静脈不全を治療する方法をこれから記載するが、その方法において静脈は治療区域に沿って少なくとも部分的に圧迫される。そのように圧迫することで、望ましい臨床結果によるが、単に閉塞するよりも部分的または完全に静脈を虚脱することをより一層確保できる。理論に束縛されるものではないが、静脈の虚脱は内皮細胞の二つ以上の管腔表面を反対側の接触点に互いに配置して、繊維組織の増殖を刺激し、結果的に再疎通や静脈の再開放のリスクが低い状態で静脈の長期的閉鎖の改善を生むものである。一部の実施例において、展開カテーテルはアクセス部位から静脈に経皮的に導入され、静脈内の治療域に経管腔的かつ遠位に前進される。展開カテーテルの静脈を遠位に虚脱させるために、外部からの圧迫がかけられる。次に、閉塞の近位側にある閉塞された静脈の開始部分にまでカテーテルの遠位端を前進させ、カテーテルと閉塞された静脈の間に「溜まった」血液を最小限に抑える。栓形成媒体がカテーテルの遠位端から伝達された後、カテーテル端部での閉塞によって静脈閉塞物質は、カテーテル挿入地点に向かって静脈内に逆行性に(近位に)流れ、遠位に流れる力と血管内での血液との混合を減少するように強制される。この方法によって、静脈閉塞媒体がカテーテルと閉塞された静脈の間に「溜まった」血液に取り代わり、血液との混合を最小限に抑えつつ静脈内で閉塞栓を形成することも可能となる。静脈閉塞媒体と静脈の間の接着強度を高める可能性があるため、この混合の減少は特定の実施例において有利でありうる。治療区域への遠位の外部からの圧迫は、随意に削除するか全てまたは一部の手技全体にわたって残すことができる。外部からの圧迫はまた、上述のように静脈を虚脱させるために、栓形成媒体が伝達される静脈領域周辺に発生する可能性がある。カテーテルはその後、継続的に、または治療全域への最初のボーラス注入から一定または不規則な間隔を空けた個別のボーラス注入のいずれかによって静脈閉塞物質を投与しつつ、近位に格納される。外部からの圧迫は、上述のように静脈を確実に虚脱させるために、静脈閉塞物質が投与される箇所に近位に継続することができる。カテーテルはその後抜去され、アクセス部位は従来の技法を用いて閉じられる。方法を下記で詳細に説明する。
追加的な実施例において、無線周波装置またはレーザーボックスの資本財の購入を必要としない静脈閉鎖システムについて説明する。簡易かつ非侵襲的な静脈閉鎖システムの使用方法が提供されており、一部の実施例において、方法は膨潤麻酔の適用または圧迫ストッキングの着用を必要としない。本明細書に記載する静脈閉鎖システムに対する患者の受入れおよび要求は、既存の装置および技法よりも相当に高いであろう。
一部の実施例において、例えば塞栓接着剤などの静脈閉塞物質を送達する静脈閉鎖装置は、三つの構成部品を含む。第一の構成部品は、大伏在静脈またはより小さな伏在静脈のできる限り低い位置から正確な超音波誘導により伏在静脈に配置することを実現する外側カテーテルまたはイントロデューサ鞘である。静脈閉鎖装置はまた、閉塞対象の静脈内へと正確に遠位端で配置されるように構成される。一部の実施例において、鞘は複数のサイズ幅で利用でき、配置部位によるが3fr〜7frのID、25cm〜100cmの長さを含む。一部の実施例において、鞘は超音波観察によって音波を発生し、それゆえ、伏在静脈大腿静脈接合部の下に正確に配置されうる。鞘には、0.2、1、2、または5cmでの増分など、鞘に沿って増分を示す複数の目盛りの他に計測印が設けられていてもよい。目盛りおよび印は、伏在静脈に沿って正確で監視の行き届いたプルバックの動きの提供を支援する。
静脈閉鎖システムの第三の部分は、内側カテーテルに取り付けられるグルーガンまたは別の接着剤を導入する装置である。一部の実施例において、接着剤導入装置は、制御性と精度をもって接着剤を血管に投与できる手動の液体ディスペンサーガンである。一つのかかるディスペンサーガンは、Gruendeman et al.に付与された米国特許第6,260,737号で開示されており、その全体を参照により本明細書に組み込む。グルーガンの別の実施例について、下記で更に詳細を説明する。
展開可能な閉塞装置を構成する静脈閉塞システムの構成部品に関連する実施例について、これから説明する。図36は、本発明の一つの実施例に従って、血管閉塞システムで使用されうる構成部品を示す。システムは、例えば、静脈内の望ましい位置に挿入するための展開可能な閉塞装置100、閉塞装置100の送達の他に静脈を閉じるための一つ以上の物質送達の経路としての役目を果たす管状部材でありうるカテーテル200、およびカテーテル200に結合されカテーテルから物質を静脈に起動させる注入器300を含む。

Claims (21)
- 静脈を治療するシステムであって、
静脈内に静脈閉塞物質を送達するための注入器であって、該注入器は近位端と、前記静脈閉塞物質を含んだ注射器と結合するように形成された遠位端と、を備えた取り外し可能なチャンバを具備し、該取り外し可能なチャンバは該チャンバの近位端および遠位端において開口部を備えた、収容された中央内腔を具備し、前記注入器は可動壁と、該可動壁を所定の距離だけコントロールの起動によって前記チャンバの内腔内において移動させるように形成されたアクチュエータと、をさらに具備し、前記注入器がコントロールに操作可能に接続可能とされ、前記コントロールの起動によって前記注入器が約0.05mL〜3mLの静脈閉塞物質を前記静脈内にボーラス注入する結果を生み、前記アクチュエータが前記可動壁を所定の距離だけ移動させ、これによって前記注射器が前記チャンバの遠位端に結合されたときに前記静脈閉塞物質のボーラスを吐出した場合に、前記注入器が略均等に間隔を空けて自動的に前記静脈閉塞物質の複数のボーラス注入を行うように構成された、注入器と、
近位端と、遠位開口部と、側壁と、を備えたカテーテルであって、前記カテーテルの近位端は、前記注射器が前記チャンバの遠位端と結合された場合に、前記注射器を介して前記注入器に接続されるように操作可能に構成され、前記静脈内の治療全域に遠位に前進するように構成されたカテーテルと、
を含むシステム。 - 前記注入器がグルーガンを含む、請求項1に記載のシステム。
- 前記カテーテルが、前記注入器に操作可能に接続するためのルアーロックを含む、請求項1に記載のシステム。
- 静脈閉塞物質の容量を更に含む、請求項1に記載のシステム。
- 前記静脈閉塞物質の容量が約1mL〜約20mLである、請求項4に記載のシステム。
- 前記静脈閉塞物質がシアノアクリレートを含む、請求項1に記載のシステム。
- 外部から前記静脈を圧迫するように構成された圧迫要素を更に含む、請求項1に記載のシステム。
- 前記コントロールが前記カテーテルの側壁と静脈壁の間で前記カテーテル周囲で近位に前進させるのに十分な容量の媒体を前記遠位開口部を通して導入するよう前記注入器を起動するように構成された、請求項1に記載のシステム。
- 閉塞部材がフレーム部分とバリア部分を更に含む、請求項1に記載のシステム。
- 前記カテーテルは、末端の浅大腿静脈から浅静脈接合部まで延びる十分な長さを有している、請求項1に記載のシステム。
- 少なくとも部分的にそれを通じて前記カテーテルを収容するように形成された鞘をさらに具備し、該鞘は25cm〜100cmの長さを有し、該鞘の内径は3fr〜7frである、請求項1に記載のシステム。
- 前記チャンバはアダプタのロック端部、注射器のロック端部、および前記注射器を受容するための注射器のロック端部の保持スロットを具備している、請求項1に記載のシステム。
- 前記アダプタのロック端部は、回転に伴い前記注入器の保持セグメントで受け取り可能な一つ以上のカラーまたはフランジを具備している、請求項12に記載のシステム。
- 前記アダプタのロック端部は開口部またはスロットを具備し、前記可動壁を具備した前記注入器のプランジャの遠位端は前記開口部またはスロットを通じて挿入されることが可能である、請求項13に記載のシステム。
- 前記保持スロットは、前記注射器の前記保持スロットへの回転に伴って前記注射器が前記保持スロット内にしっかりと受け取られるように形成されている、請求項12に記載のシステム。
- 前記注射器のロック端部内の保持スロットは樽に翼が付いたような形状である、請求項15に記載のシステム。
- 前記チャンバの内腔の長さは50.8mm〜127mm(2インチ〜5インチ)である、請求項12に記載のシステム。
- 前記注射器のロック端部はT字型の開口部を具備している、請求項12に記載のシステム。
- 前記チャンバは、該チャンバの外径と前記内腔との間に配置された少なくとも1つの収容空間をさらに具備している、請求項1に記載のシステム。
- 前記チャンバは該チャンバの遠位端にスロットを具備し、該スロットは前記注射器の注射器フランジと結合するように形成されており、前記注射器フランジは互いに径方向に反対側に延びた第1および第2翼を具備し、前記スロットは前記注射器フランジを収容するようなサイズとされた遠位開口部、および内部チャネルであって、該チャネル内に前記注射器の翼の回転が可能なように形成された内部チャネルを備え、前記チャネルは前記注射器の翼の回転を制限するための停止面を備えている、請求項1に記載のシステム。
- 前記カテーテルは音波を発生する先端を具備している、請求項1に記載のシステム。
