And the distal end side of the firing block 5 is provided with a firing plate 51 and a cutter 9 corresponding to the anastomotic nail, the firing plate 51 and the cutter 9 are at least partially positioned in the nail bin 31, the anastomotic nail is positioned on the distal end side of the firing plate, and the position of the cutter 9 corresponds to the cutter groove 313 of the nail bin 31.
In the present invention, the distal end side and the proximal end side are the proximal end side with respect to the operator, the end closer to the operator, the end farther from the operator, that is, the end closer to the operation position, the direction along the axis of the suture cutting device is the axial direction, that is, the direction from the distal end side to the proximal end side of the suture cutting device, or the direction from the proximal end side to the distal end side of the suture cutting device. In the view of fig. 1, the distal side 11 of the housing 1 is the left end side of the housing 1, and the fixed handle 12 of the housing 1 is located on the proximal side of the housing 1. In fig. 1, the S1 direction is the proximal-to-distal direction, and the S2 direction is the distal-to-proximal direction. In the view of fig. 14, the distal side 41 of the rail 4 is the left end side thereof, and the proximal side 42 of the rail 4 is the right end side thereof. In the present invention, the up-down direction is defined as the up-down direction in the view angle of fig. 1, that is, the D1 direction is the upward direction in fig. 1. The left-right direction is defined by the direction D2 in fig. 21. In the present invention, the inner side and the outer side of a member are the inner side with respect to the axis of the member, and the outer side with respect to the axis.
As shown in fig. 1 and 2, in this embodiment, the drive handle 71 pushes the firing block 5 toward the distal direction of the lancing device when the drive handle 71 is gripped for movement in a first direction. As shown in fig. 1, the driving handle 71 includes a firing engagement portion 711, a pivoting portion 712, and a holding portion 713 arranged in this order, the pivoting portion 712 being rotatably connected to the housing 1, the firing engagement portion 711 being rotatably connected to the proximal end side of the firing block 5. In this embodiment, as shown in fig. 3, the lancing device further includes a transmission member 6, and both ends of the transmission member 6 are rotatably connected to the proximal side of the firing block 5 and the firing engagement portion 711 of the driving handle 71, respectively. The nail anvil 2 and the firing assembly are both close to the inner side wall of the casing 1, so as to enlarge the field of view and the operation space in the casing 1, and the firing assembly and the pivoting part 712 are located at two sides of the center line of the casing 1, so as to obtain higher firing force transmission efficiency. In this embodiment, the firing assembly is located above the centerline of the housing 1 and the pivot 712 is located below the centerline of the housing 1. In another alternative embodiment, the firing assembly may be located below the centerline of the housing 1, the pivot 712 may be located above the centerline of the housing 1, etc., all falling within the scope of the present invention.
As shown in fig. 2, the proximal side of the housing 1 is provided with a proximal opening 15, and the firing assembly enters the interior of the housing 1 from the proximal opening 15. The operator can also see the inside of the suturing device through the proximal opening 15 and use surgical tools such as forceps to manipulate the tissue to be anastomosed. In this embodiment, the anvil 2 and the cartridge assembly 3 are both located above the inside of the housing 1, the window 13 is opened on the upper surface of the housing 1, and the position of the window 13 corresponds to the position of the anvil 2, so that the tissue to be anastomosed is conveniently pulled into between the anvil 2 and the cartridge assembly 3 through the window 13. In other alternative embodiments, the window 13 may be provided at other positions of the side wall of the housing 1, not limited to the position shown in fig. 2.
As shown in fig. 3, in this embodiment, the distal end side of the housing 1 is provided with a distal end opening 111, the distal end opening 111 avoids a sealed space formed between the distal end of the suture cutting device and the tissue cavity when the suture cutting device is placed into the tissue, and avoids the influence caused by the normal secreted gas or liquid and contents in the digestive tract. In this embodiment, the distal opening 111 is located in the middle of the distal side surface of the housing 1, i.e. the distal opening 111 is aligned with the axis of the slitting device and is a circular opening. In other alternative embodiments, the distal opening 111 may be formed at other positions, and may have other shapes, for example, the distal opening 111 may be a circular opening or an oval opening formed in at least two through distal side walls of the distal side wall, which are all within the scope of the present invention.
As shown in fig. 1 and 2, the distal surface of the housing 1 may be an arc-shaped surface for guiding, i.e., the distal diameter of the housing 1 is smaller than the diameter of the main body portion thereof, so that the distal side thereof forms an approximately conical guide portion, which can more conveniently guide the insertion of the housing 1 into the inside of the rectum through the anus or guide the housing 1 more easily into other lesion sites of the digestive tract while protecting the mucosal layer from damage.
As shown in fig. 5 and 6, the distal end side of the nail cartridge 31 is a nail cartridge surface 312, the nail cartridge surface 312 is provided with a plurality of nail holes 3121, the nail holes 3121 may be arranged in one row or a plurality of rows, and when the nail holes 3121 are at least one row, the nail holes 3121 at the inner ring and the projection of the nail holes 3121 at the outer ring along the diameter direction may overlap. Fig. 6 shows the positional relationship of the cutter 9 and the cartridge 31 in an initial state, in which the firing block 5 is omitted. Fig. 5 shows the structure of the firing block 5 mated with the cutter 9. The cutter 9 is fixedly arranged in a containing groove 59 on the distal end side of the firing block 5, and the bottom surface of the containing groove 59 is a push-type knife surface 58 matched with the cutter 9. The distal side of the firing block 5 is provided with a firing tab 51 and a cutter 9, the firing tab 51 corresponding to the position of the staple hole 3121 for pushing staples out through the staple hole 3121 when the stapling device is fired. The push surface 58 is located on the proximal side 92 of the cutter 9. When the firing block 5 moves towards the distal direction, the cutter 9 is driven to synchronously move towards the distal direction. As shown in fig. 7, the staples 33 include a fixed end 331 and a pointed end 332, and the fixed end 331 of the staples 33 is fixed to the inside of the cartridge 31. When the suturing and cutting device is started, the triggering piece 51 of the triggering block 5 pushes the spike end 332 of the anastomotic nail 33 to penetrate through the nail hole 3121, the tissue is penetrated and then reaches the nail anvil 2, the nail anvil surface 22 of the nail anvil 2 is formed, and the fixed end 311 is separated from the nail bin 31 and fixed on the tissue, so that the anastomosis of the tissue is realized. The shape and configuration of the staples in fig. 7 are merely examples, and in other alternative embodiments, the staples may take other shapes and configurations and remain within the scope of the present invention. For example, in another alternative embodiment, the staple is a "B" shaped staple, the staple includes two tissue-piercing legs on either side, and when the staple is formed on the anvil, the legs of the staple press against the grooves of the anvil to bend and deform the staple into a "B" like shape. The nail anvil 2 is further provided with a cutting matching portion, the cutting matching portion corresponds to the position of the cutter groove 313, and when the stitching and cutting device is triggered, the cutting matching portion is matched with the cutter 9, so that the cutting effect of the cutter 9 on tissues is improved. The cutting engagement may be a knife anvil or a distally extending recess in the anvil 2. Specifically, the knife anvil may be a hard plastic sheet fixedly disposed at a corresponding position on the nail anvil 2, or a silicon sheet or the like in a manner disclosed in the prior art.
As shown in fig. 5, the cutter slot 313 is a linear slot or an arc slot, the nail holes 3121 are all located on the same side of the cutter slot 313, and the nail holes 3121 are disposed close to the inner wall of the housing 1 with respect to the cutter slot 313. I.e. the cutters 9 are located on the inner side away from the side wall of the housing 1, and the staple holes 3121 are located on the outer side close to the side wall of the housing 1.
The firing process of the surgical stapling device is described in detail below with respect to FIGS. 1-17. As shown in fig. 1 to 9, the surgical stapling device according to this embodiment is schematically configured in an initial state (i.e., unfired state). As shown in fig. 10 to 16, the surgical incision device according to this embodiment is schematically configured at the time of firing. Fig. 17 is a schematic view showing the structure of the surgical stapling device in this embodiment after the completion of firing.
In this embodiment, the cartridge 31 has a first position and a second position, which is located on the distal side of the first position, in different states. The cartridge 31 gradually approaches the anvil 2 and gradually covers the window 13 as the cartridge 31 moves from the first position to the second position. The firing block 5 has a third position, a fourth position, and a fifth position, the fourth position being distal to the third position, and the fifth position being distal to the fourth position.
As shown in fig. 3 and 5, in the initial state, the cartridge 31 is positioned at the first position, which is located at the proximal end side of the window 13. As shown in fig. 1 and 5, in the initial state, the driving handle 71 is not held, the cartridge 31 is in the initial first position, the firing block 5 is in the initial third position, the distal end side 91 of the cutter 9 is located inside the cutter slot 313 and does not protrude from the cartridge surface 312, and the tissue is not cut. At this time, the cartridge 31 may be located entirely on the proximal side of the window 13, i.e., not overlapping the window 13, or the cartridge 31 may be located partially on the proximal side of the window 13, i.e., partially overlapping the window 13, but entirely on the opposite proximal side of the window 13. In the initial position, the second elastic member 73 ensures that the driving assembly stays at a position stably, and is not unfired by external force.
As shown in fig. 4 to 8, in this embodiment, the surgical stapling device further includes a guide rail 4 extending along an axial direction of the stapling device, and the anvil 2, the cartridge assembly 3, and the firing block 5 are sequentially sleeved on the guide rail 4 from a distal end side to a proximal end side of the stapling device. The guide rail 4 can guide and limit the axial movement of the cartridge assembly 3 and the firing block 5, and keeps the cartridge assembly 3 and the anvil 2 aligned in the axial direction, so that the corresponding position on the anvil surface 22 of the anvil 2 can be formed after the staples in the cartridge assembly 3 are pushed out. The distal side 41 of the rail 4 may be secured to the interior of the housing 1 and the anvil 2 secured to the interior of the housing 1 to maintain positional stability of the rail 4 and anvil 2 relative to the housing 1, although the invention is not so limited. After the firing block 5 is separated from the housing 1, the firing block 5 and the cartridge assembly 3 may be removed from the proximal side 42 of the guide rail 4.
In this embodiment, as shown in fig. 4 to 8, the surgical stapling device includes two guide rails 4, and the two sides of the anvil 2, the staple cartridge 31, and the firing block 5 are respectively sleeved on the guide rails 4. In other alternative embodiments, the guide rails 4 are not limited to two, but may be provided in one or more, which falls within the scope of the present invention. The two sides of the nail anvil 2 are respectively provided with a nail anvil guiding part 21 which is sleeved on the two guide rails 4, and a nail anvil surface 22 is arranged between the two nail anvil guiding parts 21. The nail bin 31 comprises two nail bin guiding parts 311 which are respectively sleeved on the two guide rails 4, and the nail bin surface 312 is arranged between the two nail bin guiding parts 311.
As shown in fig. 10 and 11, when the driving handle 71 is held in the first direction (i.e., S3 direction in fig. 10), the driving handle 71 rotates in the first direction, and the firing engagement portion 711 pushes the firing block 5 in the distal direction (i.e., S1 direction in fig. 1), so that the firing block 5 moves from the third position to the fourth position, i.e., to the position shown in fig. 12, during the process of moving the firing block 5 from the third position to the fourth position, the firing block 5 and the staple cartridge 31 are linked, and the staple cartridge 31 simultaneously moves to the second position to the distal side under the pushing action of the firing block 5, where the second position is the surgical position where the anvil 2 and the staple cartridge 31 clamp tissue, i.e., the closure of the stapling device is achieved. At this time, the second elastic member 73 is pressed by the driving handle 71 to have a certain elastic deformation amount. At this time, the distal end side 91 of the cutter 9 moves a distance in the distal direction along with the firing block 5, but is still located inside the cutter groove 313, and does not change relative position to the cartridge 31, but does not protrude from the cartridge surface 312, and does not cut tissue.
When the firing block 5 is in the fourth position, the driving handle 71 is continuously held along the first direction, and the driving handle 71 drives the firing block 5 to continuously move to the fifth position in the distal direction, namely the position of the firing block 5 as shown in fig. 13, and at the moment, the firing block 5 moves to the distal side for a certain distance. As shown in fig. 13 and 14, in the process of moving the firing block 5 from the fourth position to the fifth position, the firing block 5 is not linked with the staple cartridge 31, the position of the staple cartridge 31 is kept unchanged in the second position, the firing block 5 continues to move to the distal side and enters the inside of the staple cartridge 31, so that the distal side of the firing block 3 can push out the staples in the staple cartridge 31 towards the direction of the anvil 2 and drive the cutter 9 to move towards the direction of the anvil 2, and the staples pass through the cutter slot 313 to reach the cutting matching part of the anvil 2, namely reach the position shown in fig. 15 and 16 relative to the staple cartridge 31, and cut tissues to complete the firing of the stapling device. At this time, the second elastic member 73 is further pressed by the driving handle 71 to have a larger elastic deformation amount. Thus, the present invention provides for the closing and firing of the lancing device by means of a single actuation handle 71.
As shown in fig. 17, after the completion of the firing of the lancing device, the operator releases the driving handle 71, that is, the gripping force of the driving handle 71 is removed, and at this time, the driving handle 71 rotates in a second direction (i.e., S4 direction in fig. 17) from the firing position under the elastic deformation restoring force of the second elastic member 73, and rebounds to its initial position, so that the restoration of the lancing device is achieved, and the second direction is opposite to the first direction. Further, since the driving handle 71 is rotatably connected with the firing assembly, when the driving handle 71 rotates along the second direction, the whole firing assembly is driven to move towards the proximal side of the stapling device, so that the staple cartridge 31 returns to the first position.
Only one structure of the second elastic member 73 is shown in this embodiment. In other alternative embodiments, the second elastic member 73 may be disposed between the fixed handle 12 and the firing engagement portion 711 of the driving handle 71, or may be disposed between the side wall of the housing 1 and the firing engagement portion 711 or the holding portion 713 of the driving handle 71, which falls within the scope of the present invention. In other alternative embodiments, the second elastic member 73 may also adopt an elastic structure such as a compression spring, a tension spring, a torsion spring, or the like.
As shown in fig. 6, 8 and 14, in this embodiment, the firing assembly further includes a first elastic member 32, and two ends of the first elastic member 32 respectively abut against the staple cartridge 31 and the firing block 5. The first elastic member 32 is located inside the staple cartridge 31, the distal end side of the first elastic member 32 abuts against the distal end side of the staple cartridge 31, and the proximal end side of the first elastic member 32 abuts against the distal end side of the firing block 5.
As shown in fig. 6, in this embodiment, the first elastic member 32 includes two compression springs that are respectively provided in two cartridge guide portions 311 of the cartridge 31 and respectively fit over the two guide rails 4. The first elastic member 32 is provided between the distal end side of the firing guide 52 and the distal end side of the staple cartridge 31.
In the initial state (i.e. the state of fig. 4), the staple cartridge 31 is in the first position, the firing block 5 is in the third position, at this time, the first elastic member 32 may be in an uncompressed state or may be in a state with a certain initial compression amount, at this time, the driving handle 71 is held, the pushing force of the driving handle 71 on the firing block 5 is insufficient to overcome the force of the first elastic member 32 against deformation, so that the staple cartridge 31 and the firing block 5 implement linkage through the first elastic member 32, the firing block 5 pushes the staple cartridge 31 to move synchronously to the anvil 2 side, and the firing block 5 moves to the fourth position, and the staple cartridge 31 moves to the second position (i.e. the state of fig. 12). At this time, since the tissue is clamped together with the anvil 2 and the driving handle 71 is continuously gripped, the tissue is subjected to the resistance to the continuous distal movement of the staple cartridge 31, and therefore, it is necessary to increase the gripping force of the driving handle 71, the pushing force of the driving handle 71 to the firing block 5 is increased, the force of the first elastic member 32 against the deformation is overcome, the first elastic member 32 is compressed, and the deformation of the first elastic member 32 is suppressed, and at this time, the staple cartridge 31 is no longer moved in the distal direction, and the firing block 5 is continuously moved in the distal direction to the fifth position (i.e., the state of fig. 13 and 14). The firing plate 51 of the firing block 5 protrudes from the distal side of the firing guide 52, so that the firing plate 51 contacts with the staples in the staple cartridge 31 and pushes the staples out toward the anvil 2, and the cutter 9 is pushed out from the cutter slot 313 toward the anvil 2 to reach the cutting mating part of the anvil 2, thereby realizing the firing of the stapling device.
After the completion of the firing of the stapling device, when the driving handle 71 is rotated and reset in the second direction under the action of the second elastic member 73, the driving handle 71 drives the staple cartridge assembly 3 to move back to the first position in the initial state in the proximal direction of the stapling device. Meanwhile, when the driving handle 71 is rotationally reset in the second direction, the first elastic member 32 is no longer pushed in the distal direction, the firing plate 51 is pushed in the proximal direction by the elastic restoring force of the first elastic member 32, and the firing block 5 is retracted to the third position inside the staple cartridge 31 in the initial state with respect to the staple cartridge 31 by the first elastic member 32. In various embodiments, the first elastic member 32 may be a spring, a metal elastic sheet, or a flexible material block that can be compressed when subjected to an external force, such as a rubber block, a silicone block, etc., which falls within the scope of the present invention.
In another alternative embodiment, instead of the first elastic member 32, a friction damping mechanism (for example, a friction damping layer of rubber particles is disposed on the inner wall of the proximal end side of the staple cartridge 31 or the outer wall of the distal end side of the staple cartridge 5) may be disposed between the outer wall of the distal end side of the staple cartridge 5 and the inner wall of the proximal end side of the staple cartridge 31, and in the initial state, the driving handle 71 is held, and the driving handle 71 does not push the staple cartridge 5 sufficiently to overcome the friction force of the friction damping mechanism, so that the staple cartridge 31 and the staple cartridge 5 are linked by the friction damping mechanism, the staple cartridge 31 is pushed by the staple cartridge 5 to move synchronously to the anvil 2 side, and the staple cartridge 31 moves to the fourth position (i.e., the state of fig. 12). At this time, since the cartridge 31 and the anvil 2 clamp the tissue together and the driving handle 71 is continuously held, the tissue is subjected to the resistance to the continuous distal movement of the cartridge 31, and therefore, it is necessary to increase the holding force of the driving handle 71, the pushing force of the driving handle 71 to the firing block 5 is increased to overcome the friction force of the friction damping mechanism, and at this time, the cartridge 31 is no longer moved in the distal direction by the reaction force of the tissue and the firing block 5 is continuously moved in the distal direction to the fifth position (i.e., the state of fig. 13). The firing piece 51 of the firing block 5 protrudes from the distal side of the firing guide 52, so that the firing piece 51 contacts with the staples in the staple cartridge 31 and pushes the staples out toward the anvil 2, and pushes the cutter 9 out toward the anvil 2 to reach the cutting mating part of the anvil 2, thereby realizing the firing of the stapling device.
Fig. 18 and 19 illustrate the mating arrangement between the drive handle 71, the housing 1 and the firing assembly. As shown in fig. 18, the fixed handle 12 is provided with a mounting groove 121, the pivoting portion 712 of the driving handle 71 is located in the mounting groove 121, and the pivoting portion 712 is rotatable in the mounting groove 121. Specifically, the side wall of the mounting groove 121 is provided with a first through hole 122, the pivoting portion 712 of the driving handle 71 is provided with a second through hole 7121, the driving assembly 7 further includes a second connecting member 75, and the second connecting member 75 is disposed in the first through hole 122 and the second through hole 7121 of the pivoting portion 712 in a penetrating manner, so that the pivoting portion 712 can rotate around the second connecting member 75. In this embodiment, the fixed handle 12 is at an angle to the axial direction of the sewing device, which is convenient for the operator to hold, and does not form a shield to the proximal opening 15 of the housing 1, so that the operator can more conveniently observe the interior of the housing 1 from the proximal opening 15. In other alternative embodiments, the fixed handle 12 may also be disposed to extend in the axial direction of the sewing device, or may take other shapes and/or other connection means with the housing 1, all falling within the scope of the present invention. In other alternative embodiments, the mounting groove may not be provided on the fixed handle 12, the outer side wall of the driving handle 71 is attached to the outer side wall of the fixed handle 12, or the mounting groove is provided in the driving handle 71, and the fixed handle 12 is inserted into the mounting groove, which falls within the protection scope of the present invention.
In this embodiment, the second link 75 may be disengaged from the first and second through holes 122, 7121 when the firing assembly needs to be replaced without using the surgical stapling apparatus, and the firing assembly may be entirely disengaged from the housing 1 when the second link 75 is disengaged from the first and/or second through holes 122, 7121. At the same time, the firing assembly may be separated from the housing 1 such that the drive handle 71 and the firing assembly may be separated together from the housing 1. The second connector 75 includes an operation portion 751 and a connection portion 752, the connection portion 752 is provided to penetrate the first through hole 122 and the second through hole 7121, the operation portion 751 is located outside the driving handle 71, and the connection portion 752 is pulled out of the first through hole 122 and the second through hole 7121 by performing a pulling-out operation on the operation portion 751.
As shown in fig. 18 to 22, the firing block 5 is detachably connected to the housing 1 by a first connecting member 8, a guide groove 56 extending in the axial direction of the lancing device is provided on one side surface of the firing block 5, the first connecting member 8 is fixed to the housing 1, the first connecting member 8 is partially located in the guide groove 56, and the first connecting member 8 is movable in the extending direction of the guide groove 56, so that the firing block 5 can move in the axial direction of the lancing device relative to the housing 1, and the guide groove 56 can also provide guidance for the axial movement of the firing block 5. The guide groove 56 is a through groove for communicating the upper side surface and the lower side surface of the firing block 5.
Specifically, as shown in fig. 20 and 21, the first connecting member 8 includes a first mating portion 81, a connecting portion 82, and a second mating portion 83 that are sequentially arranged, the widths of the first mating portion and the second mating portion are greater than the width of the guide slot, the first mating portion 81 is fixedly connected with the housing 1, and the connecting portion 82 is disposed in the guide slot 56 in a penetrating manner, so that the first mating portion 81 and the second mating portion 83 are respectively located at the inner side and the outer side of the firing block 5, and the first mating portion 81 and the second mating portion 83 are respectively located at the inner side and the outer side of the firing block 5, which means respectively located at the upper side and the lower side of the firing block 5. As shown in fig. 18, the housing 1 is provided with a connector mounting groove 16, and the first mating portion 81 is fixedly embedded in the connector mounting groove 16. In other alternative embodiments, the first mating portion 81 and the housing 1 may be fixedly connected by other means, such as a bolt, a rivet, a pin, a buckle, or the like, to fixedly connect the first mating portion 81 and the side wall of the housing 1. In this embodiment, the first engaging portion 81 extends in the width direction (i.e., the direction D2 in fig. 21) of the firing block 5, but the present invention is not limited thereto, and in other alternative embodiments, the first engaging portion 81 may be provided in other shapes.
In another alternative embodiment, the first mating portion 81 may also be connected to a portion of the housing 1, and specifically, a first connector mounting portion (for example, adopting a bump, a beam, etc. structure) is provided on the housing 1, and the first mating portion is connected to the first connector mounting portion. For example, the first connector mounting portion is a bump, and the first fitting portion is provided with a groove that mates with the bump, or the first fitting portion is screwed, riveted, or the like with the first connector mounting portion. The width of the first fitting portion may be smaller than or equal to the width of the guide groove 56, and the connection of the housing 1 and the first fitting portion keeps the first fitting portion from entering the inside of the guide groove 56.
Further, as shown in fig. 20 and 21, in order to achieve the detachable connection of the first link 8 and the firing block 5, a distal end side of the guide groove 56 is provided with a guide groove hole 561, a width of the guide groove hole 561 is larger than a width of the second engagement portion 83, and when the second engagement portion 83 moves to the guide groove hole 561, the second engagement portion 83 can be separated from the guide groove 56 through the guide groove hole 561, so that the firing block 5 is separated from the housing 1. The width here refers to the width in the direction D2 in fig. 21.
Fig. 21 and 22 show the configuration of the first link 8 mated with the firing block 5 in the initial and fired conditions, respectively. As shown in fig. 21, in the initial state, the firing block 5 is positioned at the third position, and the second engaging portion 83 of the first link 8 is positioned at the proximal end side of the guide slot hole 561. Since the firing block 5 is not displaced in the proximal direction by the holding action of the driving handle 71, the second engaging portion 83 is not separated from the guide groove 56 by entering the guide groove hole 561, thereby ensuring a relatively stable connection of the firing block 5 to the housing 1 in the initial state. As shown in fig. 22, in the firing state, the firing block 5 moves to the fourth position in the distal direction, the first connecting member 8 moves toward the proximal side 562 of the guide groove 56, and the second engaging portion 83 is not separated from the guide groove 56 during the movement, thereby ensuring a relatively stable connection of the firing block 5 to the housing 1 during the firing.
When the surgical stapling device is not in use and the firing assembly is to be removed, the second coupling member 75 is first pulled out, the drive handle 71 is separated from the fixed handle 12, the drive handle 71 can pull the firing block 5 in a proximal direction such that the first coupling member 8 moves relative to the firing block 5 in a distal direction of the guide slot 56 such that the second coupling member 83 can be separated from the guide slot 56 through the guide slot hole 561 when the second coupling member 83 moves to the guide slot hole 561, thereby removing the drive handle 12 together with the firing block 5 and the cartridge assembly 3 from the housing 1.
As shown in fig. 18, in this embodiment, the proximal end side of the firing block 5 is provided with a first pivot shaft 54, and one end of the transmission member 6 includes a hooking portion 61, and the hooking portion 61 is rotatably hung on the first pivot shaft 54. The firing mating part 711 of the driving handle 71 is provided with a second pivot shaft 7111, and the second end of the transmission member 6 includes a first connection hole 62, and the first connection hole 62 is rotatably sleeved on the second pivot shaft 7111. Thus, after the firing block 5 is separated from the housing 1, the firing block 5 can be further separated from the driving handle 71 by removing the hooking portion 61 from the first pivot shaft 54.
As shown in fig. 18, in this embodiment, a stopper 57 is provided on the proximal end side of the firing block 5, the stopper 57 is located on the distal end side of the first pivot shaft 54, and the proximal end side of the stopper 57 is disposed opposite to the distal end side of the hooking portion 61. The stop 57 may define the rotational movement between the transmission 6 and the firing block 5. Further, the proximal side of the stopper 57 and the distal side of the hooking portion 61 are respectively inclined guide surfaces.
As shown in fig. 23 to 24, a structure of a surgical suture apparatus according to another embodiment of the present invention is shown. As in the embodiment shown in fig. 1 to 22, when the second engaging portion 83 moves to the guide groove hole, the second engaging portion 83 may be separated from the guide groove 56 by the guide groove hole 561. This embodiment differs from the embodiments shown in fig. 1 to 22 in that: the firing assembly may be separately separated from the housing 1 and replaced separately. One end of the transmission member 6 comprises a second connecting hole 63, a transmission member matching part is arranged on the proximal end side of the firing block 5, a third connecting hole 510 is formed in the transmission member matching part, and the second connecting hole 63 and the third connecting hole 510 are rotationally connected through a connecting pin 9.
One end of the connecting pin 9 is inserted into the second connecting hole 63 and the third connecting hole 510, and the other end is located outside the second connecting hole 63 and the third connecting hole 510, so that the operator can hold the connecting pin in a hand. The connection pin 9 may be separated from the second connection hole 63 and the third connection hole 510. When the firing assembly needs to be replaced, the cartridge assembly and the firing block 5 can be disengaged from the housing 1 by operating the connecting pin 9 such that the connecting pin 9 is disengaged from the second connecting hole 63 and/or the third connecting hole 510. Thus, in this embodiment, the firing assembly can be replaced separately without having to separate the drive handle 71 from the housing 1.

Claims (19)

1. A surgical stapling device, comprising:
a hollow shell, wherein a window is formed in the side wall of the shell;
An anvil fixed inside the housing and located on a distal side of the window;
the trigger assembly is positioned at the proximal end side of the nail anvil, the inside of the trigger assembly is provided with a anastomat and a cutter, the trigger assembly comprises a nail bin assembly and a trigger block, the trigger block is at least partially detachably connected with the shell through a first connecting piece, the trigger assembly can move along the axial direction of the suturing and cutting device relative to the shell, the trigger block is provided with a guide groove extending along the axial direction of the suturing and cutting device, the first connecting piece comprises a first matching part, a connecting part and a second matching part which are sequentially arranged, the first matching part is fixedly connected with the shell, the width of the second matching part is larger than that of the guide groove, the connecting part is arranged in the guide groove in a penetrating mode, the first matching part and the second matching part are respectively located at the inner side and the outer side of the firing block, and the first connecting piece can move along the extending direction of the guide groove;
and when the stitching device is triggered, the driving assembly pushes the triggering assembly to move towards the distal end side direction of the stitching device, and the anastomat and the cutter in the triggering assembly are pushed out towards the nail anvil.
2. The surgical stapling device of claim 1 wherein said anvil, said staple cartridge, and said firing block are sequentially aligned in a distal-to-proximal direction along an axial direction of said stapling device, said staples are disposed in said staple cartridge, said firing block is provided with a cutter, and said firing block is detachably connected to said housing.
3. The surgical stapling device of claim 2 further comprising a rail extending in an axial direction of the stapling device, the anvil and the firing assembly being nested on the rail, the firing assembly being separable from the rail from a proximal side of the rail when the firing assembly is separated from the housing.
4. A surgical stapling device according to claim 3, comprising two guide rails, wherein cartridge guides are provided on both sides of the cartridge, respectively, which are sleeved on the guide rails, and wherein the staples and the cutter are arranged between the cartridge guides.
5. The surgical stapling device of claim 2 wherein a distal side of the staple cartridge is provided with a cutter slot and a staple hole, a distal side of the firing block is provided with a firing tab, the firing tab corresponds to the position of the staple hole, and the cutter corresponds to the position of the cutter slot.
6. The surgical stapling device of claim 5 wherein the cutter slot is a linear slot or an arcuate slot, the staple holes are all located on the same side of the cutter slot, and the staple holes are disposed adjacent to the inner wall of the housing relative to the cutter slot.
7. The surgical stapling device of claim 5 wherein the anvil is further provided with a cutting engagement portion corresponding to the position of the cutter slot.
8. The surgical stapling device of claim 7 wherein the cutting engagement portion is a knife anvil or a distally extending recess in the anvil.
9. The surgical stapling device of claim 5 wherein in an initial state, said firing assembly is positioned on a proximal side of said window and a distal side of said cutter is positioned within said cutter slot, said firing assembly progressively covering said window as said drive assembly pushes said firing assembly in a distal direction of said stapling device.
10. The surgical stapling device of claim 2 wherein,
The shell is provided with a connecting piece mounting groove, and the first matching part is fixedly embedded in the connecting piece mounting groove.
11. The surgical stapling device of claim 2 wherein a first connector mounting portion is provided on the housing, the first mating portion being connected to the first connector mounting portion.
12. The surgical stapling device according to claim 10 or 11, wherein a distal end side of the guide slot is provided with a guide slot hole having a width larger than a width of the second engaging portion, the second engaging portion being separable from the guide slot through the guide slot hole when the second engaging portion moves to the guide slot hole.
13. The surgical stapling device of claim 12 wherein said drive assembly comprises a drive handle comprising, in order, a firing mating portion, a pivot portion and a grip portion, said pivot portion being rotatably connected to said housing, said firing mating portion being rotatably connected to said firing assembly, said grip portion being gripped for rotation in a first direction, said firing mating portion driving said firing assembly toward said anvil.
14. The surgical stapling device of claim 13 wherein a proximal side of the housing is provided with a fixed handle, the pivoting portion being detachably connected to the fixed handle by a second connector;
The fixed handle is provided with a first through hole, the pivoting part is provided with a second through hole, the second connecting piece is arranged in the first through hole and the second through hole in a penetrating mode, and the second connecting piece can be separated from the first through hole and the second through hole; when the second connecting piece is separated from the first through hole and/or the second through hole, the whole firing assembly is separated from the shell.
15. The surgical stapling device of claim 13, further comprising a transmission member having two ends rotatably coupled to a proximal side of the firing assembly and a firing mating portion of the drive handle, respectively;

The surgical stapling device of claim 13, further comprising a transmission member having a second connecting hole at one end thereof, a third connecting hole being provided at a proximal side of the firing block, the second connecting hole and the third connecting hole being rotatably connected by a connecting pin, the connecting pin being disengageable from the second connecting hole and the third connecting hole, the other end of the transmission member being rotatably connected to a firing mating portion of the drive handle; after the connecting pin is separated from the second connecting hole and/or the third connecting hole, the firing assembly can be separated from the shell.

The surgical stapling device of claim 13 wherein said drive assembly further comprises a second resilient member positioned between said drive handle and said housing, said drive handle driving said firing assembly to move in a distal direction of said stapling device when said drive handle is rotated in a first direction by an external force, said second resilient member being resiliently deformed, said drive handle rotationally returning in a second direction under a resilient restoring force of said second resilient member after said external force is removed.

The surgical stapling device of claim 2 wherein said staple cartridge has a first position and a second position, said staple cartridge being in said first position in an initial state, said drive assembly pushing said staple cartridge through said firing block toward a distal side of said stapling device to said second position when said stapling device is fired;
when the staple cartridge is positioned at the second position, the driving assembly continuously pushes the firing block to move towards the distal end side direction of the stitching and cutting device, and the firing block pushes staples in the staple cartridge out towards the staple anvil.

The surgical stapling device of claim 18 wherein said firing assembly further comprises a first resilient member positioned within said staple cartridge, a distal side of said first resilient member abutting a distal side of said staple cartridge, a proximal side of said first resilient member abutting a distal side of said firing block;
When the nail bin is positioned at the first position, the firing block is at least partially positioned in the nail bin, the first elastic piece is not compressed or has an initial compression amount, the nail bin is positioned at the second position, and when the driving assembly continues to push the firing block to move towards the distal end side direction of the suture cutting device, the firing block compresses the first elastic piece so that the firing block continues to enter the inside of the nail bin.
