ceph-2-em cpds. of formula (I) (where R1 is H, a metal cation, 1-4 C alkyl, phenyl-(1-4C alkyl) or benzhydryl in which the phenyl rings may be substd., or tetrahydropyran-2-yl, R2 and R3 are both H or R2 is -N=CH-R5 (R5 = 1-4C alkyl or opt. substd. phenyl) and R3 is -N-CH-R5, 1-4C alkyl, 2-4C alkenyl, benzyl, 1-4C alkoxy or -COOR6 (R6 = 1-4C alkyl or phenyl-(1-4C alkyl)), and R4 is 1-4C alkyl, 2-4C alkenyl, phenyl-(1-4c alkyl) opt. substd. in the phenyl ring, -COOR7 (R7 = 1-4C alkyl or benzyl) or a group -CO-R8 in which R8 is opt. substd. phenyl or a residue of formula (Ia)) are prepd. by reacting a cpd. of formula (I: R4=H) (in which R1 may not be H) with a base of the formula M-A (where A is H, -OR11 (R11=1-4C alkyl) or -NR9R10 (R9, R10 = 1-6C alkyl, 3-6C cycloalkyl or trimethylsilyl) and M is an alkali atom) and subsequently reacting with a compound of the formula R4-X (where X is Cl, Br or R) or with dimethyl or diethyl sulphate, a compound (I; R1 = alkali cation) opt. being converted into (I; R1=H).
