Claims (82)
- a)近位頂部捕捉部分であって、

- 前記ステントの少なくとも一部分が、端部でつなぎ合わされて頂部を形成するストラットを備える、請求項34に記載のステントグラフトシステム。
- 前記ステントが、少なくとも一部において超弾性金属で形成される、請求項35に記載のテレスコープ型ステントグラフトシステム。
- a)ルーメン状グラフト構成要素と、
b)前記ルーメン状グラフト構成要素の近位端から延在するベアステントと、
c)前記ベアステントの近位端から遠位に延在する少なくとも1つの近位バーブと、
d)前記ベアステントの遠位端から遠位に延在する少なくとも1つの遠位バーブであって、前記ルーメン状グラフト構成要素の主軸に沿った前記近位バーブと前記遠位バーブとの間の距離が、約6mm〜約40mmの範囲である、遠位バーブと、
を含む、ステントグラフトシステム。 - 前記ルーメン状グラフト構成要素が二分岐である、請求項37に記載のステントグラフトシステム。
- 前記バーブの少なくとも一部分が、前記ベアステントから約20度〜約60度の範囲の角度をなして延在する、請求項38に記載のステントグラフトシステム。
- 前記ベアステントが、少なくとも一部において超弾性金属で形成される、請求項39に記載のステントグラフトシステム。
- a)バレルと、
b)前記バレルから前記バレルの主軸に沿って延在するスプールと、
c)前記スプールの端部にあるリムであって、前記スプールの直径より大きいが、前記バレルの直径より小さい直径を有するリムと、
を含む、脚部クラスプ。 - 少なくとも一部において、ステンレス鋼、ポリエステル、ポリエーテルエーテルケトン及びアクリロニトリルからなる群から選択される少なくとも1つの構成成分で形成される、請求項41に記載の脚部クラスプ。
- 前記リムが径方向に延在するスポークを備える、請求項42に記載の脚部クラスプ。
- a)脚部クラスプであって、
i.バレルと、
ii.前記バレルから前記バレルの主軸に沿って延在するスプールと、
iii.前記スプールの端部にあるリムであって、前記スプールの直径より大きいが、前記バレルの直径より小さい直径を有するリムと、
を備える脚部クラスプと、
b)前記バレルに固定され、且つ前記バレルから前記スプールと反対方向に延在する支持チューブと、
c)前記バレル径より大きい内径を有し、且つ前記スプール及び前記リムを被覆する第1の位置と、前記スプール及び前記リムを露出させる第2の位置との間を摺動自在に動くことが可能なシースと、
を含む、ステントグラフトデリバリーシステム。 - 前記リムが径方向に延在するスポークを備える、請求項44に記載のステントグラフトデリバリーシステム。
- ステントグラフトであって、前記シースが前記第1の位置にあるとき、前記ステントグラフトのステントが前記リムに干渉する関係で前記スプールの周囲に延在し、及び前記ステントが固定されるルーメン状グラフトが、前記リムと前記シースとの間に延在し、前記シースが前記第1の位置から前記第2の位置に動くと、前記脚部クラスプから前記ステントグラフトが解放される、ステントグラフトをさらに備える、請求項45に記載のステントグラフトデリバリーシステム。
- a)ルーメン状グラフト構成要素と、
b)近位頂部と遠位頂部とによってつなぎ合わされた斜めストラットのベアステントであって、前記ルーメン状グラフト構成要素の近位端から延在するベアステントと、
c)前記ベアステントに隣接し、且つ前記ルーメン状グラフト内にある近位ステントであって、頂部によってつなぎ合わされた斜めストラットを備える近位ステントと、
d)前記ルーメン状グラフト構成要素に取り付けられている前記ベアステントから遠位に延在する少なくとも1つのバーブと、
を含む、ステントグラフトシステム。 - 前記近位ステントが、前記ベアステント内に入れ子にされる、請求項47に記載のステントグラフトシステム。
- 前記ベアステントが前記ルーメン状グラフト構成要素内に固定され、前記バーブが遠位頂部から前記ルーメン状グラフト構成要素の中を通って延在する、請求項48に記載のステントグラフトシステム。
- a)二分岐の第1のステントグラフトであって
i.二分岐の第1のルーメン状グラフト構成要素と、
ii.前記二分岐の第1のルーメン状グラフト構成要素の2本の脚部のうちの一方の表面に沿って延在し、且つそこに固定される複数のステントと、
を備える二分岐の第1のステントグラフトと、
b)第2のステントグラフトであって、
i.第2のルーメン状グラフト構成要素と、
ii.前記第1のルーメン状グラフト構成要素の表面に沿って延在し、且つそこに固定された複数のステントと、
を備え、前記第2のステントグラフトを、前記二分岐の第1のルーメン状グラフト構成要素の2本の脚部構成要素のうちの第1の脚部の前記遠位端に挿入し、前記第1のステントグラフト及び前記第2のステントグラフトの各々の少なくとも2つのステントを重ね合わせることができる、第2のステントグラフトと、
c)前記二分岐の第1のルーメン状ステントグラフトの第2の脚部の表面に沿って延在し、且つそこに固定された複数のステントと、
を含む、テレスコープ型ステントグラフトシステムであって、
前記第1の脚部が前記第2の脚部より短く、前記第1の脚部が、前記第2のステントグラフトの少なくとも2つのステントが重なり合うのに必要な数より少なくとも1つ多いステントを備える、テレスコープ型ステントグラフトシステム。 - 前記二分岐の第1のステントグラフトが、前記二分岐の第1のルーメン状グラフト構成要素の第1の脚部の2つの外側ステントの間にある内側ステントをさらに備え、前記外側ステントのうちの一方が前記二分岐の第1のルーメン状グラフト構成要素の遠位端にあり、前記内側ステントが、前記二分岐の第1のルーメン状グラフト構成要素の内表面に固定される、請求項50に記載のテレスコープ型ステントグラフトシステム。
- 前記二分岐の第1のステントグラフトが、前記第2の脚部の2つの外側ステントの間にある第2の内側ステントをさらに備え、前記外側ステントのうちの一方が前記第2の脚部の遠位端にあり、前記第2の内側ステントが前記第2の脚部の内表面に固定される、請求項51に記載のテレスコープ型ステントグラフトシステム。
- i.第3のルーメン状グラフト構成要素と、
ii.前記第3のルーメン状グラフト構成要素の外表面に沿って延在し、且つそこに固定される複数の外側ステントと、
を備える第3のステントグラフトであって、
前記第3のステントグラフトを挿入すると、前記二分岐の第1のルーメン状グラフト構成要素の前記第2の脚部の前記遠位端に挿入されて前記第2の脚部の少なくとも2つのステントに重なり合う、第3のステントグラフトをさらに備える、請求項52に記載のテレスコープ型ステントグラフトシステム。 - 前記二分岐の第1のステントグラフトの前記第1の脚部及び前記第2の脚部の前記内側ステントの少なくとも1つが、前記第1のルーメン状グラフト構成要素内にほぼ近位に向いた複数のバーブを備え、前記第2のステントグラフト又は前記第3のステントグラフトを前記二分岐の第1のステントグラフトのそれぞれの脚部の前記遠位端に挿入すると、前記バーブの少なくとも一部分の間に前記第2のステントグラフト又は前記第3のステントグラフトのルーメン状グラフト構成要素のステントに干渉する関係が生じる、請求項54に記載のテレスコープ型ステントグラフトシステム。
- 腹部大動脈瘤の治療方法であって、
a)シース及びデリバリーシステムの遠位先端を、患者の動脈を通じて前記患者の腹部大動脈瘤に送り込むステップであって、前記シースが二分岐ステントグラフトを格納している、ステップと、
b)前記シースと螺着自在に連結されている前記デリバリーシステムの送りねじ用ナットを回転させ、それにより前記二分岐ステントグラフトから前記シースを少なくとも部分的に引き込むステップと、
c)前記送りねじ用ナットを前記シースと螺着自在に連結しておきながら、前記送りねじ用ナットを前記デリバリー装置のハンドル本体に沿って摺動させ、それにより前記シースをさらに引き込むステップであって、前記二分岐ステントグラフトが前記腹部大動脈瘤内で少なくとも部分的に展開され、それにより前記腹部大動脈瘤が治療される、ステップと、
を含む、方法。 - 前記デリバリー装置の遠位端にあるクラスプを開き、前記二分岐ステントグラフトの近位端にあるベアステントを解放するステップをさらに含む、請求項55に記載の方法。
- 前記クラスプが開いて前記ベアステントが解放されたときに、前記二分岐ステントグラフトの第1の脚部の一部分を前記シース内に保持するステップをさらに含む、請求項56に記載の方法。
- ステントを前記第1の脚部の遠位端で前記シースと脚部クラスプとの間に固定することにより、前記二分岐ステントの前記第1の脚部が保持される、請求項57に記載の方法。
- 前記第1の脚部が、前記二分岐ステントの2本の脚部のうちの長いほうである、請求項58に記載の方法。
- 前記シースが前記ベアステントを露出させるのに十分なだけ引き込まれたとき、前記クラスプが、前記ベアステントのストラットを前記デリバリーシステムの主軸に向かって拡げる、請求項59に記載の方法。
- 前記第1の脚部が少なくとも部分的に前記シース内にある間に前記二分岐ステントの第2の脚部に拡張ステントグラフトによるカニュレーションを行うステップをさらに含む、請求項60に記載の方法。
- 保持されている前記脚部が、カニュレーションが行われる脚部より長い、請求項61に記載の方法。
- 前記カニュレーションが行われる脚部が、前記拡張ステントグラフトとテレスコピックな関係にある、請求項62に記載の方法。
- 前記カニュレーションが行われる脚部が、前記カニュレーションが行われる脚部及び前記拡張ステントグラフトの各々の少なくとも2つのステントによって前記拡張ステントグラフトに重なる、請求項63に記載の方法。
- 前記カニュレーションが行われる脚部が、前記カニュレーションが行われる脚部及び前記拡張ステントグラフトの各々の2つのステントによって前記拡張部の脚部に重なるのに必要な数より少なくとも1つ多いステントを備える、請求項64に記載の方法。
- 前記カニュレーションが行われる脚部の前記遠位端から2番目のステントが、前記二分岐ステントグラフトの前記グラフト内にある、請求項65に記載の方法。
- 前記二分岐グラフトの前記遠位端から2番目のステントが、前記ステントから内側近位に延在するバーブを備える、請求項66に記載の方法。
- 前記脚部クラスプから前記二分岐ステントグラフトを解放するステップと、次に前記デリバリー装置の残りの部分からスライダ及び前記シースを切り離して、前記スライダ及びシースを実質的にその場に置いておきながら前記装置の残り部分を前記患者から引き抜くステップとをさらに含む、請求項67に記載の方法。
- 第2の拡張部をシースを通じて前記第1の脚部まで送り込み、前記第2の拡張部により前記第1の脚部のカニュレーションを行うステップをさらに含む、請求項68に記載の方法。
- 前記カニュレーションが行われる第2の脚部が、前記カニュレーションが行われる第1の脚部及び前記第2の拡張部の各々の少なくとも2つのステントによって前記拡張ステントグラフトに重なる、請求項69に記載の方法。
- 前記カニュレーションが行われる第1の脚部が、前記カニュレーションが行われる第1の脚部及び前記第2の拡張部の各々の2つのステントによって前記拡張部の脚部に重なるのに必要な数より少なくとも1つ多いステントを備える、請求項70に記載の方法。
- 前記カニュレーションが行われる第1の脚部の前記遠位端から2番目のステントが、前記二分岐ステントグラフトの前記グラフト内にある、請求項71に記載の方法。
- 前記二分岐グラフトの前記遠位端から2番目のステントが、前記ステントから内側近位に延在するバーブを備える、請求項72に記載の方法。
- a)頂部捕捉装置アセンブリであって、
1)近位頂部捕捉部分であって、
ii.前記近位捕捉部分の主軸と実質的に平行な少なくとも1つの径方向拘束体を画定するノーズと、
iii.前記ノーズから遠位に延在する複数の爪部であって、最近位の径方向拘束体に対して径方向に、且つ前記主軸と実質的に平行に前記主軸の周りに径方向に配設された爪部と、
を備える近位頂部捕捉部分と、
2)前記主軸の周りに径方向に配設されたスロットを画定する遠位頂部捕捉部分であって、前記スロットが、前記近位頂部捕捉部分と前記遠位頂部捕捉部分とを前記主軸に沿って互いに動かすことにより前記爪部(time)と嵌合可能である、遠位頂部捕捉部分と、
3)前記ノーズと前記遠位頂部捕捉部分との間に前記主軸から径方向に延在し、且つ前記爪部が前記スロットと嵌合関係になる動きに干渉しない関係で前記主軸に沿って前記スロットと整列する複数のボスと、
4)前記遠位頂部捕捉部分が固定される細長部材であって、前記近位頂部捕捉部分及び前記複数のボスを貫通して延在する細長部材と、
5)前記近位頂部捕捉部分が固定され、前記細長部材が貫通して延在するルーメンであって、前記ルーメンを動かすことにより、前記爪部が前記スロットと嵌合して前記ボス上に載置される第1の位置と、前記爪部が前記スロットと嵌合せず前記ボス上に載置されない第2の位置との間で前記近位頂部部分が前記主軸に沿って動く、ルーメンと、
6)頂部によって連係されたストラットを備えるベアステントであって、前記爪部が前記スロットと嵌合されると、前記ストラットが前記爪部の間に延在し、前記頂部の一部分が前記ボスと前記遠位頂部捕捉部分との間に延在する、ベアステントと、
7)前記ステントから前記径方向拘束体の中に延在する少なくとも1つの腎臓上バーブと、
を備える、頂部捕捉装置アセンブリと、
b)前記細長部材及び前記ルーメンが貫通して延在する脚部クラスプであって、
1)バレルと、
2)前記バレルから前記バレルの主軸に沿って延在するスプールと、
3)前記スプールの端部にあるリムであって、前記スプールの直径より大きいが、前記バレルの直径より小さい直径を有するリムと、
を備える脚部クラスプと、
を含む、ステントグラフトデリバリー装置。 - a)xバルブと、
b)前記xバルブを支持するガスケットと、
を含む、xバルブアセンブリ。 - 前記ガスケットが、外周支持体(peripherial support)と、前記外周支持体(peripherial support)から内側に延在する少なくとも1本のアームとを備える、請求項75に記載のxバルブアセンブリ。
- 前記ガスケットが、交差する主軸に沿った少なくとも2対のアームを備える、請求項76に記載のxバルブアセンブリ。
- アームの各対が整列している、請求項77に記載のxバルブアセンブリ。
- 前記軸のうちの少なくとも2つが互いに垂直である、請求項78に記載のxバルブアセンブリ。
- 前記一対のアームが一平面内にある、請求項79に記載のxバルブアセンブリ。
- 前記ガスケットが超弾性金属を含む、請求項80に記載のxバルブアセンブリ。
