前記生体情報値と、前記活動量とに基づいて、前記対象者の異常が継続していると判定する機能と、を実現させることを特徴とする。

[1.1 システム全体]
図1は、本発明の異常判定装置を適用した異常判定システム1の全体概要について説明するための図である。図1に示すように、異常判定システム1は、ベッド10の床部と、マットレス20の間に載置される検出装置3と、検出装置3より出力される値を処理するため処理装置5を備えて構成されている。この検出装置3、処理装置5とで異常判定システム(異常判定装置)を構成している。
つづいて、異常判定システム1の機能構成について、図2を用いて説明する。本実施形態における異常判定システム1は、検出装置3と、処理装置5とを含む構成となっており、各機能部(処理)は、生体信号取得部200以外についてはどちらで実現されても良い。すなわち、これらの装置を組み合わせることにより、異常判定装置として機能する。
・生体情報値算出部300により患者の生体情報値が取得(算出)
・睡眠状態判定部350により患者の睡眠状態が取得
されている。
呼吸数 8〜25回/分
心拍数 50〜110回/分
であり、この範囲から外れた場合は、異常値として判定する。なお、この正常範囲は患者の年齢、体重等によって変化してもよい。また、患者毎に通常の生体情報値を正常範囲と設定してもよい。
つづいて、第2実施形態について説明する。第1実施形態では、患者状態推測部700において、入力された生体情報を、生体情報値及び「患者の動き(活動量)」「呼吸振幅の変動(呼吸イベント指数)」「周期的な患者の動き(周期性体動指数)」の全部、いずれか一つ又はそれらの組合せに基づいて判定することで患者の異常状態、とくに患者の死期を判定することとして説明した。
(2)心拍数120[回/分]以上又は40[回/分]以下が一定時間以上継続
(3)夜間睡眠の開始から終了にかけて心拍数または呼吸数のトレンドが上昇(10%以上)
(4)夜間(21:00〜6:59)の呼吸数または心拍数のばらつき(標準偏差、変動係数)が一定値以上
(5)呼吸障害指数もしくは周期性体動指数が有意に減少
(6)呼吸障害指数もしくは周期性体動指数が有意に増加、もしくは一定値以上(夜間)
(7)活動量が有意に増加もしくは減少
(8)睡眠判定が一定時間以上継続、夜間の覚醒判定が95%以上
これらの特徴点を1又は複数組み合わせることにより、特徴ベクトルが出力される。なお、特徴点として説明したものは1例であり、当該値に限定されるものではない。例えば、(1)を例に取ると、呼吸数25[回/分]以上であってもよいし、10[回/分]以下であってもよい。このように、各値は、説明の都合上の値である。そして、該当する特徴点は「1」、非該当の特徴点は「0」が出力されでも良いし、確率変数が出力されても良い。
つづいて、第3実施形態について説明する。第3実施形態は、第1実施形態の図2の機能構成を、図5に置き換えたものである。
上述した実施形態を利用することにより、患者の状態(死期が迫っていること)を判定する実施例について説明する。図7〜図18は、患者日誌の一例を説明するための図である。ここで、図7〜図9及び図10〜図12は患者が回復した場合の患者日誌であり、図13〜図15及び図16〜図18は患者が死亡した場合の患者日誌の一例である。
まず、第1の例として回復例1について説明する。例えば、図7、図8、図9は、ある介護施設で生活している要介護高齢者の患者日誌である。図7が睡眠日誌、図8が心拍日誌、図9が呼吸日誌である。図7の患者日誌のグラフには、上段に活動量(黒縦棒)、下段に睡眠状態が示されている。また、図8の心拍日誌には、上段に心拍数の変動、下段に睡眠状態が示され、図9の呼吸日誌には、上段に呼吸数の変動、下段に睡眠状態が示されている。
つづいて、第2の例として回復例2について説明する。回復例2は、図10が睡眠日誌、図11が心拍日誌、図12が呼吸日誌である。
つづいて、第3の例として、死亡例1について説明する。死亡例1は、図13が睡眠/活動量日誌、図14が呼吸日誌、図15が心拍日誌である。図13では、睡眠状態と活動量とが重畳して表示されている。すなわち、黒縦棒が活動量を示している。

Claims (5)
- 対象者の生体信号を取得する生体信号取得部と、制御部とを備えた異常判定装置であって、
前記制御部は、
前記生体信号取得部から取得された前記生体信号に基づいて、前記対象者の生体情報値を算出し、
前記生体信号から、前記対象者の活動量を算出し、
前記生体情報値と、前記活動量とに基づいて、前記対象者の異常が継続していると判定し、
前記生体情報値と、前記活動量とに基づいて、前記生体情報値が異常値となる状態が第1判定時間継続した後に、当該生体情報値が正常範囲となる状態が第2判定時間継続した場合に、前記第2判定時間において、前記活動量が低い場合に、前記対象者の死期が迫った状態であると判定する、
ことを特徴とする異常判定装置。 - 前記第2判定時間において、更に前記対象者が離床していない場合に、前記対象者の死期が迫った状態であると判定する、
ことを特徴とする請求項1に記載の異常判定装置。 - 前記生体情報値は、前記取得された前記生体信号から、前記対象者の心拍数及び呼吸数が算出されることを特徴とする請求項1又は2に記載の異常判定装置。
- 前記制御部は、
前記対象者の活動量、呼吸障害指数、周期性体動指数の少なくとも何れか1つ又はこれらの組み合わせに基づいて、前記対象者の異常が継続していると判定することを特徴とする請求項1から3の何れか一項に記載の異常判定装置。 - 対象者の生体信号を取得する生体信号取得部が接続されたコンピュータに、
前記生体信号取得部から取得された前記生体信号に基づいて、前記対象者の生体情報値を算出する機能と、
前記生体信号から、前記対象者の活動量を算出する機能と、
前記生体情報値と、前記活動量とに基づいて、前記対象者の異常が継続していると判定する機能と、
前記生体情報値と、前記活動量とに基づいて、前記生体情報値が異常値となる状態が第1判定時間継続した後に、当該生体情報値が正常範囲となる状態が第2判定時間継続した場合に、前記第2判定時間において、前記活動量が低い場合に、前記対象者の死期が迫った状態であると判定する機能と、
を実現させることを特徴とするプログラム。
