Description
米国特許出願公開第2007/0191930号明細書は、グラフトアダプター上側部品部材とグラフトアダプター下側部品部材とを規定するグラフトアダプター本体を備える、腹部大動脈瘤内での使用のための血管内グラフトアダプターを開示する。グラフトアダプター上側部品部材とグラフトアダプター下側部品部材とは、それぞれ、その側壁を貫いて形成された一対の切開部を有する。切開部の両方の対はそれぞれ、右腎動脈と左腎動脈とに位置調整される。グラフトアダプター上側部品部材は、その近位端から遠位端へ延びる上側軸方向貫通通路を有する。グラフトアダプター下側部品部材は、その近位端から遠位端へ延びる下側軸方向貫通通路を有する。上側軸方向貫通通路と下側軸方向貫通通路とは、それぞれ互いに位置調整される。
国際公開第98/26731号は、好ましい構造において、支持がステントであり、PTFEなどのフッ素重合体製の管がステント体の内部を貫通し管部自身の上をステントを覆って引き返して折り返しを形成する、多段補綴物を開示する。この組立部品はその後加熱され、外側の層が内側の層に接触して合体し、連続した継ぎ目の無い端部を有する折り畳まれた袋体の内部でステント体を緊密に取り囲む。
たとえば血管内動脈瘤を含む血管の欠損を修復するための補綴物は周知である。そのような病状を治療するための一般的な補綴物は、ステントグラフトである。たとえば、上腸間膜動脈および腸骨動脈のような複数の血管に補綴物がまたがる必要があるとき、そのような補綴物をモジュール形式で設けることが、また知られている。同様に、治療される動脈またはその他の血管の寸法が補綴物の一端から他端へ実質的に変わる場合に、モジュール式の補綴物が設けられることがある。さらに他の例は、治療される患部の長手に沿って異なる種類の補綴物を使用することが病状により必要とされる場合である。
本発明は、補綴物の結合装置と、改良された補綴物組立部品とを提供しようとするものである。
図1を参照して、腎動脈14,16の真下の上腸間膜動脈12の内部に設置され、総腸骨動脈18,20内へ分枝する、二つの分枝を有するステントグラフト10が示される。ステントグラフト10は、大腿動脈22への二回目の分枝をする。
Claims (13)
- 補綴物用の結合装置(28)であって、一方が他方の内側に同心に配置された内側結合要素(60)および外側結合要素(62)と、少なくとも前記外側結合要素(62)を覆う膜と、を含み、前記外側結合要素(62)は、自己拡張型であり、前記内側結合要素(60)と前記外側結合要素(62)とは、使用時に前記外側結合要素が拡張して前記結合装置が血管の壁へ当接することを可能にする一方、前記内側結合要素は内方へ伸長して前記結合装置が補綴物モジュールの一つ以上と結合することを可能にするように、異なる径に拡張して、拡張されたときに互いに間隔を空けられることができる、補綴物用の結合装置(28)。
- 前記外側結合要素は血管の壁に連結可能であり、前記内側結合要素は補綴物に連結可能である、請求項1に記載の装置。
- 前記内側結合要素(60)は自己拡張型である、請求項1または2に記載の装置。
- 前記内側結合要素(60)および/または前記外側結合要素(62)は、ステントである、請求項1から請求項3のいずれかに記載の装置。
- 前記膜は、グラフト材料製である、請求項1から請求項4のいずれかに記載の装置。
- 前記内側結合要素(60)と前記外側結合要素(62)との両方を覆う膜を含む、請求項1から請求項5のいずれかに記載の装置。
- 前記膜は、いずれかの端部において結合された二重同心管の形状を有し、前記内側結合要素(60)と前記外側結合要素(62)とは、それぞれ前記二重管の両方の端部の間に設置される、請求項6に記載の装置。
- 前記膜は、第一の管状部(54)と第二の管状部(52)とから形成され前記第一の管状部(54)が前記第二の管状部(52)よりも大きな径を有する、単一の構造であり、前記第一の管状部と前記第二の管状部との間に連結片(58)を含み、前記第二の管状部は、前記二重同心管を設けるために、めくり返すことが可能である、または前記第一の管状部の内部にめくり返されている、請求項6または請求項7に記載の装置。
- 少なくとも前記外側結合要素(62)は前記膜の外側表面上に設置される、請求項1から請求項8のいずれかに記載の装置。
