Abstract

PURPOSE:To facilitate insertion and alignment of a suture prosthetic material adequate for use in an automatic suture device in surgical operation and to solve problems of mispositioning, etc., after mounting by forming a cloth body to a cylindrical shape, thereby forming the prosthetic material described above. CONSTITUTION:This suture prosthetic material is formed by superposing non- woven fabrics 3 consisting of an in-vivo decomposable absorbent blank and a power net fabric 4 on each other and forming these fabrics to a cylindrical shape by subjecting both edge parts to chain stitching 5, 6. Both ends 7 of the sewing yarn are extended. The in-vivo decomposable absorbent blank includes a polyglycolic acid, polylactic acid, a copolymer of the glycolic acid and lactic acid, paradioxanone, polycarprolactone, chitin, etc. The prosthetic material is mounted to the frame part 10 and cartridge part 9, respectively, of the suture device at the time of use. Both are then integrally fixed by staples in the state of holding the body tissues of the lesion between non-woven fabrics 3, 3 and the lesion 13 is ported. The power net fabric 4 is removed outside the body together with the lesion 13 by pulling the yarn ends 7.

The present invention is a tubular suture prosthesis material. In the first invention, since a stretchable material is used as a part of the constituent material, the stretchable material prevents the suture prosthesis from being attached to an automatic suturing device. Further, since the adhesion after mounting is increased, there is no displacement and the cutting of the prosthesis material, which is performed at the same time as the cutting of the affected area, is in tension, so that it can be performed easily and reliably.

Further, in the second invention, since the whole is a cloth-like body made of a biodegradable and absorbable material, it is not necessary to remove the non-decomposable substance after surgery, and since the removal of the substance is forgotten, Accidents such as foreign bodies remaining in the body can also be prevented.

The stretchable knitted or woven fabric of the first aspect of the present invention is appropriately knitted or woven together with rubber threads, polyurethane-based elastic threads, crimps, bulky processed threads, etc. in its structure. Although it has elasticity in the direction, its structure is not particularly limited, but from the viewpoints of ease of sewing such as cutting and integration, and stability of form, polyurethane yarn is covered with nylon yarn, for example. A particularly preferable example is a power net fabric knitted into a warp knit with a thread obtained by ringing.

On the other hand, as the cloth-like body made of a biodegradable and absorbable material, polyglycolic acid, polylactic acid, a copolymer of both, or a mixture thereof, and further paradioxanone, polycaprolactone, chitin, etc. have already been used as medical materials. Examples of such materials are knitted fabrics, woven fabrics, non-woven fabrics, films, and the like, but in particular, flexibility, breathability, appropriate waist, thickness, blood absorption, ease of passing staples, degradability, etc. From the Japanese Patent Publication No. 5-18579, which was previously invented by the applicant,
Examples of suitable materials include a knitted and woven knitted fabric made of polyglycolic acid disclosed in JP-A-79586, and a non-woven fabric manufactured by needle punching a woven fabric. In addition, such a nonwoven fabric has a property of poor stretchability.

According to the first aspect of the present invention, when the knitted fabric having a stretchability and the cloth and the cloth-like body are integrated, they are both cut to a required size (preferably the same size for integration work). Put the cloths on top of each other, and both ends (both ears)
Are integrated by, for example, sewing, bonding or the like. As a particularly preferable means of integration, a coarse pitch comb stitch that allows easy separation of both can be exemplified, and the thread end thereof is extended to an appropriate length in order to facilitate the above separating operation. It is desirable to put it out. Examples of the sewing thread include sewing thread for sewing clothing, suture thread for medical use, nylon,
It is composed of a non-biodegradable and absorbable material such as polyester, vinylon, cotton and silk, and any of the above-mentioned biodegradable and absorbable materials. As a suture, it is preferable that the suture is made of the same material as the biodegradable and absorbable material of the present invention in consideration of the case where the thread is caught and remains in the body. Further, it is important for achieving the above-mentioned object that the inner diameter of the tube formed by the above integration is equal to or smaller than the outer diameter of the suture instrument to be applied, and therefore, the size, shape, and shape of the cloth-like body to be used, It is desired that the sewing means for integration and the like meet such purposes.

In the second aspect of the present invention, the tubular body made of the biodegradable and absorbable material is appropriately tubular according to the desired size, that is, the size of the staple implantation site of the automatic suturing device to be applied. The cloth-like body cut into a size is joined by an arbitrary method such as stitching or adhering. For example, rough provisional stitching or use of provisional attachment is advantageous in workability after the suturing operation described later. Therefore, it is particularly preferable. However, a knitted fabric knitted in a high density in a tubular shape may be used as a suture prosthesis material so as to be suitable for the use.

As the non-woven fabric (3), 12 filaments, 33 denier polyglycolic acid yarns were knitted by an 18-gauge circular knitting machine with a basis weight of 45 g / m 2 , and four needle-punched in the wale direction were prepared. Then, they were alternately laminated so that the course direction and the wale direction were orthogonal to each other, and the whole was needle punched to be integrally manufactured. The power net fabric (4) is made of 280 denier polyurethane yarn and 70 denier, 24 filament nylon yarn, and is made into a power net structure using a 48 gauge warp knitting machine so that the mixing ratio is 15:85. It is organized. The sewing thread can be exemplified by a 4-0 size polyglycolic acid suture thread.

The present invention has the characteristic of preventing air leakage from the resection site, tearing of soft tissue, etc., especially in the operation using a staple type automatic suturing device, and in combination with an elastic material, that is, its combination. Elongation makes it easy to attach and fix to various sizes of automatic suturing devices. Also, once attached, it will not be displaced due to the tightening force, and cutting operation after fixing to staples. Even in the case, it has a feature that it can be easily cut because it is under tension. In addition, it is economical because it requires less expensive biodegradable and absorbable material. When a sewing thread having elasticity is applied, it can be combined with a low elasticity cloth.
