noteworthy changes:
- fix handling of empty account key (#15)
- drop monkeypatching of acme (needs newer acme versions)
- cleanup challange after validation
- don't create account if subcommand run with --help (#13)
- add support for revocation reasons
- added support for remote webroot validation over SFTP (kudos Luka Matijevic!)
- add option to symlink SAN names to the created cert/key
- change default minimal validity to 1 week
