Description
本願は、出典明示して本明細書の一部とみなす、2008年12月15日に出願した米国仮特許出願61/122,520号に基づく優先権を主張する。
本発明は、一般的に組織修復用の補綴デバイスに関し、より詳細には、安定なニット構造を利用する外科的なシルク製メッシュデバイスに関する。
ヘルニアおよび腹壁異常に最初に用いる外科的なメッシュが、鍵板修復、骨盤底機能障害、および再建または美容外科手術のような他のタイプの組織修復に現在用いられている。2010年においては、8,000,000を超えるヘルニア手術、800,000を超える鍵板修復、3,000,000を超える骨盤脱修復(pelvic prolapse repair)、600,000を超える尿失禁修復(urinary incontinence repair)、および1,500,000を超える再建または審美的な形成外科手術が行われることが計画されている。これらの手術の大部分は、C.R.BardによるBard Mesh(ポリプロピレン);Synecture/US SurgicalによるDexon(ポリグリコール酸);W.L.GoreによるGore-Tex(ポリテトラフルオロエチレン);EthiconによるProlene(ポリプロピレン)、Prolene Soft(ポリプロピレン)、Mersilene Mesh(ポリエステル)、Gynemesh(ポリプロピレン)、Vicryl Knitted Mesh(ポリグラクチン910)、TVT(ポリプロピレン);American Medical SystemsによるSparcテープ(ポリプロピレン);およびTYCO Healthcare InternationalによるIVSテープ(ポリプロピレン)を含む現在市販されている移植可能な外科的なメッシュデバイスを用いるようである。
現在の外科的なメッシュデバイスの欠点に鑑み、生体適用性かつ吸収性で、宿主コラーゲンに存する生理学的ストレスに耐える能力を有し、組織侵食、フィステルまたは癒着を最小限化する外科的なメッシュに対する要望が存在し続けている。したがって、本発明の態様による形態は、軟組織および硬組織の修復に使用するための生体適用性の外科的なシルク製メッシュ補綴デバイスを提供する。軟組織修復の例には、ヘルニア修復、鍵板修復、美容外科、膀胱スリングの実施などが含まれる。骨修復のような硬組織修復の例には、再建形成外科手術、オルト外傷(ortho trauma)などが含まれる。

ニットパターンが、第1のセットの糸がノードにおいて第2のセットの糸と大きな摩擦の接触を経験するように第2のセットの糸よりも直径において事実上より大きくなる、第1の糸におけるたるみを含むことを特徴とする、補綴デバイス用のニットメッシュを製作する方法。 - 第1、第2および第3のセットの糸が20ないし1000μmの直径である請求項1記載の方法。
- 第1、第2および第3のセットの糸が実質的に一定の直径を有する請求項1記載の方法。
- 第1のセットの糸をウェール方向で両板機の第1の針床に適合し;
第2のセットの糸をウェール方向で両板機の第2の針床に適合し;ついで、
第3のセットの糸をニットメッシュの所定数のコース毎に適合することを含む、補綴デバイス用のニットメッシュを製作する方法であって、第3のセットの糸の適合はニットメッシュに開口部を規定し、
ここに、第1のセットの糸および第2のセットの糸は交互に適合して、各々、第1の針床および第2の針床にねじれ形のループを形成し、第1のセットの糸は第2のセットの糸とインターレースしてニットメッシュのノードを規定し、第1のセットの糸および第2のセットの糸の交互の適合は第1のセットの糸が第2のセットの糸に対してノードにおいて異なる張力を有することを生じ、張力における差が、ニットメッシュがノードにおいて解けることを実質的に妨げることを特徴とする該方法。 - 第1、第2および第3のセットの糸がシルクから形成される請求項4記載の方法。
- 第1、第2および第3のセットの糸が20ないし1000μmの直径である請求項5記載の方法。
- 第1、第2および第3のセットの糸が実質的に一定の直径を有する請求項4記載の方法。
- 平板機(flat needle bed machine)上で、ニットメッシュの第1のセットのコースの各々に沿って第1の一連のジャージループを形成し;ついで、
平板機上で、ニットメッシュの第2のセットのコースの各々に沿って第2の一連の交互のタックループおよびジャージループを形成することを含み、第2のセットのコースは第1のセットのコースと交互になり;
ここに、第2のセットのコースは第1のセットのコースよりも大きな張力を有し、第2のセットのコースに沿ったタックループは第1のセットのコースのジャージループと結合し、ニットメッシュがタックループにおいて解けることを実質的に妨げることを特徴とする、補綴デバイス用のニットメッシュを製作する方法。 - 連続する糸が第1のセットのコースおよび第2のセットのコースを形成する請求項8記載の方法。
- 第1のセットのコースおよび第2のセットのコースが異なる糸によって形成される請求項8記載の方法。
- 第1のセットのコースおよび第2のセットのコースが異なる直径を有する異なる糸によって形成される請求項10記載の方法。
- ニットメッシュがシルクから形成される請求項1記載の方法。
