Description
従って、依然として、糖尿病性神経因性疼痛を治療・予防するための新規治療法および治療薬の開発が求められている。
一方、マキサディランと神経因性疼痛との関連性に関しては、これまでに明らかにされていなかった。
[1] 配列番号1で示されるアミノ酸配列において、23位〜46位のアミノ酸のうちの1〜24個のアミノ酸が欠失しているアミノ酸配列からなるポリペプチドを有効成分とする神経因性疼痛を治療および/または予防するための医薬組成物。
[2] ポリペプチドが、
(i)配列番号2で示されるアミノ酸配列からなるポリペプチド、または
(ii)配列番号2で示されるアミノ酸配列において、1もしくは数個のアミノ酸が欠失、置換もしくは付加されたアミノ酸配列を有し、かつ、神経因性疼痛を治療および/または予防する活性を有するポリペプチド、
である、[1]の医薬組成物。
[3] 以下のいずれかのポリペプチドをコードする核酸または該核酸を含む発現ベクターを有効成分とする神経因性疼痛を治療および/または予防するための医薬組成物:
(a)配列番号1で示されるアミノ酸配列において、23位〜46位のアミノ酸のうちの1〜24個のアミノ酸が欠失しているアミノ酸配列からなるポリペプチド;
(b)配列番号2で示されるアミノ酸配列からなるポリペプチド、または
(c)配列番号2で示されるアミノ酸配列において、1もしくは数個のアミノ酸が欠失、置換もしくは付加されたアミノ酸配列を有し、かつ、神経因性疼痛を治療および/または予防する活性を有するポリペプチド。
[4] 神経因性疼痛が、脳下垂体アデニル酸シクラーゼ活性化ポリペプチド受容体I型(PAC1受容体)媒介性の神経因性疼痛である、[1]〜[3]のいずれかの医薬組成物。
[5] 神経因性疼痛が、糖尿病性神経因性疼痛である、[4]の医薬組成物。
本発明は、マキサディランの部分ペプチドを有効成分とする、神経因性疼痛を治療および/または予防するための医薬組成物に関する。
(1)配列番号1で示されるアミノ酸配列において、23位〜46位のアミノ酸のうちの1〜24個のアミノ酸が欠失しているアミノ酸配列または配列番号2で示されるアミノ酸配列をコードする核酸;
(2)上記(1)の核酸において、1〜数個のヌクレオチドの欠失、置換、付加または挿入を有し、かつPAC1受容体に結合して神経因性疼痛を治療および/または予防する活性を有するペプチドをコードする核酸;
(3)上記(1)の核酸とストリンジェントな条件下でハイブリダイズし、かつ神経因性疼痛を治療および/または予防する活性を有するペプチドをコードする核酸;あるいは
(4)上記(1)の核酸とBLAST等(例えば、デフォルトすなわち初期設定のパラメータ)を用いて計算したときに、80%以上、より好ましくは90%以上、最も好ましくは95%以上の相同性を有する塩基配列からなり、かつPAC1受容体に結合して神経因性疼痛を治療および/または予防する活性を有するペプチドをコードする核酸。
ここで、(2)において「1〜数個」の範囲は特には限定されないが、例えば、1から20個、好ましくは1から10個、より好ましくは1から7個、さらに好ましくは1から5個、特に好ましくは1から3個、あるいは1個または2個である。また、(3)において、ストリンジェントな条件とは、いわゆる特異的なハイブリッドが形成され、非特異的なハイブリッドが形成されない条件をいい、例えば、ナトリウム濃度が、10mM〜300mM、好ましくは20〜100mMであり、温度が25℃〜70℃、好ましくは42℃〜55℃での条件をいう。
哺乳動物としては、非限定的にサル、イヌ、ウシ、ウマ、ヒツジ、ブタ、ウサギ、マウス、ラット、モルモット、ハムスター等が挙げられるが、好ましくはヒト、特に糖尿病性神経因性疼痛に罹患しているヒトである。
(実施例)
max.d.4をコードするプラスミドM65-pGEX-2Tを用いて大腸菌を形質転換し、形質転換体を得た。得られた形質転換体をLB培地(2ml)にて一晩に培養し、さらに300mlのLB培地を加えてOD=0.6〜0.8になるまで2〜3時間培養した。その後、IPTG液を3ml加えて、さらに4時間培養した。培養物を遠心分離(4℃、10000rpm、10分)し、上清を除去して沈渣を回収した。沈渣よりGST-タグ付きタンパク質を、B-Per GST Fusion Protein Purification kit(Thermo scientific 社)を用いて溶出した。溶出物に1M CaCl2溶液(最終濃度2.5mM )とトロンビン(1Uで100μg処理)を加えて、37℃にて一晩消化した。トロンビン消化物をSep-Pakカラム(Waters社)を用いて精製した後、逆相HPLC[カラム:ミリポア社製 RESORCE RPC;分離条件:AからBへの80分リニアグラディエント;A]水:アセトニトリル:10%TFA=90:10:1;B]水:アセトニトリル:10%TFA=40:60:1]に供しmax.d.4を得た。
髄腔内投与用には、max.d.4をACSF(人工脳脊髄液)に10pmol/l となるように溶解して調製した。ACSFの組成を以下に示す。

塩化カリウム 0.112g
塩化カルシウム水和物 0.092g
塩化マグネシウム水和物 0.101g
D-グルコース 0.09g
蒸留水 Fill up to 500ml
合計 500ml
皮下投与用には、生理食塩水中25%デキストラン液100μlに5nmolのmax.d.4を添加し混合して調製した。
750mg/kgのStreptozotocin(SIGMA)(以下STZ)を8〜9週齢のddYマウス(日本クレア社から購入)に腹腔内投与し、2週間後にマウスの尾から血液を採取し、小型血糖測定機グルテストEII、自己検査用グルコースキットグルテストセンサーを使用して血糖値を測定した。この時、対照マウス(STZ非投与マウス)の血糖値は平均103mg/dlであるのに対し、糖尿病モデルマウス(STZ投与マウス)の血糖値は平均507mg/dlであり、有意に増加していた(図2)。また、糖尿病モデルマウスでは対照マウスと比較すると体重の減少が確認された(図3)。
STZを投与して2週間後の血糖値が300mg/dl以上のマウスを糖尿病モデルマウスと判断した。
糖尿病モデルマウスにおける痛覚過敏の検定を行った。また、糖尿病モデルマウスの髄腔内に、max.d.4(50pmol)を投与し1日後に機械的刺激に対する痛みの閾値の変化を観察した。機械的刺激に対する評価はテイルピンチ法を使用し、以下の式を用いてMPE(Maximal possible effect)値によって評価した。
糖尿病モデルマウスの温熱に対する感受性の低下の有無を明らかにする為、STZを投与して4週間後に髄腔内へ、max.d.4(50pmol)を投与し1日後に熱刺激に対する侵害反応を観察した。熱刺激に対する評価は、ホットプレート法により行った。
max.d.4が炎症性の痛みに対して効果があるかどうかを検討するためにホルマリンテストを行った。
(1)アロディニアの評価
神経因性疼痛であるアロディニアの症状の有無を確認するため、糖尿病モデルマウスにおける非侵害刺激の検討(von Frey試験)を行った。STZを投与する1週前にvon Frey試験を全てのマウスに実施し、非侵害刺激に対する痛みの閾値が同程度になるように4つのグループに分け、糖尿病モデルマウスの髄腔内にmax.d.4を投与し、非侵害刺激に対する痛みの閾値を以下のように計算し、変化を観察した。

髄腔内へmax.d.4を投与した時の痛みの経時的変化を検討した。max.d.4(50pmol)を髄腔内へ投与すると、3時間後よりも24時間後においてアロディニアの症状がより改善することが確認された(図12)。
髄腔内へmax.d.4を投与した時の痛みの用量依存的効果を、5pmol、16.7pmol、50pmolの3段階の投与量において検討した。その結果、用量依存的に、アロディニア症状が改善することが確認された(図13)。
上記実施例6より、max.d.4の髄腔内への投与によって、糖尿病モデルマウスにおけるアロディニアの症状の改善が確認された。そこで、髄腔よりもより簡便な投与法である皮下投与でもアロディニアの症状の改善が確認されるかを検討した。max.d.4はペプチドであるため、max.d.4単体での皮下投与では、すぐに分解されてしまうことが考えられるため、デキストランとmax.d.4を混合して皮下投与した。また、髄腔とは異なり皮下投与は直接神経へ投与しているわけではないため、髄腔の投与量の100倍に相当する5nmolのmax.d.4を投与した。その結果、max.d.4の皮下投与後3時間でアロディニアの症状の改善が有意に確認されたが、24〜48時間に改善効果は消失した(図14)。
Claims (2)
- 以下の(i)〜(iii)のいずれかのポリペプチドを有効成分とする糖尿病性神経因性疼痛を治療および/または予防するための医薬組成物:
(i)配列番号2で示されるアミノ酸配列からなるポリペプチド、
(ii)配列番号2で示されるアミノ酸配列において、1もしくは数個のアミノ酸が欠失、置換もしくは付加されたアミノ酸配列を有し、かつ、糖尿病性神経因性疼痛を治療および/または予防する活性を有するポリペプチド、または
(iii)配列番号2で示されるアミノ酸配列と90%以上の同一性を有するアミノ酸配列を有し、かつ、糖尿病性神経因性疼痛を治療および/または予防する活性を有するポリペプチド。 - 以下のいずれかのポリペプチドをコードする核酸または該核酸を含む発現ベクターを有効成分とする糖尿病性神経因性疼痛を治療および/または予防するための医薬組成物:
(i)配列番号2で示されるアミノ酸配列からなるポリペプチド、
(ii)配列番号2で示されるアミノ酸配列において、1もしくは数個のアミノ酸が欠失、置換もしくは付加されたアミノ酸配列を有し、かつ、糖尿病性神経因性疼痛を治療および/または予防する活性を有するポリペプチド、または
(iii)配列番号2で示されるアミノ酸配列と90%以上の同一性を有するアミノ酸配列を有し、かつ、糖尿病性神経因性疼痛を治療および/または予防する活性を有するポリペプチド。
