Abstract
Description
ソースが無線周波数(RF)、直流(DC)抵抗又はレーザーであるか否かに関わらない熱エネルギは、直接的な圧力において標的接合領域と接触する血管又は組織を取り付け又は接合することがよく知られている。これは、しばしば、顎型圧縮熱送達装置を用いて行われる。また、このようなバルーン、金属ケージ及びバスケットのような半径方向に拡張可能な装置は、RF又はDC抵抗又はバルーンの場合には加熱された生理食塩水の形で、エネルギと結合され、組織を焼灼し、出血を停止し、狭窄を形成するために管腔内で使用されることが知られている。
Claims (29)
- 動静脈(AV)瘻孔を形成するための装置であって、
長尺状部材と、
前記長尺状部材に対して移動可能に前記長尺状部材に接続され、テーパ遠位端を有する遠位部材と、
前記長尺状部材及び前記移動可能な遠位部材のうち一つの平滑テーパ面上に配置される第1の加熱部材と、
前記長尺状部材及び前記移動可能な遠位部材のうちの他の平滑テーパ面上に配置された第2の加熱部材と、を含み、
前記両加熱部材は、前記瘻孔を形成するために組織を切断するように構成されている装置。 - 請求項1に記載の装置において、
前記長尺状部材は、長尺状外管を含む装置。 - 請求項1に記載の装置において、
さらに、前記遠位部材を前記長尺状部材に連結するためのシャフトを含み、
前記シャフトは前記長尺状部材に対して前記遠位部材を延長及び後退させるために延長及び後退可能である装置。 - 請求項3に記載の装置において、
前記シャフト及び前記遠位部材の一つは、可撓性材料で形成される装置。 - 請求項1に記載の装置において、
前記近位の長尺状部材上の前記平滑テーパ面は、遠位テーパ面を含み、
前記遠位部材上の前記平滑テーパ面は、近位テーパ面を含み、
さらに、前記近位テーパ面及び前記遠位テーパ面は互いに整合する装置。 - 請求項5に記載の装置において、
前記第1の加熱部材は、前記近位テーパ面上に配置される装置。 - 請求項6に記載の装置において、
前記第2の加熱部材は、前記遠位テーパ面上に配置される装置。 - 請求項1に記載の装置において、
前記第1及び第2の加熱部材の一方が能動的であり、他方は受動的である装置。 - 請求項8に記載の装置において、
前記能動的加熱部材はDC抵抗エネルギによって通電される装置。 - 請求項8に記載の装置において、
前記受動的加熱部材は、受動的な熱伝導性表面を含む装置。 - 請求項8に記載の装置において、
前記能動的加熱部材は、楕円形の形状を有する装置。 - 請求項1に記載の装置において、
前記遠位部材はテーパ状で柔軟である装置。 - 請求項1に記載の装置において、
前記加熱部材のうち一つに関連した組織を保持するための構造体をさらに含む装置。 - 請求項13に記載の装置において、
前記構造体は前記加熱部材の一つの面に隣接して配置された複数の突起要素を含む装置。 - 請求項13に記載の装置において、
前記長尺状部材及び前記遠位部材の少なくとも一つが前記構造体によって保持された組織を受容するためのキャビティを含む装置。 - 請求項15に記載の装置において、
前記キャビティは、前記加熱部材の一つ内に配置されている装置。 - 請求項5に記載の装置において、
組織接着を最小限にするための前記近位及び遠位テーパ面のそれぞれの上に配置されるコーティングを含む装置。 - 請求項17に記載の装置において、
前記コーティングはPTFEを含む装置。 - 請求項5に記載の装置において、
前記近位及び遠位テーパ面のそれぞれは、約25〜100マイクロインチの滑らかな表面仕上げを有するように構成されている装置。 - 請求項1に記載の装置において、
前記遠位部材は、前記長尺状部材に対して回転可能である装置。 - 請求項1に記載の装置において、
さらに、前記加熱部材により生成された熱を拡散し、前記加熱部材から外側に放射される温度勾配を形成するために、前記加熱部材の少なくとも一つより上、下又は内に配置された伝導材料を含む装置。 - 隣接する第1及び第2の血管間のAV瘻孔を形成する方法であって、
前記第1の血管から前記第2の血管内にガイドワイヤを挿入する工程と、
遠位部材のテーパ遠位先端部が選択された吻合部位に接触するように、前記ガイドワイヤ上の近位長尺状部材及び遠位部材を備えるカテーテルを挿入する工程と、
前記2つの血管間の開口部を拡大するために、前記長尺状部材が前記第1の血管内に残るように、前記長尺状部材の平滑テーパ遠位面が第1の血管の組織壁に接触するまで、前記遠位部材を前記第2の血管内に前進させる工程と、
前記長尺状部材の前記平滑テーパ遠位面及び前記遠位部材上の対応する平滑テーパ近位面間の前記開口部を囲む組織をクランプするために前記遠位部材及び長尺状部材をともに移動させる工程と、
前記2つの血管の間の所望の瘻孔を形成するために、前記開口部を切断及び形成し、かつその縁部を接合するように前記遠位部材及び前記長尺状部材上の1つの加熱部材にエネルギを印加する工程とを含む方法。 - 請求項22に記載の方法において、
さらに前記エネルギ印加工程中に前記開口部が切断される位置において150℃〜300℃の温度を維持する工程を含む方法。 - 請求項23に記載の方法において、
前記エネルギ印加工程は、前記2つの血管の壁を一つに接合するために、約1〜30秒間に亘って維持される方法。 - 請求項22に記載の方法において、
前記移動する及びクランプする工程は、さらに、前記長尺状部材上の平滑テーパ遠位面及び前記遠位部材上の対応する平滑テーパ近位面間に公知の制御された圧力を印加することを含み、
前記公知の制御された圧力は約100mN/mm2〜400mN/mm2の範囲内である方法。 - 請求項22に記載の方法において、
さらに前記遠位部材の摩擦抵抗を低減するために、前記前進工程中に、前記遠位部材を回転させる工程を含む方法。 - 請求項22に記載の方法において、
さらに前記加熱部材に関連付けられた構造体を用いて切断組織を保持する工程を含む方法。 - 請求項27に記載の方法において、
前記構造体が、キャビティを含む方法。 - 請求項28に記載の方法において、
前記構造体は、さらに、平滑テーパ面及び取り囲む前記キャビティの少なくとも一つから延出する複数の突出要素を含む方法。
