Description
Technical Field
The invention belongs to the technical field of Chinese herbal medicines, and particularly relates to a composition and a preparation for treating psoriasis caused by external cold and internal heat and a preparation method thereof.
Background
Psoriasis, commonly known as "psoriasis", is a common chronic inflammatory skin disease that is prone to recurrence, characterized by red papules or plaques covered with multiple silvery-white scales. The incidence of the disease is the most in the young and the strong, the incidence of the disease is more in males than in females, the incidence of the disease is more in the north than in the south, the disease is easy to develop or aggravate in autumn and winter, and the disease is relieved in summer.
The disease is considered in western medicine, and the etiology of the disease is related to heredity, infection, metabolic disorder, immune dysfunction, endocrine disorder, mental trauma, trauma or operation and the like. Psoriasis was initially recognized in western medicine as an epidermal disease caused by biochemical or cellular defects of keratinocytes. In the early 80's of the 20 th century, several biochemical mediators, enzymes and pathways were found to be abnormal in psoriasis, including cAMP, eicosanoids, protein kinase C, phospholipase C, polyamines, transforming growth factor alpha, and the like. The conventional western medicines are also hormone medicines or cyclosporine medicines and the like, the medicines have high treatment efficiency but have unsatisfactory effect, and long-term use of the medicines can not only cause drug resistance of the skin of a patient, but also even aggravate diseases.
The three diseases of classic psoriasis, namely 'blood heat', 'blood dryness' and 'blood stasis', are the results of pathological changes, and a plurality of prescriptions are also treated in a symptomatic way.
The related technical scheme discloses the combination of 25-35 parts of rhubarb root and rhizome, 25-35 parts of golden larch bark, 25-35 parts of common burreed rhizome, 25-35 parts of zedoary, 15-25 parts of safflower, 25-35 parts of red sage root, 25-35 parts of bletilla, 15-25 parts of pseudo-ginseng, 35-45 parts of wild chrysanthemum flower, 10-20 parts of pericarpium zanthoxyli and 25-35 parts of Chinese mugwort leaf, thus achieving the purposes of promoting blood circulation, removing blood stasis, killing parasites, relieving itching, clearing away heat and toxic materials, relieving swelling and alleviating pain. The prescription adopts a mode of 'symptomatic medication', and treats the disease of 'blood stasis' by activating blood and dissolving stasis. Meanwhile, the prescription carries out a certain treatment mode on the surface layer of the skin by using a bathing mode. The mode has slow response and poor effect on the treatment of psoriasis.
In traditional Chinese medicine, the syndrome of external cold and internal heat (cold wrapped fire) refers to patients with heat or blood heat in their organism, and the disease is mostly caused by the close of hair orifices after autumn and winter, no external heat is dissipated, and the internal heat is dissipated in a skin-peeling manner to form erythema and scale.
The traditional Chinese medicine has a long time for treating psoriasis, but the existing traditional Chinese medicine treatment scheme is mostly 'symptomatic medicine administration', few medicine formulas aiming at 'medicine administration for the reasons' are available, and the traditional Chinese medicine also has an excellent treatment result.

Clinical cases:
patient 1: male, 40 years old, han nationality, zheng Zhou, 4 months and 24 days of first diagnosis in 2019. The main complaints are: the whole body is scattered on the squamous erythema and pimple for more than 10 years, and the weight is increased for 1 year. The current medical history: the patients have no obvious reason to cause the disease before more than 10 years, have been diagnosed in a certain three hospitals in Zheng Zhou, and have uncertain specific medication and poor effect. The skin damage is serious in winter and mild in summer for 10 years, and the process is repeated. Before 1 year, no obvious cause or aggravation exists, and the self-conscious effect is mild when the Chinese medicament is taken orally and externally used for compound clobetasol propionate ointment and the like in a certain three hospitals. The symptoms are as follows: the whole body has the symptoms of dripping squamous erythema, pimple with itching, much outside of the limbs, sleep-receiving, loose stool, small stool, irritability, dry mouth without desire for drinking, pale tongue, reddened tip, thin and white fur and weak pulse. And (3) diagnosis: and (3) Western diagnosis: psoriasis. Traditional Chinese medicine diagnosis: and (4) the psoriasis. Syndrome type: yang deficiency with external cold, spleen deficiency with liver hyperactivity, and skin stagnant heat. Therapeutic method: warming yang to dispel cold, invigorating spleen to regulate liver, cooling blood and clearing heat. The product obtained from example 7 was used, 2 times per day. And (2) second diagnosis: after the medicine is taken, most of the skin lesions disappear, the stool returns to normal, the tongue is pale, the coating is thin and white, the pulse is weak on the right and is flat on the left. Three diagnoses: after the medicine is applied, most of the skin lesions disappear, the residual skin lesions are thick, dark and red, the stool is normal, the tongue is pale, the tongue coating is thin and white, and the pulse is deep. Four diagnosis: the skin loss basically subsides, several dark red papules remain, loose stools, a pale tongue with teeth marks on the edges, a thin and white coating, and a deep and slightly wiry pulse. The exterior cold is removed and the stagnated heat of the skin is basically removed, so the method is to warm and tonify the spleen and stomach, sooth the liver, clear heat, cool blood and remove toxicity.
The disease course of the patient is as long as 10 years, the pathogenesis of the patient is complex, and the disease is not only mixed of deficiency and excess, but also related to a plurality of internal organs. Severe winter with severe skin damage and mild summer, pale tongue with weak pulse, which is the condition of yang deficiency and external cold; loose stool, dry mouth without desire for drinking, spleen deficiency with dampness; the skin lesion is in the shape of drops, red in color, red in tip of tongue and with the existence of stagnant heat in skin; the spleen-qi is irritable and irritative, which is the symptom of stagnated heat in the liver meridian. The origin of the disease of the patients with the slight thought lies in the deficient yang-qi of the body, the natural deficiency of defensive yang, the easy invasion of exogenous pathogenic wind-cold, the contraction and stagnation of the cold, the wind-cold pathogen tends to block the body surface, the failure of the yang-qi of the skin surface to reach, the aggravation of the stagnated heat of the skin, and the skin damage attack. Therefore, the patients in this example are distinguished by yang deficiency with external cold, spleen deficiency with liver hyperactivity and skin stasis and heat, and the treatment is performed by warming yang for dispelling cold, invigorating spleen for soothing liver, clearing heat and cooling blood. If the patient only takes bitter and cold herbs, or the disease is improved in the early stage, but the middle-yang will be damaged for a long time, which leads to the occurrence of variable syndrome. And a doctor with slight brightness sees that yang deficiency and blood heat coexist in the body, adopts a method of warming yang qi and clearing heat and cooling blood, neglects the existence of pathogenic factors of wind cold to block the exterior disease mechanism, does not release the exterior cold to block the exterior of the muscle, is difficult to communicate between qi activities in the body and the outside of the body even if the yang is warmed, is difficult to remove the skin stagnated heat, does not get the disease state better, even aggravates the skin stagnated heat, and has the phenomenon of skin damage aggravation. Therefore, the best treatment method for the patient is to use both cold and warm, to coordinate the exterior and interior and to treat both the principal and secondary aspects of the disease.
The back condition before treatment is shown in fig. 1; FIG. 2 shows an abdominal condition before treatment; FIG. 3 shows the back condition after treatment; figure 4 shows the abdominal condition after treatment.

And (3) diagnosis: psoriasis, dialectical: yang deficiency with external cold and damp-heat in the interior. The product obtained from example 7 was used, 2 times per day. And B, diagnosis: the skin lesions all subsided, no new eruption, pale and swollen tongue with reddish tip, white coating and greasy coating, and weak pulse.
Patient 3: for men, the age of 64 years is 64 years, the patients have no obvious inducement to the appearance of erythema scattered in drops on the whole body before telling 1 year, the patients are diagnosed as psoriasis in local hospitals, the treatment is unknown, the skin lesions recur within 2 months after being cured, the treatment effect is poor by continuing the previous method, the skin lesions gradually increase within 1 year and obviously worsen before 2 months. The symptoms at the time of clinic are as follows: the scaly erythema is thick on the back and the limbs, a large amount of scales are dry, the skin lesions on the extending sides of the upper limbs, the thighs and the buttocks are fused into hypertrophic patch, the skin lesions are dark red, the pruritus at night is obvious, the skin lesions are aversive to cold, and the skin lesions are aggravated in cold days; if the food is taken orally, bitter taste in the morning, dry stool, 1 time in 2-3 days, pale tongue with thin and white coating, and wiry pulse. And (3) Western diagnosis: psoriasis. Traditional Chinese medicine diagnosis: and (4) the psoriasis. Syndrome type: syndrome of yang deficiency with external cold and skin stasis and heat. Therapeutic method: warm yang, relieve exterior syndrome, cool blood and clear heat. The product obtained from example 7 was used, 2 times per day.
