Abstract
본 발명은 면역 조절능이 향상된 중간엽 줄기세포 및 이의 용도에 관한 것으로서, 구체적으로 후성유전자 조절제인 메틸화 저해제 (Methylation inhibitor) 및 히스톤 탈아세틸화 저해제 (Histone deacetylase inhibitor)를 처리하여 면역조절능이 향상된 중간엽 줄기세포와 이를 포함하는 면역관련 질환의 예방 또는 치료용 약학 조성물에 관한 것이다.
Description
본 발명은 면역 조절능이 향상된 중간엽 줄기세포 및 이의 용도에 관한 것으로서, 구체적으로 후성유전자 조절제인 메틸화 저해제 (Methylation inhibitor) 및 히스톤 탈아세틸화 저해제 (Histone deacetylase inhibitor)를 처리하여 면역조절능이 향상된 중간엽 줄기세포와 이를 포함하는 면역관련 질환의 예방 또는 치료용 약학 조성물에 관한 것이다.
면역체계는 해로운 외부물질인 항원으로부터 신체를 보호하는 역할을 한다. 이러한 항원의 종류로는 박테리아, 바이러스, 독소, 암세포, 다른 사람 혹은 동물의 혈액과 조직이 이에 해당된다. 면역체계는 이러한 해로운 물질에 반응하여 이를 제거하기 위하여 항체를 생산한다. 그러나 자가 면역에 이상이 생긴 경우, 면역체계는 자신의 건강한 신체 장기와 해로운 항원을 구분하지 못하여, 정상적인 조직을 파괴하게 되며, 이러한 반응으로 인해 야기되는 자가 면역 질환 (Autoimmune disease)이 발생하게 된다. 자가 면역에 이상이 생긴 경우에는 신체의 정상적인 조직을 대상으로 반응이 일어나게 된다. 자가면역 이상의 원인은 확실치 않으나, 박테리아와 같은 미생물 또는 약물이 자가 면역 질환에 걸리기 쉬운 유전자를 가지고 태어난 사람들에게 병을 유발하는 것이라는 가설이 있다.
자가면역 치료를 위해 다수의 화학적 및 생물학적 면역치료 방법이 개발되었다. 전형적 치료방법은 스테로이드 같은 화학제 또는 항-면역 세포항체를 이용하여 신체의 면역반응을 전체적으로 약화시키는 것이다. 예를 들어 CD22, CD20, CD19, CD74 또는 HLA-DR 항원에 결합하는 항체를 투여하여, B 세포의 정상기능을 포함한 신체의 전체적 면역반응을 약화시키는 것이다.
대한민국 공개특허공보 제2008-0109705호는 면역억제는 이 증가된 간엽줄기세포 매개 자가유래 수지상세포에 관한 것으로, T-세포 억제능이 향상된 수지상 세포 및 이의 자가면역질환 용도를 개시한다. 미국특허 제 7,074,403호는 B 세포를 표적으로 하는 항체를 이용한 자가면역 질환의 치료법에 관한 것으로, B-세포 항원에 특이적으로 결합하는 항체를 개시한다. 미국특허 제7,4735,592호는 다른 대상체 유래의 세포를 사용한 면역질환 치료용 세포 조성물에 관한 것으로, 항-CD3, 항-CD28 항체로 코팅된 T 세포 조성물를 개시한다. 종래 자가 면역질환 치료는 신체의 전반적 면역시스템을 약화시키는 것으로, 외부의 침입에 대응할 수 있는 신체의 면역기능을 전반적으로 떨어뜨려 다른 질환에 걸리게 하는 부작용을 초래하였다.
한편, 중간엽 줄기세포 (Mesenchymal Stem Cell; MSC)는 지방세포, 뼈세포 및 연골세포와 같은 여러 세포계통으로 분화할 수 있는 골수를 포함하는 전신에 존재하는 줄기세포이다. 중간엽 줄기세포는 인간, 마우스, 쥐, 개, 염소, 토끼 및 고양이를 포함하는 다수의 종으로부터 분리되었다. 최근 중간엽 줄기세포가 in vitro 및 in vivo에서 여러 T 림프구 활성을 억제하여 면역조절능력을 발휘한다는 보고가 있으나, T 림프구에 대한 중간엽 줄기세포의 면역조절 활성에 관련된 작용기전은 완전하게 규명되어 있지는 않은 상태이다.
이에, 본 발명자들은 메틸화 저해제 (Methylation inhibitor) 및 히스톤 탈아세틸화 저해제 (Histone deacetylase inhibitor)가 처리되어 면역조절능이 향상된 중간엽 줄기세포를 유효성분으로 포함하는 면역관련 질환의 예방 또는 치료용 약학 조성물을 완성하게 되었다.
본 발명의 목적은 면역억제제의 사용 없이도 효과적으로 면역질환을 예방 또는 치료할 수 있는 세포치료제를 제공하는 것이다.
본 발명의 또 다른 목적은 메틸화 저해제 및 히스톤 탈아세틸화 저해제가 처리되어 면역조절능이 향상된 중간엽 줄기세포를 유효성분으로 포함하는 면역관련 질환의 예방 또는 치료용 약학 조성물을 제공하는 것이다.
본 발명의 또 다른 목적은 중간엽 줄기세포 또는 그 배양물에 메틸화 저해제 및 히스톤 탈아세틸화 저해제를 처리하여 면역조절능이 향상된 중간엽 줄기세포를 포함하는 면역 조절제로서,
상기 메틸화 저해제 및 히스톤 탈아세틸화 저해제는 1:1 내지 1:6000 몰 비율로 처리 하는 것인 면역 조절제를 제공하는 것이다.
본 발명의 또 다른 목적은 중간엽 줄기세포 또는 그 배양물에 메틸화 저해제 및 히스톤 탈아세틸화 저해제를 처리하여 면역조절능이 향상된 중간엽 줄기세포를 제조하는 방법으로서,
상기 메틸화 저해제 및 히스톤 탈아세틸화 저해제는 1:1 내지 1:6000 몰 비율로 처리하는 것인 방법을 제공하는 것이다.
본 발명의 목적인 면역억제제의 사용 없이도 효과적으로 면역질환을 예방 또는 치료할 수 있는 세포치료제를 제공을 위해서, 본 발명자들은 면역조절능이 향상된 중간엽 줄기세포를 포함하는 면역조절 관련 질환의 예방 또는 치료용 약학 조성물로서, 상기 중간엽 줄기세포는 면역조절능을 갖는 것인 약학 조성물을 개발하였다.
본 발명에 따른 후성 유전자 조절제의 처리에 의해 면역 조절능이 향상된 중간엽 줄기세포는, 항염증성 사이토카인인 IL-10, 및 IDO 로 이루어지는 군에서 선택된 1종 이상의 발현이 증가한 세포일 수 있다.
본 발명에서 상기 "세포 치료제"란 세포와 조직의 기능을 복원시키기 위하여 살아있는 자가 (autologous), 동종 (allogenic), 이종 (xenogenic) 세포를 체외에서 증식ㆍ선별하거나 여타한 방법으로 세포의 생물학적 특성을 변화시키는 등의 일련의 행위를 통하여 치료, 진단 및 예방의 목적으로 사용되는 의약품을 말한다. 미국은 1993년부터, 우리나라는 2002년부터 세포치료제를 의약품으로 관리하고 있다. 이러한 세포치료제는 크게 두 분야로 분류할 수 있으며 그 첫 번째는 조직재생 혹은 장기기능 회복을 위한 "줄기세포 치료제"이며, 두 번째는 생체 내 면역반응의 억제 혹은 면역반응의 항진 등 면역반응 조절을 위한 면역세포 치료제로 분류할 수 있다.
이하, 본 발명을 상세히 설명하고자 한다.
일 구현예로, 본 발명은 면역조절능이 향상된 중간엽 줄기세포를 포함하는 면역조절 관련 질환의 예방 또는 치료용 약학 조성물로서, 구체적으로 메틸화 저해제 (Methylation inhibitor) 및 히스톤 탈아세틸화 저해제 (Histone deacetylase inhibitor)가 처리되어 면역조절능이 향상된 중간엽 줄기세포를 유효성분으로 포함하는 면역관련 질환의 예방 또는 치료용 약학 조성물을 제공한다.
상기 "메틸화'는 화학적으로는 기질에 존재하는 수소원자가 메틸기(Methyl group)로 치환되는 반응으로 알킬화 (Alkylation)의 한 형태이며 생물학적 시스템에서 메틸화는 효소에 의한 촉매반응으로 이루어진다. 생물체 내 유전자발현 및 단백질기능의 조절과 RNA물질대사 (RNA metabolism)에 메틸화반응이 중요한 역할을 담당한다. DNA 메틸전달효소 (DNA methyltransferase)에 의해 일어나는 이 반응은 척추동물의 DNA 내 CpG 위치 (시토신 이후 구아닌의 나란한 배열)의 시토신에 발생하여 Me-CpG의 형태를 띤다. 단백질의 메틸화반응은 히스톤 (Histone)에서 많이 연구가 되어있다. 히스톤메틸전달효소 (Histonemethyltransferase)가 S아데노실메티오닌 (S-adenosyl methionine)으로부터 히스톤에 메틸기를 전달한다.
아르기닌 잔기는 펩티딜아르기닌메틸전달효소(Peptidylarginine methyltransferase; PRMT)에 의해 단일메틸아르기닌(monomethylarginine) 또는 2메틸아르기닌(Dimethylarginine)으로 메틸화 될 수 있으며 이중 2메틸아르기닌은 하나의 질소원자에 2개의 메틸기가 결합된 비대칭구조(Asymmetric) 또는 두 개의 질소원자에 각각 메틸기가 결합된 대칭구조(Symmetric)를 형성할 수 있다. 리신 잔기에는 리신메틸전달효소(Lysinemethyltransferase)에 의해 메틸기가 3개까지 결합할 수 있다.
상기 "아셀틸화"는 화합물에 아세틸작용기를 치환하는 반응. 탈아세틸화는 아세틸기를 제거하는 반응이다. 화합물에 아세틸기의 활성수소원자를 아세틸기로 치환하는 반응으로 수산기의 수소원자를 아세틸기로 치환하면 에스테르 (ester), 아세트산염 (acetate)을 얻을 수 있다. 생물학에서 아세틸화는 히스톤, p53, 튜불린과 같은 단백질의 수식화 과정에서 일어난다.
NAT는 A, B, C 세 종류가 있으며 각기 다른 기질 (substrate)에 작용한다. 이들은 리보좀에 있으며 새로 만들어진 폴리펩티드를 아세틸화시킨다. 사람에서는 NatA와 NatB가 발견되고 연구되었다. NatA의 subunit은 저산소증과 β-카테닌(β-catenin) 신호전달과 같은 암과 관련된 분야에서 연구되어 왔고 유두갑상샘암종 (papillarythyroid carcinoma)와 신경모세포종 (neuroblastoma)과 같은 암에서 과발현 되어진다고 보고된 바가 있다.
라이신 (lysine) 아세틸화와 탈아세틸화: 히스톤 단백질은 유전자조절의 일환으로 N 말단에 라이신 잔기에서 아세틸화/탈아세틸화가 일어난다. 일반적으로 이러한 반응은 히스톤 아세틸기전이효소 (histoneacetyltransferase, HAT)나 히스톤 탈아세틸효소 (histone deacetylase, HDAC)에 의해 촉매되는데 이들은 비히스톤 단백질의 아세틸화에도 영향을 준다. 전사인자, 작동인자 단백질, 분자샤페론, 세포골격 단백질의 아세틸화/탈아세틸화의 조절은 인산화효소의 작용으로 인한 인산화나 인산의 작용으로 인한 탈인산화와 유사하게 중요한 번역 후 조절 메커니즘을 보여준다.
상기 "탈아셀틸화"는 아세틸화의 반대작용으로 히스톤, p53, 튜불린과 같은 단백질에 수식된 아세틸기를 떼어내는 반응을 의미한다.
상기 "저해제"는 효소의 촉매작용을 저해하는 물질. 저해란, 효소의 구조를 불가역적으로 변화시켜 그 활성을 잃게 하는 불활성화와는 달리, 효소의 활성중심 또는 활성발현과 관련된 다른 부분에 결합함으로써 효소활성을 억제하는 것이다. 그 작용의 차이에 따라서 길항형 저해물질(competitive inhibitor), 비길항형 저해물질(noncompetitiveinhibitor) 등으로 분류된다. 저해물질이라고도 한다.
본 발명에서 "메틸화 저해제"는 단백질 또는 핵산의 메틸화를 시키는 효소의 촉매작용을 저해하는 물질을 의미하고, 여기에는 5-azacytidine, 또는 Decitabine이 포함될 수 있으나, 이에 제한되는 것은 아니다.
본 발명에서 "탈아세틸화 저해제"는 는 단백질의 아세틸화를 시키는 효소의 촉매작용을 저해하는 물질을 의미하고, 여기에는 TSA, 또는 Valproic acid이 포함될 수 있으나, 이에 제한되는 것은 아니다.
본 발명에서 상기 "중간엽 줄기세포(Mesenchymal Stem Cell; MSC)"는 골수(bone marrow), 혈액, 진피 및 골막에서 분리되는 줄기세포로서, 다양한 세포 예컨대 지방세포, 연골세포 및 뼈세포 등으로 분화할 수 있는 전능성 (pluripotent) 또는 다능성(multipotent) 세포를 의미한다.
상기 중간엽 줄기세포는 인간 또는 포유류의 조직으로부터 분리해 낸 미분화된 성체 줄기세포의 한 종류로서, 다양한 조직에서 유래할 수 있다. 성체 줄기세포 중 조혈모세포는 주로 부유상태(non-adherent)로 존재하지만 중간엽 줄기 세포는 주로 부착성 세포들이다. 일 예에서, 제대 유래 중간엽 줄기세포, 제대혈 유래 중간엽 줄기세포, 골수 유래 중간엽 줄기 세포, 지방 유래 중간엽 줄기 세포, 근육 유래 중간엽 줄기 세포, 신경 유래 중간엽 줄기 세포, 피부 유래 중간엽 줄기 세포, 양막 유래 중간엽 줄기 세포, 및 태반 유래 중간엽 줄기 세포 등으로 이루어진 군에서 선택된 1종 이상인 것일 수 있으나, 이에 제한되는 것은 아니다.
상기 중간엽 줄기 세포는 포유류, 예컨대 인간 유래의 중간엽 줄기세포일 수 있으며, 적용되는 대상으로부터 유래하는 중간엽 줄기 세포 (자가 유래 중간엽 줄기세포), 또는 적용되는 대상과 동종 유래의 중간엽 줄기세포일 수 있다.
중간엽 줄기세포는 골수 등에 매우 적은 양으로 존재하지만, 이를 분리 및 배양하는 과정은 당업계에 잘 알려져 있으며 (예컨대, 미국특허 제5,486,359호에 개시되어 있으며), 또한, 상기 중간엽 줄기세포는 공지된 방법에 따라 골수의 조혈모세포로부터 부착특성에 의해 분리한 후 분화능력을 잃지 않은 상태에서 증식시켜 얻을 수 있다.
중간엽 줄기세포를 얻는 과정을 구체적으로 설명하면 다음과 같다: (1) 인간 또는 마우스를 포함하는 포유동물, 바람직하게는, 인간의 중간엽 줄기세포 소스, 예컨대, 혈액 또는 골수로부터 중간엽 줄기세포를 분리하는 단계 (상기 골수는 경골, 대퇴골, 척수 또는 장골로부터 추출할 수 있다.); (2) 분리된 세포를 적합한 배지에서 배양하는 단계; 및 (3) 배양과정에서 부유 세포를 제거하고 배양 플레이트에 부착된 세포들을 계대 배양하여, 최종적으로 구축된 중간엽 줄기세포를 수득하는 단계를 통하여 얻을 수 있다.
상기 과정에서 이용되는 배지로는, 줄기세포의 배양에 이용되는 일반적인 어떠한 배지도 이용할 수 있다. 바람직하게는, 혈청 (예컨대, 우태아 혈청, 말 혈청 및 인간 혈청)이 함유된 배지이다. 본 발명에서 이용될 수 있는 배지는, 예를 들어, RPMI 시리즈, Eagles's MEM (Eagle's minimum essential medium, Eagle, H. Science 130:432(1959)), α-MEM (Stanner, C.P. et al., Nat. New Biol. 230:52(1971)), Iscove's MEM (Iscove, N. et al., J. Exp. Med. 147:923(1978)), 199 배지 (Morgan et al., Proc. Soc. Exp. Bio. Med., 73:1(1950)), CMRL 1066, RPMI 1640 (Moore et al., J. Amer. Med. Assoc. 199:519(1967))를 포함하나, 이에 한정되는 것은 아니다. 상기 배지에는, 다른 성분, 예를 들어, 항생제 또는 항진균제 (예컨대, 페니실린, 스트렙토마이신) 및 글루타민 등이 포함될 수 있다.
중간엽 줄기세포의 확인은 유세포 분석을 통하여 할 수 있다. 이러한 유세포 분석은, 중간엽 줄기세포의 특이한 표면 마커를 이용하여 실시된다. 예컨대, 중간엽줄기세포는 CD44, CD29 및/또는 MHC 클래스 I에 대하여 양성 반응을 나타내므로, 이를 통해 중간엽 줄기세포를 검증할 수 있다.
일 구현예로, 본 발명은 메틸화 저해제 (Methylation inhibitor) 및 히스톤 탈아세틸화 저해제 (Histone deacetylase inhibitor)가 처리되어 면역조절능이 향상된 중간엽 줄기세포를 유효성분으로 포함하는 면역관련 질환의 예방 또는 치료용 약학 조성물을 제공하며,
상기 메틸화 저해제 및 히스톤 탈아세틸화 저해제는 1:1 내지 1:6000 몰 비율로 처리하는 것을 특징으로 할 수 있으나, 이에 한정되는 것은 아니다.
본 발명에 따른 상기 후성 유전자 조절제의 처리에 의해 면역 조절능이 향상된 중간엽 줄기세포를 면역조절 관련 세포 (예컨대, Th17 세포)와 공조배양한 경우 그 세포의 증식이 억제될 수 있고, 또한 염증성 사이토카인인 IL-2, IL-17, 및 IFN-g로 이루어지는 군에서 선택된 1종 이상의 발현이 감소할 수 있다.
중간엽 줄기세포에 후성 유전자 조절제를 메틸화 저해제 및 히스톤 탈아세틸화 저해제를 처리할 경우, 후성 유전자 조절제를 처리하지 않은 군에 비해 항염증성 유전자 (예컨대, IL-10, IDO)의 발현 정도가 현저하게 증가할 수 있다.
다른 구현예로, 본 발명은 중간엽 줄기세포 또는 그 배양물에 메틸화 저해제 및 히스톤 탈아세틸화 저해제를 처리하여 면역조절능이 향상된 중간엽 줄기세포를 제조하는 방법으로서,
상기 메틸화 저해제 및 히스톤 탈아세틸화 저해제는 1:1 내지 1:6000 몰 비율로 처리하는 것인 방법을 제공한다.
본 발명에서 상기 면역조절능이 향상된 중간엽 줄기세포는 100 mm dish에 2.5×105 ~ 7.5×105의 중간엽 줄기세포를 seeding하고, 상기 중간엽 줄기세포 또는 그 배양물에 메틸화 저해제 및 히스톤 탈아세틸화 저해제를 1:1 내지 1:6000 몰 비율로 처리하고, 18~27 시간 동안 (28~42℃ 배양)의 조건으로 배양하는 방법으로 제조할 수 있다.
상기 메틸화 저해제 및 히스톤 탈아세틸화 저해제의 몰 비율은 1:1 내지 1:10000, 1:1 내지 1:8000, 1:1 내지 1:5000 1:2 내지 1:8000, 1:2 내지 1:5000, 1:5 내지 1:5000. 바람직하게는, 1:1 내지 1:6000인 것일 수 있으나, 이에 한정되는 것은 아니다.
본 발명에서 상기 메틸화 저해제 및 히스톤 탈아세틸화 저해제는 중간엽 줄기세포 또는 그 배양물에 처리한 것일 수 있으며, 상기 메틸화 저해제 및 히스톤 탈아세틸화 저해제의 중간엽 줄기세포 또는 그 배양물에 처리는 메틸화 저해제 및 히스톤 탈아세틸화 저해제를 각각 단독으로 처리하거나 섞어서 동시에 처리하는 것일 수 있다.
상기 메틸화 저해제 및 히스톤 탈아세틸화 저해제의 함량은 사용 목적, 제형의 형태 등에 따라서 적절하게 조절 가능하며, 예컨대, 약학 조성물의 전체 중량 기준으로 0.001 내지 99중량%, 0.001 내지 90중량%, 0.001 내지 50중량%, 0.01 내지 50중량%, 0.1 내지 50중량%, 또는 1 내지 50중량%일 수 있으나, 이에 제한되는 것은 아니다.
본 발명에서 상기의 방법으로 제조한 면역조절능이 향상된 중간엽 줄기세포는 일반적으로 줄기세포를 배양하는 방법을 사용하여 배양할 수 있으며, 상기 중간엽 줄기세포를 배양하는 방법이 제한되어 있는 것은 아니다.
한편, 본 발명에 따른 상기의 방법으로 제조한 면역조절능이 향상된 중간엽 줄기세포는 당업계에 공지된 방법에 따라 증식 및/또는 배양될 수 있다.
적절한 배지는 동물 세포 및 특히, 포유동물 세포의 배양을 위해 개발되거나, 또는 동물 세포 성장에 필요한 적절한 성분, 예컨대 동화성 탄소, 질소 및/또는 미량 영양소와 함께 실험실 내에서 제조될 수 있는 임의의 이용 가능한 배지를 사용할 수 있다.
상기 배지는 동물 세포 성장에 적절한 임의의 기본 배지, 비제한적인 예로서, 일반적으로 배양에 이용되는 기본 배지로는 MEM(Minimal Essential Medium), DMEM(Dulbecco modified Eagle Medium), RPMI(Roswell Park Memorial Institute Medium), K-SFM(Keratinocyte Serum Free Medium)이 있으며, 이 외에도 당해 업계에서 이용되는 배지라면 제한없이 사용할 수 있다. 바람직하게는, α-MEM 배지(GIBCO), K-SFM 배지, DMEM배지(Welgene), MCDB 131배지(Welgene), IMEM배지(GIBCO), DMEM/F12 배지, PCM 배지, M199/F12(mixture)(GIBCO), 및 MSC 확장배지(Chemicon)로 구성된 군에서 선택될 수 있다. 이러한 기본 배지에, 탄소, 질소 및 미량 영양소의 동화성 공급원, 비제한적인 예로서, 혈청 공급원, 성장 인자, 아미노산, 항생제, 비타민, 환원제, 및/또는 당 공급원이 첨가될 수 있다. 그러나 당업계에서 통상의 지식을 가진 자가 다양한 조직 기원 유래 줄기세포에 가장 적합한 배지를 선택 또는 조합하여 공지의 방법으로 적절히 배양할 수 있음은 자명할 것이다.
또한, 이 분야의 통상의 지식에 기초하여 상기의 중간엽 줄기세포에 대하여적합한 배양 환경, 시간, 온도 등의 조건을 조절하면서 배양할 수 있음은 자명하다.
본 발명은 상기와 같은 방법으로 제작한 메틸화 저해제 및 히스톤 탈아세틸화 저해제가 처리되어 면역조절능이 향상된 중간엽 줄기세포를 유효성분으로 포함하는 면역관련 질환의 예방 또는 치료용 약학 조성물을 제공한다.
또한, 본 발명은 중간엽 줄기세포 또는 그 배양물에 메틸화 저해제 및 히스톤 탈아세틸화 저해제를 처리하여 면역조절능이 향상된 중간엽 줄기세포를 포함하는 면역 조절제로서,
상기 메틸화 저해제 및 히스톤 탈아세틸화 저해제는 1:1 내지 1:6000 몰 비율로 처리하는 것인 면역 조절제를 제공한다.
본 발명에 따른 상기 후성 유전자 조절제의 처리에 의해 면역 조절능이 향상된 중간엽 줄기세포는, 항염증성 사이토카인인 IL-10, 및 IDO 로 이루어지는 군에서 선택된 1종 이상의 발현이 증가할 수 있다.
본 발명에 따른 상기 후성 유전자 조절제의 처리에 의해 면역 조절능이 향상된 중간엽 줄기세포를 면역조절 관련 세포 (예컨대, Th17 세포)와 공조배양한 경우 그 세포의 증식이 억제될 수 있고, 또한 염증성 사이토카인인 IL-2, IL-17, 및 IFN-g로 이루어지는 군에서 선택된 1종 이상의 발현이 감소할 수 있다.
본 발명에서 상기 "면역 조절제 "란 면역요법에 사용하는 치료제이며, 생체가 항진한 면역응답능은 억제하고, 저하한 면역응답능은 부활증강하여 정상적인 면역응답능에는 아무런 영향을 주지 않는 작용을 하는 치료약제를 의미한다. 면역조절제는 크게 생물학적 물질과 합성화학약제로 분류한다. 기초실험성적을 보면 면역담당세포의 기능에는 증강적으로 작용함과 동시에 세포의 분화에도 증강, 촉진적으로 작용한다. 또한, 면역 조절제는 생체의 면역반응을 억제시키거나, 활성시키는 역할을 하며, 면역계통의 활성을 억제시키는 것을 면역 억제제라고 하고, 질병을 일으키는 특정원인에 대해 공격을 하게 유발하는 것을 면역 활성제라고 한다.
본 발명에 따른 중간엽 줄기세포 또는 그 배양물에 메틸화 저해제 및 히스톤 탈아세틸화 저해제가 처리되어 면역 조절능이 향상된 중간엽 줄기세포를 유효성분으로 포함하는 면역 조절제는 면역세포 증식억제 효과가 있을 수 있다.
예컨대, 본 발명에 따른 중간엽 줄기세포 또는 그 배양물에 메틸화 저해제 및 히스톤 탈아세틸화 저해제가 처리되어 면역 조절능이 향상된 중간엽 줄기세포를 면역조절 관련 세포 (예컨대, Th17 세포)와 공조배양한 경우 그 세포의 증식이 억제될 수 있다.
상기 면역조절 관련 세포는 Th17 세포, Th0세포, 및 Th1세포로 이루어진 군에서 선택된 어느 하나 이상인 것일 수 있으나, 이에 제한되는 것은 아니다.
본 발명의 일례는 메틸화 저해제 및 히스톤 탈아세틸화 저해제가 처리되어 면역조절능이 향상된 중간엽 줄기세포를 유효성분으로 포함하는 면역관련 질환의 예방 또는 치료용 약학 조성물을 제공한다.
본 발명에서 상기 "면역질환"은 포유류 면역계의 구성 성분에 의해 야기, 또는 매개된 포유류의 병리상태를 의미한다. 면역 반응의 자극 또는 중단으로 인한 질환을 포함할 수 있는데, 본 발명에의 면역질환은 과민성 면역반응으로 인해 야기되는 질환 예를 들면, 자가면역질환; 염증성질환; 및 세포, 조직 또는 기관의 이식거부 (transplantation rejection) 반응을 포함한다.

본 발명에서 사용된 용어 "염증성 질환"이란 염증유발인자 또는 방사선조사 등 유해한 자극으로 인해 인체 면역체계를 과도하게 항진시켜 대식세포와 같은 면역세포에서 분비되는 TNF-α (tumor necrosis factor-α), IL-1 (interleukin-1), IL-6, 프로스타글란딘(prostagladin), 루코트리엔(luecotriene) 또는 산화질소(nitric oxide, NO)와 같은 염증 유발물질(염증성 사이토카인)에 의해 유발되는 질환을 말한다.
상기 염증성 질환은 셀리악 병, 크론 병, 및 궤양성 대장염으로 이루어진 군에서 선택된 1종 이상인 것일 수 있으나, 이에 제한되는 것은 아니다.
본 발명에서 사용된 용어 "자가면역질환"이란 자기관용의 유도 또는 계속적 유지에 문제가 발생하여, 자기항원에 대한 면역반응이 일어나고, 이로 인해 자신의 조직을 공격하는 현상이 발생하는데 이러한 과정에 의해 발생되는 질환을 의미한다.

본 발명에서 상기 자가면역질환은 류마티스 관절염, 및/또는 이식거부질환 이 포함될 수 있으며, 상기 이식거부 질환에는 인슐린 의존성 당뇨병, 크론병, 다발성 경화증, 피부경화증, 중증근육무력증, 하시모토병, 자가면역 혈구감소증, 쇼그렌 증후군 및 전신홍반루푸스로 이루어진 군에서 선택된 1종 이상인 것일 수 있으나, 이에 한정되는 것은 아니다.
본 명세서에서 제공되는 유효성분 (메틸화 저해제 및 히스톤 탈아세틸화 저해제가 처리되어 면역조절능이 향상된 중간엽 줄기세포) 또는 이를 포함하는 약학 조성물은 경구 투여 또는 비경구 투여의 다양한 투여 경로로 투여 대상에게 투여될 수 있으며, 예컨대, 투여 대상의 병변 부위에 주입 또는 이식되거나, 혈관투여 (정맥투여 또는 동맥투여), 피하투여 등의 비경구 투여 경로로 투여될 수 있으나, 이에 제한되는 것은 아니다.
상기 약학 조성물의 투여 대상은 인간, 원숭이 등의 영장류, 래트, 마우스 등의 설치류 등을 포함하는 포유동물, 예컨대, 류마티스 관절염 등의 자가면역 질환을 앓고 있는 포유동물 (e.g., 인간), 및/또는 상기 동물에서 유래한 (분리된) 조직, 세포 또는 이들의 배양물 일 수 있으나, 이에 제한되는 것은 아니다.
본 발명에서 제공되는 면역관련 질환 또는 자가면역 질환의 예방 또는 치료 방법에 있어서, 치료 대상은 인간, 원숭이 등의 영장류, 래트, 마우스 등의 설치류 등을 포함하는 포유동물 또는 인간을 제외한 포유 동물일 수 있다.
본 명세서에서 제공되는 약학 조성물은, 통상의 방법에 따라 제형화 된, 산제, 과립제, 정제, 캡슐제, 현탁액, 에멀젼, 시럽, 에어로졸 등의 경구형 제형, 또는 현탁제, 유제, 동결건조 제제, 외용제, 좌제, 멸균 주사 용액, 이식용 제제 등의 비경구용 제형 등으로 제형화하여 사용될 수 있다.
본 발명의 약학 조성물에 포함되는 약제학적으로 허용되는 담체는 제제 시에 통상적으로 이용되는 것으로서, 락토스, 덱스트로스, 수크로스, 솔비톨, 만니톨, 전분, 아카시아 고무, 인산 칼슘, 알기네이트, 젤라틴, 규산 칼슘, 미세결정성 셀룰로스, 폴리비닐피롤리돈, 셀룰로스, 물, 시럽, 메틸 셀룰로스, 메틸히드록시벤조에이트, 프로필히드록시벤조에이트, 활석, 스테아르산 마그네슘 및 미네랄 오일 등을 포함하나, 이에 한정되는 것은 아니다. 본 발명의 약제학적 조성물은 상기 성분들 이외에 윤활제, 습윤제, 감미제, 향미제, 유화제, 현탁제, 보존제 등을 추가로 포함할 수 있다. 적합한 약제학적으로 허용되는 담체 및 제제는 Remington's Pharmaceutical Sciences (19th ed., 1995)에 상세히 기재되어 있다.
본 발명의 약학 조성물은 경구 또는 비경구로 투여할 수 있고, 바람직하게는 비경구로 투여된다. 비경구로 투여되는 경우, 본 발명의 약학 조성물은 정맥내 주입, 피하 주입, 근육 주입 및 복강 주입 등으로 투여할 수 있다. 본 발명의 약학 조성물은 적용되는 질환의 종류에 따라, 투여 경로가 결정되는 것이 바람직하다. 예컨대 타입 I 당뇨병에 적용되는 경우, 복강투여가 가장 바람직하고, 이는 투여된 수지상세포가 희석되지 않고 효과적으로 췌장으로 이동할 수 있기 때문이다. 또한, 본 발명의 약학 조성물이 관절염 환자에 적용되는 경우에는 정맥내 주사로 투여될 수 있지만, 가장 바람직하게는 국부적으로 관절내 주입으로 투여된다.
본 발명의 약학 조성물의 적합한 투여량은 제제화 방법, 투여 방식, 수용자의 연령, 체중, 성, 병적 상태, 음식, 투여 시간, 투여 경로, 배설 속도 및 반응 감응성과 같은 요인들에 의해 다양하게 처방될 수 있다. 본 발명의 약학 조성물의 투여량은 바람직하게는 1일 당 1 x 103- 1 x 1012 세포/kg 이다.
발명의 약학 조성물은 당해 발명이 속하는 기술분야에서 통상의 지식을 가진 자가 용이하게 실시할 수 있는 방법에 따라, 약제학적으로 허용되는 담체 및/또는 부형제를 이용하여 제제화함으로써 단위 용량 형태로 제조되거나 또는 다용량 용기 내에 내입시켜 제조될 수 있다.
본 발명은 면역 조절능이 향상된 중간엽 줄기세포 및 이의 용도에 관한 것으로서, 구체적으로 중간엽 줄기세포 또는 그 배양물에 메틸화 저해제 (Methylation inhibitor) 및 히스톤 탈아세틸화 저해제 (Histone deacetylase inhibitor)를 처리하여 면역조절능이 향상된 중간엽 줄기세포를 포함하는 약학 조성물을 면역관련 질환의 예방 또는 치료에 활용할 수 있다.

골수 유래 중간엽 줄기세포는 가톨릭세포치료 사업단으로부터 구입하여 사용하였다. 중간엽 줄기세포는 DMEM/F12 (Gibco)에 10% FBS와 antibiotics를 첨가하여 37℃, 5% 이산화탄소, 습도 95% 배양기에 배양하였다.

100 mm dish에 5 X 105의 중간엽 줄기세포를 seeding하고, 메틸화 저해제 및 히스톤 탈아세틸화 저해제를 하기 표 1과 같은 다양한 조합으로 처리하였다.

평가
실시예 1에서 얻은 후성 유전자 조절제로 처리한 중간엽 줄기세포에 대해서, 면역조절능 관련 유전자 인 IDO 및 IL-10 (항염증성 유전자)의 발현을 하기와 같은 real-time PCR 방법을 사용하여 확인하여 중간엽 줄기세포의 면역조절능 향상 여부를 평가하여 실험결과를 상기 표 2에 나타냈다.
구체적으로, 100 mm dish에 5 X 105의 중간엽 줄기세포를 seeding하고, 메틸화 저해제 및 히스톤 탈아세틸화 저해제를 표 1과 같은 다양한 조합으로 처리하여 처리하였다. 3일동안 중간엽 줄기세포를 배양한 후, 0.25% trypsin을 이용하여 세포를 수확하여 Trizol (Invitrogen)을 이용하여 RNA를 분리하였고, cDNA를 First-Strand cDNA Synthesis Kit for Real-Time PCR (Thermo Fisher)를 이용하여 합성한 후, real time PCR을 45 cycle, Tm 60℃에서 수행하였다. Real-tiem PCR을 수행하기 위해 사용된 primer 서열은 하기와 같다.

상기 표 1에 나타낸 바와 같이, 중간엽 줄기세포에 후성 유전자 조절제를 메틸화 저해제 및 히스톤 탈아세틸화 저해제는 1:1 내지 1:5000 몰 비율로 처리 처리한 경우, 후성 유전자 조절제를 처리하지 않은 군에 비해 항염증성 유전자인 IL-10, IDO 의 발현 정도는 5~16배 정도 증가한 것으로 나타났다.
상기 표 1에 나타난 바와 같이, 중간엽 줄기세포에 아무런 저해제를 처리하지 않은 대조구 (시료번호 1), 2 uM 5-Azacytidine을 처리 (시료번호 2), 500 nM TSA (시료번호 3)를 처리. 100 nM Decitabine (시료번호 4)를 처리, 5 mM VPA (시료번호 5)를 처리했을 때에 비하여, 100 nM Decitabin과 500 nM TSA를 병용처리한 경우 (시료번호 6), 2 uM 5-Azacytidine과 5 mM VPA를 병용처리한 경우 (시료번호 7), 및 2 uM 5-Azacytidine과 10 mM VPA를 병용 처리한 경우 (시료번호 7)에서 항염증성 유전자인 IL-10, 및 IDO의 발현 정도가 현저하게 증가했다.
구체적으로, 중간엽 줄기세포에 아무런 저해제를 처리하지 않은 대조구 (시료번호 1)에 비해서, 100 nM Decitabin과 500 nM TSA를 병용처리한 경우 (시료번호 6) IL-10의 발현 정도는 13.5배 증가했고, IDO의 발현 정도는 16배 증가했으며, 2 uM 5-Azacytidine과 5 mM VPA를 병용처리한 경우 (시료번호 7)는 IL-10의 발현 정도는 5.5배 증가했고, IDO의 발현 정도는 10.4배 증가했으며, 2 uM 5-Azacytidine과 10 mM VPA를 병용 처리한 경우 (시료번호 8) IL-10의 발현 정도는 5.4배 증가했고, IDO의 발현 정도는 9.4배 증가했다.
따라서 상기의 결과를 토대로 본 발명자들은 실시예 1의 방법으로 후성 유전자 조절제를 중간엽 줄기세포에 처리한 시료번호 6 내지 8 의 경우, 대조군 (시료번호 1)에 비해서, IDO 및 IL-10 발현이 현저하게 증가하는 것을 알 수 있었다.

중간엽
줄기세포의 영향
3-1: CD4+ T 세포 분리 및 배양
건강한 성인으로부터 말초혈액 단핵세포 (Peripheral blood mononuclear cells, PBMC)를 얻고, 여기에서 CD4 antibody를 사용하여 CD4+ T 세포를 다음과 같은 방법으로 분리하여 얻었다.
건강한 성인 혈액을 PBS (Phosphate-Buffered Saline)와 1:1로 섞어 Ficoll과 혈액+PBS을 1:4의 비율로 Ficoll층이 흐트러지지 않게 50 mL 튜브에 천천히 띄운 다음 혈액을 2000 rpm, 30분간 원심 분리하였다. Buffy coat층만을 따서 새 튜브에 담은 후 PBS로 세척하고 CD4+ T 세포를 분리하기 위해 monoclonal anti-human CD4 antibody가 붙어있는 microbeads (MicroBeads; Miltenyi Biotech, Bergisch Gladbach, Germany)를 이용하여 분리 후 세포 수를 세어 5 x 105개씩을 48 well plate (NUNC, Denmark)에 배양하였다.

후성 유전자 조절제가 처리된 중간엽 줄기세포에 의한 면역 조절 능력을 확인하기 위해, 건강한 성인 말초 혈액 단핵세포에 대하여 T 세포를 활성화시키는 Th0 조건, Th17 세포를 분화시킬 수 있는 Th 17조건으로 자극한 것에 대하여 실시예 1에 따라 후성 유전자 조절제가 처리된 중간엽 줄기세포를 공조배양하여, 하기와 같은 방법으로 실험을 수행하였다.
CD4+ T세포는 특정 사이토카인과 전사인자의 발현에 따라 Th0 (T helper type 0), Th1 (T helper type 1), Th2 (T helper type 2), CD4+CD25+면역조절 T세포 (regulatory T cell)와 Th17 세포 (T helper 17 cell) 등의 아형으로 구분될 수 있다. 이중 T세포 수용체 자극제로 T 세포 활성화될 수 있는 Th0 세포는 염증성 질환의 병인에서, 특이적인 사이토카인 IL-17을 많이 생산하는 Th17세포은 자가면역 질환인 류마티스 관절염 병인에서 동물모델과 인간세포 연구 결과들을 통해 직접적인 역할과 병원체에 대한 숙주 방어 또는 이상 면역반응 유도 등의 역할이 규명되어 있다.
구체적으로, 실시예 3-1에서 분리한 말초혈액 단핵세포를 T 세포 수용체 자극제로 T 세포를 자극하는 Th0 조건 (Anti-CD3 (BD Phamingen, USA, #555329), anti-CD28 (BD Phamingen, USA, #555725), IL-17을 생산하는 Th17 세포 분화 조건 (anti-CD3 1 ug/ml (BD Phamingen, USA, #555329) anti-CD28 1 ug/ml (BD Phamingen, USA, #555725), IL-1b 20 ng/ml (R&D systems, USA, #201-LB-025), IL-6 20ng/ml (R&D systems, USA, #206-IL-010), IL-23 20ng/ml (R&D systems, USA, #1290-IL-010), anti-IL-4 2ug/ml (R&D systems, USA, #MAB304-100), anti-IFN-r 2ug/ml (R&D systems, USA, #MAB2851-100)으로 자극하고, 실시예 1에 따라 후성 유전자 조절제가 처리된 중간엽 줄기세포와 공조배양하였다.
3-3: CD4+ T 세포의 사이토카인 함량 분석
염증성 Th0 또는 Th17 세포 분화조건으로 자극 후 중간엽 줄기세포만 또는 실시예 1에 따라 후성 유전자 조절제가 처리된 중간엽 줄기세포를 3일간 공조배양한 후 자극 된 CD4+ T 세포의 항염증성 및 염증성 사이토카인 변화를 하기와 같은 유세포분석기 (FACS) 및 ELISA 방법으로 확인하였다.
FACS 방법으로 자극 된 CD4+ T 세포의 사이토카인 변화를 확인하기 위해서 구체적으로, 상기 자극된 세포에서 생성되는 사이토카인 분비를 세포내에 유지하도록 Golgi-stop (Pharmingen, San Diego CA)으로 4시간 동안 처리하여 PE/cy7 anti-human CD4 (Biolegend, USA, #300512), APC anti human CD25 (BD Phamingen, USA, #555434)로 세포 표면을 staining하고 세포내 사이토카인 측정을 위해 Cytofix/Cytoperm (BD ParMingen, USA, #554723)를 이용하여 fixation /permeabilization (BD ParMingen, USA, #554714) 시킨 후 PE Conjugated Anti human IL-17A (eBioscience, USA, #12-7179-42), FITC Conjugated Anti-human IFN-g (eBioscience, USA, #11-7319-82), APC anti human IL-10 (Biolengend, USA, #506807), PE anti-Foxp3 (eBioscience, USA, #12-4776-42)로 세포에 각각 넣고 4℃에서 30분간 반응시켰다. 0.5% (v/v) BSA/PBS buffer로 씻어준 후 유세포 분석기(FACS Calibur; Becton Dickinson, San Diego, CA)로 CD4+ T 세포내 사이토카인을 측정하였다.
그 결과, 도 1의 A, B, 및 C에 나타난 것과 같이 염증성 Th0 조건을 처리한 CD4+ T 세포에 대하여 중간엽 줄기세포를 공조배양하지 않았을 때를 기준으로 상기 염증성 Th0 조건을 처리한 CD4+ T 세포와 아무런 처리를 하지 않은 중간엽 줄기세포를 공조배양한 것, 또는 상기 염증성 Th0 조건을 처리한 CD4+ T 세포에 대하여 실시예 1에 따라 후성 유전자 조절제 (상기 표 1의 시료번호 6 내지 8)가 처리된 중간엽 줄기세포를 공조배양한 조건에서 사이토카인의 함량을 분석한 결과, IL-17 (도 1의 A)의 발현은 염증성 Th0 조건을 처리한 CD4+ T 세포에 대하여 중간엽 줄기세포를 공조배양하지 않았을 때의 함량에 비해서 염증성 Th0 조건을 처리한 CD4+ T 세포에 대하여 MSC를 공조배양했을 때 40% 감소, 염증성 Th0 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V5)를 공조배양했을 때 63% 감소, 염증성 Th0 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V10)를 공조배양했을 때 63%감소, 염증성 Th0 조건을 처리한 CD4+ T 세포에 대하여 MSC(D100T500)를 공조배양했을 때 71%로 감소하였으며, IFN-g (도 1의 B)는 염증성 Th0 조건을 처리한 CD4+ T 세포에 대하여 중간엽 줄기세포를 공조배양하지 않았을 때의 함량에 비해서 염증성 Th0 조건을 처리한 CD4+ T 세포에 대하여 MSC를 공조배양했을 때 34% 감소, 염증성 Th0 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V5)를 공조배양했을 때 52% 감소, 염증성 Th0 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V10)를 공조배양했을 때 63%감소, 염증성 Th0 조건을 처리한 CD4+ T 세포에 대하여 MSC(D100T500)를 공조배양했을 때 71%로 감소하였으나, 면역조절 T세포인 CD25+Foxp3+ (도 1 C) 함량은 유지됨 (도 1의 C)을 확인하였다.
또한, 도 2의 B, C, 및 D에 나타난 것과 같이 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 중간엽 줄기세포를 공조배양하지 않았을 때를 기준으로 상기 염증성 Th17 조건을 처리한 CD4+ T 세포와 아무런 처리를 하지 않은 중간엽 줄기세포를 공조배양한 것, 또는 상기 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 실시예 1에 따라 후성 유전자 조절제 (상기 표 1의 시료번호 6 내지 8)가 처리된 중간엽 줄기세포를 공조배양한 조건에서 사이토카인의 함량을 분석한 결과, IL-17 (도 2의 B)의 발현은 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 중간엽 줄기세포를 공조배양하지 않았을 때의 함량에 비해서 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC를 공조배양했을 때 21% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V5)를 공조배양했을 때 65% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V10)를 공조배양했을 때 58% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(D100T500)를 공조배양했을 때 51%로 감소하였으며, IFN-γ (도 2의 C)는 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 중간엽 줄기세포를 공조배양하지 않았을 때의 함량에 비해서 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC를 공조배양했을 때 7% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V5)를 공조배양했을 때 22% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V10)를 공조배양했을 때 19% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(D100T500)를 공조배양했을 때 29%로 감소하였으나, 면역조절 T 세포인 CD25+Foxp3+ (도 2 D)는 감소하지 않음을 확인하였다.
염증성 Th17 세포 분화조건으로 자극 후 중간엽 줄기세포만 또는 실시예 1에 따라 후성 유전자 조절제가 처리된 중간엽 줄기세포를 3일간 공조배양한 후 자극 된 CD4+ T 세포의 항염증성 및 염증성 사이토카인 변화를 하기와 같은 sandwich ELISA 방법으로 확인했다.
구체적으로, Sandwich ELISA용 96 well plate (NUNC, Denmark)에 단클론성 IL-17(R&D systems, USA, #MAB317), IFN-r (R&D systems, USA, #MAB2851), IL-10(R&D systems, USA, #MAB2172), IL-2 항체(R&D systems, USA, #MAB202를 각각 4 ug/mL로 50 ul/well씩 넣고 4℃에서 밤새 반응시킨 다음 차단용액 (1% (w/v) BSA/ PBS (Phosphate-Buffered Saline), 0.05% (v/v) tween 20/PBS)을 200 uL/well씩 넣고 실온에서 2시간 반응시켰다. 대조군으로는 재조합 IL-17, IFN-r, IL-10, IL-2 (R&D, USA)를 이용 하여 각각 5 ng/mL∼78 pg/mL 농도를 측정하였다. 표준시료와 함께 측정할 분리한 환자 혈청을 50 uL/well씩 넣고 실온에서 2시간 반응시켰다. 반응용기를 세척용액 (0.05% (v/v)Tween 20/PBS)으로 4번 세척하고 biotinylated IL-17, IFN-r, IL-10, IL-2 항체를 200 ng/mL로 희석하여 50 uL/well씩 넣어 실온에서 2시간 반응시킨 후 세척용액으로 4번 세척하였다. 마지막으로는 Extravidin-Alkaline phosphatase conjugate (SIGMA, USA, #E2636)를 1：2,000으로 희석하여 50 uL/well씩 넣고 실온에서 2시간 반응시키고 세척 후 phosphate disodium salt hexahydrate (PNPP, Fluka)/Diethanolamine 용액(DEA, 97Ml, NaN3 0.2 g, MgCl2H2O 0.1 g, 1차 증류수 800 mL)을 1 mg/mL 농도로 녹여 50 uL/well씩 넣어 30분 후 0.2 M NaOH로 반응을 멈추고 405 nm 파장에서 흡광도를 측정하였다.
그 결과 도 3에서 나타난 것과 같이, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 중간엽 줄기세포를 공조배양하지 않았을 때를 기준으로 상기 염증성 Th17 조건을 처리한 CD4+ T 세포와 아무런 처리를 하지 않은 중간엽 줄기세포를 공조배양한 것, 또는 상기 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 실시예 1에 따라 후성 유전자 조절제 (상기 표 1의 시료번호 6 내지 8)가 처리된 중간엽 줄기세포를 공조배양한 조건에서 사이토카인의 함량을 분석한 결과, IL-17 (도 3의 A)의 발현은 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 중간엽 줄기세포를 공조배양하지 않았을 때의 함량에 비해서 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC를 공조배양했을 때 35% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V5)를 공조배양했을 때 70% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V10)를 공조배양했을 때 61% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(D100T500)를 공조배양했을 때 73%로 감소하였으며, IFN-γ (도 3의 B)는 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 중간엽 줄기세포를 공조배양하지 않았을 때의 함량에 비해서 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC를 공조배양했을 때 46% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V5)를 공조배양했을 때 33% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V10)를 공조배양했을 때 36% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(D100T500)를 공조배양했을 때 63%로 감소하였으며, IL-2 (도 3의 C)는 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 중간엽 줄기세포를 공조배양하지 않았을 때의 함량에 비해서 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC를 공조배양했을 때 11% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V5)를 공조배양했을 때 28% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(A2V10)를 공조배양했을 때 42% 감소, 염증성 Th17 조건을 처리한 CD4+ T 세포에 대하여 MSC(D100T500)를 공조배양했을 때 92%로 감소하였으나, 항염증성 사이토카인 IL-10의 생성은 감소하지 않았다 (도 3의 D).
3-4: CD4+ T 세포의 증식 측정 (Mixed Lymphocytes Reaction)
또한, 염증성 Th17 세포 분화조건으로 자극 한 CD4+ T 세포와 실시예 1에 따라 후성 유전자 조절제가 처리된 중간엽 줄기세포 혹은 염증성 Th17 세포 분화조건으로 자극 한 CD4+ T 세포와 중간엽 줄기세포만을 3일간 공조배양한 후 염증관여 Th17 세포의 증식을 T 세포 증식 측정 방법 (mixed lymphocytes reaction)으로 확인하였다.
구체적으로, 상기 실시예 3-2의 Th17 세포 분화조건으로 48시간 배양 후 12시간 전에 3H-thymidine처리하여 Thymidine Incorporation assay (PerkinElmer Life Sciences, USA)방법으로 동위원소측정을 counts per minute (cpm)로 나타내었다.
그 결과 도 2의 A에 나타난 것과 같이, 염증성 Th17 세포 분화조건으로 자극 한 CD4+ T 세포와 실시예 1에 따라 후성 유전자 조절제가 처리된 중간엽 줄기세포를 공조배양한 경우 염증성 Th17 세포 분화조건으로 자극 한 CD4+ T 세포와 중간엽 줄기세포만을 공조배양한 경우에 비하여 더 효과적으로 Th17 세포의 증식이 억제되었다.

줄기세포의 영향
류마티스 관절염 환자의 관절 활막내 단핵세포에 대하여 실시예 3-2와 같은방법으로 Th0 자극한 후에 실시예 1과 같은 방법으로 후성 유전자 조절제가 처리된 중간엽 줄기세포를 공조배양하고, 그 영향을 살펴보았다.
구체적으로, 류마티스 관절염 환자로부터 관절 활막내 단핵세포 (synivial fluids mononuclear cells, SFMC)는 다음과 같은 방법으로 얻었다. 류마티스 관절염 환자의 활액을 PBS (Phosphate-Buffered Saline)와 1:5로 섞어 Ficoll과 활액+PBS을 1:4의 비율로 Ficoll층이 흐트러지지 않게 50 mL 튜브에 천천히 띠운 다음 혈액을 2000 rpm, 30분간 원심 분리하였다. Buffy coat층만을 따서 새 튜브에 담은 후 PBS로 세척하고 CD4+ T cell을 분리하기 위해 monoclonal monoclonal anti-human CD4 microbeads (MicroBeads; Miltenyi Biotech, Bergisch Gladbach, Germany, #130-045-101)를 이용하여 분리 후 세포 수를 세어 5 x 105개씩을 48 well plate (NUNC, Denmark)에 배양하였다.
실시예 3-2와 같은 방법으로 활막 단핵세포를 염증성 Th0 세포 분화조건으로 자극후, 실시예 1에서 제조한 후성 유전자 조절제가 처리된 중간엽 줄기세포 혹은 그 대조군인 중간엽 줄기세포를 3일간 공조배양하였다. 그런 후에, 실시예 2-3와 같은 방법으로 Th0 세포의 사이토카인 변화를 유세포분석기 (FACS)를 사용하여 확인하였다.
그 결과 도 4에 나타난 것과 같이, 류마티스 관절염 환자의 관절내 활막단핵세포에 대하여 염증관여 Th0 조건을 처리한 한 후에 중간엽 줄기세포만 공조배양한 경우에 비해서, 상기 관절내 활막단핵세포에 대하여 Th0 조건을 처리한 한 후 실시예 1과 같이 후성 유전자 조절제 (상기 표 1의 시료번호 6 내지 8)가 처리된 중간엽 줄기세포와 공조배양한 조건에서 IL-17 (도 4 A), 또는 IFN-g (도 4 B)의 발현은 감소한 반면 면역조절 T세포인 CD25+Foxp3+ (도 4의 C)는 감소하지 않음을 확인하였다.
상기 결과는 류마티스 관절염 환자의 병인세포들이 많이 모여있는 관절내 활막 단핵세포와 상기 실시예 1과 같이 후성 유전자 조절제가 처리된 중간엽 줄기세포를 공동배양한 경우에도 면역조절능 효과가 있음 나타낸다.

Claims (16)
- 면역조절능이 증가된 중간엽 줄기세포를 유효성분으로 포함하는, IL-17, IFN-g, IL-2, IL-10, 및 IDO으로 이루어진 군에서 선택된 1종 이상의 사이토카인 매개성 염증 질환의 예방 또는 치료용 약학 조성물로서,
상기 면역조절능이 증가된 중간엽 줄기세포는, 중간엽 줄기세포 또는 그 배양물에 메틸화 저해제 (Methylation inhibitor) 및 히스톤 탈아세틸화 저해제 (Histone deacetylase inhibitor)를 1:1 내지 1:6000 몰 비율로 처리하여 얻어진 것이며, 상기 메틸화 저해제 및 히스톤 탈아세틸화 저해제는 (a)Decitabine 및 TSA, 또는 (b)5-azacytidine 및 VPA이고,

- 중간엽 줄기세포 시료 또는 그 배양물 시료에, 메틸화 저해제 및 히스톤 탈아세틸화 저해제를 1:1 내지 1:6000 몰 비율로 처리하여, 면역 조절능이 증가된 중간엽 줄기세포를 얻는 방법으로서,
상기 메틸화 저해제 및 히스톤 탈아세틸화 저해제는 (a)Decitabine 및 TSA, 또는 (b)5-azacytidine 및 VPA이고,
상기 면역 조절능이 증가된 중간엽 줄기세포는 Th17 세포, Th0세포, 및 Th1세포로 이루어진 군에서 선택된 어느 하나 이상 면역세포의 증식 억제,
IL-10, 및 IDO으로 이루어진 군에서 선택된 1종 이상의 항염증 인자의 발현을 증가, 또는
IL-17, IFN-g, 및 IL-2로 이루어진 군에서 선택된 1종 이상의 염증인자의 발현을 감소하는 것인, 면역 조절능이 증가된 중간엽 줄기세포를 얻는 방법.
