Description
SET OF FIXED JOINT PROSTHETIC IMPLANT ELEMENTS HAVING A SINGLE AND UNIQUE FORCED POSITION
The present invention relates to a set of elements for osteo- integrated endossea implantology.
More particularly, the invention concerns to a set of elements of the above kind conceived in such a way to make it easy and safe the coupling, without any possibility of coupling error between the assembled components during the copying between patient oral cavity in the doctor studio and the technician to carry out the prosthetic handmade product, and vice versa, and the positioning in a single and unique forced position during the mounting of the prosthetic handmade product within the patient oral cavity.
This invention makes the result of the work extremely precise, avoiding the possible cumulative error due to the doctor, to the technician and to the difficult conditions of the submucosa implants, thus reducing possible professional frictions and disputes, directly increasing the quality of the co-operation between the doctor and the technician with a noticeable reduction of the costs and discomforts with a direct advantage for the patient.
The implantologic sector within which the present invention is inserted is having a large development, and many solutions have been suggested for elements suitably studied to try to satisfy all the needings.
As it is well known, this kind of implantology is carried out in two phases: the first one, or surgical phase, provides the insertion of the implant within the maxillary bone of the patient, and the second one, of prosthetic phase, providing the realisation and the manufacturing and installation of the esthetical/masticator prosthesis resting on the implants inserted within the bone. Rehabilitation problems connected with this implant-prosthetic field are many and are due to the many different situations that can occur: missing dental elements, different anatomical zones, different functional zones, having different anatomical-functional peculiarities and noticeable differences between the various patients. Among the noticeable problems, it is included the possibility of coupling errors of the various elements during the copying of the implantare position from the oral cavity to the technician laboratory and vice versa. This kind of errors can be repeated and cumulated, being it
difficult to correct the same, and if they are not timely noted and corrected it is possible to induce an irreversible damage to the implant, seriously jeopardising the global picture, along with serious complications and discomforts for the patient health. The error can be generated during the surgical insertion phase: due to a high number of implants, variable not parallel direction among the various kinds of implants. The error can be generated during the position impression creation to copying the position of the implant within the oral cavity from the doctor to the dental technician to the gypsum model within the laboratory. The error can be generated during the mounting of the rehabilitative construction on the implantari pillars within the oral cavity.
Probability that the position copying/coupling error occurs increases both with the increasing of the number of implants applied for each rehabilitative structure, and with the increase of the sides of the polygon of each implant. For example, for illustrative but not limitative purposes; in case of rehabilitation of a single and unique missing tooth: central incisive with esthetical-functional needing, in order to avoid damages to the contiguous teeth. The election conservative therapy is for the single and unique crown mono-implant, applying within the maxillary bone a single and unique prosthetised implant with a single and unique stump pin resting on a single and unique crown.
An implant available on the market for these clinical needings is a biphasic cylindrical implant (a. surgical phase, b. prosthetic phase) providing on the occlusal end an inner or outer fixed joint having a polygonal shape. During the impression position phase within the oral cavity and during the manufacturing and mounting phase of the stump pin on the gypsum model within the laboratory, or during the mounting of the stump pin on the single and unique implant within the oral cavity, it is possible that positioning errors occur. It must be taken into consideration the vertiginous increasing, which is not proportional to the error probability factor in relation to the number of polygonal sides of the implant fixed joint of the single and unique implant. In a single and unique fixed joint equilateral triangular implant, wherein the angle between the sides is 360°/3 = 120°, and the error probability is 2 or 3 for each passage, while the proper position is only one, while in a single and unique regular hexagonal fixed joint implant the angle among the sides is 360 6 = 60°, and the error probability is 5 or 6 for each passage; while in a single and
unique regular dodecagon polygonal fixed joint the angle between the 12 sides is of 360 12 = 30°, and the error probability is 11 or 12 for each passage.
Analysing the increase of the error probability for the coupling/copying in the clinical rehabilitation cases of a bridge supported by at least two implant pillars with polygonal inlet. In a bridge supported by two polygonal fixed joint implants having a triangular equilateral shape (3 x 3 - 1) = 8 wrong positioning possibility will exist, with only one proper position. Instead, in a bridge supported by two polygonal fixed joint implants having a regular hexagonal equilateral inlet (6 x 6 - 1) = 35 wrong positioning possibility will exist, with only one proper position. In a bridge supported by two polygonal fixed joint implants having a regular octagonal inlet (8 x 8 - 1) = 64 wrong positioning possibility will exist, with only one proper position. In a bridge supported by two polygonal fixed joint implants having a regular dodecagon inlet (12 x 12 - 1) = 143 wrong positioning possibility will exist, with only one proper position. It can be deduced that the vertiginous not proportional increase of the probable error factor in function of the number of the polygonal sides of the implant fixed joint, and in function to the increase of the number of implants involved in the prosthetic rehabilitation.
The solution offered by the present invention reduces the error possibility during all the working phases at a zero level. In this situation, the Applicant has conceived and realised a new set of elements which is highly specific, allowing to be able to work with the maximum reliability and simplicity for any kind of rehabilitative prosthetic solution on the dental endossei implants, during the copying to the dental technician laboratory, of the single and unique and forced implant fixed joint within the oral cavity, and vice versa.
Furthermore, the solution according to the invention allows to have an implant perfectly osteo-integ rated into the maxillary bone of the patient with a removable or fixed stump, with a single and unique position having an anti-rotation effect with zero probabilities that copying and/or positioning error occurs. The solution according to the invention allows to have a series of implants perfectly osteo-integ rated within the maxillary bone of the patient with a bridge or removable or fixed occlusal meso- and eso- structure or "overdenture", with a single and unique position having an anti-rotational effect with zero possibility of a copying/positioning error.
The solution according to the invention allows to have a copying system from the oral cavity implant to the laboratory model simulating the position of the implant (s) within the oral cavity perfectly within a single and unique position, having an anti-rotational and without errors. The solution according to the invention allows to have a system of surgical insertion of the implant into the maxillary bone of the oral cavity by single and unique position insertion wrench(s) having an anti-rotational effect. The solution according to the invention allows to have a simulation system for the endo-osseus implant with the laboratory with an analogousus implant having a single and unique anti-rotational effect. The solution according to the invention allows to have a screwing system of the prosthetic part to the lower implants in a not parallel condition and position, by the use of the not parallel compensatory/adapter.
It is therefore specific object of the present invention a set of implant-prosthetic elements with a single and unique forced fixed joint position, comprising at least a depth or extension endo-osseus implant, installed within the maxillary bone of the patient, said implant providing a connector neck having a coupling geometrical system with an inner or outer fixed joint with a single and unique forced position, with an anti- rotation effect, corresponding to the neck of said implant, prosecutes within the implant a threaded or unthreaded blunt channel, the implant having a single and unique forced coupling position system with a mono- position bum out stump pin manufactured or fused by titanium or other alloy of a suitable metal, coupled with the implant in a single and unique forced position and fixed to the implant by cement/ dental glue, aiming to be a prosthetic support of the dental crown rest on the implant pillar.
Otherwise, it can be provided a cannulated cylindrical or frusto- conical stump, comprised of titanium or burn out material for its casting, coupable in a single and unique forced position with the implant and fixed to the same by a screw, aiming to be a composable or not prosthetic support of the dental crown rest on the implant pillar. Otherwise, it can be provided a cannulated connector/compensatory, comprised of titanium or burn out material for its casting, coupable in a single and unique forced position with the implant and fixed to the same by a screw, aiming to prosthetically unify/compensate an axial not parallel situation exiting between two or more implants, or between the implant and the natural tooth.
Otherwise, it can be provided a copying system of the position of the implant (s) placed within the oral cavity, to the gypsum model simulating the oral cavity with the dental technician laboratory, by a metal cannulated double fixed joint system, with a single and unique forced coupling position with the implant on a lower part, and in the upper part a single and unique forced fixed joint coupling position with an impression cap, in order to obtain a perfect and precise copying without any error during the copying of the implant position from the oral cavity for the subsequent working of the prosthetic handmade product with the laboratory.
Otherwise, it can be provided a metal implant with inner/outer fixed joint neck, having a single and unique forced position for the simulation on the gypsum model of all the parameters of the endo-osseus implant(s) during the working of the prosthetic handmade product. In a first embodiment of the present invention, said implant and/or components substantially provides an inner or outer coupling fixed joint, having a connector neck provided with geometrical coupling system with a single and unique forced anti-rotation coupling position.
The connector provided with the geometrical single and unique forced coupling position system provides the asymmetrical exploitation of the poli-angular shape, modifying at least one side with respect to the other sides of the polygon, or chamfering the sides of the poli-angular element in a different way each other, or moving at least an eccentric angle with respect to the axis passing through the other angles of the implant fixed joint. To simplify this mono-position, error free coupling concept between the implant and the prosthesis system and/or copying, it will follow, for illustrative, but not limitative purposes, an elliptically egg- shaped fixed joint, having the basis and the opening angle different with respect to the top, or a chamfered, or not chamfered, triangular shaped fixed joint, wherein an inner angle is different with respect to the other two inner angles, or a chamfered, or not chamfered, quadrangular shaped fixed joint, wherein at least an inner angle is different with respect to the other three inner angles, or a chamfered, or not chamfered, pentagonal shaped fixed joint, wherein at least an inner angle is different with respect to the other four inner angles, or a chamfered, or not chamfered, hexagonal shaped fixed joint, wherein at least an inner angle is different with respect to the other five inner angles, and so on, increasing with the
increasing of the number of the angles. Modifying a single and unique inner angle with respect to the other angles of the polygon, thus conditioning any poli-angular shape in a single and unique coupling position with its specular shape. It is a measure that technically allows an absolute precision and simplicity for an error free coupling during any rehabilitation and working prosthetic phase. Furthermore, according to the invention, it can be provided a cannulated connector, having different heights, comprised of titanium or any other bio-compatible material or burn out material for its casting, said connector geometrically providing toward the implant the specular image of the implant fixed joint coupled to the same implant in a single and unique forced position, and fixed to the implant by a screw passing within the connector, while in the direction starting from the occlusal plane, a plane with a through hole is provided, without any fixed joint, aiming to prostheticaJly unify and/or compensate a not parallel situation between two or more implants and the natural tooth.
Compensation of not parallel is obtained when the subject not parallel implants are coupled with connectors provided fiat, without any fixed joint, in the occlusal part, but only plane and holed resting supports, allowing a simple reconstruction on the implants provided below and screwed on the same.
In a first embodiment of the present invention, said depth cylindrically shaped implant, of butterfly extension implant having symmetrical wings, or asymmetrical implants with a single and unique wing, ail these implant shapes provide on the implant neck a polygonal coupling system, that is preferably comprised of an inner threading and a chamfered isosceles triangular fixed joint having inner grooves with different thickness.
Furthermore, according to the invention, the frusto-conical cylindrical stump pin with a single and unique fixed joint position, cast by suitable metal alloy, is geometrically the specular image of the implant isosceles triangular fixed joint, coupled with the implant in a single and unique position and fixed by cement/dental glue, aiming to be the prosthetic support of the dental crown rest on the implant pillar.
Furthermore, according to the invention, a titanium cannulated frusto-conical cylindrical stump to be cast, geometrically is the specular image of the implant isosceles triangular fixed joint, coupled with the implant in a single and unique position and fixed to the implant by a screw
passing within the stump, aiming to be the prosthetic support of the dental crown rest on the implant pillar.
Furthermore, according to the invention, a cannulated cylindrical connector/compensatory having various heights, comprised of titanium or other bio-compatible metal, or burn out material for its casting, geometrically is the specular image of the implant isosceles triangular fixed joint, coupled with the implant in a single and unique position and fixed to the implant by a screw, aiming to prosthetically unify and/or compensate a not parallel situation between two or more implants and the natural tooth.
Furthermore, according to the invention, a coupling - copying system, allowing to have a copying system from the implant within the oral cavity to the laboratory model simulating the position of the implant(s) within the oral cavity in a perfect way, in a single and unique forced position, with an anti-rotational effect and error free.
The copying system is comprised of the following three particulars: a. Copying for metallic implant having a double fixed joint cannulated shape, that in the apical portion restrains within the implant of the analogousus implant, and in the occlusal portion restrains with the impression fixed joint. Fixed joints have a single and unique forced position. b. copying for metallic implant having a cannulated cylindrical and perimetrally milled shape with an anti-rotation effect, while its apical portion restrains in the fixed joint. Fixed joint has a single and unique forced position. c. a long metallic screw, with a blunt 90° head, blocking together the impression fixed joint with the implant copying to the implant during the impression taken. Furthermore, according to the invention, an analogousus of the implant, comprised of metal and having a neck with an isosceles triangular inner fixed joint with a single and unique forced position, in order to obtain a precise copy of the mouth implant on the gypsum model.
Furthermore, according to the invention, a coupling metallic digital wrench, having a specular geometrical shape of the implant isosceles triangular fixed joint, coupled with the implant in a single and
unique position, aiming to introduce/remove the implant from the maxillary bone during the surgical phase.
The present invention will be now described, for illustrative but not limitative purposes, according to its preferred embodiments, with particular reference to the figures of the enclosed drawings, wherein: figures 1a, b and c show the lateral view of the endo-osseus implant for: a. depth, b. symmetrical extension, c. asymmetrical extension.
The difference of the implant shape helps to better exploit the bone anatomy of each case, said anatomy varying for each patient, while the prosthetic coupling is unified and standardised.
Figure 2a is a cross-section of the joint/coupling neck of the implant of figure 1 , while figure 2b is the top view of the same. According to the invention, this geometrical shape provides above a flared isosceles triangular shaped fixed joint having three grooves, as well as an inner thread, in such a way to technically have a prosthetic coupling joint with a single and unique forced position standardised with respect to all the other elements of the set of elements according to the invention.
Figure 3 is an isometric lateral view of the stump pin, showing the section of the isosceles triangle with a single and unique forced position fixed joint. This stump pin is comprised of bum out plastic or of titanium, or other bio-compatible, providing a fixed joint neck 3 corresponding to the implant fixed joint of figure 2.
With respect to what is already known, this stump pin provides two parts: a. male pin coupling with the implant b. frusto-conical cylindrical stump.
Plastic stump pin already realised and modelled on the basis of the anatomy of the missing tooth, is sent to the laboratory for its fusion. Plastic evaporates and a titanium or gold or other alloy pin is realised. Stump pin is fixed to the implant pillar within the oral cavity in a single and unique position of the pin by the cement, and above this the missing esthetical functional crown is realised.
Figure 4 shows a burn out plastic or titanium or other bio- compatible material cannulated stump provided with a fixed joint neck corresponding to the implant fixed joint of figure 2. With respect to what already known, this stump provides two parts:
4' - male cannulated fixed joint, coupling with the implant. 4" - frusto-conical cylindrical stump. Plastic stump previously realised,
modelled on the basis of the anatomy of the missing tooth, is sent to the laboratory for its fusion. Plastic evaporates and a titanium or gold or other alloy stump is realised. Cannulated stump has cannulated isosceles triangular male which is a specular image of the implant fixed joint shown in figure 2 in the apical portion, and is fixed to the implant pillar in a single and unique possible position of the fixed joint for its coupling, while stump is anchored to the implant by a passing screw with a 90° flared head acting as blocking screwed between stump and implant, above this stump the missing esthetical functional crown is realised. Figure 5 is an isometric lateral view of the connector compensating the not parallel, with the possible height variation from 0.5 mm to 10mm, which is indispensable in case of bridge rest on two or more not parallel implant pillars. The cylindrical compensatory connector is coupled from the apical side (5') to the implant and provides a isosceles triangular fixed joint, with a single and unique fixed joint position for the implant, while in the occlusal plane provides a holed plate plane (5") acting as rest plane for the prosthetic structure; it also has a through hole that allows the screwing of the prosthetic structure on the implant by a through screw. Figure 6 is an isometric lateral view of the single and unique position copying system. A copying of the position of the implant within the mouth to the gypsum model with the laboratory: the components of the set of elements is shown, to be used during the copying phase. This important device is comprised of 3 separated parts 6a and of a cannulated metallic cylindrical copying, providing in its apical part a single and unique position isosceles triangular male fixed joint with the implant, while in the crown portion this copying provides a single and unique position isosceles triangular male fixed joint for the perfect coupling with the anti-rotation impression cap 6b. Both devices are separated in a sequential manner from the implant by the 90° flared through screw (6c). Once mounted on the copying fixed joint implant (6a) and on the same the cap of the impression (6b), the whole is fixed by the through screw (6c) and the impression is taken with a precision material, until the same is hardened. The exceeding hardened impression material on the impression cap is taken away and the through screw is unscrewed. Taking away the impression spoon from the oral cavity along with the impression cap englobed within the same impression inserting the single and unique
position fixed joint - implant copying (6a) assembly in the single and unique forced position fixed joint of the impression cap (6b) englobed within the impression and on the other side inserting the analogous sole forced position fixed joint implant (figure 7) provided at the end of the implant copying (6a) fixing and blocking the whole by a through screw (6c). Pouring the gypsum on this assembly the analogous of the implant is obtained (figure 7) englobed within the gypsum in the same identical tridimensional position that the implant will find within the oral cavity.
Figure 7 is a lateral view with the fixed joint section of the laboratory analogous of the implant. The analogous is made up of metal, has a cylindrical neck having the same size and identical isosceles triangular cavity, prosecuting with a threaded blind hole as the implant, while outside, beyond the neck, a grooved hexagonal shaped is provided in order to guarantee an anti-rotational stability within the gypsum. The implant analogous shown in figure 7, with the laboratory allows to those that have to model and realise the prosthesis, to known the implant borders, its depth, its relation with the bone and the gingiva, in such a way to realise a perfect prosthetic handmade product on the basis of precise and proper parameters. Figure 8 is a lateral view of the insertion key. It is a digital key for the insertion of the cylindrical implant with direct force developed by the thumb, with the guarantee of not damaging the implant during the introduction phase. This key provides a male isosceles triangular fixed joint lower part 8' which is a specular image of the inner isosceles triangular fixed joint of the implant neck, while the upper part 8" provides a hexagonal shaped widening of 10 mm, for its handling.
Figure 9 shows a clinical solution of two single and unique implants, both prostetised with a single and unique crown. The first one, on the right side of the patient: R) it is a stump pin cemented in a single and unique position of the implant to support the upper crown, while on the side L it is shown an isosceles triangular single and unique forced position cannulated stump. The latter device allows a prosthetic solution that can be screwed instead of cemented.
Figure 10 shows a bridge rest on two not parallel implant pillars wherein it is evident the indispensability of the not parallel compensatory device, without which the bridge cannot enter in the underlying implant fixed joint. Figure 10 puts into evidence a lateral view of the compensatory

connector coupled with the implants in case of a structure of a bridge rest on two or more not parallel implant pillars. Only a plane rest without inner cavities fixed joints or without outer projecting fixed joints can guarantee an easy and safe removability of the prosthetic superstructure, with respect to the implants, in case it is necessary an intervention by the doctor: for exemplificative purposes, to inspect the implant, for maintenance purposes of the implant and of the bridge after a long time, to introduce modifications to the bridge due to a further loss of other teeth due to implant pathologies requiring this kind of intervention. The present invention has been described for illustrative, but not limitative purposes with particular reference to two preferred embodiments of the endossei implants with their single and unique forced position anti-rotational triangular fixed joint, and the relevant components with their single and unique forced position anti-rotational triangular fixed joint coupled with the implant and their precise copying in a single and unique forced error free position of the implant position from the mouth to the laboratory, and vice versa, but it is to be understood that modifications and/or changes can be introduced by those skilled in the art without departing from the relevant scope as defined in the enclosed claims.
