Description

COTYL FOR TOTAL HIP PROSTHESIS
The present invention relates to an uncemented acetabulum for a total hip prosthesis.

Total hip prostheses are composed of two parts hinged together, namely a femoral part, that is to say fixed to the femur, and an acetabular part or acetabulum fixed in the pelvis of the wearer of the prosthesis. The acetabulum is fixed either by cementing with an acrylic cement or by a mechanical process such as screwing the acetabulum, fixing the acetabulum by screws, expansion or impaction of the acetabulum in lotos.

While cemented acetabular cups are usually made of a single block of high density polyethylene, non-cemented acetabular cups are composed of two parts, namely an external metal cup, fixed to the bone, and an internal part or insert, in polyethylene, which is articulated with the head of the femoral part of the prosthesis. The polyethylene insert has an external surface of convex, variable shape, adapting to the internal surface of the metal cup of the acetabulum. The concave inner surface of the insert has a hemispherical shape to receive the head of the femoral component.

This insert has, at the base of its convex external part, a flange or an annular rim projecting outwards, in a substantially diametral plane of the spherical joint. The edge of the polyethylene insert may be circular but it is rather frequently asymmetrical so as to be more projecting in one of its parts which is intended to be oriented either upwards or backwards, to effectively oppose the risks of instability and dislocation of the prosthetic head.

The connection between the two parts of the hip prosthesis, namely the external metal cup and the polyethylene insert, must therefore meet three requirements, namely they must be firmly linked to each other, orientable and removable and reusable.

Various means of embedding the polyethylene insert in the cup are currently known. Some are of the geometric type, with a notch provided on both the insert and the cup, others have a circumferential relief snapping into a groove or a circumferential metal wire springing and registering in a groove . These known mounting means have, to varying degrees, several drawbacks. Indeed, if the connection between the insert and the cup is too loose, there is a risk of mobilization of the polyethylene insert, that is to say of the appearance of a clearance between it and the cup, play causing a certain "floating" of the insert. Furthermore, if the connection between the insert and the cup is on the other hand too tight, it is difficult if not impossible to modify the relative position of the insert with respect to the cup without destroying the insert. Furthermore, the notch forecast, if it makes it possible to modify the angular position of the insert relative to the cup from 20 to 200 or from 10 to 100, limits the possibilities of angular orientation of the insert and prevents choosing freely the angular position of the insert in the cup. Finally, the complexity of the mechanism for fixing the insert, according to the previous solutions, significantly increases the cost of manufacturing the acetabulum.

The object of the present invention is to remedy these drawbacks by providing a cup of particularly simple design, making it possible to obtain a perfectly free positioning of the insert in the metal cup, by choosing as best as possible the orientation of the projecting part. of the edge of the insert, a very good hold of the insert in the cup without risk of migration of this insert or modification of its position, thanks to a very effective pliers effect, and the possibility of extracting the insert without damaging it and put it back in position, if necessary changing the orientation of the protruding rim.

To this end, this non-cemented acetabulum for a total hip prosthesis, comprising a metal external cup intended to be fixed to the bone by a mechanical process, and a plastic insert, in particular made of polyethylene, having an external surface complementary to the internal surface of the cup so that it can be force-fitted therein, the insert comprising a spherical internal surface intended to receive the head of the femoral prosthesis and, around its opening, a flange or an external annular rim and transverse intended to come to bear against the annular edge of the cup surrounding the opening thereof, the external surface of the insert comprising a circular cylindrical part extending from its rim, coaxial with the latter, and engaging in a complementary cylindrical part, of the same radius, of the internal surface of the cup extending from its opening, characterized in that q ue the cylindrical part of the inner surface of the cup is corrugated in the circumferential direction and it has, on its corrugations, longitudinal grooves.

An embodiment of the present invention will be described below, by way of non-limiting example, with reference to the attached drawing in which
- Figure 1 is a perspective view of a cup according to the invention comprising a cup and an insert separated from each other, before their mutual nesting.

- Figure 2 is an elevational view, partially in radial section, of the cup and the insert shown in Figure 1, before their mutual engagement.

The acetabulum for total hip prosthesis which is represented in FIGS. 1 and 2, is composed of two parts, namely an outer metallic part or cup 1, intended to come into contact with the bone, and an inner part or insert 2 in polyethylene (or other equivalent plastic) intended to form a joint with the head of the femoral prosthesis.

The cup 1 is made of a metallic or biocompatible material, in particular of titanium or titanium alloy.

La surface cylindrique 5 délimite un bord annulaire la de la cupule 1.It has an external surface 3 which has the shape of a hemisphere with an axis xx 'but which could be constituted by any other form of suitable surface. The cup 1 is hollow and it has an internal surface, of revolution around the axis xx 'and which consists of two parts, namely, in the bottom of the cup 1, a concave and smooth surface 4, for example in shape of spherical cap, of axis xx ', and around its opening, a smooth internal surface of cylindrical base 5, of circular cross section, to which the spherical cap 4 is connected and therefore of the same radius as the base of this cap spherical. The cylindrical surface 5 delimits an annular edge 1a of the cup 1.

The insert 2 has an external shape complementary to the internal shape of the cup 1.

In other words, the convex external surface of the insert 2 comprises a smooth spherical cap 6 of the same radius of curvature and of the same height as those of the internal spherical cap 4 of the cup 1, then a smooth cylindrical surface 7 of the same radius and of the same height as the internal cylindrical surface 5 of the cup 1. Therefore, the insert 2 can fit tightly inside the cup 1. The insert 2 ends in a flange or an annular external rim 8, of axis xx ', which is intended to come to bear against the annular edge 1a of the cup 1 when the insert 2 is fitted therein.

The insert 2 also has a smooth hemispherical internal surface 9, intended to receive the head of the femoral prosthesis.

According to the invention, the internal cylindrical base surface 5 of the cup 1 is corrugated regularly in the circumferential direction, all around the axis of revolution xx ', and this corrugated surface 5 advantageously has longitudinal ridges Sa extending in planes perpendicular to the axis xx '. For the assembly of the two parts of the acetabulum, the insert 2 is slightly forcefully inserted into the cup 1, in the direction of the axis xx ', as shown by the arrows f in FIGS. 1 and 2 This causes a certain deformation of the cylindrical external surface 7 of the polyethylene insert 2 which ensures an effective fixing resistant to tearing and rotation of the insert 2 relative to the cup 1. The corrugated surface 5 and advantageously striated acts as a multitude of anchors in the manner of a multitude of multi-grip pliers opposing the rotation and tearing of the insert.

The corrugated and advantageously striated surface 5 can be produced by contouring with a devastating cutter on a machining and turning center.

This particularly simple and economical method of fixing the insert 2 in the cup 1 also allows the surgeon to modify the angular position of the insert 2 in the cup 1, by extraction and repositioning, without deterioration of the 'insert and without reduction of fixing efficiency.
