Abstract
The invention aims to provide a ureteral guide sheath dilator which can realize that a guide wire and a guide sheath tube can be simultaneously arranged in a cavity, is convenient for a doctor to carry out secondary guide sheath, does not occupy the space in a sheath, is convenient for the guide sheath to cut into the guide wire, enters the cavity along with the guide wire and improves the operation efficiency of the doctor, and a ureteral guide sheath with the ureteral guide sheath dilator.
Description
Technical Field
The invention belongs to the field of ureteral guide sheaths, and particularly relates to an improved dilator and a ureteral guide sheath.
Background
The method for entering the cavity by the traditional ureter guide sheath comprises the following steps: 1. carrying out cavity guiding firstly and then; 2. inserting the tail end of the guide wire left outside into the central channel of the guide sheath; 3. the guide wire guides the guide sheath to enter the cavity; 4. when the guide sheath reaches the target position, a doctor needs to take out the guide wire and the dilator at the same time, otherwise, the guide wire and the dilator leave a central channel in the sheath, and the space is occupied, so that other instruments cannot enter the sheath conveniently.
The above-mentioned apparatus and guiding method have the disadvantage that in the process of using the sheath, if the guiding sheath falls off or deviates from the target position, the guiding sheath needs to be pulled out; and most of all, because the guide wire is pulled out, a doctor needs to use the endoscope again to put the guide wire into the cavity, and then the guide step is repeated to put the ureter into the guide sheath again, so that the operation is very complicated and the use time is long.
Disclosure of Invention
The invention aims to provide a ureteral guiding sheath dilator which can realize that a guide wire and a guiding sheath tube can be simultaneously arranged in a cavity, is convenient for a doctor to carry out secondary guiding sheath, does not occupy the space in the sheath, is convenient for the guiding sheath to cut into the guide wire, enters the cavity along with the guide wire and improves the operation efficiency of the doctor, and a ureteral guiding sheath with the ureteral guiding sheath dilator.
In order to achieve the above object, according to a first aspect, the present invention provides a ureteral access sheath, comprising:
a guide sheath having a central channel;
the dilator runs through the central channel and is provided with an extending part embedded in the central channel and an extending part exposed out of the central channel, a guide wire groove for a guide wire to enter from the outer side is formed on the extending part of the dilator, and the guide wire groove is configured to enable the guide wire to enter so as to guide the guide sheath tube and the dilator to enter the cavity and be separated from the guide wire when the dilator is pulled out of the guide sheath tube.
Preferably, the extension of the dilator extends from the head end to the rear end to form an upwardly open guide wire channel for the guide wire to pivot through.
Preferably, the guide wire groove is provided with an opening which is opened upwards, so that the guide wire can be rapidly cut into or pulled out of the guide wire groove.
Preferably, the opening has a flared section and a narrowed section in communication with the flared section, the narrowed section extending from the head end of the dilator.
Preferably, the leading end of the guiding sheath forms a stop ring at its intersection with the projection to facilitate separation of the guide wire groove from the guide wire.
Preferably, the dilator adopts a flexible piece at least in the extending part.
Preferably, the dilator includes a dilator sidewall and a dilator central channel defined by the dilator sidewall and open front and back, the dilator has a head end opening and a back end opening, the dilator sidewall forms a notch extending from the head end opening to the back side, the notch communicates with the dilator central channel to form a guidewire channel.
Preferably, the dilator side wall is provided with a slit from the head end to the rear end to form a narrow opening section, and the narrow opening section extends backwards to form a flaring section communicated with the narrow opening section.
In a second aspect, the invention also provides a ureteral guide sheath dilator, and the dilator adopts the dilator.
In a third aspect, the invention further provides a ureteral access sheath assembly, which comprises a guide wire and the access sheath.
According to the invention, the traditional method that a guide sheath enters a cavity channel is abandoned, and a guide wire groove is formed on the outer wall of the dilator to guide the guide sheath tube and the dilator, so that the space in the sheath is not occupied; and can remain the seal wire in the chamber simultaneously with the sheath pipe, can put into the guide sheath fast the secondary when the sheath pipe appears and drops or need put into under the condition of many sheath pipes simultaneously, improve doctor's operation efficiency.
In addition, the guide groove provided by the invention can be used for any section of the guide wire to pivot, so that the defect that the guide wire is too long and needs to be guided in from the tail end is avoided, and the working efficiency is further improved.
The guide wire groove provided by the invention is convenient for the guide sheath to rapidly cut into the guide wire and enter the cavity channel along with the guide wire.

Detailed Description
The invention or the technical scheme of the invention is further specifically described by the specific embodiment and the accompanying drawings of the specification;

Referring to fig. 1, the present invention provides a ureteral access sheath assembly, which includes an access sheath tube 2, a dilator 1 and a guidewire 6;
the guiding sheath 2 has a central channel; the dilator 1 penetrates through the central channel and is attached to the inner wall of the guiding sheath tube; the distal end of the guiding sheath tube 2 is provided with a sheath tube seat 3, the tail end of the expander is provided with a buckle 4 for connecting with the sheath tube seat 3, the buckle is also provided with a buckle handle 4-1, and the tail end of the expander is also provided with an expander joint 5;
the dilator 1 is provided with an extending part embedded in the central channel and an extending part 1-2 exposed out of the central channel;
in the prior art, a guide wire is pivoted through a central channel of an expander to guide a guide sheath tube and the expander into a cavity channel, the process occupies the space in the sheath tube, the guide wire needs to be pulled out after the guide wire completes the guide function, and the guide wire needs to enter the central channel from the tail end; the above operation is complicated and the guide function of the guide wire can only be realized once; on the basis of this, the method is suitable for the production,
in view of the above existing defects, the dilator provided by the embodiment of the invention has a guide wire groove 1-1 formed on the protruding part for the guide wire 6 to enter from the outside, the guide wire groove 1-1 is configured to enable the guide wire to enter to guide the guiding sheath and the dilator into the lumen and to be separated from the guide wire 6 when the dilator 1 is pulled out from the guiding sheath 2;
the embodiment abandons the cutting into the guide wire 6 from the inner central channel, creatively selects the cutting into the guide wire from the outer side to realize the guide wire guiding function, and the space in the sheath is avoided being occupied; and further considering that the dilator needs to be pulled out of the sheath after the sheath enters the lumen, in order to achieve rapid separation of the guide wire from the guide wire groove, the present invention preferably arranges the guide wire groove at a protruding portion of the dilator exposed out of the sheath instead of the outer wall of the sheath, so that the guide wire can be rapidly separated from the guide wire groove when the dilator is pulled out.
According to the ureteral guide sheath dilator 1, the guide wire can be quickly cut into the guide wire groove 1-1 on the outer side of the dilator, the guide wire is prevented from entering the guide wire from a center hole of the dilator, therefore, the guide wire does not occupy space in the sheath when a guide function is performed, the guide wire does not need to be pulled out when the dilator is pulled out, the guide wire and the sheath tube can be simultaneously reserved in a cavity channel, the guide sheath can be quickly and secondarily placed under the condition that the sheath tube falls or a plurality of sheath tubes need to be simultaneously placed, and the operation efficiency of a doctor is improved.
In some embodiments, the extension portion of the dilator 1 extends from the head end to the rear end to form a guide wire groove 1-1 which is open upward for the guide wire to pass through, so that any section of the guide wire can be cut into the guide wire groove, compared with the conventional method in which a free section at the tail end is inserted into the central hole of the dilator for guidance, the guide wire groove provided in this embodiment can be cut into any section of the guide wire to achieve the guidance function, because the length of the guide wire is long enough, if a doctor finds the free end for a long time, if the doctor can cut any section of the guide wire located outside into the guide wire groove for guidance function, the guidance time can be greatly saved and the doctor can conveniently operate;
referring to fig. 2 and 3, the guide wire groove 1-1 has an opening opened upward for the guide wire to rapidly cut into or come out of the guide wire groove;
in some embodiments, referring again to fig. 2, the guide wire groove 1-1 is opened upward to form a flared section; for the guide wire to cut into or leave the guide wire groove rapidly; thus, after the guide wire 6 is cut into the guide wire groove 1-1, the sheath tube is pushed to lead the guide wire to be guided in parallel along the guide wire groove, so that the guide wire can smoothly guide the guide sheath to enter the cavity; optionally, the width of the opening opened upwards is consistent, and the guide wire can be quickly cut into the guide wire groove and then push the sheath tube along the direction guided by the guide wire; if the guide wire is required to be separated from the guide wire groove, the guide wire groove is manually deviated from the guide direction of the guide wire, so that the separation of the guide wire groove and the guide wire can be quickly realized; the whole operation process is smooth and quick, and the operation efficiency is greatly improved.
In other embodiments, referring to fig. 3, the upward opening of the guide wire groove 1-1 has a flared section and a narrow section communicating with the flared section, it can be understood that the narrow and flared sections of the present invention are defined relative to the outer diameter of the guide wire, the width of the flared section is greater than or equal to the outer diameter of the guide wire so that the guide wire can directly enter the guide wire groove through the flared section, the width of the narrow section is smaller than the outer diameter of the guide wire, and the narrow section needs to have a certain deformation capability to cut into the guide wire;
the narrow-mouth section provided by the embodiment extends from the head end of the dilator, at least the extending part of the dilator adopts a flexible piece, and the flexibility of the guide wire groove can allow the narrow-mouth section to deform, so that a guide wire can enter and exit from the narrow-mouth section and the flaring section;
at the moment, the tail end of the guide wire can be cut into the guide wire groove by a flaring section, and any section of the guide wire can also be cut into the guide wire groove by the flexibility of a narrow section;
the opening is set into a flaring section and a narrow section, so that when the rapid incision is realized, the narrow section is set to avoid sudden and unexpected separation of the guide wire and the dilator in the guiding process;
it is understood that the upward opening of the guide wire groove 1-1 can be integrally set as a narrow opening section, and the guide wire groove is cut in or pulled out through the flexibility of the narrow opening section;
further, in order to avoid the narrow opening section from preventing the guide wire from being rapidly separated from the guide wire groove, the head end of the guiding sheath tube forms a baffle ring at the joint of the guiding sheath tube and the extending part, and the process also takes the premise that the narrow opening section of the guide wire groove of the dilator has flexibility, and the existing guide wire is a hard guide wire, and the baffle ring provides a thrust force to promote the guide wire 6 to be rapidly separated from the guide wire groove 1-1 by utilizing the hard characteristic and the flexibility of the cut section.
In the embodiment, the dilator 1 comprises a dilator side wall and a dilator central channel defined by the dilator side wall and opened in the front and back directions, the dilator 1 is provided with a head end opening and a back end opening, the dilator side wall is provided with a notch extending from the head end opening to the back side, and the notch is communicated with the dilator central channel to form a guide wire groove; at the moment, the guide wire 6 is directly cut into a guide wire groove (a central channel at the tail end of the dilator) through the incision to realize a guiding function, and then the guide wire is quickly separated from the dilator through the incision when the dilator is pulled out, so that the guide wire and the sheath tube can be simultaneously left in the cavity channel; optionally, the side wall of the dilator is provided with a cutting slit from the head end to the rear end to form a narrow opening section, and the narrow opening section extends backwards to form a flared section communicated with the narrow opening section; the slit-shaped narrow opening section can effectively prevent the sheath from being separated from the wire guide groove in the process of guiding the guide sheath by the guide wire, so that the sheath tube can be stably guided into the cavity; the dilator provided by the embodiment has the following advantages: the ureteral sheath dilator can be sleeved in from any section of the guide wire without being sleeved in from the tail end like the traditional sheath, thereby being beneficial to improving the operation efficiency; 2. after the dilator is taken out, the guide wire can be placed in the cavity channel to be used as a safe guide wire, so that the subsequent operation of an operator is facilitated.

1. A ureteral access sheath, the access sheath comprising:
a guiding sheath (2) having a central channel;
the dilator (1) penetrates through the central channel and is provided with an extending part embedded in the central channel and an extending part (1-2) exposed out of the central channel, and the extending part of the dilator extends from the head end to the rear end to form a guide wire groove which is opened upwards for a guide wire to pass through; the guide wire groove is provided with an opening which is opened upwards so that a guide wire can be rapidly cut into or pulled out of the guide wire groove; the opening is provided with a flaring section and a narrow section communicated with the flaring section, and the narrow section extends from the head end of the dilator; the head end of the guiding sheath tube (2) forms a baffle ring which promotes the separation of the guide wire groove and the guide wire at the joint of the head end and the extending part;
the guide wire groove (1-1) can enable a guide wire (6) to enter to guide the guide sheath tube (2) and the dilator (1) to enter a cavity, and the guide wire groove can be separated from the guide wire (6) when the dilator (1) is pulled out of the guide sheath tube (2).
2. The ureteral access sheath according to claim 1, characterized in that said dilator (1) is provided with flexible elements at least for its extension.
3. The ureteral access sheath of claim 1, wherein the dilator comprises a dilator sidewall and a dilator central channel defined by the dilator sidewall that is open front to back, the dilator having a head end opening and a back end opening, the dilator sidewall forming a notch extending posteriorly from the head end opening, the notch communicating with the dilator central channel to form a guidewire channel.
4. The ureteral access sheath of claim 1, wherein the side wall of the dilator is slit from the head end to the rear end to form a narrowed opening section, the narrowed opening section extending rearwardly to form a flared section communicating therewith.
5. A dilator for a ureteral access sheath, characterized in that the dilator of any one of claims 1 to 4 is used as the dilator.
6. A ureteral access sheath assembly comprising a guide wire and an access sheath according to any of claims 1 to 4.
