Claims (21)
- 心臓弁の修復のためのデバイスであって、
患者の脈管系を通して遠隔的に心臓内の心臓弁に隣接した位置にまで搬送され得るよう構成されたカテーテルアセンブリと;
前記カテーテルアセンブリ内を通って互いに独立に並進移動可能とされた少なくとも2つの操作可能な把持部材であるとともに、前記カテーテルアセンブリから先端・基端方向にかつ横向きに延出可能なものとされ、これにより、2つの弁小葉のそれぞれの再配置部分を把持して近接状態とし得るような、少なくとも2つの操作可能な把持部材と;
前記カテーテルアセンブリを通って並進移動可能とされ、さらに、前記2つの弁小葉の前記近接状態とされた部分どうしを固定し得るものとされた、固定機構と;
を具備し、
前記固定機構が、横向きに延出可能とされ、さらに、前記カテーテルアセンブリの少なくとも1つのオリフィスを通して前記カテーテルアセンブリに対して係合解除可能とされ、
前記各把持部材が、延出の向きとは逆向きにすなわち基端向きに変形した状態で、すなわち反転曲がりを形成した状態で、前記カテーテルアセンブリの先端部から延出され、これにより、弁小葉の心室側表面をクリップし得るようになっていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記カテーテルアセンブリが、湾曲した先端部を有した単一のカテーテルを備え、
これにより、弁に対しての大動脈からの逆行的なアクセスが可能とされていることを特徴とするデバイス。 - 請求項2記載のデバイスにおいて、
前記先端部の少なくとも一部が、形状記憶材料から構成されていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記カテーテルアセンブリが、互いにスライド可能に連結された一対をなす2つのカテーテルから構成され、
各カテーテルには、1つの前記把持部材が配置され、
各カテーテルが、少なくとも1つのオリフィスを備え、
前記固定機構が、前記オリフィスを通して延出可能とされ、さらに、前記カテーテルアセンブリに対して係合解除可能とされていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記固定機構が、ステープルまたはリベットを備え、
前記ステープルまたはリベットが、小葉を変位させることなく、小葉のうちの、前記把持部材によって把持された部分を貫通して挿入可能とされていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記固定機構の少なくとも一部が、形状記憶材料から構成されていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記カテーテルアセンブリが、前記少なくとも1つのオリフィスの近傍に、退避可能部分を備え、
この退避可能部分が、退避状態においては、ノッチを形成し、
このノッチを使用して、前記固定機構が、前記カテーテルアセンブリに対して係合解除可能とされていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記カテーテルアセンブリを通って互いに独立に並進移動可能とされた一対をなす操縦可能な把持部材を具備し、
前記各把持部材が、前記カテーテルアセンブリの互いに反対側に位置した少なくとも2つのオリフィスの一方から横向きに延出可能とされ、
前記各把持部材の延出が、前記各把持部材の変形を調節可能な態様で行われ、これにより、弁小葉の心房側表面を把持し得るようになっていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記各把持部材が、前記カテーテルアセンブリの互いに反対側に位置した少なくとも2つのオリフィスの一方から延出可能とされ、
前記各把持部材の延出が、前記各把持部材を変形させつつ行われ、これにより、弁小葉の心室側表面を把持し得るようになっていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記各把持部材が、前記カテーテルアセンブリの互いに反対側に位置した少なくとも2つのオリフィスの一方から延出可能とされ、
前記各把持部材の延出が、前記各把持部材の変形を調節可能な態様で行われ、これにより、弁小葉の心房側表面を把持し得るようになっていることを特徴とするデバイス。 - 請求項10記載のデバイスにおいて、
前記把持部材の各先端部には、一対をなす操作可能なジョーが配置されていることを特徴とするデバイス。 - 請求項10記載のデバイスにおいて、
前記把持部材の各先端部には、真空吸引アセンブリが配置されていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
前記把持部材の少なくとも一部が、形状記憶材料から形成されていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
さらに、前記カテーテルアセンブリを前記心臓弁に向けて搬送するための導入カテーテルを具備していることを特徴とするデバイス。 - 請求項14記載のデバイスにおいて、
前記導入カテーテルが、前記カテーテルアセンブリを前記心臓弁の近傍位置にまで操作可能とするための案内アームを備えていることを特徴とするデバイス。 - 請求項14記載のデバイスにおいて、
前記導入カテーテルが、左心室内において前記導入カテーテルの位置を一時的に安定化させるための安定化手段を備えていることを特徴とするデバイス。 - 請求項16記載のデバイスにおいて、
前記安定化手段が、1つまたは複数の接触部材を有し、
この接触部材が、前記導入カテーテルから先端・基端方向に延出可能とされ、
前記接触部材が、心臓キャビティの内表面に対して1つまたは複数のポイントにおいて接触可能とされていることを特徴とするデバイス。 - 請求項17記載のデバイスにおいて、
前記1つまたは複数の接触部材が、形状記憶材料から形成されていることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
さらに、前記心臓弁の近傍領域を撮影し得るような向きとされた撮影デバイスを具備していることを特徴とするデバイス。 - 請求項1記載のデバイスにおいて、
さらに、保持トリガー機構を具備し、
この保持トリガー機構が、前記把持部材に対しての弁小葉の位置を検出するための1つまたは複数のセンサを備え、
前記把持部材が、弁小葉が前記把持部材から所定距離以内に位置したことが検出されたことに応答して延出されることを特徴とするデバイス。 - 心臓弁の修復のためのデバイスであって、
患者の脈管系を通して遠隔的に心臓内の心臓弁に隣接した位置にまで搬送され得るよう構成されたカテーテルアセンブリと;
前記カテーテルアセンブリ内を通って互いに独立に並進移動可能とされた少なくとも2つの操作可能な把持部材であるとともに、前記カテーテルアセンブリから先端・基端方向にかつ横向きに延出可能なものとされ、これにより、2つの弁小葉のそれぞれの再配置部分を把持して近接状態とし得るような、少なくとも2つの操作可能な把持部材と;
前記カテーテルアセンブリを通って並進移動可能とされ、さらに、前記2つの弁小葉の前記近接状態とされた部分どうしを固定し得るものとされた、固定機構と;
を具備し、
前記固定機構が、横向きに延出可能とされ、さらに、前記カテーテルアセンブリの少なくとも1つのオリフィスを通して前記カテーテルアセンブリに対して係合解除可能とされ、
前記カテーテルアセンブリが、互いにスライド可能に連結された一対をなす2つのカテーテルから構成され、
各カテーテルには、1つの前記把持部材が配置され、
各カテーテルが、少なくとも1つのオリフィスを備え、
前記固定機構が、前記オリフィスを通して延出可能とされ、さらに、前記カテーテルアセンブリに対して係合解除可能とされていることを特徴とするデバイス。
