Abstract
本发明公开了一种自固化磷酸钙根管填充剂及其应用方法。本发明将磷酸钙盐与合适的显影剂混合,并且引入具有治疗效果的药物,特别是抗厌氧菌药物,然后用无水无毒溶剂调和成牙膏状或稠糊状,将此稠糊状物灌入到注射器中,即可用于牙根管的注射治疗。整个糊剂的主要成分为磷酸钙复合粉末。在水溶液中呈中性,其变慢过程是缓慢的中性水化反应过程,没有其他的刺激物,水化后的最终成分为羟基磷灰石,与人体硬组织的无机成分相似,生物相容性好,无刺激性。因此在填入根管后即使超充也不会产生根充反应,超充于牙槽骨中的部分可逐步被牙槽骨吸收,引导新生骨的形成,其效果优于现有的其他类型根管充填产品,改变了根管填充的传统概念。

技术领域
本发明属于医用材料领域,涉及一种治疗牙髓炎、牙尖周炎等疾病的新型根管填充剂。
背景技术
根管充填是治疗根尖周炎和牙髓病的有效方法之一,治疗中根管充填材料的性能直接影响着根管充填的效果,成为根管治疗成功的关键。随着技术的进步,传统的酚醛类充填材料逐渐被摒弃,新的糊剂类材料在临床获得了应用。目前研究和临床应用中涉及较多的有以下几种:
1 氧化锌丁香油糊剂 氧化锌糊剂在临床上的应用已有较长的时间,其中的丁香油酚具有抗菌性能,对多数兼性菌和少数厌氧菌均有不同程度的抑制作用(Torabinejad M,et al.,Jendod,1995,21(8):403),然而对其毒性的进一步研究表明,氧化锌糊剂达不到理想的根管充填材料所要求的惰性和无副作用;
2 Ca(OH)2根管充填糊剂 文献(Trope M.,Endod DentTraumatol,1992,8(5):183)报道氢氧化钙可使局部环境呈偏碱性状态,降低吸收细胞的活力,促进硬组织形成;另外钙离子可与牙本质中的磷灰石基质反应形成磷酸钙晶体,封闭牙本质小管,减少细菌的存在。氢氧化钙的细胞毒性小于氧化锌丁香油糊剂,具有可吸收性,但其只能用于年轻恒牙的根尖诱导成形;
3 碘仿糊剂 碘仿糊剂适用于脓液多、渗出物多、根尖未发育完全的根管及乳牙根管的填充。碘仿与醇、组织液、脂肪或细菌产生接触后,缓慢分解产生游离碘,从而使细菌产物氧化,产生杀菌作用;另外,运用碘仿糊剂根管充填可能引起病人的全身过敏反应。
磷酸钙骨水泥(CPC)是一种新型的硬组织修复材料,由固液两相组成,可调和成糊状物,根据缺损部位任意塑形。它的生物相容性好,在人体的环境和温度条件下自行固化,其水化成分最终转化为羟基磷灰石(hydroxyapatite HAP),能够引导骨生长,逐步被组织吸收并产生骨头再生效果,目前骨科临床应用结果令人满意(陈统一等,中华创伤杂志,1999,15(3):184-86)。
材料的超充一直是根管治疗的主要难题,传统材料超充后会引起肿胀、疼痛,是引起根管治疗失败的主要原因;而CPC材料超充部分可被组织吸收,无刺激反应,并起到引导根尖周骨组织再生的作用,改变了根管治疗的传统概念。
本发明需要解决的技术问题是公开一种自固化磷酸钙根管填充剂,该根管填充剂可采取注射的方式进行填充,方便医生操作,提高治疗效果,减轻甚至消除根充反应,解决根管填充治疗问题。
本发明的技术构思是这样的:磷酸钙骨水泥与根管壁密合性好,可扩散至管壁牙本质小管内。在根管内自行固化,不发生体积变化,增加牙根的机械强度,降低根折的机率,可有效地起到了引导根尖周围骨组织再生的作用。
将磷酸钙盐与合适的显影剂混合,并且引入具有治疗效果的药物,特别是抗厌氧菌药物,然后用无水无毒溶剂调和成牙膏状或稠糊状,将此稠糊状物灌入到注射器中,即可用于牙根管的注射治疗。
本发明详细的技术方案如下所述:
本发明的可注射自固化磷酸钙根管填充剂的组分包括复合磷酸钙盐、有效量的显影剂和稳定剂,其优选的比例为:
复合磷酸钙盐∶显影剂∶药物∶稳定剂=10∶0~10.0∶0~3.0∶0.05~1.0,质量比;
所说的复合磷酸钙盐是由几种磷酸钙按一定比例混合的混合物,可以按(US5525148,US5545254)公开的方法配制,可以是磷酸钙(α型或β型)、磷酸四钙中的一种或两者的混合物;磷酸八钙,磷酸氢钙(有结晶水或无结晶水,DCPD或DCPA),羟基磷灰石,氟磷灰石,焦磷酸钙中的一种或它们的混合物,其中磷酸钙盐粉末的颗粒大小应小于10微米。
所说的复合磷酸钙盐粉末中磷酸四钙的粒径为1~20微米,DCPA的粒径为1~10微米。
所说的显影剂为碘仿、硫酸钡等,其中以碘仿为佳。它不但具有显影作用,同时还能够杀菌。所有粉末的颗粒要求小,不得大于100μm。
所说的药物为抗菌消毒药,如妥布霉素、泰能、甲硝唑和替硝唑,其中以替硝唑最佳。
所说的稳定剂为羟乙基或羟丙基纤维素、聚乙烯醇、聚乙烯吡咯烷酮等,其中以聚乙烯吡咯烷酮最佳。
本发明的根管填充剂与溶剂混合调和成为糊状,即可采用填充的方式,也可以采用注射器进行直接注射填充,其中以可注射的方式为最佳。
根管填充剂与溶剂比例如下:
根管填充剂∶溶剂=10∶2.0~10.0,质量比;
所说的溶剂为丙三醇、聚乙二醇或二甲基硅酮油等,其中以二甲基硅油最佳。
本发明有如下优点:
1 无水的膏体进入人体后,通过与周围组织的组织液交换,磷酸钙盐粉末一遇水后即可慢慢变硬,可有效的封闭根管且对根管有一定的强化作用,同时是在非水介质中逐步固化,固化体强度较低,在必要时也较容易将糊剂取出,利于后续治疗操作。
2 整个糊剂的主要成分为磷酸钙复合粉末。在水溶液中呈中性,其变慢过程是缓慢的中性水化反应过程,没有其他的刺激物,水化后的最终成分为羟基磷灰石,与人体硬组织的无机成分相似,生物相容性好,无刺激性。因此在填入根管后即使超充也不会产生根充反应,超充于牙槽骨中的部分可逐步被牙槽骨吸收,引导新生骨的形成,其效果优于现有的其他类型根管充填产品,改变了根管填充的传统概念。
3 根管中含有部分抗菌消毒药物,可以局部长效缓释药物,局部浓度较好,但全身药物浓度很小,从而达到抗菌,消毒作用,简化根管治疗疗程,缩短治疗周期。
4 本发明根管糊剂粒径小,浆体流动性和稳定性好,在注射针管中长期保存不分层,不分相,可用于注射根管填充治疗。当需使用时可采用注射的方法进行。注射时在外力作用下可剪切稀化、流动性好、阻力小,注入根管中时注射针尖只需插入到根管的三分之一处。在注射压力下糊剂仍可很好地到达牙根管根尖,有效地填充封闭根管,达到消灭死腔的目的。即使是细小根管、弯曲根管、分支根管也是如此,大大方便临床操作,降低扩根及填充操作技术要求,提高治疗效果。
具体实施方式
下面将结合实例进一步阐明本发明的内容,但这些实例并不限制本发明的保护范围。
实施例1
称取含有磷酸氢钙、磷酸四钙和羟基磷灰石的粒径小于10μm的复合磷酸钙盐粉末10g,替硝唑0.5g,聚乙二醇-400 0.1g组成混合物,用10g无水的二甲基硅酮油将其调和成糊状物,加入到注射器中。该糊状物易于注射,稳定性好;注射后不分层、不分相,有效地封闭根管,起到了杀菌、防腐收敛的作用。
实施例2
称取由磷酸氢钙、磷酸四钙和羟基磷灰石组成的粒径小于10μm的复合磷酸钙盐粉末10g,替硝唑0.5g,甘油0.1g组成混合物,用10g无水的二甲基硅酮油将其调和成糊状物,加入到注射器中。该糊状物易于注射,稳定性好;注射后不分层、不分相,有效地封闭根管,起到了杀菌、防腐收敛的作用。

将实施例1所述的根管糊剂送入牙根管内,对患牙髓病和根尖周病的108个病例进行了充填,共有121颗牙,221个牙根。其中女性65例,男性43例,年龄9-66岁,平均年龄41.52岁,随访时间12-25个月。临床上均未发生不良反应,牙齿不变色,根管内充填未发生体积变化。即使超充的部分牙齿也均无术后疼痛,局部牙龈充血、水肿等根冲反应出现,材料有效地起到了引导根尖周围骨组织再生的作用。
Claims (4)
1.一种自固化磷酸钙根管填充剂,其特征在于包括复合磷酸钙盐、有效量的显影剂、药物、稳定剂和溶剂,各个组分的质量比比例为:复合磷酸钙盐∶显影剂∶药物∶稳定剂=10∶0~10.0∶0~3.0∶0.05~1.0;
根管填充剂与溶剂比例如下:
根管填充剂∶溶剂=10∶2.0~10.0,质量比;
溶剂为丙三醇、聚乙二醇或二甲基硅酮油;
所说的稳定剂为羟乙基或羟丙基纤维素、聚乙烯醇、聚乙烯吡咯烷酮。
2.根据权利要求1所述的根管填充剂,其特征在于所说的显影剂为碘仿或硫酸钡。
3.根据权利要求1所述的根管填充剂,其特征在于所说的药物为抗菌消毒药。
4.根据权利要求3所述的根管填充剂,其特征在于,药物选自妥布霉素、泰能、甲硝唑和替硝唑。
