技术领域
本发明涉及一种可植入的医疗器械,尤其涉及一种闭塞左心耳,阻断左心耳内血栓流出效果佳,可重复定位,固定稳定的,耐疲劳的左心耳封堵器。
背景技术
目前,随着介入器械的发展,许多外科手术无法治疗的治病得到了医治。这些治病的治疗相对于外科手术的治疗方法,介入治疗法的创伤小,恢复快,疗效佳。经过将近半个世纪的发展,介入治疗法得到了迅猛的发展,也产生了各种各样的介入医疗器械产品。使得越来越多的治病和患者得到治疗。
心房颤动(房颤)是最常见的心律失常之一,除了心悸症状带来的不适,血栓栓塞等并发症的发生是房颤最主要的危害。非瓣膜性和非风湿性房颤患者中,约90%的血栓源自左心耳。每年约有5%房颤患者发生血栓栓塞事件和脑卒中。而脑卒中引起的致死率高达50%。因此,如何预防和治疗房颤血栓引起的脑卒中具有重要的临床意义。目前预防和治疗房颤血栓的方法有外科手术、药物治疗和介入治疗方法。其中外科手术法闭塞左心耳,由于其创伤大、风险高等原因未得到广泛的应用。目前,房颤病人预防脑中风的方法有的最普遍的方法是长期口服抗凝血药物,但抗凝血药物有导致出血的风险,约有14%~44%的患者因存在禁忌证而无法接受长期药物抗凝治疗。近年来国内外发展了一种最先进的介入治疗法,通过细小的导管将特制的左心耳封堵器植入到左心耳内,闭塞左心耳,预防和治疗房颤患者发生血栓事件和脑卒中。这种治疗方法创伤小,风险低,恢复快,疗效佳,从而成为预防和治疗房颤引起的脑卒中首选的治疗方法。
目前该类器械从结构来看,可大体分为编织型,切割型和混合型。这些类 型的左心耳封堵器都可以通过导管介入的方法将封堵器送入左心耳内,实现左心耳闭塞。但是都存在相当的局限性。每个人的左心耳解剖结构复杂,形状各异,有椭圆状,花生状等等。左心耳内壁凹凸不平,形状不规则,左心耳壁薄。所以难以在既不刺穿心壁,又能稳固锚定在左心耳内不脱落,最终达到理想的封堵效果。
如发明专利CN200610037789和CN200580002183所设计的编织型左心耳封堵器。他们利用封堵器的固定盘表面的凹凸不平的结构来锚定封堵器,封堵盘在左心耳口封堵,达到闭塞的效果。这种结构的设计优势在于结构简单,加工难度小,成本低。但是编织的固定盘的锚定力度较低,很难适应形状各异的左心耳形态。即使在固定盘上缝合锚定钩,在固定盘受到挤压变形的情况下依然很难刺入左心耳壁中,实现稳定固定。而且固定盘在左心耳内受到挤压变形后会影响封堵盘的形状,导致封堵效果不佳。
如发明专利CN200410059976和CN99815453所涉及的切割型左心耳封堵器。他们将镍钛金属管切割,热处理成型成所设计的形状。在与左心耳内壁接触的部位设计有向外翘起的锚定刺,刺入左心耳内壁,实现稳定固定。通常在切割型的网状框架上缝合有阻流膜,实现左心耳闭塞的效果。这种结构设计的优势在于,整个封堵器一体成型,刚性较强,容易在左心耳内壁固定,所设计的锚定刺容易刺入左心耳内壁中,稳定固定,防止封堵器脱落。但是切割型左心耳封堵器在形状上形变能力有限,无法适应形状各异的左心耳形态,使得它的使用受到很大的限制。切割型左心耳封堵器在塞如左心耳后,在左心房内的部分通常会与心房壁比形成凹陷,形成新的“左心耳”区域,增加新的血栓形成的风险。切割型左心耳封堵器的锚定刺通常是刺向左心房,起到稳定封堵器,防止封堵器脱落。但是刺向左心房的锚定刺在重复定位,回收封堵器时会卡在鞘管壁边沿,使得封堵器无法回收,进行重复定位。
如发明专利CN201110146287和CN103099652A所涉及的混合型左心耳封堵器。在这些设计中封堵盘采用编织的型式,通常在封堵盘内缝合有阻流膜,用于闭塞左心耳。而固定盘有镍钛金属管切割,热处理成型成所设计的形状。在与左心耳内壁接触的部位设计有向外翘起的锚定刺,刺入左心耳内壁,实现稳 定固定。混合型左心耳封堵器有效的解决了切割型左心耳封堵器的大部分问题。但是如在CN103099652A中固定盘的一个锚定刺通常是刺向左心房,起到稳定封堵器,防止封堵器脱落。但是刺向左心房的锚定刺在重复定位,回收封堵器时会卡在鞘管壁边沿,使得封堵器无法回收,进行重复定位。且这种混合型左心耳封堵器的固定盘通常形变较大,以翻转锚定刺的方向,导致固定盘的张力和形变量达到材料的极限值。这种器械在永久植入中的疲劳强度差,容易发生断裂,刺破心脏等其他组织,危及生命。
因此,综合各种左心耳封堵器的优缺点,亟待发展一种可有效闭塞左心耳的,可重复定位的,稳定固定的,可永久植入的左心耳封堵器。
发明内容
本发明的目的是提供一种闭塞左心耳,阻断左心耳内血栓流出效果佳,可重复定位,固定稳定的,耐疲劳的左心耳封堵器。
为了实现上述目的,本发明提供的技术方案为:提供一种左心耳封堵器,包括封堵盘及与封堵盘连接的锚定盘,所述封堵盘采用镍钛合金丝编织成网,经热处理形成盘状结构,其一端使用钢套固定,与锚定盘进行连接,另一端使用栓头固定并设有螺纹,与输送装置的推送杆连接,在所述封堵盘的内部缝合有经化学改性的PET阻流膜;所述锚定盘包括一根镍钛管切割而成的多根支撑杆,所述支撑杆从锚定盘的中心端部的圆圈的一侧发出,跨越中心端部的中心到该圆圈的对侧,呈弯曲状,多根所述支撑杆呈交叉放射状排列,其中,至少一根所述支撑杆上开有锚定刺,所述锚定刺向外张开,朝向封堵盘方向,所述支撑杆的尾端向内弯曲,末端设置有圆形的球头,防止锚定盘在推出鞘管时刺伤心耳内部。
所述封堵盘的直径稍大于锚定盘,其中,封堵盘的直径大于锚定盘大约2-6毫米,适用于单叶型左心耳,封堵盘的直径大于锚定盘大约6-12毫米,适用于双叶型左心耳。
所述锚定盘展开时,所述锚定盘抵住左心耳内壁,所述锚刺刺入左心耳内壁,以稳定所述锚定盘在左心耳内的固定。
所述锚定刺的根部在支撑杆上,利用激光切割出锚定刺形状,利用模具热处理定型出锚定刺的角度,锚定刺在不受压力的情况下自动向外张开,压缩时该锚定刺可回收到支撑杆上。
所述支撑杆上还可以设置有空洞用于在锚定盘上缝合经化学改性的PET阻流膜,阻止左心耳内血栓的流出。
所述PET阻流膜为经过化学改性的阻流膜,该PET阻流膜利用酯基置换成酰胺基的反应,将一侧带有磺酸钠基的分子嫁接在阻流膜表面,在PET阻流膜上吸附带有正电荷的染料甲苯胺蓝,用于降低PET阻流膜与水滴的接触角,增强亲水性。
所述PET阻流膜的化学改性方法如下:在250mL的圆底烧杯中放入50片的PET阻流膜,加入1200mL的体积比为5:1的水及1,4-二氧六环的混合溶剂,加入末端分别为伯胺基和磺酸基的烷烃0.5g、催化剂1g,在50-100摄氏度下搅拌2-8小时,反应结束后将阻流膜取出,反复使用去离子水和无水酒精清洗5次。
所述磺酸基的烷烃的为3-氨基丙烷磺酸,4-氨基丁烷磺酸,5-氨基戊烷磺酸或者6-氨基己烷磺酸。
所述催化剂为氢氧化钠,氢氧化钾或氨水。
与现有技术相比,本发明有如下优点:
本发明的左心耳封堵器的锚定盘结构形变柔和,稳定,永久植入的耐疲劳性能极佳。可适应各种形态和尺寸的左心耳腔体。
本发明的左心耳封堵器的两段式设计,使得该封堵器可适应各种形态和尺寸的左心耳腔体。
本发明的左心耳封堵器可同时具备锚定稳固,且有效闭塞左心耳的优点。
本发明的左心耳封堵器不仅可用于封堵单叶型左心耳,还可用于封堵双叶型左心耳。
本发明的左心耳封堵器可重复定位。在一些情况下如果放置效果不佳,在没有解脱输送推杆的情况,可将封堵器收回鞘管内,重复定位和放置,直至达到满意的锚定和闭塞的效果,大大降低手术风险。
本发明的左心耳封堵器可使用较小的输送系统,进一步减小手术过程输送装置对血管的损伤。
本发明的左心耳封堵器与推送杆的柔性连接,大大减小手术过程中推送杆对该封堵器施加的张力,使得封堵器放置的位置更加准确。
本发明的左心耳封堵器的阻流膜经过表面化学处理,使得表面负离子化,不仅降低封堵盘表面的血小板粘附,还提升阻流膜的亲水性和生物相容性,能够实现快速内皮化,降低封堵器相关血栓形成风险。
通过以下的描述并结合附图,本发明将变得更加清晰,这些附图用于解释本发明的实施例。

具体实施方式
现在参考附图描述本发明的实施例,附图中类似的元件标号代表类似的元件。
实施例1
如图1,图2a和图2b所示,本发明所涉及左心耳封堵器包括有封堵盘106,与封堵盘106连接的锚定盘101,其中封堵盘106用镍钛丝编织成编织网,经热处理成盘状,两端用钢套109和栓头110固定,封堵盘106的一端和锚定盘101用钢套连接,另一端设置有螺纹,与输送装置的推送杆连接。封堵盘106的直径大于锚定盘101大约2-6毫米,封堵盘106内部缝合有经化学改性的PET阻流膜108。其中的锚定盘101是用一根镍钛管切割而成的多根支撑杆102,使用模具经热处理成图中所示的形状。锚定盘101的中心端部107经焊接与封堵盘106连接。虽然在实施例一中支撑杆102有9根,但是可根据实际力学性能和规格的要求,可改变支撑杆的数量。这九根支撑杆从中心端部107圆圈的一侧发出,跨越中心端部107的中心到该圆圈的对侧,并呈弯曲状,多根支撑杆一起呈交叉放射状排列。在支撑杆102上还可以设置有空洞用于在锚定盘101上缝合PET阻流膜,阻止左心耳内血栓的流出。在支撑杆102上,利用激光切割出锚定刺104形状,利用模具热处理定型出锚定刺104的角度。该锚定刺104向外张开,朝向封堵盘106的方向,锚定刺104的根部在支撑杆102上,锚定刺104在不受压力的情况下自动向外张开,压缩时该锚定刺104可回收到支撑杆102上。支撑杆102的末端105向内弯曲状。支撑杆102的末端可设置球形的圆头111,放置锚定盘101在推出鞘管时刺伤心耳内部。
如图5a,图5b和图5c所示,锚定盘101在进入鞘管115和从鞘管115里出来的过程有如下特点:为了能清晰说明锚定盘101进出鞘管的过程,图5a,图5b和图5c中将锚定盘101的支撑杆102简化为两根,支撑杆102在鞘管内被拉直并呈交叉 状,锚定刺104处朝向中心端部107的相反的斜内侧方向。在鞘管115内推送过程中不会刮伤鞘管内壁。在推送杆的推力作用下,支撑杆102的末端的弯头105慢慢推出鞘管115(图5a),沿方向A展开。继续推送推送杆,支撑杆102继续沿方向A展开呈如图5b状态。此时,锚定刺104翻转约90度,接近垂直于推送杆。推送杆继续施加推送力,将锚定盘101完全推出鞘管115,支撑杆102继续沿方向A展开呈如图5c状态。此时,锚定盘101完全展开,锚定刺104再翻转约90度,处于锚定盘101外侧,朝向中心端部107的斜外侧方向,刺向左心耳内壁。固定在左心耳腔内。在回收锚定盘101时的过程与锚定盘101推出鞘管115的过程各个阶段相反,锚定盘101沿方向B被推送杆拉入鞘管115,显著的特点是,锚定刺104由展开时朝向中心端部107的斜外侧方向翻转朝向中心端部107的相反的斜内侧方向。这个过程保证锚定刺104不会刮伤左心耳壁并能顺利进入鞘管115。本实施例的左心耳封堵器不仅能保证锚定盘101在左心耳内的可重复定位功能,还大大减少器械对左心耳壁的损伤,降低手术的风险和并发症。
实施例2
如图4a所示,为实施例1在单叶型左心耳内的放置形态,选择合适的规格可完全封堵单叶型左心耳。但是,在使用实施例1封堵双叶型左心耳时,往往选择合适的锚定盘的时候,封堵盘无法完全封盖左心耳开口;或者选择合适的封堵盘的时候,锚定盘通常较大无法适用,这种情况参考图4b所示。在本实施例中,如图3所示,本发明所涉及左心耳封堵器包括有封堵盘106,与封堵盘106连接的锚定盘101,其中封堵盘106用镍钛丝编织成编织网,经热处理成盘状,两端用钢套109和栓头110固定,封堵盘106的一端和锚定盘101用钢套连接,另一端设置有螺纹,与输送装置的推送杆连接。封堵盘106的直径大于锚定盘101大约6-12毫米,封堵盘106内部缝合有经化学改性的PET阻流膜108。其中的锚定盘101是用一根镍钛管切割而成的多根支撑杆102,使用模具经热处理成图中所示的形状。锚定盘101的中心端部107经焊接与封堵盘106连接。虽然在实施例一中支撑杆102有9根,但是可根据实际力学性能和规格的要求,可改变支撑杆的数量。这九根支撑杆从中心端部107圆圈的一侧发出,跨越中心端部107的中心到该圆圈的对侧,并呈弯曲状,多根支撑杆一起呈交叉放射状排列。在支撑杆102上还 可以设置有空洞用于在锚定盘101上缝合PET阻流膜,阻止左心耳内血栓的流出。在支撑杆102上,利用激光切割出锚定刺104形状,利用模具热处理定型出锚定刺104的角度。该锚定刺104向外张开,朝向封堵盘106的方向,锚定刺104的根部在支撑杆102上,锚定刺104在不受压力的情况下自动向外张开,压缩时该锚定刺104可回收到支撑杆102上。支撑杆102的末端105向内弯曲状。支撑杆102的末端可设置球形的圆头111,放置锚定盘101在推出鞘管时刺伤心耳内部。在这实施例中,封堵盘106比锚定盘101直径大6-12毫米,这样的封堵器可以适用于封堵双叶型左心耳,参考图4c所示。
实施例3
本发明的左心耳封堵器的封堵盘内缝合的PET阻流膜为经过化学改性的阻流膜。如图6所示,该阻流膜的化学改性利用酯基置换成酰胺基的反应,将一侧带有磺酸钠基的分子嫁接在阻流膜表面,具体操作如下:
在250mL的圆底烧杯中放入50片(50×50mm2)的PET阻流膜,加入200mL水/1,4-二氧六环(5:1)的混合溶剂,加入末端分别为伯胺基和磺酸基的烷烃0.5g,如3-氨基丙烷磺酸,4-氨基丁烷磺酸,5-氨基戊烷磺酸或者6-氨基己烷磺酸。再加入催化剂1g,如氢氧化钠,氢氧化钾,氨水。在50-100摄氏度下搅拌2-8小时。反应结束后将阻流膜取出,反复使用去离子水和无水酒精清洗5次。
参考图8所示,反应后的PET阻流膜浸入带有正电荷的染料甲苯胺蓝后立即取出,多次实验发现,未经化学改性的PET阻流膜没有吸附带有正电荷的染料甲苯胺蓝,在逐步增加化学改性的时间后,吸附带有正电荷的染料甲苯胺蓝的量逐步增加。
结合图8和图9所示的接触角测试结果,经过化学改性的PET阻流膜与水滴的接触角为76.5±3度,比为改性的PET阻流膜的接触角小41度,比超疏水的ePTFE膜的接触角小51度。从测试结果来看,改性后的PET阻流膜的亲水性大大增强。有利于提升阻流膜的亲水性和生物相容性,能够实现快速内皮化,降低封堵器相关血栓形成风险。
因此,本发明具有以下优点:
本发明的左心耳封堵器的锚定盘结构形变柔和,稳定,永久植入的耐疲劳 性能极佳。可适应各种形态和尺寸的左心耳腔体。
本发明的左心耳封堵器的两段式设计,使得该封堵器可适应各种形态和尺寸的左心耳腔体。

Claims (9)

1.一种左心耳封堵器,其特征在于:包括封堵盘及与封堵盘连接的锚定盘,所述封堵盘采用镍钛合金丝编织成网,经热处理形成盘状结构,其一端使用钢套固定,与锚定盘进行连接,另一端使用栓头固定并设有螺纹,与输送装置的推送杆连接,在所述封堵盘的内部缝合有经化学改性的PET阻流膜;所述锚定盘包括一根镍钛管切割而成的多根支撑杆,所述支撑杆从锚定盘的中心端部的圆圈的一侧发出,跨越中心端部的中心向该圆圈的对侧方向斜向上延伸后,再向下延伸呈弯曲状,多根所述支撑杆呈交叉放射状排列,其中,至少一根所述支撑杆上开有锚定刺,所述锚定刺朝向封堵盘方向向外张开,所述支撑杆的尾端向内弯曲,末端设置有圆形的球头,防止锚定盘在推出鞘管时刺伤心耳内部。
2.如权利要求1所述的左心耳封堵器,其特征在于:所述封堵盘的直径稍大于锚定盘,其中,封堵盘的直径大于锚定盘2-6毫米,适用于单叶型左心耳,封堵盘的直径大于锚定盘6-12毫米,适用于双叶型左心耳。
3.如权利要求1所述的左心耳封堵器,其特征在于:所述锚定盘展开时,所述锚定盘抵住左心耳内壁,所述锚定刺刺入左心耳内壁,以稳定所述锚定盘在左心耳内的固定。
4.如权利要求1所述的左心耳封堵器,其特征在于:所述锚定刺的根部在支撑杆上,利用激光切割出锚定刺形状,利用模具热处理定型出锚定刺的角度,锚定刺在不受压力的情况下自动向外张开,压缩时该锚定刺可回收到支撑杆上。
5.如权利要求1所述的左心耳封堵器,其特征在于:所述经化学改性的PET阻流膜,其表面通过分子嫁接带有磺酸钠基或磺酸钾基。
6.如权利要求1或5所述的左心耳封堵器,其特征在于:所述PET阻流膜为经过化学改性的阻流膜,该PET阻流膜利用酯基置换成酰胺基的反应,将一侧带有磺酸钠基或磺酸钾基的分子嫁接在阻流膜表面。
7.如权利要求6所述的左心耳封堵器,其特征在于:所述PET阻流膜的化学改性方法如下:在250mL的圆底烧杯中放入50片的PET阻流膜,加入1200mL的体积比为5:1的水及1,4-二氧六环的混合溶剂,加入末端分别为伯胺基和磺酸基的烷烃0.5g、催化剂1g,在50-100摄氏度下搅拌2-8小时,反应结束后将阻流膜取出,反复使用去离子水和无水酒精清洗5次。
8.如权利要求7所述的左心耳封堵器,其特征在于:所述磺酸基的烷烃为3-氨基丙烷磺酸,4-氨基丁烷磺酸,5-氨基戊烷磺酸或者6-氨基己烷磺酸。
9.如权利要求7所述的左心耳封堵器,其特征在于:所述催化剂为氢氧化钠,氢氧化钾或氨水。
