Claims (1)
- 【特許請求の範囲】 1.血管内の少なくとも一方向の血液の流れを遮断するための血管内に移植可能な 装置であって、 a)初めに半径方向に縮小した形状で配備され、前記血管の逓減部における管腔内 へ前記装置を移送し、続いて有効形状に拡張し、この形状において周囲の血管壁に 嵌入し、前記血管の管腔内の固定位置に装置を保持する血管嵌入部と、 b)前記嵌入部がその有効形状にあって血管壁と接触しているとき、前記管腔内の 少なくとも一方向の血液の流れを遮断する、前記嵌入部に付設される管腔遮断部 とを含んでなる装置。 2.前記血管嵌入部がワイヤフレームである請求項1の装置。 3.前記血管嵌入部が膨張可能な要素である請求項1の装置。 4.前記血管嵌入部が血管壁に埋入される突起を有する請求項1の装置。 5.前記血管嵌入部が血管に埋入されるフックを有する請求項1の装置。 6.前記血管嵌入部が血管壁と接着する接着剤を含んでなる請求項1の装置。 7.前記血管嵌入部が、中央部で接合し前記中央部から外向きに延長する多数の要 素からなり、前記嵌入部がその有効形状にあるとき、前記延長要素が半径方向外向 きの圧力を血管壁に与え、装置が血管内に移植されたとき、前記中央部に対する血 17.前記装置が更に、隣接するカテーテルに装置を引き寄せることが可能な管腔間 挿入回収器への装置の接合を容易にするための、装置上に形成された連結器を含 んでなる請求項16の装置。 18.装置の血管嵌入部が、回収器が連結器に取り付けられ且つ牽引力が回収器に与 えられた後、装置の血管嵌入部がしたがって半径方向に収縮し且つ血管壁を解放 するように構成されて隣接カテーテルへの装置の回収が容易であるである請求項 17の装置。 19.体温まで加熱されたとき前記有効形状に遷移するものであって、しかしながら、 冷却液体中に装置を浸して体温より低い形状記憶遷移温度まで装置を冷却する ことによって、その場で半径方向に収縮可能な、形状記憶材料により前記血管嵌入 部が構成される請求項1の装置。 20.前記血管嵌入部が、前記有効形状に斜傾する弾性回復性自己展開材料により構 成され、前記装置が非拘束状態で前記有効形状に弾性回復的に自己展開するもの である請求項1の装置。 21.初めに前記半径方向に圧縮した形状であって、続いて前記装置に対して圧力が 与えられることによって前記有効形状に変形可能となる塑性変形可能な材料によ り前記血管嵌入部が構成される請求項1の装置。 22.a)前記血管嵌入部が、半径方向に圧縮した形状が概ね円筒形のフレームからな り、前記フレームは第一及び第二の対向する端部を有するものであって、フレーム がその有効形状に拡張して血管壁に嵌入するとき、前記フレームが逓減する血管 壁の形状に一致する概ね錐台−円錐形状となりうるものであり、 b)前記管腔遮断部が、前記フレーム上に設けられた可撓性シールからなり、前記フ レームがその有効形状に展開し前記血管壁に嵌入するとき、前記血管腔内に横断 障壁を形成するために前記シールが形成及び配備されるものである 請求項1の装置。 23.前記円筒形フレームが、室温において前記縮小形状であって体温まで加熱され て前記有効形状に遷移する形状記憶材料により作製される請求項22の装置。 24.前記円筒形フレームが、血管壁内に埋入するようにフレーム上に形成された多 数の突起を有する請求項22の装置。 25.前記フレームがワイヤ要素により構成され、前記突起が前記ワイヤ要素内に形 成された湾曲部により構成される請求項24の装置。 26.前記可撓性シールが、前記円筒形フレームの前記第一の端部上に形成され且つ 血管腔を通過する血液の流れの方向に相対して配備されるものであって、前記シ ールに対して通常血圧を与えることにより、前記円筒形フレームの第二の端部の 更なる拡張が促進され、したがって、前記フレームの第二の端部により血管壁へ与 えられる圧力が増大するものである請求項22の装置。 27.前記円筒形フレームの第二の端部が外向きに開き、その有効形状において、前 記第二の端部が血管の逓減部位における管腔と一致し且つ嵌合する請求項26の装 置。 28.前記シールが、多孔性織物をその上に取り付てなるエラストマーメンブレン膜 である請求項22の装置。 管腔に密着するための第二の展開形状をとるように設計及び形成されるものであ り、 b)前記管腔遮断部が前記交差ワイヤ構造物の周囲に形成された微細孔性メンブレ ン膜からなり、前記交差ワイヤが前記第二の展開形状であるとき、椀状構造が形成 されるように前記メンブレン膜が前記交差ワイヤの周囲に形成されるものである 請求項1の装置。 41.前記装置が前記血管の前記管腔内に配備されるとき、血餅の形成を容易にする 板により構成され、前記内向きに斜傾する細長要素が可撓性織物を円錐形構造が 形成されるようにその上に設けてなり、前記円錐構造が遮断される血液の流れの 方向に位置するように配向させるために装置が設計及び形成されるものである請 求項1の装置。 44.前記環状ばねが前記血管の前記管腔を半径方向に圧迫し、前記管腔内に前記装 置を確実に固定せしめるように設計された請求項43の装置。 45.前記円錐構造がその最も末端頂部に形成された開口部を有し、前記開口部が第 一の閉鎖状態及び第二の開放状態をとるように設計されるものであり、前記第二 の開放状態が、遮断された血液の流れの反対側からの力が前記装置を軸方向に貫 通する場合に適応するものである請求項44の装置。 46.可撓性メンブレン膜をその周囲に設けてなる環状ばねから構成され、前記環状 b)血管遮断障壁が形成されるように前記らせん状コイルの対向端部の周囲に伸展 するように設計及び形成されるものであって、前記らせん状コイルを被覆するエ ラストマーバッグ、 から構成される請求項1の装置。 50.前記エラストマーバッグが選択的に貫通されるものであって、前記装置の前記 らせん状コイルが前記管腔内に軸方句に位置するとき、前記血液の流れが遮断さ れる前記特定部位で再通可能な軸方向通路が決定される請求項49の装置。 51.膨張可能な塞栓器と結合した非膨張性材料からなる硬質キャップから構成さ れ、前記塞栓器が非膨張状態にあるとき、前記硬質キャップ及び塞栓器が前記カテ ーテルの前記管腔に配備されるための大きさと形状を有するものであり、前記塞 栓器が膨張可能なものであるので、前記装置が前記特定部位に配備された後、前記 塞栓器が半径方向に拡張し、前記血管の前記管腔を圧迫して前記装置をその場所 に確実に固定するものである請求項1の装置。 52.補助塞栓器が固定された表面領域を有するように形成され且つ遮断される血 液の流れ方向に配向するように設計され、前記塞栓器が配備されて血液の流れに よる圧縮力を受けるとき、前記塞栓器が前記管腔を前記特定部位において更に半 径方向に圧迫するものである請求項3の装置。 53.a)前記血管嵌入部が、直径方向に圧縮した第一の形状及び直径方向に拡張した 第二の形状をとるように設計された円筒形ワイヤマトリクスからなり、前記マト リクスが第一の圧縮形状にあるとき前記カテーテルの前記管腔内に配備可能なも のであり、前記マトリクスが前記第二の拡張形状にあるとき前記血管の前記管腔 内で確実に配備可能なものであり、更に、 b)前記管腔遮断部が、前記マトリクスの各端部の周囲に軸方向に形成された、血液 の流れを遮断するためのソックスからなり、前記ソックスが血液の流れ方向に配 備されるように前記装置が血管腔内に配備されるとき、前記ソックスが前記マト リクス内へ軸方向に反転し前記第二の拡張形状となるように前記ソックスが前記 マトリクスに接合されるものである請求項1の装置。 54.前記血液の流れが遮断される前記特定部位において再通性の軸方向通路が形 成されるために前記ソックスが選択的に貫通可能なものである請求項53の装置。 55.その周囲に放射状に伸展する多数の突起を有するメッシュワイヤ中央塊を含 んでなり、前記血液の流れが遮断される前記特定部位において装置が確実に固定 維持されるように、前記突起が前記血管の前記管腔内に放射状に埋入されるよう に設計及び形成されてなる請求項1の装置。 56.前記装置の周囲における血餅生成を促進して前記装置の周囲に血管遮断塊を 形成するように前記ワイヤメッシュが設計及び形成される請求項55の装置。 57.前記突起が前記血管の前記管腔内に埋入するように配備されたフックからな る請求項55の装置。 58.第一の低温に晒されてカテーテル内に配備可能な第一の圧縮形状をとり、第二 の高温に晒されて前記血管の前記管腔を半径方向に圧迫し且つ該管腔に接着する ための第二の拡張形状をとるような形状記憶材料により作製される請求項55の装 置。 59.エラストマー被覆内に収容される球形コイルを含んでなり、第一の圧縮形状及 び第二の拡張形状をとるように前記球形コイルが設計及び形成されるものであり、 前記圧縮形状に維持された状態で、前記カテーテルの前記管腔内へ配備可能なも のであり、前記拡張形状に維持された状態で放射状に拡張して前記カテーテルの 前記管腔を圧迫することにより、前記血液の流れが遮断される前記特定部位に確 実に固定維持される請求項1の装置。 60.前記球形コイルが、第一の低温に晒されたとき前記第一の圧縮形状をとり、第 二の高温に晒されたとき前記第二の拡張形状をとるような熱膨張性材料により構 成される請求項59の装置。 61.前記球形コイルが、前記エラストマ一被覆を介して放射状に伸展する多数の細 長突起を有してなり、前記血管の前記管腔内に埋入し、前記血液の流れが遮断され る前記特定部位に該装置が固定維持されるように前記突起が設計及び形成されて なる請求項59の装置。 62.前記血管嵌入部が、中央部及び第一並びに第二の対向端部を有する円筒形管状 構造物からなり、前記第一及び第二の対向端部が前記中央部の直径よりも大きい 直径を有するものであって、このことにより、前記血管の前記管腔内に軸方向に配 備されたとき、前記エラストマーシールが血管遮断障壁を形成し、前記第一及び第 二の対向端部が前記血管の前記管腔を半径方向に圧迫して前記血液の流れが遮断 される前記特定部位において装置が確実に固定維持されるように前記第一及び第 二の対向端部が設計及び形成されるものである請求項1の装置。 63.前記円筒形管状構造物がそれら各々の中点で互いに連結する多数の支柱によ り構成され、前記支柱がそれらの中点で連結することにより、前記支柱の各端部が 放 射状外向きに斜傾して前記血管の前記管腔を圧迫する請求項61の装置。 64.前記支柱がそれらの各中点で連結することにより、前記円筒形管状構造物が第 一の陥没形状をとることが可能になり、この形状によって第一及び第二の対向端 部の直径が減少し、装置がカテーテル管腔へ配備及び回収可能となる請求項63の 装置。 65.前記支柱が互いに概ね平行に配列することにより前記円筒形管状構造物が前 記第一の陥没形状をとるものである請求項64の装置。 66.外部被覆内に収容される内部核を含んでなり、前記内部核が第一の圧縮形状及 び第二の拡張形状を選択的にとるように設計されるものであって、前記第一の圧 縮形状においてカテーテル管腔へ配備され、前記第二の拡張形状において前記特 定部位を半径方向に圧迫して確実に保持固定される請求項1の装置。 67.内方成長に抵抗し且つ周囲の血液凝固を防止する滑らかな外部表面を有する 材料により外部被覆が作製されてなる請求項66の装置。 68.熱膨張性材料で充填されたバルーンからなり、前記バルーンが第一の陥没形状 及び第二の拡張形状を選択的にとるものであって、前記熱膨張性材料が前記第一 の陥没形状をとるとき前記バルーンが前記カテーテルの前記管腔に配備可能なも のであり、前記第二の拡張形状に維持されるとき、前記バルーンは前記血管の前記 管腔内で放射状に拡張して血管遮断塊を形成し且つ前記特定部位に確実に固定保 持されるものである請求項1の装置。 69.第一の低温に晒されたとき前記第一の陥没形状をとり、第二の高温に晒された とき前記バルーンが前記第二の拡張形状をとる請求項68の装置。 70.体温まで加熱されたとき前記第二の拡張形状をとる請求項69の装置。 71.前記バルーンが、内方成長に抵抗し且つ周囲の血液凝固を防止する滑らかな外 部表面を有する可撓性材料から作製されてなる請求項68の装置。 72.a)血管の管腔の一区域に沿って選択的に配備可能な第一及び第二のバル一ン カテーテルを含んでなり、前記第一及び第二のバルーンは前記区域内の血液流動 が遮断されるように膨張可能なものであり、更に、 b)前記第一及び第二のバルーンカテーテルの間に配備された中央管腔を含んでな り、前記中央管腔は塞栓手段を注入するための多数の開口部を設けてなり、前記塞 栓手段は前記血管の前記管腔の周囲に血管遮断障壁を形成するものである請求項 1の装置。 73.前記第一及び第二のバルーンが、そこから延びる少なくとも一つの側枝管を有 する血管腔の一区域内に配備可能なものであって、前記第一及び第二のバルーン カテーテルの間に配備された前記管腔が前記少なくとも一つの側枝管を通過する 血液の流れを遮断するための塞栓手段を注入するように設計及び形成されるもの である請求項72の装置。 74.前記塞栓手段の注入に先だって前記対照媒体が前記第一及び第二のカテーテ ルバルーンの中間に配備された前記管腔の前記開口部を介して注入され、前記側 枝管を明確にする請求項72に記載の装置。 75.そこから延びる少なくとも一つの側枝管を有する血管腔の一区域内に選択的 に配備可能な三弁ステントから構成され、前記ステントの選択的配備により前記 血管及びそこから延びる少なくとも一つの側枝管内を通過する血液の流れが選択 的に制御され、前記ステントが圧縮形状をとるように設計及び形成されることに よりカテーテル管腔に配備可能なものであり、前記ステントが第二の拡張形状を とるように設計されるこ とにより管腔の前記区域内に配備されたとき、前記ステントが前記特定部位に確 実に固定維持されるように前記管腔に軸方向の力を与える請求項1の装置。 76.前記カテーテルから放出されるとき、前記血管の前記管腔に接着し且つ凝固し て血管遮断塊を形成する化学的薬剤を含んでなる請求項1の装置。 77.前記管腔に接着してその周囲に血餅を形成する血餅誘発物質からなる薬剤を 含んでなり、前記血餅が前記薬剤と融合して血管遮断塊が形成される請求項76の 装置。 78.血管腔を閉鎖する装置であって、 末端部を有する細長探針要素と、 該探針の末端部に隣接する位置で血管壁を陥没させる手段と、 血管壁を融合する血管壁融合手段とからなり、 前記探針が血管腔の封鎖が必要な位置において血管内に挿入可能なものであり、 その後、前記血管壁陥没手段が利用されて探針の末端部に隣接する血管の一部を 陥没させ、その後、前記血管壁融合手段が利用されて陥没領域の血管壁を融合し、 したがって血管腔を閉鎖する装置。 79.前記血管壁陥没手段が、探針を介して縦方向に伸展し且つ少なくとも一つの吸 引口で終結する吸引管腔であり、探針の末端部に隣接する血管腔が吸引されて血 管がその位置で陥没するものである請求項78の装置。 80.前記血管壁陥没手段が、血管壁と連結可能で且つ血管壁を陥没形状にするため に利用される装置である請求項78の装置。 81.前記血管壁融合手段が、接着剤がその中を通過可能な管腔であり、前記管腔は 接着剤放出口において終結し、そのことにより接着剤が該探針を介して注入され て、 血管壁が陥没している領域において該血管腔を封鎖するものである請求項78の装 置。 82.前記血管壁融合手段が、血管壁が陥没する領域において血管を封鎖するための エネルギーを放出するように操作可能な、探針上に形成されたエネルギー放出要 素である請求項78の装置。 83.前記エネルギー放出要素が電極である請求項82の装置。 84.前記電極が単極電極である請求項83の装置。 85.前記電極が双極電極である請求項83の装置。 86.前記少なくとも一つの吸引口及び前記少なくとも一つのエネルギー放出要素 の近位において前記探針上に取り付けられた膨張可能なバルーンを更に含んでな り、膨張した前記バルーンが血管腔を実質的に遮断し、そこを通過する血液の流れ を妨げるように前記バルーンが形成されてなる請求項79の装置。 87.血管腔を閉鎖する装置であって、 管腔間に挿入可能な細長探針を含んでなり、該探針は末端部に取り付けられた取 り外し可能な核要素、及び探針を介して縦方向に伸展するエネルギー伝播通路を 有し、前記探針が血管腔内へ挿入可能なものであることにより、血管腔の閉鎖が必 要な部位に核要素が配備されるものであり、続いてエネルギーが探針要素を介し て伝播可能なものであるので血管壁が該核要素の周囲で収縮し且つ該核要素と嵌 合し、したがって血管腔が封鎖されるものであって、その後、前記核要素が前記探 針要素から取り外し可能なものであるので血管の収縮した領域内に移植された核 要素をその場に残して探針要素が体内から引抜かれ且つ除去されるものである装 置。 88.血管腔を封鎖する装置であって、 内部を縦方向に伸展し且つ末端開口部で終結する中空管腔を有する細長カテーテ ルと、 カテーテルの末端開口部から進出可能なエネルギー搬送要素とを含んでなり、 前記カテーテルが血管内を、カテーテルの末端部が、血管が閉鎖されるべき部位と 隣接する位置まで進行可能なものであり、多量の非蛋白性エネルギー伝導流体が カテーテル末端部に隣接する血管腔を充填するようにカテーテル管腔を介して注 入可能なものであり、前記エネルギー搬送要素がカテーテルの開口部からエネル ギー伝導流体中へ進出可能なものであり、このことによりエネルギー搬送要素か ら搬送されたエネルギーはエネルギー伝導流体により周辺血管壁へ伝播され、し たがってその位置で血管の収縮及び閉鎖を生じるものである装置。
