Abstract

L'objet de l'invention est une prothèse de hanche notamment pour le traitement de fractures cervicale vraie, basi-cervicale, per-trochantérienne, cervico-trochantérienne, trochantéro-diaphysaire et sous trochantérienne, comprenant une tige fémorale (10), un col (18) support, une tête (12) prothétique, caractérisée en ce que la tige fémorale (10) comprend un corps (16) rectiligne, de longueur adaptée pour prendre une appui diaphysaire.

Description

PROTHÈSE DE HANCHE, NOTAMMENT POUR FRACTURE PROXIMALE DU FEMUR ET ANCILLAIRE DE POSE
La présente invention concerne une prothèse de hanche, notamment dans le cas de fracture proximale et plus particulièrement pour un sujet âgé. L'invention couvre aussi l'ancillaire nécessaire. Les fractures du fémur sont généralement traitées par ostéosynthèse, par 5 exemple au moyen d'ensembles vis/plaque ou de clous. Dans certains cas de fractures au niveau du col du fémur et à l'appréciation du chirurgien, il a déjà été utilisé des prothèses de hanche telles que les prothèses utilisées en arthroplastie. Il faut que le trait de fracture corresponde sensiblement à la résection généralement pratiquée en arthroplastie. 10 En effet, chez le sujet âgé plus particulièrement, il est nécessaire d'éviter les conséquences redoutables engendrées par le décubitus et de permettre un retour à une vie normale le plus rapidement possible. De plus, il est inenvisageable d'imposer à un tel sujet une marche avec esquive d'appui ou pire, sans appui. 15 Pour la fixation des prothèses, certains praticiens utilisent souvent du ciment notamment dans le cas des sujets âgés car ces praticiens estiment que la repousse osseuse et le développement des tissus sont réduits. Ce scellement au ciment peut conduire à des effets secondaires tels que des chocs au ciment dus notamment à la polymérisation dudit ciment et/ou des 20 descellements par dégradation de ce ciment.
Une solution efficace recherchée est une fixation mécanique et biologique, évitant le recours à un ciment, dont l'indication traumatologique est étendue à toutes les fractures de la région supérieure du fémur. L' invention consiste à prévoir une prothèse comportant une tige fémorale avec un appui distal diaphysaire, éventuellement complété par un verrouillage. Une prothèse totale de hanche comprend une tige fémorale avec sa tête et un cotyle destiné à recevoir ladite tête. En effet, en fonction des besoins, des patients, des pratiques chirurgicales, il pourra ou non être prévu la pose d'un cotyle simultanément à la pose de la tige fémorale.
De fait, le cotyle n'intéresse pas directement la présente invention. La présente invention concerne la tige fémorale et sa tête du point de vue géométrie, la nature des matériaux important peu, sachant que ces matériaux devront nécessairement être biocompatibles et devront répondre aux critères de résistance mécanique imposés par l'application.
Cette tige fémorale dans les prothèses connues pour l'arthroplastie dispose d'un corps prévu pour être impacté afin de le faire pénétrer dans le canal médullaire du fémur après résection adaptée de la tête du fémur. Ces prothèses s'appuient en partie haute du fémur, car les interventions sont pratiquées sur des patients dont le fémur n'a pas de traumatisme et un appui 20 métaphysaire est adapté. La présente invention prévoit une tige fémorale longue de façon à reporter et à répartir de façon homogène les efforts sur la partie saine du fémur, éloignée du traumatisme, en appui diaphysaire. Ainsi, la tige fémorale selon l'invention est droite pour suivre le canal médullaire 25 et pénétrer distalement. La tige une fois introduite dans le canal médullaire doit être stabilisée et c'est aussi un des buts de la présente invention de proposer une solution adaptée et des variantes.
La tige fémorale de la prothèse de hanche selon l'invention vise aussi à éviter toute rotation de cette tige de prothèse dans ledit canal médullaire. De plus, la prothèse autorise aussi un verrouillage dont le but est au moins une stabilisation verticale interdisant tout déplacement distal susceptible par exemple de provoquer une fracture de l'os sous un effet de tulipage. La prothèse de hanche selon l'invention répond aussi aux problèmes liés aux gammes de prothèses nécessaires pour répondre aux différentes morphologies. La prothèse selon l'invention permet un très grand nombre de combinaisons avec un nombre réduit d'éléments.
La prothèse de hanche selon la présente invention est maintenant décrite en détail suivant un mode de réalisation particulier, non limitatif. Ce mode de réalisation est décrit en regard des dessins annexés dont les différentes figures représentent: - figure 1 : une vue en perspective de la prothèse de hanche selon 15 l'invention, - figure 2 : une vue en élévation latérale de la tige fémorale de la prothèse de hanche représentée figure 1, - figure 3 : une vue en coupe longitudinale de la tige fémorale de la prothèse de hanche selon l'invention, en place, 20 -figure 4 : une vue en coupe transversale suivant la ligne 4-4 de la figure 2, - figure 5 : une vue d'une vis d'immobilisation et son ancillaire, -figures 6A à 6F : des vues schématiques de fractures susceptibles de recevoir la prothèse de hanche selon l'invention. La prothèse de hanche de la figure 1 comprend de façon connue une tige 25 fémorale 10 équipée d'une tête 12 prothétique, cet ensemble étant prévu pour être rapporté sur le fémur et un cotyle non représenté, prévu pour être rapporté sur le bassin. Ce cotyle peut aussi être supprimé au profit d'une cupule rapportée sur la tête afin de rattraper le diamètre anatomique et de lui permettre de coopérer avec le cotyle naturel du patient. Telle que représentée sur les figures 2 et 3, la tige fémorale 10 comprend un corps 16 destiné à coopérer avec le canal médullaire du fémur et un col 18 5 support de la tête 12. L'axe longitudinal du col 18 support de la tête 12 fait un angle avec l'axe longitudinal de la tige fémorale 10 proprement dite, ceci de façon connue. Le corps 16 de la tige fémorale 10 est rectiligne ainsi qu'elle apparaît sur les figures 1 et 2. 10 La section transversale de la tige fémorale 10, telle que représentée figure 4, s'inscrit dans une enveloppe sensiblement rectangulaire, avec des côtés présentant éventuellement une très légère convexité, obtenue par exemple lors du polissage. Dans au moins un des angles de cette section, en l'occurrence à chacun des 15 quatre angles, il est prévu une rainure 20 longitudinale qui se prolonge comme cela est visible sur les figures 1 et 2, sur toute la partie inférieure du corps 16 de la tige fémorale 10. Une telle rainure 20 longitudinale génère au moins deux arêtes 22 destinées à éviter tout déplacement relatif en rotation de ladite tige par rapport au fémur. 20 La partie supérieure du corps 16 de la tige fémorale 10 est munie d'un trou 24 débouchant. La partie inférieure du corps 16 de la tige fémorale 10 comprend au moins un trou 25 traversant, en l'occurrence deux trous 25 traversants de clavetage. A hauteur du trou 24, au droit de l'extrémité supérieure du corps 16 et à 25 l'extrémité du col 18 support de tête, au droit du point d'inflexion intérieur, il est prévu au moins un cran 26, en l'occurrence une série de crans. En partie supérieure du corps 16 de la tige, il est prévu un trou 28 borgne destiné à recevoir le pied 30 d'un col 18 support de tête. Ce trou 28 borgne est conique tout comme le pied 30, de profils conjugués de façon à obtenir un emboîtement conique autobloquant. Un suralésage 31 ménagé autour du trou 28 borgne permet d'augmenter la stabilité du montage. De plus, ce suralésage est dissymétrique par rapport au 5 trou borgne de façon à assurer un rôle de détrompeur. L'extrémité 32 du col 18 destinée à recevoir la tête est également conique, la tête 12 prothétique disposant également d'un trou de profil conjugué, non représenté. Cette extrémité 32 du col 18 est munie d'un taraudage 34 qui permet d'exercer 10 une traction sur le col afin de le retirer du corps 16 de la tige 10, si nécessaire. Un tel moyen évite tout marquage sur les pièces. On note dans la représentation des figures 1 et 2, que chaque tige 10 fémorale comprend un jeu de cols 18 support, en l'occurrence un jeu de trois cols 18-1, 18-2, 18-3, de longueurs différentes. Chaque col est supérieur au précédent d'une 15 longueur x. De même, il est possible de prévoir des têtes 12 prothétiques qui ont des diamètres adaptés. Les têtes 12, pour un diamètre donné, disposent de plusieurs profondeurs d'alésage y. Ainsi les combinaisons des longueurs de cols 18 avec une progression 20 x et avec les profondeurs y d'alésage des têtes permettent de réaliser des combinaisons avec une progression précise. Pour des valeurs de x de 6 mm et des valeurs dey de 4 mm, on arrive à une progression de 2mm en 2mm. A partir d'un même corps 16 de tige fémorale, on peut adopter des combinaisons 25 différentes en fonction des besoins anatomiques. En partie supérieure 36 du corps 16 de la tige fémorale 10, il est ménagé un taraudage 38 et un suralésage 40 de façon à recevoir un impacteur, non représenté, utilisé pour la pose. L'axe de ce taraudage 38 est sensiblement confondu avec l'axe longitudinal de la tige. Sur la figure 3, on a représenté la tige fémorale 10 impactée dans le canal médullaire du fémur.
On remarque que, du fait de la géométrie de la tige et de la multiplicité des arêtes 22, l'agencement génère de nombreux points de contact. Les points de contact sont répartis sur une portée de plus grande longueur, assurant ainsi une répartition des efforts sur une longueur d'os plus importante. De plus, les points de contact de l'extrémité distale se retrouvent éloignés du traumatisme, reportant aussi une grande partie des efforts loin de ce traumatisme, dans une zone de l'os particulièrement saine. La mise en place de cette tige fémorale consiste à préparer le fémur grâce à des râpes adaptées de façon connue puis à utiliser un impacteur, les chocs répétés permettant la mise en place.
Le chirurgien peut procéder aux différents essais de col 18-1, 18-2, 18-3 et de tête 12 prothétique puis une fois la combinaison retenue, ces éléments sont emboîtés à force, la portée conique assurant le blocage sur une translation de longueur négligeable, ne modifiant pas la géométrie retenue. Une fois le corps 16 de la tige introduit, les arêtes 22 issues des rainures 20 20 assurent l'immobilisation en rotation. Afin d'assurer une immobilisation en enfoncement vertical pour éviter tout effet de tulipage qui risquerait de fracturer l'os, il est prévu une clavette 42 transversale destinée à coopérer avec chaque trou 25 traversant. Cette clavette 42 est une vis 44. 25 Cette vis 44 de clavetage comprend de façon avantageuse, un fût 46 dont l'extrémité est équipée d'une pointe 48 auto foreuse suivie d'un filetage, une tête 50 incluant une rondelle 52 fixe d'appui.
La tête 50 comprend des pans de façon connue, pour assurer l'entraînement en rotation ainsi qu'un taraudage 54 central. Ce taraudage 54 central est prévu pour recevoir l'extrémité d'un axe 56 fileté d'une clé 58 de vissage constituant un ancillaire de pose de cette vis de 5 clavetage. Cette clé 58 de vissage comprend une douille 60 prévue pour coopérer avec la tête 50 et ses pans. L'axe 56 fileté est monté libre en rotation et immobilisé en translation. L'axe fileté est solidaire d'un bouton 62 de manceuvre, en l'occurrence un bouton moleté.
10 Cet agencement permet de rapporter la vis 44 sur l'outil durant la phase de mise en place et éventuellement de permettre son déverrouillage. Le praticien place la tête 50 de la vis 44 à l'entrée de la douille 60. L'extrémité de l'axe 56 fileté amorce le vissage dans le taraudage 54 ménagé dans la tête de vis.
15 L'action en rotation sur le bouton 62 provoque le vissage de cet axe 56 dans le taraudage 54 provoquant l'introduction simultanée de la tête 50 dans la douille 60. La vis est ainsi solidaire de la clé 58, facilitant la manceuvre. Le praticien doit repérer le point de perçage et la direction de perçage grâce à 20 un pointeur (non représenté), ceci de façon connue pour passer à travers le trou 25 réservé dans le corps 16 de la tige 10 fémorale. La rotation de la clé 58 permet le forage et le vissage de la pointe 48 du fût 46 de la vis 44. Le fût à travers le trou 24 assure le clavetage de la tige 10 fémorale par rapport à l'os l'extrémité de ladite vis venant dans l'os cortical 25 après être passée à travers le trou 25 de clavetage de la tige. La rondelle intégrée vient en butée. Dans le cas où le traumatisme voit une fracture trochantérienne, il est possible d'assujettir un lien 64 au grand trochanter, ce lien venant en appui de traction dans un des crans 26. Ainsi, la tige fémorale 10 sert d'ancrage du massif trochantérien. Le lien 64 peut aussi être assujetti à la tige fémorale par passage à travers le trou 24 La prothèse avec sa tige fémorale selon l'invention répond ainsi à la 5 problématique multipoints exposée en préambule. On note aussi que la pose fait appel à des techniques connues qu'il convient d'adapter. Sur les figures 6A à 6F, on a représenté schématiquement les indications de pose d'une prothèse de hanche selon l'invention. - figure 6A : fracture cervicale vraie, 10 -figure 6B : fracture basi-cervicale, - figure 6C : fracture per-trochantérienne, - figure 6D : fracture cervico-trochantérienne, -figure 6E : fracture trochantéro-diaphysaire, et - figure 6F : fracture sous trochantérienne.
15 Ces indications sont très distinctes les unes des autres et la même prothèse trouve application.

Claims (11)

REVENDICATIONS
1. Prothèse de hanche notamment pour le traitement de fractures cervicale vraie, basi-cervicale, per-trochantérienne, cervico-trochantérienne, trochantéro-diaphysaire et sous trochantérienne, comprenant une tige fémorale (10), un col (18) support, une tête (12) prothétique, caractérisée en ce que la tige fémorale (10) comprend un corps (16) rectiligne, de longueur adaptée pour prendre une appui diaphysaire.
2. Prothèse de hanche selon la revendication 1, caractérisée en ce que la tige fémorale (10) dispose d'une section sensiblement rectangulaire, et comprend au moins une rainure (20) longitudinale ménagée dans au moins un angle de ladite section de façon à générer au moins une arête (22).
3. Prothèse de hanche selon la revendication 1 ou 2, caractérisée en ce que le col (18) support de tête (12) est amovible.
4. Prothèse de hanche selon l'une quelconque des revendications précédentes, caractérisé en ce que la partie supérieure du corps (16) de la tige fémorale (10) est muni d'un trou (28) borgne conique et en ce que le col (18) support de tête comprend un pied (30) conique, de profil conjugué de celui clu trou (28) borgne de façon à obtenir un emboîtement conique autobloquant.
5. Prothèse de hanche selon la revendication 4, caractérisée en ce que !a partie supérieure du corps (16) de la tige fémorale (10) comprend un suralésage (31) ménagé autour du trou (28) borgne.
6. Prothèse de hanche selon l'une quelconque des revendications précédentes, caractérisé en ce que l'extrémité (32) du col (18) est munie d'un taraudage (34) de façon à pouvoir exercer une traction.
7. Prothèse de hanche selon l'une quelconque des revendications 25 précédentes, caractérisée en ce que la partie supérieure (36) du corps (16) de l,atige fémorale (10) comprend un taraudage (38) et un suralésage (40) de façon à recevoir un impacteur, l'axe de ce taraudage (38) étant sensiblement confondu avec l'axe longitudinal de la tige fémorale (10).
8. Prothèse de hanche selon l'une quelconque des revendications précédentes, caractérisé en ce que le corps (10) comprend au moins un cran (26).
9. Prothèse de hanche selon l'une quelconque des revendications précédentes, caractérisé en ce que le corps (10) comprend au moins un trou (25) traversant de clavetage.
10. Vis (44) pour la prothèse de hanche selon l'une des revendications précédentes, caractérisée en ce qu'elle comprend un fût (46) dont l'extrémité est équipée d'une pointe (48) auto foreuse suivie d'un filetage, une tête (50) à pans incluant une rondelle (52) fixe d'appui et comportant des pans et un taraudage (54) central, ladite vis (44) étant destinée à coopérer avec chaque trou (25) traversant de clavetage de la prothèse de hanche.
11. Ancillaire de pose associé à la prothèse de hanche selon l'une des revendications 1 à 9, destiné à poser la vis (44) selon la revendication 10, caractérisé en ce qu'il comprend une clé (58) de vissage avec une douille (60) prévue pour coopérer' avec la tête (50) et ses pans, un axe (56) fileté monté libre en rotation et immobilisé en translation, cet axe fileté étant solidaire d'un bouton (62) de manoeuvre.
