Description
本発明は、概して、手術を行なうためのシステム及び方法、より詳しくは、腹腔鏡手術のための方法及びデバイスに関係する。
本願は、いずれのタイトルも「マイクロ腹腔鏡システム及び方法」である2010年9月19日に出願された米国仮出願第61/384,288号及び2011年7月4日に出願された米国仮出願第61/493,423号に基づく35USC 119(e)優先権の利益を主張する。
腹腔鏡又は低侵襲手術には、腹部へ通じるいくつかの比較的小さなポートを利用することが含まれ、これらのポートから様々なタイプの器具類及び付属品が導入され、様々な外科的介入に利用される(内視鏡による映像下において通常実施される)。観血的手術はいくつかの観点から優れていると通常考えられているが、5から15mmのポートをいくつか使用することには依然として局所的な痛み、瘢痕、及びおそらくポートに関係する合併症(例:瘢痕におけるヘルニア)を引き起こし、そして外科医の他に一人又は二人以上の補助者を必要とすることになる。

Claims (15)
- 腹腔鏡システム(1000、2000、8000)であって、
ニードルユニット(1200)のニードル部(1210)の遠位端に着脱可能なエンドエフェクタ(1300)を配置し、管腔と、前記管腔内に配置され、体腔と外部環境との間に封止された連通路を提供する可逆性ポートシールとを有する腔鏡ポート(1100、1100’、2200、3000、8200)と、
前記腹腔鏡ポート(1100、1100’、2200、3000、8200)の前記管腔に装着するように構成された外径を有し、前記ポートシールを無効化している間に前記腹腔鏡ポート(1100、1100’、2200、3000、8200)を通じて導入可能である長尺管状部材を有するガイドカニューレ(1400、1400’、2100、3100/3200、4000、4000’、8100)と、を有し、
前記長尺管状部材は、遠位開口部から近位開口部まで軸方向に延在するカニューレ管腔を少なくとも1つ収容しており、
前記カニューレ管腔の少なくとも1つは、前記遠位開口部及び/又は前記近位開口部から前記ニードル部(1210)及び腹腔鏡デバイスを受け入れるように構成されており、
前記長尺管状部材は、前記ニードル部(1210)の遠位端の任意の場所に伸縮自在に延在可能であり、これによって、前記ニードル部(1210)の遠位端と前記腹腔鏡ポート(1100、1100’、2200、3000、8200)との間に連通路が得られ、
前記ガイドカニューレ(1400、1400’、2100、3100/3200、4000、4000’、8100)は、前記ニードル部(1210)の前記遠位端を捕獲するように構成され、これによって、前記ニードル部(1210)の前記遠位端への前記エンドエフェクタ(1300)の配置のために、前記ニードル部(1210)の前記遠位端が前記ガイドカニューレを通じて前記外部環境に向けてガイドされる、腹腔鏡システム。 - シール部材(2140)が前記カニューレ管腔内に配置される、請求項1に記載の腹腔鏡システム。
- 前記シール部材(2140)は、前記遠位開口部から前記近位開口部への方向で封止する構成である、請求項2に記載の腹腔鏡システム。
- 前記シール部材(2140)は、プラグを備え、
前記プラグは、前記カニューレ管腔の近位侵入口にぴったりと合う構成である、請求項2又は3に記載の腹腔鏡システム。 - 前記長尺管状部材の前記遠位開口部は、前記ニードル部(1210)を前記長尺管状部材内から前記シール部材を通じて外へガイドする、選択的に拡張可能及び/又は収縮可能なエッジ(4050、7000、8124、8322)を有する部分を備える、請求項2から4のいずれかに記載の腹腔鏡システム。
- 前記拡張可能及び/又は収縮可能なエッジは、拡張時には少なくとも部分的に漏斗形状になる、請求項5に記載の腹腔鏡システム。
- 前記拡張可能及び/又は収縮可能なエッジは、作動手段を使用して選択的に変化可能である、請求項5又は6に記載の腹腔鏡システム。
- 前記作動手段は、外側管状部(8112)を備え、前記外側管状部は、それに対応して装着した内側管状部(8122)に対してスライド可能であり、
前記外側管状部は、前記拡張可能及び/又は収縮可能なエッジの部分を選択的に覆ったり覆わなかったりする構成である、請求項7に記載の腹腔鏡システム。 - 前記拡張可能及び/又は収縮可能なエッジは、自己拡大可能である、請求項5から8のいずれかに記載の腹腔鏡システム。
- 前記拡張可能及び/又は収縮可能なエッジは、円筒形状に収縮可能である、請求項5から9のいずれかに記載の腹腔鏡システム。
- 前記拡張可能及び/又は収縮可能なエッジは、
複数の重複花弁様部材(10i)を含む虹彩絞り(8124a)か、
漏斗形状から円筒形状へ対称に折り畳みが可能な複数の折り目(24i)を、備える、
請求項5から10のいずれかに記載の腹腔鏡システム。 - 前記長尺管状部材は、前記ニードル部の前記遠位端を当該ニードルの侵入口において前記体腔にはめ込むように構成された最大突出長を有する、請求項1に記載の腹腔鏡システム。
- 前記ガイドカニューレを通じて提供され、前記ガイドカニューレの中に移動すると共に前記ガイドカニューレから外へ移動して、前記ニードル部(1210)の前記遠位端を捕獲し、前記外部環境へ前記ガイドカニューレを通じて前記体腔の外側に引き出す捕獲部材を更に有する、請求項1に記載の腹腔鏡システム。
- 前記捕獲部材は、少なくとも1つのループを備える、請求項13に記載の腹腔鏡システム。
- 長尺マニピュレータに取り付けでき、そして、直接映像化した方向で、前記体腔において前記遠位端の捕獲及び/又は引っ張りを操作できる着脱可能なカメラヘッドを備える、
請求項13に記載の腹腔鏡システム。
