Description
特許文献1により、通常の剛性の縦通材の代わりに個々のらせんバネ要素と相互に結合されているいくつかのチューリップ状の椎弓根スクリューから成る脊椎固定装置が周知である。らせんバネの長さは調節可能であるが、それによって、らせんバネ要素のプリテンションの変更のみが達成される。
特許文献2により、通常の剛性の縦通材の代わりに個々の制動要素と相互に結合されているいくつかのチューリップ状の椎弓根スクリューから成る別の脊椎固定装置が周知である。この制動要素における不利点は、その製造が生体適合性材料によるため、制動要素が線形のバネ特性を有することである。
いくつかの椎弓根スクリューと、脊椎の長手方向軸に対して平行にそれぞれ2つの椎弓根スクリュー間に配置されている弾性の結合部とを有する別の脊椎固定装置が特許文献3により周知である。この周知の装置は、漸進的なバネ特性を有する弾性結合を含んで成る。これらの弾性手段は、その中央穴が粘弾性の材料で充填されているネジバネを含んで成る。この周知の装置における不利点は、異なるバネ率を有する要素を有する弾性手段の構造によって、弾性手段の高価な製造工程が必要となることである。
本発明は、請求項1の特徴を有する椎体の弾性の安定化のための装置で上記の課題を解決する。
本発明によって達成される利点は、本発明による装置により、
−小さな圧力で十分に大きな弾性の自由度および制動が保証されている。それによって、この範囲での脊椎の十分な運動の自由度が可能とされ、
−高い圧力負荷またはピーク負荷に際して圧力を受入れる大きなバネ範囲が必要ではないため、後部要素の過負荷を阻止することができ、
−生体適合性金属材料、例えばチタンによって製造された唯一のバネ要素によって、無段階に漸進的なバネ特性が達成可能である
点において実質的に確認される。
漸進的なバネ率を有するネジバネの別の構造は、例えば、
−長手方向軸に対して平行に測定されると、2つの隣接したバネ巻き線間に少なくとも1つの異なるスリット幅xを有するネジバネの製造によって可能であり、または
−長手方向軸に対して直交の断面が長手方向軸に対して平行に測定されると、少なくとも2つの隣接したバネ巻き線間に異なる高さhを有するバネ材料のネジバネの製造によって可能である。
他の実施形態においては、弾性手段は、長手方向軸に沿って連続して配置されているバネ巻き線を有する一様の蛇行形状のバネとして構成されており、ここで各バネ巻き線は曲げ軸を有するループを有する。
実施形態に応じて、曲げ軸と長手方向軸との間の間隔Lは長手方向軸の両側で一定であり、またはバネ巻き線は、長手方向軸の左側に配置されている少なくとも1つの曲げ軸と長手方向軸との間の距離L1が、長手方向軸の右側に配置されている少なくとも1つの曲げ軸と前記長手方向軸との間の距離L2に対して異なるように実施されうる。
カムの構造によって、少なくとも1つのカムによって制限された許容バネ範囲が少なくとも2つのバネ巻き線で異なりうる。
蛇行状のバネのバネ特性がn個のバネ巻き線を有し、ここで各バネ巻き線はバネ率iを有し、すなわち、例えば、i1<ij<inである。
別の実施形態においては、バネ材料の肉厚が少なくとも2つのループ間で異なるため、これら2つのバネ巻き線に対するバネ率は異なる。
さらに別の実施形態においては、弾性手段はその軸方向の外側端において、弾性手段の骨固定手段における固定に適している結合部を含んで成る。例えば、2つの結合部の1つは、長手方向軸に対して同軸のバーで構成されうるが、第2の結合部は、棒状の縦通材を受入れる長手方向軸に対して同軸の中央穴を有するケースとして構成されうる。他方では、ヒンジ結合によって、例えば棒状の縦通材と結合可能である少なくとも1つの結合でもありうる。
本発明および本発明の発展形態を以下、複数の実施例の部分的概略図を用いてさらに詳細に述べる。
図1は、複数の椎体を安定化する本発明による装置の実施形態を示す図である。
図2は、本発明による装置の実施形態の弾性手段を示す縦断面図である。
図3は、本発明による装置の他の実施形態の弾性手段を示す図である。
図4は、本発明による装置の図2に示されている実施形態の弾性手段の応力ひずみ線図におけるバネ特性を示す図である。
図4bに示されているバネ特性20は、蛇行状のバネ29としての弾性手段5の実施形態によって実現される。図4bによるバネ特性20は非連続性であり、バネ範囲s=s1において頂点を有し、これは例えば、バネ巻き線19の停止手段22の閉鎖によって実現されている。

Claims (17)
- 椎体を弾性に安定化するための装置(1)であって、
A)それぞれ1つの中心軸(3)および1つの頭部(4)を有する少なくとも2つの骨固定手段(2)と、
B)長手方向軸(12)を有し、かつ前記長手方向軸(12)が前記中心軸(3)に対して横方向に伸びるように、2つの隣接した骨固定手段(2)の頭部(4)と結合可能である弾性手段(5)と
を有し、ここで
C)前記弾性手段(5)が圧力負荷に際して漸進的な傾きを伴ったバネ特性(20)を有する装置(1)において、
D)前記弾性手段(5)が金属材料で製造され、かつバネ要素を1つのみ含んで成り、
E)前記バネ要素がバネ巻き線の一巻き部分(19)を含んで成り、そのうち前記バネ巻き線の少なくとも2つの一巻き部分(19)が少なくとも1つの幾何学的寸法を有し、前記バネ巻き線の少なくとも2つの一巻き部分(19)の幾何学的寸法がそれぞれ互いに異なり、
F)前記弾性手段(5)がその端(3、4)において、前記弾性手段(5)の骨固定手段(2)における固定に適している結合部(16)を含んで成り、そして
G)第1の結合部(16a)が、前記長手方向軸(12)に対して同軸のバー(8)として構成され、かつ第2の結合部(16b)が、前記長手方向軸(12)に対して同軸の中央穴(18)を有するケース(17)として構成されることを特徴とする装置(1)。 - 前記弾性手段(5)の前記バネ特性(20)が、応力ひずみ線図において無段階の漸進的過程を有することを特徴とする請求項1に記載の装置(1)。
- 前記弾性手段(5)が前記バネ巻き線(19)の可変ピッチδを有するネジバネ(11)として構成されていることを特徴とする請求項1または2に記載の装置(1)。
- 前記ネジバネ(11)がバネ線で製造されており、該バネ線の前記断面領域(13)は前記長手方向軸(12)に対して平行に測定されるとバネ巻き線の隣接した各2つの一巻き部分(19)において相異なる高さhを有することを特徴とする請求項3に記載の装置(1)。
- 前記弾性手段(5)が、前記長手方向軸(12)に沿って連続して配置されているバネ巻き線の一巻き部分(19)を有する一様の蛇行形状のバネ(29)として構成されていることを特徴とする請求項1に記載の装置(1)。
- バネ巻き線の各一巻き部分(19)が曲げ軸(24)を有するループ(21)を有することを特徴とする請求項5に記載の装置(1)。
- 前記曲げ軸(24)と前記長手方向軸(12)との間の間隔Lが前記長手方向軸(12)の両側で一定であることを特徴とする請求項6に記載の装置(1)。
- 前記長手方向軸(12)の左側に配置されている少なくとも1つの曲げ軸(24a)と前記長手方向軸(12)との間の距離L1が、前記長手方向軸(12)の右側に配置されている少なくとも1つの曲げ軸(24b)と前記長手方向軸(12)との間の距離L2に対して異なることを特徴とする請求項6に記載の装置(7)。
- バネ巻き線の各一巻き部分(19)が、前記バネ巻き線の各一巻き部分(19)の許容バネ可動範囲を制限する停止手段(22)をそれぞれ含んで成る請求項5〜8のいずれか1つに記載の装置(1)。
- 前記停止手段(22)が、前記対応するバネ巻き線の各一巻き部分(19)の許容バネ可動範囲を制限する少なくとも1つのカム(23)をバネ巻き線の各一巻き部分(19)ごとに含んで成ることを特徴とする請求項9に記載の装置(1)。
- 前記少なくとも1つのカム(23)によって制限された許容バネ可動範囲がバネ巻き線の少なくとも2つの一巻き部分(19)で異なることを特徴とする請求項10に記載の装置(1)。
- 前記蛇行状のバネ(29)がバネ巻き線のn個の一巻き部分(19)を含んで成り、かつ前記バネ巻き線の各一巻き部分(19)はそれぞれ異なるバネ率iを有し、ここでi1<ij<inであることを特徴とする請求項5〜11のいずれか1つに記載の装置(1)。
- 前記バネ材料の肉厚が少なくとも2つのループ(21)間で異なることを特徴とする請求項6〜12のいずれか1つに記載の装置(1)。

- 少なくとも1つの結合部(16b)が、前記長手方向軸(12)に対して同軸の中央穴(18)を有するケース(17)であることを特徴とする請求項14または15に記載の装置(1)。
- 少なくとも1つの結合部(16)がヒンジ結合を含んで成ることを特徴とする請求項14〜16のいずれか1つに記載の装置(1)。
