Abstract
Description
The present invention relates to a new multifunctional surgical device.
When it is necessary, during a closed stomach surgery, to aspirate body fluids, to irrigate the operated area, to establish and maintain a pneumoperitoneum, to incise or electro-coagulate, as many accessories and instruments must be successively inserted into a probe placed for this purpose as there are functions to be fulfilled. This results in a detrimental loss of time, a loss of anatomical marks and a loss of inert gas used in the pneumoperitoneum. Added to this are significant risks of infection as a result of the succession of introduction and withdrawal of the instruments.
The invention aims to remedy these drawbacks, which goal is achieved in that it provides a multifunctional surgical device which comprises a tube simultaneously determining a space for circulation of fluid (s) and guide of instrument (s ) surgical.
More precisely, the interior space of the tube is subdivided into several sub-spaces for circulation of fluid (s), each determined by a conduit, for example, for the irrigation of the operated area, the supply of sterile inert gas, the suction of liquid (s). In practice, the suction duct can be defined by the tube itself thanks to the space left vacant in the latter by the other ducts and the instrument or instruments which it contains.
As an instrument, the tube can comprise, for example, a retractable electric scalpel, possibly transformable into a monopolar electrocoagulating tip.
The proximal end of each of the conduits and instrument (s3 emerges through the wall of the tube and that of a handle surrounding the tube at the emergence zone. This allows the ends of the conduits to be connected to the power supplies desired as well as accessing the control of the surgical instrument (s) and making, if necessary, their electrical connection.
The proximal end of the tube can, for its part, be joined to a suction.
This provides a multifunctional device in the sense that it can ensure - aspiration of peritoneal and cystic fluids whatever
whatever their density, - irrigation by injection of solutes ensuring toilet
your complete peritoneal, - a pneumatic grip, - an intervention using an electric scalpel or a
monopolar electrocoagulant tip.
The device allows the suction of liquids of high density: more or less organized clots, cellular debris, false membranes, endometriotic cysts; it is finally and above all an ideal device in the original technique of treatment of ectopic pregnancies by laparoscopy.
In a preferred embodiment, the irrigation conduit comprises, between its proximal end and the level where it enters the tube, a deformable zone capable of being flattened. It is thus possible to reduce or interrupt pressure irrigation by acting on this area.
The advantage of the device lies in the structure which makes it possible to combine the means necessary for the performance of several functions in a small diameter tube, thereby eliminating the loss of time and the risks of infection mentioned above. This tube can have a diameter of around 8mm and a length of around 50cm.
3-3 de la figure 1.An embodiment of the invention is described below by way of nonlimiting example, with reference to the appended drawing, in which - FIG. 1 represents an overall view of the apparatus, - FIG. 2 is an enlarged view of the handle
open the apparatus of Figure 1, and - Figure 3 is an enlarged section taken along the line
3-3 in Figure 1.
As can be seen from FIG. 1, the device according to the invention comprises a handle 1 traversed, right through, by a tube 2. The tube 2, for example made of stainless steel, has a bevelled end 3 and an end that can a tip 4.
The structure of the handle 1 and of the content contained therein is more clearly visible in FIG. 2-. This handle is in the form of a housing in two parts, only one of which is shown. The two parts of the housing are joined by screws (not shown) passing through the holes 6. At 7, 8 and 9, respectively, the tube 2 receives a conduit 10 for introducing sterile inert gas, a conduit 11 for irrigation and a scalpel 12 electrically insulated by a polytetra fluoroethylene sheath and protected by a sleeve 13. The irrigation conduit 11 has a flexible section 14 disposed in a cavity 15 provided in the wall of the housing 5. This cavity 15 opens onto the upper face of the housing through a passage 16 in which a rod 17, integral with a lever formed by a spring leaf 18, is capable of penetrating. Another spring leaf 19 at the end forming a cam and hook is also provided to cooperate with the former. The proximal ends of the conduits 10 and 11 are each provided with a nozzle 20.
Une fenêtre 26 prévue dans le boîtier permet le déplacement de la prise 22 lorsque l'on agit sur le poussoir 23.The proximal end of the scalpel 12 is provided with a head 21 on which are mounted a socket 22 and a plunger 23. This plunger acts against the force of a spring 24 which is received like the head 21 , in a cavity and which bears, on the one hand, on the head 21 and, on the other hand, on a shoulder 25 formed by this cavity. A window 26 provided in the housing allows displacement of the socket 22 when the pusher 23 is acted on.
As can be seen from FIG. 3, the conduits 10 and 11 and the scalpel 12 extend into the tube 2, leaving a vacant space there. The conduits 10 and 11 are welded side by side; the distal end of the duct 10 is located approximately lem behind the bevel 3, while that of the duct 11 is substantially flush with this bevel.
The use of the device is as follows
The lever 18 is lowered until its free end, after sliding along the cam surface of the blade 19 exceeds it and is locked in the lowered position. In doing so, the rod 17 was made to penetrate the passage 15 and the flexible section 14 of the conduit 11 was pressed against the underside of the cavity 15, so as to close the conduit 11.
The end piece 20 of line 10 is then connected to a supply of sterile inert gas, the end piece 20 of line 11 to a container of pressurized irrigation solution, the end piece 4 of tube 2 to a suction and the intake of current 22 to a power supply.
The beveled end 3 of the tube 2 is introduced into the abdominal cavity by an incision made for this purpose.
To establish a pneumoperitoneum, inert gas is injected through the conduit 10 up to a given pressure which will be kept constant despite the inevitable losses by regulated introduction of gas.
To proceed to a peritoneal toilet, the lever 18 is released and thus lets the irrigation solution from the pressure tank pass.
This solute and the liquids to be evacuated are sucked into the vacant space 27 of the tube 2.
The tip of the electrosurgical unit is normally retracted: to project it out of the bevel 3 of the tube 2, it suffices to act on the pusher 23 which, as soon as it is released, is brought back into position by the spring 24.
As appears from the description above, the device according to the invention, thanks to its original structure, makes it possible to maintain the pressure of a pneumoperitoneum and to perform several functions which, otherwise, would require the use of a kit of 'instruments.
One can thus make surgical gestures with a closed belly in conditions of 'speed, technical safety and optimal aseptic, which reduces the risks of complications and operative shock.
