- 【特許請求の範囲】 1.個人の腕および手における生理学的機能をアセスメントする装置であって、 (a)該個人の手首の近位側において刺激を生成および付与する刺激器であっ て、該刺激の付与は該手首を通る神経を刺激する、刺激器と、 (b)該個人の該手首の近位側において、筋電性電位を検出する検出器であっ て、該筋電性電位は、該刺激に応答して該個人の該手の筋肉によって生じるもの であり、該筋肉は該神経と連絡状態にある、検出器と、 (c)該刺激および該筋電性電位に応答して、生理学的機能を評価するコント ローラと、 を備えた装置。 2.前記コントローラは、前記刺激の付与から前記筋電性電位の検出までの間の 遅延を決定するようにさらに調節されている、請求項1に記載の装置。 3.前記コントローラは、前記遅延を手根管症候群の有無に相関させるようにさ らに調節されている、請求項2に記載の装置。 4.前記手首の近位側において前記腕上に配置され、かつ前記刺激器と電気的連 絡状態になるように調節された刺激電極をさらに備えた、請求項1に記載の装置 。 5.前記手首の近位側において前記腕上に配置され、かつ前記検出器と電気的連 絡状態になるように調節された検出電極をさらに備えた、請求項1に記載の装置 。 6.前記手首の近位側において前記腕上に配置され、かつ前記コントローラと電 的連絡状態になるように調節された基準電極をさらに備えた、請求項1に記載の 装置。 7.前記コントローラと電気的連絡状態にある温度センサをさらに備えた、請求 項1に記載の装置。 8.前記刺激は電気的刺激である、請求項1に記載の装置。 9.前記刺激は磁気的刺激である、請求項1に記載の装置。 10.前記コントローラと電気的連絡状態にあるインジケータであって、該コン トローラから指標を受け取り、前記生理学的機能を示すインジケータをさらに備 えた、請求項1に記載の装置。 11.前記インジケータは、手根管症候群の有無を示すようにさらに調節されて いる、請求項10に記載の装置。 12.前記インジケータは、発光ダイオードを備えている、請求項10に記載の 装置。 13.前記装置と外部デバイスとの間に連絡状態を確立するための連絡ポートを さらに備えた、請求項1に記載の装置。 14.個人の腕における神経伝導パラメータをアセスメントする方法であって、 (a)該個人の手首の近位側において刺激を付与するステップであって、該刺 激の付与は該手首を通る神経を刺激する、ステップと、 (b)該個人の該手首の近位側において、筋電性電位を検出するステップであ って、該筋電性電位は、該刺激に応答して該個人の手の筋肉によって生じるもの であり、該筋肉は該神経と連絡状態にある、ステップと、 (c)該刺激および該筋電性電位を処理して、処理結果を得るステップと、 (d)該処理結果に基づいて該神経伝導パラメータを評価するステップと、を 包含する方法。 15.該評価の結果を示す、表示ステップをさらに包含する、請求項14に記載 の方法。 16.前記処理ステップは、前記刺激の付与から前記筋電性電位の検出までの間 の遅延を決定するステップをさらに包含する、請求項14に記載の方法。 17.前記評価ステップは、前記神経伝導パラメータを予想値と比較するステッ プをさらに包含する、請求項14に記載の方法。 18.前記表示ステップは、前記神経伝導パラメータと予想値との間の差を示す ステップをさらに包含する、請求項15に記載の方法。 19.前記筋電性電位の一次導関数を決定するステップをさらに包含する、請求 項14に記載の方法。 20.前記筋電性電位の1つの前記一次導関数が所定のレベルを上回るまで、刺 激持続時間を漸増させながら請求項13のステップ(a)およびステップ(b) を繰り返すステップをさらに包含する、請求項19に記載の方法。 21.前記刺激の付与から前記筋電性電位の検出までの間の遅延を決定する前記 ステップは、該筋電性電位の二次導関数を決定するステップを包含する、請求項 16に記載の方法。 22.少なくとも2回の刺激付与において前記刺激の付与から前記筋電性電位の 検出までの間の遅延を決定し、該少なくとも2回の刺激付与の各回に応答して決 定される遅延を比較するステップをさらに包含する、請求項14に記載の方法。 23.個人の腕における神経筋機能をアセスメントするための電極構成であって 、 (a)該個人の手首の近位側において刺激を付与するように位置決めされる刺 激電極であって、該刺激の付与は該手首を通る神経を刺激する、刺激電極と、 (b)個人の該手首の近位側において、該刺激に応答して該神経に繋がってい る筋肉によって生じる筋電性電位を検出するように位置決めされる検出電極と、 を備え、 該電極は、該個人の該腕の前部上に位置決めされるように大きさおよび形状が 決められている、 電極構成。 24.前記構成は、前記手首の実質的に近位側において、前記個人の前記腕に取 外し可能に固定されるように調節されている、請求項23に記載の電極構成。 25.温度センサをさらに備えた、請求項23に記載の電極構成。 26.基準電極をさらに備えた、請求項23に記載の電極構成。 27.前記刺激電極および前記検出電極は電極ハウジング内に収容されている、 請求項23に記載の電極構成。 28.前記電極ハウジングは、前記手首の実質的に近位側において前記個人の前 記腕上に配置されるように調節されている、請求項27に記載の電極構成。 29.第2の刺激電極をさらに備えた、請求項28に記載の電極構成。 30.前記刺激電極および前記第2の刺激電極は、実質的に前記電極ハウジング の中央に位置決めされる、請求項29に記載の電極構成。 31.前記刺激電極および前記第2の刺激電極は、該刺激電極の一方が前記電極 ハウジングの一方の端部に位置決めされ、かつ該刺激電極の他方が該電極ハウジ ングの反対側の端部に位置決めされるように構成されており、これにより、該電 極ハウジングが前記腕の前記前部上に配置されたときに、該刺激電極の一方が前 記手首の直ぐ近位側に位置し、該刺激電極の他方が該手首のより近位側の位置に 位置する、請求項29に記載の電極構成。 32.第2の検出電極をさらに備えた、請求項27に記載の電極構成。 33.前記検出電極および前記第2の検出電極は、該検出電極の一方が前記電極 ハウジングの一方の端部に位置決めされ、かつ該検出電極の他方が該電極ハウジ ングの反対側の端部に位置決めされるように構成されており、これにより、該電 極ハウジングが前記腕の前記前部上に配置されたときに、該検出電極の一方が該 腕の内側近傍に位置し、該検出電極の他方が該腕の外側近傍に位置する、請求項 32に記載の電極構成。 34.前記電極ハウジングを前記個人の前記腕に固定するための取付け機構をさ らに備えた、請求項27に記載の電極構成。 35.前記電極ハウジングは、請求項1の装置との電気的連絡状態を得るように 調節されたコネクタを備えている、請求項27に記載の電極構成。 36.前記電極ハウジングは使い捨でである、請求項27に記載の電極構成。 37.個人の腕における神経伝導パラメータをアセスメントする方法であって、 (a)該個人の手根管の近位側において、該手根管を通る神経に複数の刺激を 付与するステップと、 (b)該複数の刺激に応答して該個人の手の筋肉によって生じる複数の筋電性 電位を検出するステップであって、該複数の筋電性電位のそれぞれは各刺激に応 答して生じるものである、ステップと、 (c)前記複数の刺激の付与から前記複数の筋電性電位の検出までの間の複数 の遅延を決定するステップと、 (d)該複数の遅延の統計値を計算するステップと、 を包含する、方法。 38.前記神経伝導パラメータと予想値との間の差の有無を示すステップをさら に包含する、請求項37に記載の方法。 39.前記複数の刺激のそれぞれは、所定のレートで一度に1つずつ送達される 、請求項37に記載の方法。 40.前記複数の刺激は、前記神経に所定のレートで対で送達される、請求項3 7に記載の方法。 41.前記対の各送達は、所定の時間間隔に分けられる、請求項40に記載の方 法。 42.前記統計値のそれぞれは、前記複数の遅延の平均値と標準偏差とを含む、 請求項37に記載の方法。 43.前記統計値は、前記複数の遅延の各遅延間の差の平均値と標準偏差とを含 む、請求項37に記載の方法。 44.少なくとも2回の刺激付与について、前記刺激の付与から前記筋電性電位 の検出までの間の遅延の差を計算するステップをさらに包含する、請求項42に 記載の方法。 45.ステップ(a)の前にノイズのレベルを測定するステップをさらに包含す る、請求項37に記載の方法。 46.(a)前記個人の前記腕の皮膚温度を測定するステップと、 (b)下記の式(A)および(B) (A)mcorrected=muncorrected+k1T+k2 (B)Scorrected=Suncorrected+k1T+k2 但し、muncorrectedは該皮膚温度において決定された平均値であり、 Suncorrectedは該皮膚温度において決定された標準偏差であり、mcorrectedは 該皮膚温度に関して調節された平均値であり、Scorrectedは該皮膚温度に関し て調節された標準偏差であり、Tは該皮膚温度であり、k1およびk2は定数であ る に従って、該皮膚温度に関して前記平均値と前記標準偏差とを調節するステップ と、 をさらに包含する、請求項42に記載の方法。
