Description
実施例1
この例では、家畜用ブタに、ブタの胃食道接合部(GEJ)を再構築する様式で、内視鏡的管腔内粘膜形成術(ELM)を行った。1番目のブタ(ブタ#1)は、(GEJ)の管腔の直径を約2分の1小さくするために、噴門のELMを受けた。これを達成すべく、Duette(登録商標)Multi-band Mucosectomy 装置(米国ノースカロライナ州ウインストン・セーラムのWilson-Cook Medical, Inc.社、CookTM Endoscopy)を装備した内視鏡システムを使用し、噴門の周り円周方向に3個所の部位で組織を縛って切除したが、これは噴門の周囲の約70%に相当する。2番目のブタ(ブタ#2)は、5週間後に伸延バルーンを使用して処置を逆行させたことを除き、同様に治療した。それら実験用ブタは、対照群と共に、エサの摂取量を無制限とし、水分は自由に摂取できるようにした。ブタは、研究期間中、健康活発であった。この研究の結果を図13、図14、図15にグラフで示している。見て分かるように、治療を受けた2匹のブタは、対照群のブタに比較し、体重増加が著しく少なかった。更に、ブタ#2は、処置の逆行後は、体重増加が加速した。
実施例2
この実施例では、内視鏡的管腔内粘膜形成術(ELM)を使用し、家畜用ブタに胃底皺襞形成術を行った。実施例1と同様に、Duette(登録商標)Multi-band Mucosectomy 装置を装備した内視鏡システムを使用した。結紮と切除を繰り返して、胃の底部の粘膜及び粘膜下組織を10平方センチメートル除去した。胃の最も拡張する部分である胃底は、切除後の治癒と同時に縮んで再構築されていた。このブタは、対照群共々、5週間に亘り、エサの摂取量を無制限とし、水分は自由に摂取できるようにした。ブタは、研究期間中、健康活発であり、治療を受けたブタは、対照群のブタに比較し、体重増加が著しく少なかった。
Claims (33)
- 患者を治療するための方法において、
前記患者の体内通路が再構築される結果となる治癒反応を引き起こすことを目的として、前記体内通路の管腔表面から組織を切除する段階から成る、方法。 - 前記切除する段階は、粘膜組織及び粘膜下組織の1つ又はそれ以上の切除を施す段階を含んでいる、請求項1に記載の方法。
- 前記切除する段階は、前記体内通路の目標領域内で円周方向に分散した複数の切除を施す段階を含んでいる、請求項1に記載の方法。
- 前記体内通路は食道である、請求項1に記載の方法。
- 前記再構築は、前記患者の胃食道逆流を軽減するのに効果的である、請求項4に記載の方法。
- 前記体内通路は胃である、請求項1に記載の方法。
- 前記再構築は、前記胃に狭い部分を作成し、前記患者の満腹感を修正するのに効果的である、請求項6に記載の方法。
- 前記切除中に、内視鏡で前記体内通路を視覚化する段階を更に含んでいる、請求項1に記載の方法。
- 前記内視鏡は、作業チャネルを有しており、前記切除する段階は、前記チャネルを通して実施される、請求項8に記載の方法。
- 前記切除する段階は、電気焼灼スネアのループを前記作業チャネルに通す段階と、組織を前記ループ内に捕縛する段階と、前記捕縛した組織を前記ループで切断する段階と、を含んでいる、請求項9に記載の方法。
- 前記組織を前記ループ内に捕縛する前に、前記組織を帯で縛る段階を更に含んでいる、請求項10に記載の方法。
- 前記縛る段階は、真空によって容器内に組織の疑似ポリープを作成する段階と、前記疑似ポリープの周りに帯を展開配備する段階を含んでいる、請求項11に記載の方法。
- 前記患者を、胃食道逆流障害を患っていると診断する段階と、
前記診断に基づき、前記切除作業を実施する段階と、を含んでいる、請求項1に記載の方法。 - 前記患者を、肥満の治療が必要であると診断する段階と、
前記診断に基づき、前記切除作業を実施する段階と、を含んでいる、請求項1に記載の方法。 - 前記切除する段階は、前記体内通路の下層の筋肉組織を実質的に傷付けること無く、前記体内通路から管腔組織を切除する段階を含んでいる、請求項1に記載の方法。
- 前記体内通路は、食道である、請求項14に記載の方法。
- 前記体内通路は胃である、請求項14に記載の方法。
- 前記組織は、非病変組織である、請求項1に記載の方法。
- 前記組織は、粘膜と粘膜下組織を備えている、請求項1に記載の方法。
- 前記切除する段階の前に、前記体内通路の直径を測定する段階を更に含んでいる、請求項1に記載の方法。
- 体内管腔を再構築するためのシステムにおいて、
前記体内管腔と前記体内管腔内に位置する少なくとも1つの既知の寸法の基準の画像を生成することができるように構成されている内視鏡と、
前記画像内での前記基準と前記体内管腔との比較に基づき、前記体内管腔の寸法を計算するためのソフトウェアと、
前記体内管腔から組織を除去するための管腔内外科装置と、を備えているシステム。 - 前記内視鏡外科装置はスネアを備えている、請求項21に記載のシステム。
- 前記スネアは、スネア本体部分とスネアループを有しており、前記内視鏡は作業チャネルを有しており、前記スネア本体部分と前記スネアループは、前記作業チャネルを通して受け入れることができ、その状態で、前記体内管腔から組織を除去するように操作することができる、請求項22に記載のシステム。
- 前記内視鏡の遠位端上に装着され、前記作業チャネルに掛けた真空によって引き出された組織をその中に受け入れる働きをすることができる、筒を更に備えている、請求項23に記載のシステム。
- 前記筒は、前記筒上に預けられた1つ又は複数の結紮帯を備えている、請求項24に記載のシステム。

内視鏡結紮器筒と、
前記結紮筒上に預けられた1つ又は複数の結紮帯と、
前記1つ又は複数の結紮帯と作動的に関係付けられた1本の牽引糸であって、前記1つ又は複数の結紮帯を前記結紮筒から展開配備させるように操作することができる牽引糸と、を備えている医療装置。 - 更に、内視鏡を備えており、前記内視鏡結紮器筒は前記内視鏡上に取り付けられている、請求項26に記載の医療装置。
- 前記結紮器筒は、近位端と遠位端を有しており、前記遠位端には斜端加工が施されている、請求項26に記載の医療装置。
- 医療装置において、
近位端と斜端加工を施した遠位端を有する内視鏡結紮器筒と、
前記結紮筒上に預けられている1つ又は複数の結紮帯であって、前記斜端加工を施した遠位端から展開配備させることができる結紮帯と、を備えている医療装置。 - 前記1つ又は複数の結紮帯と作動的に関係付けられた1つ又は複数の牽引糸であって、前記1つ又は複数の結紮帯を前記結紮筒から展開配備させるように操作することができる牽引糸を更に備えている、請求項29に記載の医療装置。
- 患者を治療するための方法において、
前記患者の体内通路が再構築される結果となる治癒反応を引き起こすことを目的として、前記体内通路の管腔表面から組織を除去する段階から成り、前記除去する段階では、下層の筋肉組織の層を実質的に傷付けること無く粘膜組織及び粘膜下組織が除去される、方法。 - 前記除去する段階は、粘膜組織及び粘膜下組織を前記筋肉組織から分離し、その後、前記粘膜組織と前記粘膜下組織を除去する段階を含んでいる、請求項31に記載の方法。
- 前記分離する段階は、前記粘膜組織と前記粘膜下組織を筒の中に引き込む段階を含んでいる、請求項32に記載の方法。
