Abstract
본 발명은 PIBF (progesterone induced blocking factor) 단백질을 유효성분으로 함유하는 염증성 질환 예방 또는 치료용 약학 조성물에 관한 것이다. 본 발명은, 폐경 후 천식 동물 모델에, PIBF 단백질을 처리하여 기도 염증 감소를 실험적으로 확인하였는 바, 폐경 후 여성 천식 환자들에게 유용하게 사용할 수 있다.
Description
본 발명은 PIBF (progesterone induced blocking factor) 단백질을 유효성분으로 함유하는 염증성 질환 예방 또는 치료용 약학 조성물에 관한 것이다.
천식은 기도의 만성 염증과, 만성 기도 폐쇄를 일으키는 병으로, 국내 유병률이 5% 내외인 비교적 흔한 만성 질환이다. 만성 폐쇄성 기도 질환은, 최근 WHO가 지정한 4개 비 감염성 질환군에 포함된, "특별한 관리가 필요한 질환군"으로, 전 세계적으로 유병률과 사망률이 가장 빠르게 증가하는 질환이다.
천식은 인종, 성별, 나이, 지역에 따라 유병률 뿐 아니라 질환의 중증도와 예후가 다양한 질환으로 알려져 있으나, 전 세계적으로 그 유병률은 증가하는 추세이며, 이로 인한 질환 부담 또한 증가하고 있다. 특히 천식은 여성에서 지속적으로 유병률이 높으며, 사회적 취약층인 노인 인구에서도 증가하고 있다. 이 중, 폐경 후 노인 여성에서 천식의 유병률이 10% 이상 급격한 증가를 보이며 발병률 증가 및 중증의 증상 발현이 관찰되나, 그 원인은 불명확하다.
특히 노인 여성천식은 고용량의 흡입 스테로이드제에 대한 증상 조절이 잘 되지 않고, 지속적인 천식 악화 및 폐 기능 감소가 진행되는 중증천식 환자의 비율이 높다. 천식 발병률이 높은 서구에서는 중증 천식환자의 비율은 전체 천식환자의 10% 가량 차지하며, 한국 및 일본에서도 역시 5-10% 가량 차지한다. 중증 천식은 전체 천식의 5~10% 정도 차지하나, 이들이 소모하는 의료비용은 천체 천식 관련 의료비용의 60% 이상 차지한다.
중증 천식은 현재 확실한 치료법은 없으며, 일상생활에 막대한 지장을 초래하며 특히 노인 여성의 경우 스테로이드제 사용에 따른 부작용의 위험성이 더욱 높다. 장기간 전신 또는 흡입 스테로이드 제재의 사용 시, 골다공증에 의한 골절, 우울증, 불면증, 비만, 당뇨, 관상동맥질환, 의인성 쿠싱 증후군의 발생이 증가한다고 알려져 있으며, 이러한 부작용으로 인한 사회적 비용 또한 문제이다.
성별에 따른 천식 유병률 및 질환의 중증도 등 임상 양상의 차이는 여러 역학조사에서 확인되며, 여성호르몬의 영향이 질병의 병인에 관여한다는 가설이 설득력을 얻고 있으나 여성호르몬 조절을 통한 측정 질병의 치료법으로써의 활용은 아직 입증된 바가 없으며, 폐경 후 여성호르몬의 장기 투여는 암 발생, 혈전증 등 상당한 부작용 발생의 위험성이 큰 문제로, 평생 지속되는 만성질환 치료에 적용할 수 있는 약물요법이 아니다.
상기 종래기술의 문제점을 해결하기 위하여, 본 발명은 PIBF (progesterone induced blocking factor) 단백질을 유효성분으로 함유하는 염증성 질환 예방 또는 치료용 약학 조성물을 제공한다.
또한 본 발명은 PIBF (progesterone induced blocking factor) 단백질을 유효성분으로 함유하는 염증성 질환 예방 또는 개선용 식품 조성물을 제공한다.
그러나, 본 발명이 이루고자 하는 기술적 과제는 이상에서 언급한 과제에 제한되지 않으며, 언급되지 않은 또 다른 과제들은 아래의 기재로부터 당업자에게 명확하게 이해될 수 있을 것이다.
본 발명은, PIBF (progesterone induced blocking factor, 프로게스테론 유도억제인자) 단백질을 유효성분으로 함유하는 염증성 질환 예방 또는 치료용 약학 조성물을 제공한다.
상기 "예방"은 동물의 병리학적 세포의 발생 또는 세포의 손상, 소실의 정도의 감소를 의미한다. 예방은 완전할 수 있으며 또는 부분적일 수도 있다. 이 경우에는 개체 내의 병리학적 세포의 발생 또는 신경세포의 사멸이나 손실 등이 상기 염증성 질환의 예방 및 치료용 조성물을 사용 하지 않은 경우와 비교하여 감소하는 현상을 의미할 수 있다.
또한, 상기 "치료"는 치료하고자 하는 대상 또는 세포의 천연 과정을 변경시키기 위하여 임상적으로 개입하는 모든 행위를 의미하며, 임상 병리 상태가 진행되는 동안 또는 이를 예방하기 위하여 수행할 수 있다. 목적하는 치료 효과는 질병의 발생 또는 재발을 예방하거나, 증상을 완화시키거나, 질병에 따른 모든 직접 또는 간접적인 병리학적 결과를 저하시키거나, 전이를 예방하거나, 질병 진행 속도를 감소시키거나, 질병 상태를 경감 또는 일시적 완화시키거나, 예후를 개선시키는 것을 포함할 수 있다. 즉, 상기 치료는 상기 약학 조성물에 의해 염증성 질환의 증세가 호전되거나 완치되는 모든 행위를 포괄하는 것으로 해석될 수 있으나, 특별히 이에 제한되는 것은 아니다.
상기 조성물은 폐경 후 여성의 염증성 질환 예방 또는 치료용으로 사용될 수 있다.
상기 PIBF 단백질은, 반감기 연장 및 안정화 등 약물 최적화를 위해 PIBF에 FC가 결합된 재조합 단백질 형태로 적용할 수 있으며, 여기서 상기 재조합 단백질은 서열번호 1로 표시되는 염기서열 또는 서열번호 2로 표시되는 아미노산 서열로 구성될 수 있다.
또한 상기와 같은 재조합 단백질의 경우 FC 뿐만 아니라, 알부민(albumin), 알부민 결합 단백질(albumin binding protein),IgG, 페길레이션(PEGylation) 등으로 재조합될 수 있다.
상기 PIBF 단백질은 25 ~ 45kDa의 분자량으로 이루어질 수 있고, 바람직하게는 30 ~ 40 kDa의 분자량으로 이루어질 수 있다. 더욱 바람직하게는 35 kDa의 분자량으로 이루어질 수 있다.
상기 염증성 질환은 천식, 자가 면역 질환, 만성 염증, 만성 전립선염, 사구체신염, 과민증, 염증성 장 질환, 골반 염증성 질환, 류마티스 관절염, 셀리악병, 사코이드증, 간질성 방광염 또는 혈관염일 수 있다.
상기 조성물은 항염증제, 기관지 확장제, 항히스타민제, 충혈제거제 및 진해제로 이루어진 군에서 선택된 하나 이상의 약물을 추가로 포함할 수 있다. 예를 들어, A2A 효능제, A2B 길항제, 항히스타민제, 베타-2 아드레날린 수용체 효능제, 카스파제 억제제, LTB4 길항제, LTD4 길항제, PDE4 억제제, 점액용해제, 매트릭스 메탈로프로테이나제 억제제(matrix metalloproteinase inhibitor; MMPi), 류코트리엔(leukotriene), 항생제, 항-신생물제(anti-neoplastics), 펩타이드, 백신, 엘라스타제(elastase) 억제제 또는 소듐 크로모글리케이트(sodium cromoglycate) 등을 추가로 포함할 수 있다. 이러한 화합물들은 PIBF 단백질의 치료 활성의 강화제로서, 또는 PIBF 단백질의 요구 투여량 또는 잠재적인 부작용을 감소시키기 위한 수단으로서 추가로 함유될 수 있다.
상기 약학 조성물은 경구형 제형, 외용제, 좌제 또는 멸균 주사용액의 형태로 제형화될 수 있다. 상기 경구형 제형은 산제, 과립제, 정제, 캡슐제, 현탁액, 에멀젼, 시럽, 흡입제 또는 에어로졸 등의 형태일 수 있으며, 상기 제형들은 통상의 방법에 따라 제조될 수 있다.
상기 조성물을 제제화할 경우에는 희석제, 담체 또는 부형제 등을 함께 사용할 수 있다.
본 발명에서 사용가능한 담체, 부형제 또는 희석제로는, 락토즈, 덱스트로즈, 수크로스, 솔비톨, 만니톨, 자일리톨, 에리스리톨, 말티톨, 전분, 아카시아 고무, 알지네이트, 젤라틴, 칼슘 포스페이트, 칼슘 실리케이트, 셀룰로즈, 메틸 셀룰로즈, 미정질 셀룰로스, 폴리비닐 피롤리돈, 물, 메틸히드록시벤조에이트, 프로필히드록시벤조에이트, 탈크, 마그네슘 스테아레이트 또는 광물유 등을 들 수 있다.
경구투여를 위한 고형제제에는 정제, 환제, 산제, 과립제, 캡슐제 등이 포함되며, 이러한 고형제제는 상기 화합물은 적어도 하나 이상의 부형제, 예를 들면, 전분, 칼슘카보네이트(calcium carbonate), 수크로스(sucrose) 또는 락토오스(lactose), 젤라틴 등을 섞어 조제될 수 있다.
또한, 단순한 부형제 이외에 마그네슘 스테아레이트, 탈크 같은 윤활제들도 사용될 수 있다. 경구를 위한 액상 제제로는 현탁제, 내용액제, 유제, 시럽제 등이 해당되는데 흔히 사용되는 단순희석제인 물, 리퀴드 파라핀 이외에 여러 가지 부형제, 예를 들면 습윤제, 감미제, 방향제, 보존제 등이 포함될 수 있다.
비경구 투여를 위한 제제에는 멸균된 수용액, 비수성용제, 현탁제, 유제, 동결건조 제제, 좌제가 포함될 수 있다. 수성용제, 현탁제로는 프로필렌글리콜(propylene glycol), 폴리에틸렌 글리콜, 올리브 오일과 같은 식물성 기름, 에틸올레이트와 같은 주사 가능한 에스테르 등이 사용될 수 있다. 좌제의 기제로는 위텝솔(witepsol), 마크로골, 트윈(tween) 61, 카카오지, 라우린지, 글리세로제라틴 등이 사용될 수 있다.
상기 약학조성물의 유효성분인 PIBF (progesterone induced blocking factor) 단백질의 염증성 질환의 예방 또는/및 치료를 위한 유효량은 투여경로, 환자의 연령, 성별, 질환의 정도에 따라서 다르지만, 통상 10 내지 1000 ㎍/㎏의 양을 일일 1회 내지 수회 투여할 수 있다.
본 발명에 따른 약학조성물은 쥐, 생쥐, 가축, 인간 등의 포유동물에 다양한 경로로 투여될 수 있다. 투여는 통상의 방법에 따라 이루어질 수 있다. 예를 들면, 경구, 직장, 정맥, 근육, 피하, 기관지내 흡입에 의해 투여될 수 있다.
또한 본 발명은, PIBF (progesterone induced blocking factor) 단백질을 유효성분으로 함유하는 염증성 질환 예방 또는 개선용 식품 조성물을 제공한다.
본 발명에 따른 식품 조성물은 당업계에 공지된 통상적인 방법에 따라 다양한 형태로 제조할 수 있다. 일반 식품으로는 이에 한정되지 않지만 음료(알콜성 음료 포함), 과실 및 그의 가공식품(예: 과일통조림, 병조림, 잼,마아말레이드 등), 어류, 육류 및 그 가공식품(예: 햄, 소시지 콘비이프 등), 빵류 및 면류(예: 우동, 메밀국수, 라면, 스파게이트, 마카로니 등), 과즙, 각종 드링크, 쿠키, 엿, 유제품(예: 버터, 치이즈 등), 식용 식물 유지, 마아가린, 식물성 단백질, 레토르트 식품, 냉동식품, 각종 조미료(예: 된장, 간장, 소스 등) 등에 본 발명의 PIBF을 첨가하여 제조할 수 있다. 또한, 영양보조제로는 이에 한정되지 않지만 캡슐, 타블렛, 환 등에 본 발명의 PIBF 단백질을 첨가하여 제조할 수 있다. 또한, 본 발명의 PIBF 단백질 자체를 차, 쥬스 및 드링크의 형태로 제조하여 음용(건강음료)할 수 있도록 액상화, 과립화, 캡슐화 및 분말화하여 섭취할 수 있다. 또한, 본 발명의 본 발명의 PIBF 단백질을 식품 첨가제의 형태로 사용하기 위해서는 분말 또는 농축액 형태로 제조하여 사용할 수 있다. 본 발명에 따른 PIBF 단백질은 전체 조성물 총 중량에 대하여 0.01 내지 50 중량%로 포함될 수 있으나, 이에 한정되는 것은 아니다.
상기 식품 조성물은 여러 가지 영양제, 비타민, 전해질, 풍미제, 착색제, 펙트산, 펙트산의 염, 알긴산, 알긴산의 염, 유기산, 보호성 콜로이드 증점제, pH 조절제, 안정화제, 방부제, 글리세린, 알코올 또는 탄산화제 등을 함유할 수 있다. 이러한 첨가제의 비율은 크게 중요하진 않지만 본 발명의 조성물 100 중량부당 0.01 ~ 0.1 중량부의 범위에서 선택되는 것이 일반적이다.
본 발명은 PIBF (progesterone induced blocking factor) 단백질을 유효성분으로 함유하는 염증성 질환 예방 또는 치료용 약학 조성물을 제공할 수 있다. 특히 본 발명은 폐경 후의 천식 동물 모델에, PIBF 단백질을 처리하여 기도 염증 감소를 실험적으로 확인하였는 바, 폐경 후의 여성 천식 환자들에게 유용하게 사용할 수 있다.

이하, 실시예를 통하여 본 발명을 보다 상세하게 설명한다. 본 발명의 목적, 특징, 장점은 이하의 실시예를 통하여 쉽게 이해될 것이다. 본 발명은 여기서 설명하는 실시예에 한정되지 않고, 다른 형태로 구체화될 수도 있다. 여기서 소개되는 실시예는 본 발명이 속하는 기술 분야에서 통상의 지식을 가진 자에게 본 발명의 사상이 충분히 전달될 수 있도록 하기 위해 제공되는 것이다. 따라서 이하의 실시예에 의해 본 발명이 제한되어서는 안 된다.
본 발명은, 하기 실시예를 통하여 폐경을 유도한 female 천식 마우스 모델에서 PIBF 35KDa 단백질의 항천식염증효과를 확인하였다. 지금까지 천식 기전 연구에 있어 주로 염증세포의 양상이나 그 역할에 대한 연구 및 몇몇 염증성 사이토카인의 작용기전 연구에 집중되어 왔다. 천식의 임상적 다양성에 근거하여 최근 천식 치료는 임상 아형에 따른 맞춤형 치료제 개발이 이상적인 방향으로 제시되고 있다. 폐경 후 노인 여성에 발생하는 천식은 하나의 독특한 임상형이며, 이에 대한 맞춤형의 천식 치료가 절실히 필요하고 본 발명이 가장 적합한 기술일 수 있다. 또한 여성 노인 천식환자에 특히 부작용의 위험성이 높은 스테로이드의 사용을 회피할 수 있는 점이 본 발명이 가지는 가장 큰 장점일 수 있다.
<실시예>
1. 실험방법
1-1: 폐경 후 female 천식 동물 모델에서 기관지폐포세척액 내 염증세포 및 폐조직 분석
폐경 후 여성천식을 반영하는 동물모델 획립을 위해, 6 주령의 암컷 C57BL/6 마우스(오리엔트바이오)에 난소 절제술을 시행하여 폐경을 유도하였으며, 안정화 후, 급성 알레르기 천식 동물모델을 유도하였다. 또한, 본 모델에서 여성호르몬이 알레르기 천식의 임상형에 끼치는 영향을 확인하기 위해, 도 1의 프로토콜과 같이 여성호르몬을 주기적으로 주입하여 그 영향을 비교, 평가하였다. 모델 확립 24일 째, 복강 마취 및 흉부 절개 후 26 게이지 주사기를 이용하여 심장채혈을 하여 안락사와 동시에 혈액샘플을 채취하였다. 이후, 카테터를 기도에 삽입하고, 2 ml PBS 및 주사기를 이용하여 기관지폐포세척액(bronchoalveolar lavage fluid, BALF)을 확보하였다.
이어 생리식염수로 재관류(reperfusion) 후 폐조직(lung tissue)을 얻어 좌폐는 조직병리검사를 위해 10% 포르말린에 고정하였고 나머지는 단백질, RNA 추출용으로 분리하여 섭씨 -80도에 보관하였다. BALF는 혈구계산판(hemocytometer)을 이용하여 총 세포수를 구하였고, 즉시 세포 원심 분리(cytospin) 한 다음 diff quick 염색 후 differential count를 시행하였다. 폐조직은 하루 동안 고정시킨 후 섭씨 61도 이하의 온도로 녹인 파라핀을 이용하여 포매한 후 박절기를 사용하여 4 ㎛ 두께로 박절하여 슬라이드를 제작하였고 폐조직 내 염증소견을 H&E 염색으로 확인하였다 급성 천식 동물모델은 다음과 같이 유도하였다. 0일 및 14일에 난알부민 20 ug과 Alum 2 mg을 PBS와 섞어 100 ul로 만든 후, 복강내로 투여하여 OVA에 대한 전신 감작을 유도하였다. 21일째, 22일째 및 23일째에 네블라이저를 이용하여 5% OVA를 기도로 흡입시켜 천식모델을 제작하였다.
1-2: PIBF 유전자 반 결손 형질전환 마우스를 이용한 천식모델 기도염증 효과 분석
여성호르몬에 의해 유도되는 PIBF의 천식 표현형 내 영향 확인을 위하여 폐경 후 여성 천식 동물모델을 정상마우스 및 PIBF 반 결손 마우스에 유도 및 여성호르몬 처리 후 기관지폐포세척액 내 세포 유입 분석을 수행하였다. PIBF 반 결손 마우스를 적용한 점을 제외하고는 상기 1-1과 동일한 방법으로 실험을 수행하였다.
1-3: PIBF 처리를 통한 기관지폐포세척액 및 폐 조직 내 염증세포 유입 결과 분석
정상 마우스 및 난소절제술을 통한 폐경 유도 마우스에 급성 천식 동물 모델을 확립하고, 도 6의 프로토콜과 같이 PIBF(35 kDa PIBF1 재조합 단백질, 서열번호 1)를 처리하여 그 효과를 확인하였다. 도 6의 프로토콜과 같이 PIBF를 처리하는 점을 제외하고는 상기 1-1과 동일한 방법으로 실험을 진행하였다. 상기 PIBF 재조합 단백질은 PIBF에 FC가 결합된 형태로서, (주)와이바이로직스에 의뢰하여 제조하였다. 제조된 재조합 단백질은 서열번호 1의 염기서열(도 8a, PIBF1+FC) 또는 서열번호 2의 아미노산 서열(도 8b, PIBF1+FC)로 구성된다(도 8).
2. 실험결과
2-1: 폐경 후 female 천식 동물 모델에서 기관지폐포세척액 내 염증세포 및 폐조직 분석 결과
정상 마우스 대비 폐경 유도 후 천식유도 마우스의 경우, 기도 내 염증세포 침윤이 증가함을 보였으며, 이 때 여성호르몬을 처리한 군의 경우, 기도염증이 다시 감소하는 것으로 확인되었다(도 2).
조직학적 분석 결과, 난소절제술을 시행한 마우스에서 정상마우스 대비 폐 조직 내 염증세포 침윤이 증가하였으며, 다시 여성호르몬 처리 시, 감소하는 것으로 확인되었다(도 3).
2-2: PIBF 유전자 반 결손 형질전환 마우스를 이용한 천식모델 기도염증 효과
도 4에서 나타내는 바와 같이, PIBF 반 결손 시, 염증세포 유입이 증가하는 것으로 확인되었다. 또한 폐조직 분석 결과, PIBF 반 결손 마우스 내 천식 모델 유도 마우스의 경우, 정상 마우스 대비, 여성호르몬 처리에도 불구하고 기도 벽이 더 두꺼우며, 염증세포 침윤도 증가하는 것으로 확인되었다(도 5).
2-3: PIBF 처리를 통한 기관지폐포세척액 및 폐 조직 내 염증세포 유입 결과
분석 결과, 정상 마우스 대비 폐경 유도 마우스에서 천식 모델 유도 시, 기도 염증의 악화를 확인할 수 있었다. 이때, PIBF를 처리할 경우, 용량 의존적으로 그 염증이 감소함으로 확인되었다. 반면 정상 마우스에서 유도한 천식모델에서는 그 효과가 확인되지 않았고 조직 소견 또한 동일한 양상을 반영하는 것으로 확인되었다(도 7).

제조예 1. 약학적 제제의 제조

본 발명의 PIBF 단백질 0.1 g
유당 1.5 g
탈크 0.5 g
상기의 성분들을 혼합하고 기밀포에 충진하여 산제를 제조하였다.
1-2. 정제의 제조
본 발명의 PIBF 단백질 0.1 g
락토오스 7.9 g
결정성 셀룰로오스 1.5 g
상기의 성분들을 혼합한 후 직타법(direct tableting method)으로 제조하였다.
1-3. 캡슐제의 제조
본 발명의 PIBF 단백질 0.1g
옥수수전분 5g
카르복시 셀룰로오스 4.9g
상기의 성분들을 혼합하여 분말을 제조한 후, 상기 분말을 통상의 캡슐제의 제조방법에 따라 경질 캡슐에 충전하여 캡슐제를 제조하였다.
제조예 2. 건강식품의 제조
2-1. 밀가루 식품의 제조
본 발명의 PIBF 단백질 0.5 ~ 5.0 중량부를 밀가루에 첨가하고, 이를 혼합한 혼합물을 이용하여 빵, 케이크, 쿠키, 크래커 및 면류를 제조하였다.
2-4. 유제품(dairy products)의 제조
본 발명의 PIBF 단백질 5 ~ 10 중량부를 우유에 첨가하고, 상기 우유를 이용하여 버터 및 아이스크림과 같은 다양한 유제품을 제조하였다.
