Abstract
1. Способ лечени ожоговых ран степени §- Ш , предусматривающий обработку ожоговых ран антисептиком, отличающийс тем, что, с целью сокращени сроков лечени , ожоговую рану в первые двое суток помещают в абактериальную среду с периодическими колебани ми температуры 32-30 С, затем в каждые последукшще сутки снижают циклически температуру до 20-22 0, при этом в каждом цикле снижают начальную температуру на 1,5С от минимального значени температуры каждого предьщущего цикла, далее повьщ ают ее до исходного максимального значени и повторно вьщерживают, причем вьщержку температуры в каждом цикле постепенно увеличивают с 2-3 до 18-20 мин.. 2.Способ лечени ожоговых ран степени (п, предусматривающий помещение их в абактериальную среду и проведение аутодермопластики, отличающийс тем, что, с целью сокращени сроков лечени , на вторые сутки после операции температуру абактериальной среды снижают каждые последукицие сутки циклически I до 20-22 0, при этом в каждом цикле снижают начальную температуру на (/) 1,5с от минимального значени температуры каждого предьщущего цикла, далее повышают ее до исходного максимального значени и повторно вьщерживают , причем выдержку температуры в каждом цикле постепенно увеличивают с 2-3 до 18-20 мин. 3.Устройство дл лечени ожоговых ран степени и - Ш , содержащее соединенные между собой фильтр, нагнетатель воздуха, бактериальный фильтр и электронагреватель, отличающеес тем, что, с цепью сокращени сроков лечени , в него введены датчики температур, подключенные к последнему двухпозиционный регул тор нижнего предела температуры и двухпозицонный регул тор верхнего предела температуры , а также два реле рремени, каждое из которых соединено с одним из регул торов и с нагревателем.1. Изобретение относитс к медицине в частности к лечению ожоговых ран и предназначено дл использовани в стационарах ожоговых центров. Цель изобретени - сокращение сроков лечени . Пример. Больна Ж., 25 лет поступила в ожоговое отделение с поверхностными ожогами ( - Ш степе ни правой верхней конечности и правого бедра, 7% поверхности тела. После туалета ожоговых ран в области правой верхней конечности начато лечение в регулируемой среде при 30-32С. Через 24 часа от начала лечени ожогова рана на верхней конечности покрылась тонкой корочкой , полностью ликвидировалс боле вой синдром, уменьшились влени местного воспалени . Обдув пораженной поверхности абактериальным воздухом поддерживали в диапазоне 30-32 С в течение первых двух суток. С третьих суток начат термотренинг равномерным снижением минимального значени поддерживаемой температуры на 1,5С ежесуточно при сохранении максимального значени на исходном уровне 32°С до достижени 20-22 0 (на шестые сутки после начала увеличени диапазона температур), при этом врем вьщержки равномерно увеличива ли от начального значени (2 3 мин) до 18-20 мин (фиг. 2). На шестые сутки от начала термотренинга минимальна температура воздуха, обдувающего конечность, доведена до 20С (комнатной) . На восьмые сутки конечность извлекли из изол тора. Синдрома отмены в виде местного озноблени не наблюдалось . , Больна выписана . из стационара на восемнадцатые сутки с момента травмы в удовлетворительном состо нии . П р и м е р 2. Больной К, поступил с глубокими ожогами Ж степени правой голени с размером ож говой раны 4% поверхности тела. Пос туалета раны права голень больного помещена в абактериальный изол тор и начато лечение в регулируемой сре де. Температура воздуха в изол торе поддерживалась в диапазоне 30-32с. Через 48 ч от начала лечени влажны струп стал сухим, на третьи сутки уменьшились отек и перифокальное воспаление м гких тканей. На четвертые сутки нормализовалась температура тела, значительно улучшилось общее состо ние больного. На восьмой день после поступлени в стационар больному вьшолнена некрзктоми с аутодермопластикой. На вторые сутки после операции начат термотренинг по указанному режиму (фиг. 2) до 7 сут (после операции). На дес тые сутки конечность извлекли из изол тора. Явлений местного озноблени не наблюдалось. Аутолоскут полностью прижилс . На 34-е сутки со дн поступлени больной выписан в удовлетворительном состо нии домой, приступил к работе. На фиг. 1 изображена блок-схема предлагаемого устройства; на фиг. 2 термограмма потока воздуха. Устройство содержит предварительный фильтр 1, нагнетатель 2, бактериальный фильтр 3, электронагреватель 4, гибкий воздуховод 5, лечебную камеру 6 и датчик 7 температуры. Параллельно регул тору 8 температуры установлен дополнительный регул тор 9 температуры. В линии св зи регул торов 8 и 9 с электронагревателем 4 установлены реле 10 и 11 времени, причем коммутаци вьшолнена таким образом, что в соответствии с тем, какое реле времени (10 или 11) замыкает цепь управлени электронагревателем 4, последний управл ет либо цепью, содержащей регул тор 9 температуры и реле 11 времени, либо цепью, содержащей регул тор 8 температуры и реле 10 времени (положение контактов в этом случае показано на фиг. 1 пунктиром). Регул торы 8 и 9 температуры состо т из блока настройки заданной температуры, блока сравнени заданной и фактической температуры, измер емых датчиком 7, блока формировани выходного сигнала и блока ререгулировани зоны нечувствительности .
