Abstract
一種雙向性血管管路裝置,包含一管體,及一開設出一通口的連動機構。該管體包括一圍繞界定出一通道的管壁,該通道具有一遠端開口及一近端開口,該管壁開設出一連通該通道的第二開口。當體外輸液由該近端開口進入人體血管時,輸送的液體可以經遠端開口進入血管的一端內,部分血液可經由該第二開口朝血管的另一端方向輸送,防止因單向管路置入血管而阻滯血流,從而造成人體組織傷害之情事。另一方面,該連動機構可受控制部分伸出該管壁外,以將該雙向性血管管路裝置固定於適當位置,避免不慎拉扯而偏位。
Description
本發明是有關於一種導管,特別是指一種雙向性血管管路裝置。
在進行體外循環維生系統時,需要經由外部管路將血液送回人體循環系統進行生理作用,通常用來輸送血液的導管會置入人體周邊血管內,尤其常置入於腹股溝的股動脈及股靜脈內。目前常使用單向導管置入股動脈內輸送血液,在血液送入人體時,會將血液送往心臟,但如此一來原本離心方向通往遠端組織的血液被截流,無法將血液繼續輸送至遠端組織,尤其經股動脈供應血流的下肢,在長時間使用體外循環維生系統進行治療時,容易造成下肢缺血進而導致壞死之問題,並產生患者須截肢之憾事。
另一方面,習知開啟傷口插設導管時,可以確認導管是否插設至雙向血流確切位置,但開啟傷口置放導管,需要專科手術者在手術室施做,難以滿足疾病治療的即時性,且術後傷口容易滲血。為了儘快建立管路及避免傷口滲血,現時傾向以經皮穿刺的閉鎖方式進行導管設置。臨床上為了改善單向導管造成遠端組織血流供應不良的問題,有一種方法是使用額外一支導管,將其置入與原本單向導管反向的血管內,以提供前行血流灌注遠端組織,但此種方法需要額外的導管、導線及相關硬體設備及監測,施術者也需要花時間穿刺血管位置。
目前也有其他設計的單管雙向導管,例如美國第5171218號專利案、美國第2016/0121079 A1號專利案,及美國專利2018/0043085A1號,這些專利案均揭露了透過單管來達到雙向導通的導管,且這些導管也具備在置入血管後,可將導管本身固定的機構。但這些機構的結構複雜,且需在導管內部佔用不小的空間容置,故仍有很大的改善空間。
因此,本發明之目的,即在提供一種可讓血液流至遠端組織, 特別是下肢的雙向性血管管路裝置。
於是,本發明雙向性血管管路裝置,包含一管體,及一開設出一通口的連動機構。該管體包括一圍繞界定出一通道的管壁,該通道具有一遠端開口及一近端開口,該管壁開設出一連通該通道的第二開口。該連動機構可滑移地嵌設於該管壁內,並可受控制而相對於該管體在一第一位置及一第二位置間移動,當該連動機構位於該第一位置時,該通口不連通該第二開口,當該連動機構位於該第二位置時,該通口連通該第二開口,且該連動機構部分伸置出該管壁外。
本發明之功效在於:當體外輸液由該近端開口進入該管體時,輸送的液體可以經遠端開口進入血管的一端內,部分血液可經由該第二開口朝血管的另一端方向輸送,防止因管路置入血管而阻滯血流,從而造成人體組織傷害之情事。另一方面,當本發明置於適當位置時,可控制該連動機構使其由該第一位置移動至該第二位置,使該連動機構部分凸伸於該管壁外而固定於血管上,使該管體能固定在適當位置,避免不慎拉扯該管體而導致該管體滑動偏位。
在本發明被詳細描述之前,應當注意在以下的說明內容中,類似的元件是以相同的編號來表示。
參閱圖1、圖2,及圖3,為本發明雙向性血管管路裝置1之一第一實施例,其中圖3是該雙向性血管管路裝置1橫切面的示意圖。該雙向性血管管路裝置1包含一管體2,及一設置於該管體2的連動機構3。該雙向性血管管路裝置1本身具可撓性。該管體2包括一圍繞界定出一通道211的管壁21。該通道211具有一遠端開口212及一近端開口213。該管壁21開設出一連通該通道211的第二開口214,及一沿該管體2的軸向方向位於該近端開口213及該第二開口214之間的切槽215。
參閱圖1、圖2,及圖4,該連動機構3包括一可沿該管體2的軸向方向滑動地嵌設於該管壁21內的滑動件31,及一連接該滑動件31且具可撓性的擋板32。該滑動件31如圖4所示地具有一鄰近該管體2之近端開口213且部分凸伸出該管壁21的操作部311、一鄰近該遠端開口212而與該操作部311相間隔的端板部312,及二沿該管體2的軸向方向延伸以連接該操作部311及該端板部312的連接部313。該操作部311、該端板部312,及該等連接部313相配合圍繞界定出一缺槽314。該端板部312開設出一通口315,及一位於該缺槽314與該通口315之間的導孔316(在圖1及圖2中,因剖視位置而未顯示)。該擋板32的一端一體連接於該端板部312上。
參閱圖2、圖4,及圖5,使用者可透過操作該操作部311而控制該連動機構3,使該連動機構3相對於該管體2在一如圖2所示的第一位置,及一如圖5所示的第二位置間移動。當該連動機構3位於該第一位置時,該滑動件31之導孔316對準該第二開口214,該通口315未對準該第二開口214,因此該導孔316連通該第二開口214,該通口315則不連通該第二開口214。此時該擋板32位於該管壁21內且伸置於該缺槽314內。當使用者朝該近端開口213(見圖1)的方向推動該操作部311時,會使該滑動件31沿該管體2的軸向方向朝該近端開口213滑動,以使該連動機構3移動至該第二位置,在前述過程中,該擋板32被該滑動件31帶動而由該切槽215穿伸而出。當該連動機構3移動至該第二位置時,該導孔316不對準該第二開口214,該通口315對準該第二開口214,因此該導孔316不連通該第二開口214,該通口315則連通該第二開口214。此時該擋板32彎曲地向外延伸而凸伸出該管壁21外。
參閱圖2、圖6,及圖7,本第一實施例可透過一較硬的組織擴張器A插入一血管B中(該組織擴張器A由該近端開口213插入,並封閉該遠端開口212),此時該連動機構3位於該第一位置,該擋板32未凸出於該管壁21外,使該管壁21外表面呈光滑狀而利於在人體中滑移。當該導孔316(見圖4)進入該血管B中時,血液會由該導孔316經該第二開口214流入該通道211中。如此便可透過檢視該管體2內是否有血液來確認該通口315及該第二開口214是否確實進入血管B中。參閱圖5、圖6,及圖7,確認後便可透過推動該操作部311,驅使該連動機構3移動至該第二位置,使得該擋板32向外伸出以卡抵血管B內壁,達到固定該管體2之作用,避免因意外而不慎拉動該雙向性血管管路裝置1時,該管體2因移動而偏位之情形。就定位後,將該組織擴張器A取出,並由該近端開口213體外輸液至該通道211內,輸送的液體可由該遠端開口212進入血管B的一端內,而部分血液則由該第二開口214及該通口315朝血管B的另一端輸送,防止因管路置入血管B而阻滯遠端血流,從而造成人體組織傷害之情事。
參閱圖8及圖9,為本發明雙向性血管管路裝置1之一第二實施例,該第二實施例大致上是與該第一實施例相同,不同的地方在於:該端板部312未開設出該導孔316 (見圖4),而該管體2之管壁21還開設有二彼此相間隔的定位導孔216,及一位於該管壁21內且連通該等定位導孔216的內部通道217。該第二實施例之連動機構3移動至該第一位置時,該端板部312遮擋該第二開口214,使該通口315不連通該第二開口214。當該連動機構3移動至該第二位置時,該通口315對準開第二開口214,使兩者相連通。而當位於遠端的一個遠端定位導孔216A進入血管時,血液會進入該遠端定位導孔216A,並經由該內部通道217由另一個近端定位導孔216B流出,如此便可確認該通口315及該第二開口214是否已進入血管內。本第二實施例透過較細的內部通道217使血液的上升更為明顯,提升血液流入後的辨識度,以提高定位精準度。
綜上所述,本發明透過該第二開口214可使部分血液能朝血管B的另一端方向輸送,例如朝離心方向輸送,以將血液送入人體組織,從而可避免因管體2置入血管B而阻滯血流,進而傷害人體組織之情事,在該第二開口214進入血管B後,可控制該擋板32伸出,以將該管體2固定於合適位置,避免該管體2因不慎拉扯而偏位,此外,本發明還有著能透過血液的上升來確認是否進入血管B的功能,故確實能達成本發明之目的。
惟以上所述者,僅為本發明之實施例而已,當不能以此限定本發明實施之範圍,凡是依本發明申請專利範圍及專利說明書內容所作之簡單的等效變化與修飾,皆仍屬本發明專利涵蓋之範圍內。

Claims (5)
- 一種雙向性血管管路裝置,包含:一管體,包括一圍繞界定出一通道的管壁,該通道具有一遠端開口及一近端開口,該管壁開設出一連通該通道的第二開口;及一連動機構,可滑移地嵌設於該管壁內且開設出一通口,該連動機構可受控制而相對於該管體在一第一位置及一第二位置間移動,當該連動機構位於該第一位置時,該通口不連通該第二開口,當該連動機構位於該第二位置時,該通口連通該第二開口,且該連動機構部分伸置出該管壁外。
- 如請求項1所述的雙向性血管管路裝置,其中,該管壁還開設出一沿該管體的軸向方向位於該近端開口及該第二開口之間的切槽,該連動機構包括一嵌設於該管壁內的滑動件,及一連接該滑動件且具可撓性的擋板,該連動機構可沿該管體的軸向方向滑動,且開設出該通口,當該連動機構位於該第一位置時,該擋板位於該管壁內,當該連動機構位於該第二位置時,該擋板穿過該切槽而凸伸出該管壁外。
- 如請求項2所述的雙向性血管管路裝置,其中,該連動機構之滑動件具有一鄰近該管體之近端開口且部分凸伸出該管壁的操作部、一鄰近該遠端開口而與該操作部相間隔的端板部,及二沿該管體的軸向方向延伸以連接該操作部及該端板部的連接部,該端板部開設有該通口,且一體連 接該擋板,該操作部、該端板部,及該等連接部相配合圍繞出一缺槽,當該連動機構位於該第一位置時,該擋板伸置於該缺槽內。
- 如請求項3所述的雙向性血管管路裝置,其中,該滑動件之端板部還開設出一位於該缺槽及該通口之間的導孔,當該連動機構位於該第一位置時,該導孔連通該第二開口,當該連動機構位於該第二位置時,該導孔不連通該第二開口。
- 如請求項3所述的雙向性血管管路裝置,其中,該管體之管壁還開設有二彼此相間隔的定位導孔,及一位於該管壁內且連通該等定位導孔的內部通道。
