updated README

- added dynamic DebugPortOffset retrieval (thanks to mcp for the idea!)

- added some alternative code for NtClose (thanks to ahmadmansoor!)

- only log when the process is hidden

- only log NtClose when the real NtClose call failed
- added more DebugPort offsets (thanks to Insid3Code!)
