Description
本特許文献は、参照により本明細書に組み込まれた2003年10月10日に出願された仮米国特許出願第60/510,617号の35U.S.C.§119(e)による出願日の利益を主張するものである。
本発明は医療装置に関し、詳細には、血管など損傷を受けた体内の管の治療のためにヒトまたは動物の体内にインプラントするためのプロテーゼに関する。
本明細書を通して、本発明の大動脈または他の血管への適用について述べる際に、腹部装置に関する「遠位」という用語は、インプラントされる際に血液の流れに対してより下流の位置またはより下流の装置の部分を指すことを意図し、「遠位に」という用語は、血液の流れの方向またはより下流を意味する。「近位」という用語は、インプラントされる際に血液の流れに対してより上流の位置またはより上流の装置の部分を指すことを意図する。「近位に」という用語は、血液の流れと反対の方向またはより上流を意味する。
管の長さ分を取り除くという機能を行うことを妨げる。モジュール間の引き抜きまたは内部漏出(endoleak)から生じる内部漏出(endoleak)は、比較的小さなものでも、動脈瘤再加圧を生じることになる。再加圧された動脈瘤は破裂−極めて高致死率を伴う出来事−する傾向にある。
本発明の1つの態様において、一端の開口部と内面とを有する第1のプロテーゼモジュールと、一端の開口部と外面とを有する第2のプロテーゼモジュールとを含むモジュール式管腔内プロテーゼであって、前記第2のモジュールの一端を前記第1のモジュールの一端に挿入することにより前記第1および第2のモジュールが接続され、前記内面または外面のいずれかが少なくとも1つの突起を含み、前記内面または外面の他方は前記モジュールが接続される際に少なくとも1つの突起に係合する少なくとも1つの表面特徴を含むモジュール式管腔内プロテーゼがある。

本発明は損傷を受けた血管組織を除去する、および/または既存の血管機能の一部を保存するために、血管または管腔にインプラントされるモジュール式管腔内プロテーゼに関する。本発明はまた、排除しないとしても、モジュール間の引き抜きおよび漏出を最小限にすることに関する。
USAのMTS Corporationから入手可能であるMTS ALLIANCE RT/5(登録商標)張力試験機により測定可能である。MTS機は機械を制御する、データを収集および処理するために使用されるコンピュータ端末に接続される。加圧ポ
ンプシステムが、MTS機の張力アーム上に位置付けられる負荷セルに装着される。プロテーゼの一端は加圧ポンプに接続され、加圧ポンプは60mmHgの内部圧力を与え、生体内に展開されたときに血液によって装置に加えられる圧力をシミュレートする。プロテーゼの他端は密封される。プロテーゼは、平均的なヒトの体温をシミュレートするために試験中は完全に37℃の水浴に浸される。MTS機は、装置が完全に分離するまで、0.1mmずつ装置を引っ張る。コンピュータは、とりわけ、モジュールが分離に抵抗する最高の力、すなわち引き抜き力を記録することになる。

図7は、自己拡張分岐プロテーゼ120、自己拡張チューブ状プロテーゼ150(Bloomington, IndianaのCook Incorporatedから市販される製品コードTFB1からTFB5)および、医療処置時に患者の管腔にプロテーゼ120を展開するための、導入器100としても既知である、血管内展開システム100を示す。これらのアイテムはそれぞれ、PCT出願WO98/53761号でより詳細に記載される。
プロテーゼモジュールは好適には順次展開される。チューブ状プロテーゼ150と分岐プロテーゼ120との間のモジュール相互接続はそのままで形成される。まず、分岐プロテーゼ120が展開され、その後チューブ状プロテーゼ150が展開される。例えば、国際出願第WO98/53761号に記載されるように、分岐大動脈プロテーゼ120は腹部大動脈内に展開可能である。分岐プロテーゼ120は、本体部分123、短脚部160および長脚部132を有する概ね逆Y字構成を有する。プロテーゼの本体は、織ポリエステルチューブから構成される。プロテーゼ120の近位端には、プロテーゼの端を越えて延びる自己拡張ステント121があり、遠位に延びるバーブ151を有する。短脚部160はその遠位末端に突起を有する。

プロテーゼの内部を通って導入可能である。その後、腎枝延長部は、腎枝延長部の適切な長さがプロテーゼ枝内に来るまで、プロテーゼ枝に通すことにより展開可能である。この時点で、腎枝延長部は拡張が可能となるか、または能動的に拡張され、大動脈プロテーゼとのテレスコーピング接続を形成することができる。
は、相互接続プロテーゼモジュールを下腹部動脈内に設置可能であり、それらはテレスコーピング相互接続80を形成する。螺旋状プロテーゼ枝はさらに、参照により本明細書に組み込まれる、「Branched Vessel Endoluminal Device」と題される、2004年1月13日に出願され、2004年9月30日に公開された、米国出願公報第2004/0193254号に記載される。
Claims (10)
- 一端(43)に開口部と内面(46)とを有する第1のプロテーゼモジュール(40)と、
一端(47)に開口部と外面(48)とを有する第2のプロテーゼモジュール(50)とを備えるモジュール式管腔内プロテーゼであって、
前記第1および第2のモジュール(40、50)はグラフト材から形成され、前記第2のモジュール(50)の前記一端(47)を前記第1のモジュール(40)の前記一端(43)に挿入することにより接続され、
前記内面(46)または外面(48)のいずれか一方が少なくとも1つの突起(42または44)を含み、
前記内面(46)または外面(48)の他方は少なくとも1つの表面特徴(42または44)を含み、前記少なくとも1つの表面特徴(42または44)はグラフト材に形成され、前記モジュール(40、50)が接続される際に前記表面特徴(42または44)が前記少なくとも1つの突起(42または44)に係合するように前記突起(42または44)と前記表面特徴(42または46)が少なくとも実質的に相互補完的である、モジュール式管腔内プロテーゼ。 - 前記少なくとも1つの突起(42または44)は第1の隆起(42または44)であり、前記少なくとも1つの表面特徴(42または44)は第2の隆起(42または44)であり、前記第1の隆起(42または44)は少なくとも実質的に、前記第2の隆起(42または44)を補完する、請求項1に記載のプロテーゼ。
- 前記少なくとも1つの突起(42または44)は下記の(1)または(2)のいずれかであり、すなわち
(1)第1の螺旋状隆起(42または44)であり、前記少なくとも1つの表面特徴(42または44)は第2の螺旋状隆起(42または44)であり、または
(2)第1の多重の実質的に平行な螺旋状隆起(34)であり、前記少なくとも1つの表面特徴は第2の多重の実質的に平行な螺旋状隆起であり、前記第1の多重の実質的に平行な螺旋状隆起(34)は、少なくとも実質的に、前記第2の多重の実質的に平行な螺旋状隆起を補完する、請求項1に記載のプロテーゼ。 - 前記少なくとも1つの突起は第1の複数の実質的に平行な環状隆起(14)であり、前

- 前記第1のプロテーゼモジュール(40)の外側と、前記第1のプロテーゼモジュール(40)の内側のいずれか一方に配置された少なくとも第1のステント(68または70)と、前記第2のプロテーゼモジュール(50)の外側と、前記第2のプロテーゼモジュール(50)の内側のいずれか一方に配置された少なくとも第2のステント(68または70)とを含む、請求項1、請求項3または請求項4に記載のプロテーゼ。
- 前記少なくとも1つの突起と前記少なくとも1つの表面特徴は共にしわまたはひだを含む、請求項1に記載のプロテーゼ。
- 前記第1のプロテーゼモジュール(40)と前記第2のプロテーゼモジュール(50)間のテレスコーピング接続は、前記プロテーゼが60mmHgに加圧され37℃水に浸されたときに4ニュートンより大きい引き抜き力に抵抗することができる、請求項1、請求項3、請求項4または請求項9に記載のプロテーゼ。
