Abstract

Ein modulares Hüftgelenkprothesensystem, bestehend aus Prothesenschaft (1), Halseinsatz (2) und Gelenkkopf (3), ist geeignet, den Gelenkkopf (3) in Relation zum Prothesenschaft (1) sowohl radial als auch axial zu verstellen. Der Halseinsatz verfügt beidseitig über rundkonische Zapfen, welche in entsprechende konische Bohrungen am Gelenkkopf und am Prothesenschaft einsetzbar sind, sodass alle Systemteile, durch Rundkonus beliebig gegeneinander austauschbar, miteinander verbunden werden können. Das System ermöglicht somit, unterschiedliche Prothesenschäfte, Halseinsätze und Gelenkköpfe zu einer individuellen angepassten Protheseneinheit zu kombinieren und den Prothesenschaft (1) mit gezahnten Randauflagen (9) und optional einsetzbaren Knochenschrauben (6) im Oberschenkelknochen zu verankern.

Description

  • Die Erfindung betrifft eine Vorrichtung entsprechend dem Oberbegriff des Anspruchs 1. Die erfindungsgemäße Konstruktion zeigt eine modulares Hüftgelenkprothesensystem, dessen Bestandteile durch Rundkonusverbindung miteinander kombinierbar sind.
  • Hüftgelenkprothesen mit vordefinierter, starrer Gelenkkopfstellung oder verstellbarem Gelenkkopf sind in unterschiedlicher Ausführung bekannt und dienen im Falle defekter natürlicher Hüftgelenke als Ersatzgelenk zum orthopädischen Einsatz bei Menschen und Tieren.
  • Praktisch allen Modellen ist eigen, daß standardmäßige Gelenkköpfe mit unterschiedlichen Durchmessern auf die konisch geformten Zapfen der jeweiligen Prothesenhälse einfach aufgesetzt werden können.
  • Neuere Konstruktionen zeigen auch austauschbare Halseinsätze, die in unterschiedlichen Längen zur Verfügung stehen und in den Prothesenschaft, beispielsweise durch Ovalkonen unverdrehbar einsteckbar sind.
  • Die Applikation der Prothese erfolgt durch Abtrennen des Oberschenkelknochens unterhalb des Gelenkkopfs. Der Prothesenschaft wird zunächst in den geöffneten und entsprechend erweiterten Markkanal des Oberschenkelknochens eingesetzt. Da der Prothesenschaft je nach Zustand der Knochensubstanz mehr oder weniger tief in den Markkanal eindringt, um eine ausreichende Verbindung mit dem Femur zu erreichen, ist es sinnvoll den Gelenkkopf, welcher auf einen seitwärts auskragenden Zapfen des Prothesenschafts aufmontiert wird, durch entsprechende Verstellbarkeit in Relation zum Prothesenschaft in die orthopädisch günstigste Position zur beckenseitig eingesetzten Gelenkpfanne bringen zu können.
  • Die erwähnten Konstruktionen sind in Bezug auf ihre radiale Verstellbarkeit ungenügend, sodaß nur durch eine Vielzahl, in ihren Abmessungen, Winkelstellungen oder Formgebungen, unterschiedlicher Halseinsätze die erforderlichen Verstellpositionen abgedeckt werden können.
  • Aufgabe der Erfindung ist es, eine modulares Prothesensystem, bestehend aus Prothesenschaft (1), Halseinsatz (2) und Gelenkkopf (3) zur Verfügung zu stellen, welches es ermöglicht, den Gelenkkopf (1) sowohl axial als auch radial in Relation zum Prothesenschaft zu verstellen und in der geeigneten Position unverrückbar zu fixieren. D. h. es geht grundsätzlich um die künstliche Rekonstruierbarkeit der Gelenkgeometrie anhand der zur Verfügung gestellten Systemteile der Prothese unter Einsatz möglichst weniger, aber radial mehrfach verstellbarer Halseinsätze.
  • Die Aufgabe wird durch ein modulares Hüftgelenkprothesensystem mit den Merkmalen des Anspruchs 1 gelöst.
  • Das erfindungsgemäße modulare Hüftgelenkprothesensystem besteht aus 3 Systembauteilen: dem Prothesenschaft (1), dem Halseinsatz (2) und dem Gelenkkopf (3). Die Erfindung bezieht sich hauptsächlich auf die neuartige Gestaltung des Halseinsatzes (2) und seine radial verstellbare Fixierung im Prothesenschaft.
  • Der erfindungsgemäße Halseinsatz (2) weist an beiden Enden einen Rundkonus auf, welcher sowohl prothesenschaftseitig als auch gelenkkopfseitig in eine entsprechend konische Aufnahmebohrung einsteckbar ist und eine spielfreie und einfach applizierbare Konusverbindung zwischen den beteiligten Bauteilen ermöglicht.
  • Als wesentliche Besonderheit der erfindungsgemäßen Konstruktion wird ein Halseinsatz (2) vorgeschlagen, welcher sowohl prothesenschaftseitig als auch gelenkkopfseitig über einen Rundkonuszapfen verfügt und zwischen den Rundkonuszapfen vorzugsweise eine Abwinkelung aufweist, um den dort aufgesetzten Gelenkkopf (3) in eine exzentrische Position zur Verstellachse zu bringen.
  • Um den Halseinsatz (2) mit dem aufgesetzten Gelenkkopf (3) gegen Verdrehen zu sichern, ist dieser zwischen den konischen Enden mit einem vorzugsweise sternförmigen Zahnkranz (4) ausgestattet, welcher in eine entsprechende Nutkrone (5) am Rand der prothesenschaftseitigen, konischen Aufnahmebohrung (10) eingreift. Dadurch ergibt sich für den Anwender die Möglichkeit, den Halseinsatz (2) durch radiales Umstecken in eine Vielzahl von Stellungen zu bringen und ohne weiteren mechanischen Einsatz gegen Verdrehen zu sichern. Die Zeichnung zeigt beispielhaft eine 12-teilige radiale Verstellbarkeit, also umlaufend in Schritten von 30°.
  • Die axiale Verstellbarkeit des Gelenkkopfs (3) erfolgt durch Einsatz entsprechend verlängerter Halseinsätze (2). Damit ist eine 3-dimensionale Verstellbarkeit mit relativ genau einstellbaren Verstellpositionen mit ausreichendem Verstellvolumen möglich.
  • Um im Falle problematischer Knochensubstanz zusätzliche Verankerungsmöglichkeiten zur Verfügung zu stellen, weist der Prothesenschaft (1) in axialer Verlängerung zur Aufnahmebohrung (10) eine Durchgangsbohrung (7) mit Anschlag für den Kopf einer Knochenschraube (6) auf. Eine weitere Verankerungsmöglichkeit durch eine zweite, parallel verlaufende Knochenschraube (6) ergibt sich durch eine weitere Durchgangsbohrung (8) neben der Aufnahmebohrung (10) im oberen Teil des Prothesenschafts (1), sodaß beide Knochenschrauben aus Gelenkrichtung applizierbar sind.
  • Desweiteren verfügt der Prothesenschaft (1) über 2 gezahnte Randauflagen (9), welche so angeordnet sind, daß sie auf der Kortikalis aufliegen, wo eine besonders stabile Knochensubstanz zur Verfügung steht, die höheren Belastungen standhält.
  • Das distale Ende des Prothesenschafts (1) ist einerseits relativ kurz, aber andererseits blattförmig ausgebildet, um bei minimalem Resektionsvolumen, auch im Bereich der Spongiosa ausreichenden Halt zu bieten.
  • Ein weiterer Vorteil des modularen Prothesensystems ist darin zu sehen, daß es im Falle eines nachträglichen Eingriffs, ohne Ausbau des Prothesenschafts (1) erlaubt, eine aktualisierte Einstellung der Gelenkgeometrie vorzunehmen, also die Gelenkkonstruktion mit einem relativ einfachen Eingriff erneut zu justieren.
  • Die erfindungsgemäße Konstruktion erlaubt es auch grundsätzlich, die Krafteinleitung vom Beckenknochen zum Oberschenkelknochen sowohl auf den, wie beschrieben, im Markkanal verankerten, keilförmigen Prothesenschaft (1), als auch auf die beschriebenen Randauflagen (9) im Bereich der Kortikalis sowie ggf. auf die eingesetzten Knochenschrauben (6) überzuleiten.
  • Ziel ist dabei, die durch die Prothese aufgenommene Krafteinleitung auf mehrere und belastbare Verankerungsstellen am Oberschenkelknochen, aber hauptsächlich stumpf auf die Kortikalis zu übertragen, sodaß im Bereich der Spongiosa, die für die Gesunderhaltung der Knochensubstanz wichtige Durchblutung mit Nährflüssigkeiten gewährleistet bleibt.

Modulares Hüftgelenkprothesensystem bestehend aus Prothesenschaft (1), Halseinsatz (2) und Gelenkopf (3), dadurch gekennzeichnet, daß beidseitig mit Rundkonuszapfen ausgestattete Halseinsätze (2) eine radial und axial verstellbare Verbindung von Prothesenschaft (1) und Gelenkkopf (3) bilden und der Prothesenschaft (1) über gezahnte Randauflagen (9) und nutzbare Durchgänge für dort optional einsetzbare Knochenschrauben (6) verfügt.
  2. Modulares Hüftgelenkprothesensystem bestehend aus Prothesenschaft (1), Halseinsatz (2) und Gelenkopf (3), nach Anspruch 1 dadurch gekennzeichnet, daß der Halseinsatz (2) im Bereich zwischen den Rundkonen vorzugsweise einen umlaufenden, sternförmigen Zahnkranz (5) aufweist, welcher geeignet ist, in entsprechende Nutaussparungen am Rand der prothesenseitigen Aufnahmebohrung (10) radial verstellbar einzugreifen und dadurch den Halseinsatz (2) gegen radiale Verstellung zu sichern.
  3. Modulares Hüftgelenkprothesensystem bestehend aus Prothesenschaft (1), Halseinsatz (2) und Gelenkopf (3), nach Anspruch 1 dadurch gekennzeichnet, daß die konisch ausgebildete Bohrung im Prothesenschaft (1) an ihrem Rand eine Nutkrone (4) aufweist, in welche der Zahnkranz (5) des Halseinsatzes (2) radial verstellbar eingefügt werden kann.
  4. Modulares Hüftgelenkprothesensystem bestehend aus Prothesenschaft (1), Halseinsatz (2) und Gelenkopf (3), nach Anspruch 1 dadurch gekennzeichnet, daß der Halseinsatz (2) im Bereich zwischen dem gelenkkopfseitigen Konuszapfen und dem umlaufenden Zahnkranz (5) abgewinkelt ist.
  5. Modulares Hüftgelenkprothesensystem bestehend aus Prothesenschaft (1), Halseinsatz (2) und Gelenkopf (3), nach Anspruch 1 dadurch gekennzeichnet, daß die Die Achsen der beiden Konuszapfen des Halseinsatzes (2) seitlich gegeneinander versetzt angeordnet sind.
  6. Modulares Hüftgelenkprothesensystem bestehend aus Prothesenschaft (1), Halseinsatz (2) und Gelenkopf (3), nach Anspruch 1 dadurch gekennzeichnet, daß der Halseinsatz (2) gelenkseitig unterschiedliche Längen aufweisen kann, welche geeignet sind, den Gelenkkopf (3) in axialer Richtung zu verstellen.
  7. Modulares Hüftgelenkprothesensystem bestehend aus Prothesenschaft (1), Halseinsatz (2) und Gelenkopf (3), nach Anspruch 1 dadurch gekennzeichnet, daß der Prothesenschaft (1) über gezahnte Randauflagen (9) verfügt, welche im Bereich der Kortikalis aufliegen.
  8. Modulares Hüftgelenkprothesensystem bestehend aus Prothesenschaft (1), Halseinsatz (2) und Gelenkopf (3), nach Anspruch 1 dadurch gekennzeichnet, daß der Prothesenschaft (1) über Durchgangsbohrungen (7 und 8) aus Gelenkrichtung verfügt, welche geeignet sind optional Knochenschrauben (6) zur Unterstützung der Verankerung einzusetzen.
