Abstract

A device for the removal or dispersal of thromboses counts of one or more sound conducting devices inserted into the artery or vein. The sound conducting devices are connected to an ultrasonic generator at their external end. The sound conducting devices are surrounded by a protective pipe and have a suction device mounted within their circumference. The sound conducting devices are thin cannulae which form a hollow cylinder having rounded ends and with the smaller diameter suction device mounted within the cannula bundle. Infusion fluid may be introduced through this device since the cannula bundle is only loosely bound and permits transmission of liquid.

Description

Device for removing thrombi. The invention relates to a Device, which allows thrombi inside veins by means of intense ultrasonic vibrations dissolve and suck off.

So far only a few remedies are known in medicine, thrombi or To remove thrombotic substances effectively and safely: 1. Surgical (embolectomy) 2. Fibrinolysis (Sh treatment) 3. Transluminal catheter dilation Any of these methods has significant disadvantages, e.g.

keine Auflösung älterer Thromben (Embolektomie), Nichtanwendbarkeit bei Hypertonie, Magenulcus, Apoplexie usw. Außerdem muß die Behandlung so kurzfristig wie möglich nach der Thrombenbildung einsetzen.no resolution of older thrombi (embolectomy), inapplicability in hypertension, gastric ulcer, apoplexy, etc. In addition, the treatment must be so short-term as possible after thrombus formation.

All these disadvantages can be overcome with the aid of the device according to the invention for the removal of thrombi, which one or more sound conductors that can be inserted into the vein, which are connected at their rear end to an ultrasonic generator by a protective hose are surrounded and provided with a suction device, largely avoid.

Experiments have shown that relatively young thrombi on the frontal The viscosity of the end of a cannula excited to ultrasonic vibrations can be reduced that a suction through relatively thin cannulas through is possible. In the case of older and correspondingly more consistent thrombi, however this process is no longer successful, since the cavitation required for dissolution occurs can no longer be achieved at the end of the cannula. In such cases the resolution is thereby enables infusion solution to be brought to the location of the ultrasound exposure, which by means of a protective tube surrounding the cannula to the front end or the attack surface of the thrombus is supplied.

The device is expediently handled so that initially the protective tube alone, without a cannula, pushed into the vein up to the thrombus and only afterwards is the thin cannula pushed along the protective tube. This not only ensures easy sliding of the cannula up to the thrombus but also any damage to the vessel walls is avoided with certainty. On that the infusion liquid is given into the protective tube with a slight excess pressure, the cannula excited to ultrasonic vibrations and a negative pressure to suck out the dissolved thrombus is generated in the cannula.

Fig. 1 zeigt ein Ausführungsbeispiel der genannten Vorrichtung.Fig. 1 shows an embodiment of the device mentioned.

3 stellt den Schutzschlauch dar, beispielsweise aus PTFE, in der sich die Kanüle 4 befindet.1 with the wire is referred to, with 2 that located in it and to be dissolved Thrombus. 3 shows the protective tube, for example made of PTFE, in which the cannula 4 is located.

At its frontal end 5, the cannula is rounded, somewhat closed and provided with a hole 6, the diameter of which is smaller than that of the cannula 4 should be. This arrangement has the great advantage that the cannula 4 is with it Safety not clogged and a swift suction towards the bottom in the Figure indicated arrow can be done. To achieve a larger contact area, the front end can be designed to be planar or beaded accordingly. The double arrow at the top of the cannula is intended to indicate that the cannula is in their entirety is excited to longitudinal ultrasonic vibrations. She is, as already described, outside the vein in a known manner with a pierced Ultrasonic vibrator connected. Also located outside the Wire in the protective tube a side feed 7, which offers the possibility of an infusion fluid between the protective tube 3 and the cannula 4 to the Location of the action of ultrasound, i.e. at the point of contact between the end face Bring the end of the probe and the thrombus 2.

Die Fign. 2a und 2b zeigen ein weiteres Ausführungsbeispiel der Vorrichtung. The FIGS. 2a and 2b show a further embodiment of the device.

The vein is again designated with 1 ', the thrombus with 2'.

Inside the arrangement there is a suction hose 8 ', which at its front end through a slight indentation 5 'is slightly rounded and has an opening 6', the diameter of which is again smaller than the inside diameter of the tube 8 '. As an ultrasonic transmitter serve in this case, axially parallel to the suction hose 8 ' Wires 4 ', between which the infusion liquid via a lateral inlet 7' arrives at the location of the ultrasound exposure. The suction no longer takes place here via a centrally arranged ultrasonic conductor but via a flexible hose. It will be understood that this arrangement is much more flexible than that of Fig. 1, which can be very valuable in many cases.
