Description
a)骨幹端部分60に抗して結節12および/または13を固定するための縫合孔および/または
b)これらの孔を通って結節12および/または13は癒合に耐える(言い換えれば孔は結節12および/または13の効率的安定性を保証するための骨ブリッジの構成に役立つ。
骨幹端部分60の内側部分67と骨幹端部分60の外側部分63の境界線を形成する。
二頭筋溝の最大外側ずれは、近位上腕骨の髄内軸線γと解剖頚に垂直な線πに沿った上腕骨頭の突出部との間の交点近くの垂直スケールにおいて+7.5mmと−7.5mmの間に位置しているのが一般的である。二頭筋溝14経路に沿った二頭筋溝の外側ずれd6の変化から、二頭筋溝は“C”字形状をしている。
二頭筋溝14と平面ω野間の間隔d6は、前外側リブ64と平面Ψの間の間隔d7とほぼ同じであってもよい。寸法d6とd7の各々の最大寸法はほぼ同じ垂直方向高さで得られる。
(a)上腕骨頭の長さと後捻に関して不適切なプロテーゼの配置を低減するかあるいはなくす(後捻に関して、前外側リブを二頭筋溝の遠位部分と位置合せすることは、骨折状態において整向の面で正しいプロテーゼの位置決めを確実に行うのに役立ち、
(b)前頭面、横断面あるいは両方における結節の誤った位置決めを低減するかあるいはなくし、および/または
(c)欠陥のある結節の固定を低減するかあるいはなくす(結節の位置異常と移動を防止することは正常な骨折した肩の差し替えにとって重要である)。
(a)解剖学的パラメータの独立した調節性(例えば四つの解剖学的パラメータが元の場所で調節可能である)
(b)簡単な外科技術による高精度な解剖学的複製(anatomical replication)(カットアンドカバー技法を用いた高精度な解剖学的複製)
(c)革新的な関節腔(例えば半径方向の不整合最適化している間に、実質的にどんなヘッドサイズも実質的にいかなる関節腔サイズと組み合わせること)
(d)強固な意図(例えば術中の自由度(intra−operative flexibility)を外科医に与える)
(e)ずれた前外側リブ(例えば遠位二頭筋溝に位置合せするための)及び非対称な結節ベッド(例えば四箇所骨折の復元における次世代を定義すること)
(f)移植窓(例えば前外側リブを介して結節の再結合を促進するための(骨移植は再結合を始めるのに使用されてもよい))
(g)外科医が患者の解剖模型(patient anatomy)を作ることが可能になる装置および方法
(h)結節の正しい再配置(例えば長さと後傾)ができる装置および方法
(i)結節の安定性と長期にわたる固定を可能にする装置および方法
(j)患者の骨(例えば海綿骨)の(できるだけ長期の)保存が可能になる装置および方法
(k)解剖学的骨折線を考慮し、かつ結節の側方化を防止する装置および方法(例えば回旋筋腱板張力を考慮する)
患者の上腕骨幹に取付けるための幹と、
骨幹端部分と、
第一及び第二の、対向した湾曲面を備えた空洞部を定義する部材と、
フランジとを備え、
前記幹が近位端部と遠位端部を備え、
前記骨幹端部分が本体と、近位端部と、遠位端部とを備え、
前記空洞部を定義する部材が骨幹端部分本体から延びている、患者内への移植用肩プロテーゼは、
骨幹端部分の遠位端部が幹の近位端部に当接し、骨幹端部分の近位端部がフランジに当接していること、
骨幹端部分が、患者の大結節の少なくとも一部を収容するための凹状の彎曲面を有する第一陥凹部と、患者の小結節の少なくとも一部を収容するための凹状の彎曲面を有する第二陥凹部とを備えていること、
第一陥凹部の少なくとも一部が、骨幹端部分の本体から延びている空洞部を定義する部材の第一の対向した湾曲面の少なくとも一部分により定義されていること、及び
第二陥凹部の少なくとも一部が、骨幹端部分の本体から延びている空洞部を定義する部材の第二の対向した湾曲面の少なくとも一部分により定義されていることを特徴とする。
(b)骨癒合が生じる窓を備えたグループから選択される。
患者の上腕骨幹に取付けるための幹と、
骨幹端部分と、
骨幹端部分本体から延びている空洞部を定義する部材と、
フランジとを備え、
前記幹が近位端部と遠位端部を備え、
前記骨幹端部分が本体と、近位端部と、遠位端部とを備え、
前記空洞部を定義する部材が、第一彎曲面と第二湾曲面とを備えている、患者内への移植用肩プロテーゼは、
骨幹端部分の遠位端部が幹の近位端部に当接し、骨幹端部分の近位端部がフランジに当接していること、
骨幹端部分が、患者の大結節の少なくとも一部を収容するための凹状の彎曲面を有する第一陥凹部を備えていること、
骨幹端部分が、患者の小結節の少なくとも一部を収容するための凹状の彎曲面を有する第二陥凹部とを備えていること、
第一陥凹部の少なくとも一部が、骨幹端部分の本体から延びている、空洞部を定義する部材の第一の対向した湾曲面の少なくとも一部分により定義されていること、及び
第二陥凹部の少なくとも一部が、骨幹端部分の本体から延びている、空洞部を定義する部材の第二湾曲面を、骨幹端部分の本体の第二湾曲面に当接させる組合わせにより定義されていることを特徴とする。
(b)骨癒合が生じる窓を備えたグループから選択されている。
患者の上腕骨幹に取付けるための幹と、
骨幹端部分と、
骨幹端部分本体から延びている空洞部を定義する部材とを備え、
前記幹が近位端部と遠位端部を備え、
前記骨幹端部分が本体と、近位端部と、遠位端部とを備え、
前記空洞部を定義する部材が湾曲面を備えている、患者内への移植用肩プロテーゼは、骨幹端部分の遠位端部が幹の近位端部に当接し、骨幹端部分の近位端部がフランジに当接していること、
骨幹端部分が、患者の大結節および患者の小結節の少なくとも一部を収容するための凹状の彎曲面を有する陥凹部を備えていること、及び
陥凹部の少なくとも一部が、骨幹端部分の本体の湾曲面を、骨幹端部分の本体から延びている、空洞部を定義する部材の湾曲面に当接させる組合わせにより定義されていることを特徴とする。
患者の上腕骨幹に取付けるための幹と、
骨幹端部分と、
前外側リブと、
フランジとを備え、
前記幹が近位端部と遠位端部を備え、
前記骨幹端部分が本体と、近位端部と、遠位端部とを備えている、患者内への移植用肩プロテーゼは、
骨幹端部分の遠位端部が幹の近位端部に当接し、骨幹端部分の近位端部がフランジに当接していること、
前外側リブがほぼ前外側方向で骨幹端部分から延びていること、
前外側リブの自由縁部が前側方向でプロテーゼの前頭面からずれていること、
前外側リブの自由縁部が二頭筋溝の解剖学的配置の少なくとも一部分を実質的に模倣していることを特徴とする。
(b)骨癒合が生じる窓を備えたグループから選択されている。
患者の上腕骨幹に取付けるための幹と、
骨幹端部分と、
前外側リブと、
フランジとを備え、
前記幹が近位端部と遠位端部を備え、
前記骨幹端部分が本体と、近位端部と、遠位端部とを備えている、患者内への移植用肩プロテーゼは、
骨幹端部分の遠位端部が幹の近位端部に当接し、骨幹端部分の近位端部がフランジに当接していること、及び
前外側リブの自由縁部が二頭筋溝の三次元的解剖学的配置の少なくとも一部分を実質的に模倣していることを特徴とする。
(b)骨癒合が生じる窓を備えたグループから選択されている。
Claims (17)
- 患者の上腕骨幹に取付けるための幹(50)と、
骨幹端部分(60)と、
前外側リブ(64)と、
フランジ(61)とを備え、
前記幹(50)が近位端部と遠位端部を備えており、
前記骨幹端部分(60)が本体と、近位端部と、遠位端部とを備えており、
前記前外側リブ(64)が骨幹端部分(60)の主要部から延びており、
前記骨幹端部分(60)の遠位端部が幹(50)の近位端部に当接し、骨幹端部分(60)の近位端部がフランジ(61)に当接している患者内への移植用の肩プロテーゼにおいて、
前記前外側リブ(64)が第一および第二の対向した湾曲面を備えていること、
前記骨幹端部分(60)が、患者の大結節の少なくとも一部を収容するための凹状の彎曲面を有する第一陥凹部(81)と、患者の小結節の少なくとも一部を収容するための凹状の彎曲面を有する第二陥凹部(82)とを備えていること、
前記第一陥凹部(81)の少なくとも一部が、骨幹端部分(60)の本体から延びている前外側リブ(64)の第一の対向した湾曲面の少なくとも一部分により定義されていること、
前記第二陥凹部(82)の少なくとも一部が、骨幹端部分(60)の本体から延びている前外側リブ(64)の第二の対向した湾曲面の少なくとも一部分により定義されていること、および
前記第一陥凹部(81)と前記第二陥凹部(82)が非対称であることを特徴とする肩プロテーゼ。 - 第一陥凹部と第二陥凹部が異なる容積を有していることを特徴とする請求項1に記載の肩プロテーゼ。
- プロテーゼの矢状面から、前外側リブ(64)の自由縁部(65)上の点のずれ間隔が、前外側リブ(64)の自由縁部(65)上の点の垂直配置と共に変化することを特徴とする請求項1に記載の肩プロテーゼ。
- 前外側リブ(64)の肉厚が骨幹端部分(60)の本体からの間隔の関数として減少することを特徴とする請求項1に記載の肩プロテーゼ。
- 第一陥凹部(81)が大結節全体をほぼ収容するために形成されており、第二陥凹部(82)が小結節全体をほぼ収容するために形成されていることを特徴とする請求項1に記載の肩プロテーゼ。
- 第一陥凹部(81)が実質的に骨質量を取り除くことなく、大結節全体をほぼ収容するために形成され、第二陥凹部(82)が実質的に骨質量を取り除くことなく、小結節全体をほぼ収容するために形成されていることを特徴とする請求項5に記載の肩プロテーゼ。
- 第一陥凹部(81)が実質的にその形状を変えることなく、大結節全体をほぼ収容するために形成され、第二陥凹部(82)が実質的にその形状を変えることなく、小結節全体をほぼ収容するために形成されていることを特徴とする請求項5に記載の肩プロテーゼ。
- 第一陥凹部(81)が細長い形状を有しており、第二陥凹部(82)が細長い形状を有していることを特徴とする請求項1に記載の肩プロテーゼ。
- さらにプロテーゼの上腕骨頭(30)を備えていることを特徴とする請求項1に記載の肩プロテーゼ。
- プロテーゼの上腕骨頭(30)がフランジ(61)に取付けられていることを特徴とする請求項9に記載の肩プロテーゼ。
- プロテーゼの上腕骨頭(30)がモールステーパ(62)を経由してフランジ(61)に取付けられていることを特徴とする請求項10に記載の肩プロテーゼ。
- モールステーパ(62)が後側方向で中心からずれていることを特徴とする請求項11に記載の肩プロテーゼ。
- フランジ(61)が縫合糸を収容するための少なくとも一つの孔を備えていることを特徴とする請求項1に記載の肩プロテーゼ。
- 幹(50)が上腕骨幹の髄腔内に固定されるようになっていることを特徴とする請求項1に記載の肩プロテーゼ。
- 幹(50)が上腕骨幹の髄腔を把持するための少なくとも一つの溝を備えていることを特徴とする請求項14に記載の肩プロテーゼ。
- 前外側リブ(64)が二つの対向した湾曲面の間にこれらの面を貫通する少なくとも一つの開口部(72)を備えていることを特徴とする請求項1に記載の肩プロテーゼ。
- 開口部(72)が(a)縫合糸を収容するための縫合孔と(b)骨融合が生じる窓を備えたグループから選択されていることを特徴とする請求項16に記載の肩プロテーゼ。
