Description
本出願は2018年9月13日に出願された米国特許出願第62/730,713号の利益を主張し、その開示全体は、参照により本明細書に組み込まれる。
本発明は、少なくとも1つの捕捉剤を使用して、高い薬物対脂質比および高いカプセル化効率を有する鎮静薬を含む徐放性医薬組成物に関する。特許請求される医薬組成物の高い薬物対脂質比、高いカプセル化効率および徐放性プロファイルは、薬物投与の頻度を減少させ、患者のコンプライアンスを増加させ、治療結果を改善する。
上記および開示全体を通して使用されるように、以下の用語は本明細書中で別段の指示がない限り、単数形「a」、「an」および「the」は文脈が別段の指示がない限り、複数の参照を含む。
本明細書で使用される用語「リポソーム」、「リポソームの」および関連する用語は、小胞を形成する1つ以上の二重層膜によって外部媒体から隔離された内部水性空間を特徴とする。特定の実施形態では、リポソームの内部水性空間が、トリグリセリド、非水相(油相)、水-油エマルジョン、第2のリポソーム、または非水相を含有する他の混合物などの中性脂質を実質的に含まない。リポソームの非限定的な例には、50~20μm、50~450nm、50~400nm、50~350nm、50~300nm、50~250nm、50~200nm、100~500nm、100~450nm、100~400nm、100~350nm、100~300nm、100~250nmまたは100~200nmの範囲の平均直径を有する、小さな単ラメラ小胞(SUV)、大きな単ラメラ小胞(LUV)、および多ラメラ小胞(MLV)が含まれる。
リポソームの二重層膜は、さらにステロイド、好ましくはコレステロールの約55モルパーセント未満を含む。特定の実施形態では、二重層膜中のコレステロールなどのステロイドのモルパーセントが約15~55%、約20~55%、約25~55%、約15~50%、約20~50%、約25~50%、約15~45%、約20~45%、約25~45%、約15~40%、約20~40%、または約25~40%である。
本明細書で使用される「リモートローディング」という用語は、多原子イオン勾配によって、リポソームの二重層膜を横切って外部媒体から内部水性空間に薬物を移動させる手順を含む薬物充填方法である。このような勾配は、少なくとも1つの多原子イオンをリポソームの内部水性空間に捕捉剤としてカプセル化し、カラム分離、透析または遠心分離などの公知の技術によって、リポソームの外部媒体をより低い多原子イオン濃度、例えば、純水、スクロース溶液および生理食塩水で外部媒体に置き換えることによって生成される。多原子イオン勾配が、リポソームの内部水性空間と外部媒体との間に生成され、治療薬をリポソームの内部水性空間に捕捉する。捕捉剤としての例示的な多原子イオンとしては硫酸塩、亜硫酸塩、リン酸塩、リン酸水素塩、モリブデン酸塩、炭酸塩および硝酸塩が挙げられるが、これらに限定されない。例示的な捕捉剤としては、硫酸アンモニウム、リン酸アンモニウム、モリブデン酸アンモニウム、スクロースオクタ硫酸アンモニウム、スクロースオクタ硫酸トリエチルアンモニウム、および硫酸デキストランが挙げられるが、これらに限定されない。
本発明は(a)二重層膜を含む少なくとも1つの第1のリポソーム;(b)捕捉剤;および(c)鎮静薬、を含み、二重層膜が少なくとも1つの脂質を含み、脂質に対する薬物のモル比が0.02以上である徐放性医薬組成物に関する。
空のリポソームを、脂質フィルム水和-押出法によって調製した。HSPC、コレステロール、およびDSPE-PEG2000(モルパーセント59.5/39.6/0.9)をクロロホルムに溶解し、ロータリーエバポレーター中で真空下で有機溶媒を除去することによって薄い脂質フィルムを形成した。乾燥脂質フィルムを300mMの硫酸アンモニウム中、60℃で30分間再水和し、水性コアにカプセル化した硫酸アンモニウムでリポソームを形成した。液体窒素と60℃での水との間の6回の凍結融解サイクルの後、リポソームを、続いて、0.2μmの孔径を有するポリカーボネートフィルター膜を通して10回押し出した。カプセル化されていない硫酸アンモニウムを、9.4%スクロース溶液に対する透析によって除去した。
リポソーム製剤を、以下の捕捉剤:(1)75mMのスクロースオクタ硫酸トリエチルアンモニウム、(2)300mMの硫酸アンモニウム、(3)200mMのリン酸アンモニウム、および(4)7.0mMの硫酸デキストランを用いて、実施例1に従って調製した。表1は、薬物充填に対する異なる捕捉剤の効果を示す。
2つのin vitro放出システムを研究に使用し、第1のシステムは無血漿環境であり、第2のシステムはヒト血漿環境であった。第1のin vitro放出システムを設定するために、実施例2(表1の第1列)に従って調製したリポソームのデキスメデトミジン製剤0.5mL、および遊離デキスメデトミジン塩酸塩0.5mLを別々の透析バッグ(Spectra/Pro(登録商標)dialysis membrane、MWCO 50kDa、Spectrum Labs)に入れ、透析バッグの両端を密封した。第2のin vitro放出システムを設定するために、実施例1に従って調製した0.2mLのリポソームのデキスメデトミジン製剤および0.2mLの遊離デキスメデトミジン塩酸塩を、それぞれ0.8mLのヒト血漿(Valley Biomedical,Inc.)を含有する別々の透析バッグに入れ、透析バッグの両端を密封した。各透析バッグを、50mL遠心管中のpH7.4の20mLのPBSに浸漬し、37±1℃の水浴で168時間または72時間インキュベートした。インキュベーション後の指定された時点(第1のシステムの1、2、4、6、24、50、72、100および168時間、ならびに第2のシステムの1、2、4、8、24、48および72時間)で、20mLのPBSからの1.0mLアリコートをサンプリングし、1.0mLの新鮮なPBSを添加して、サンプリングアリコートを補充した。各時点でサンプリングしたアリコートの薬物濃度を、HPLCを用いて分析して、試験した製剤のin vitro放出プロファイルを作成した。
頸静脈カニューレ挿入(JVC)雌Sprague-Dawleyラット(7~8週齢)を用いて、リポソームのデクスメデトミジン製剤のin vivo PK評価を行った。ラットを、水および食物に自由にアクセスできる、12時間の明期/12時間の暗期の周期で操作される保持室に収容した。
リポソームのデクスメデトミジン製剤のin vivo PD評価を、雄モルモット(600±100g)を用いて実施した。モルモットを、水および食物へ自由なアクセスできる、12時間の明期/12時間の暗期の周期で操作される保持室に収容した。
疼痛管理のために遠隔部位に注射されるアジュバント薬剤としてのリポソームのデキスメデトミジン製剤の実現可能性を評価するための別のin vivo PD研究を、実施例5の方法に従って行った。
Claims (22)
- 医薬組成物であって、
(a)二重層膜を含む第1のリポソームであって、前記二重層膜は、第1の脂質と、コレステロールと、必要に応じて第2の脂質と、を含む、第1のリポソーム;
(b)スクロースオクタ硫酸トリエチルアンモニウム、硫酸アンモニウム、硫酸デキストランおよびそれらの組み合わせからなる群から選択される捕捉剤;および
(c)デクスメデトミジン
を含み、前記第1の脂質および必要に応じて前記第2の脂質に対する前記デクスメデトミジンのモル比が約0.19以上であり、
前記第1の脂質がHSPC、DSPCまたはそれらの組み合わせであり、前記第2の脂質がPEG-DSPE、DPPGまたはそれらの組み合わせである、医薬組成物。 - 前記第1のリポソームの平均粒径が約50nm~約20μmである、請求項1に記載の医薬組成物。
- 前記二重層膜中の前記コレステロールのモルパーセントが約15~約55%である、請求項1に記載の医薬組成物。
- 前記第1の脂質がHSPCであり、前記第2の脂質がPEG-DSPE、DPPGまたはそれらの組み合わせである、請求項1に記載の医薬組成物。
- 前記第1の脂質がDSPCであり、前記第2の脂質がPEG-DSPE、DPPGまたはそれらの組み合わせである、請求項1に記載の医薬組成物。
- 前記二重層膜中の前記第1の脂質のモルパーセントが約40~84.9%であり、前記二重層膜中の前記第2の脂質のモルパーセントが約0.1~25%である、請求項1に記載の医薬組成物。
- 前記スクロースオクタ硫酸トリエチルアンモニウムの濃度が約10~約200mMである、請求項1に記載の医薬組成物。
- 前記硫酸アンモニウムの濃度が約100~約600mMである、請求項1に記載の医薬組成物。
- 前記硫酸デキストランの濃度が約0.1~約20mMである、請求項1に記載の医薬組成物。

- 前記医薬組成物が、皮膚注射によって投与される、請求項13に記載の治療薬。
- 前記皮膚注射が、皮下、真皮下、皮内、経皮または筋肉内経路を含む、請求項15に記載の治療薬。
- 対象の疼痛を治療するための治療薬であって、
(a)請求項1~12のいずれか1項に記載の医薬組成物の治療有効量;および
(b)麻酔薬、鎮痛薬またはそれらの組み合わせ
を含む、治療薬。 - 前記麻酔薬、前記鎮痛薬またはそれらの組み合わせが、第2のリポソームにカプセル化されている、請求項17に記載の治療薬。
