Abstract
【解決手段】消化器系の特定の部位において食品の吸収を制限するための方法および装置を提供する。胃腸内埋め込み装置が、十二指腸に固定され、トライツ靭帯を超えて延びる。胃を出るすべての食物が、この装置を通過して流れる。この胃腸用装置は、当該装置を十二指腸に固定するためのアンカー(2810)、および十二指腸における栄養素の吸収を制限するための非支持の可撓スリーブ(202)を備えている。アンカーは、ステントおよび/または波状アンカーを備えることができ、カテーテルによる送入および取り出しのために畳むことが可能である。

Claims (58)
- 両端において開いており、十二指腸へと延びて十二指腸における栄養素の吸収を制限するように構成されている非支持の可撓スリーブ、および
前記スリーブの上流部へと接続され、十二指腸内に保持されるように構成されており、緩められた状態の直径が少なくとも40ミリメートルであるアンカー
を有している胃腸内埋め込み装置。 - 請求項1において、緩められた状態における前記アンカーの最小径が、少なくとも45ミリメートルである胃腸内埋め込み装置。
- 請求項1において、前記アンカーが、埋め込まれた状態において、30〜35ミリメートルの直径を有している胃腸内埋め込み装置。
- 請求項1において、前記アンカーが、埋め込まれた状態において、長さ対直径の比が1である長さおよび直径を定めている胃腸内埋め込み装置。
- 請求項1において、前記アンカーが十二指腸球部に保持されるように構成されている胃腸内埋め込み装置。
- 請求項1において、前記アンカーが折り畳み可能である胃腸内埋め込み装置。
- 請求項1において、前記アンカーが前記スリーブの上流部によって覆われている胃腸内埋め込み装置。
- 請求項4において、前記アンカーが、前記スリーブの第1の内側層と前記スリーブの第2の外側層との間に挟まれている胃腸内埋め込み装置。
- 請求項1において、前記アンカーが、ストラットからなる網目を備えているステントを含んでいる胃腸内埋め込み装置。
- 請求項1において、前記アンカーが、追従性の高い波状アンカーを含んでいる胃腸内埋め込み装置。
- 請求項1において、前記アンカーが、当該アンカーが前記スリーブの上流部を十二指腸へと固定すべく広がるときに組織へと挿入されるように構成されている手段を有している折り畳み可能なアンカーである胃腸内埋め込み装置。
- 請求項1において、前記スリーブが、胃を出て当該スリーブの上流端を通って流し込まれる糜粥がトライツ靭帯の下流端を通って当該スリーブから出る長さである胃腸内埋め込み装置。
- 請求項1において、前記スリーブの素材が、0.2未満の摩擦係数を有している胃腸内埋め込み装置。
- 請求項1において、前記スリーブがフッ素重合体で形成されている胃腸内埋め込み装置。
- 請求項11において、前記フッ素重合体が延伸ポリテトラフルオロエチレンである胃腸内埋め込み装置。
- 請求項12において、FEP層をさらに有している胃腸内埋め込み装置。
- 請求項1において、前記スリーブがポリエチレンで形成されている胃腸内埋め込み装置。
- 請求項12において、前記スリーブがシリコーンで被覆されている胃腸内埋め込み装置。

- 請求項18において、前記取り付け手段が、前記スリーブの上流部を十二指腸へと取り付けるため前記アンカーの外表面から延びる針状突起物を含んでいる胃腸内埋め込み装置。
- 請求項19において、前記針状突起物が2方向を向いている胃腸内埋め込み装置。
- 請求項20において、前記針状突起物が、前記非支持の可撓スリーブを十二指腸の筋肉組織へと固定する胃腸内埋め込み装置。

前記スリーブに組み合わされ、前記スリーブの座屈を減らすべく前記アンカーの下流から前記非支持の可撓スリーブの下流端まで延びている座屈防止装置
をさらに有している胃腸内埋め込み装置。 - 請求項1において、前記スリーブが、十二指腸において分泌された酵素が当該スリーブの外側で十二指腸を通過できるようにしている胃腸内埋め込み装置。
- 請求項1において、前記非支持の可撓スリーブを挿入するためのカテーテルと組み合わせられている胃腸内埋め込み装置。
- 請求項1において、前記非支持の可撓スリーブを取り出すための回収装置と組み合わせられている胃腸内埋め込み装置。
- 請求項1において、前記アンカーの取り外しを容易にするように構成された引き紐をさらに有している胃腸内埋め込み装置。
- 請求項26において、前記引き紐が、前記アンカーの上流部に沿って位置し、引っ張られたときに上流部の直径を減少させる胃腸内埋め込み装置。

前記外側シースの内側に位置する内側シースであって、前記アンカーの上流端を折り畳んで前記アンカーを前記外側シースに引き込むことができるようにする複数のフィンガを、下流端から延伸させている内側シース
を有している回収装置。 - 請求項49において、前記フィンガが、前記ステントに係合して前記アンカーの上流端を半径方向内向きに引くことによって前記ステントを折り畳む回収装置。
