は、穿通枝と伏在静脈との間の少なくとも1つの接続部を横断し得るように送り出される、方法。
- 請求項57ないし59の何れか1つの項に記載の方法において、伏在静脈は大伏在静脈である、方法。
- 請求項57ないし59の何れか1つの項に記載の方法において、伏在静脈は小伏在静脈である、方法。
- 第一の静脈を閉塞する方法において、
第一の静脈内に細長い閉塞装置を提供し、該細長い閉塞装置が第一の静脈と第二の静脈との間の少なくとも1つの接続部を横断するようにするステップを備える、方法。 - 請求項62に記載の方法において、第二の静脈は穿通枝又は伝達血管である、方法。
- 請求項63に記載の方法において、第一の静脈は表在静脈である、方法。
