Intensive Outpatient Program

Outpatient Program

We provide the ongoing supports & treatment needed to sustain long-term recovery while living at home, staying employed and navigating life with its daily responsibilities.  Once a participant completes a residential treatment program or has graduated from either our partial hospitalization or intensive outpatient treatment programs, our outpatient program provides the safeguard of continuum of care that includes individual & group counseling, trauma therapy, case management and wrap around services. 

What We Believe

The philosophy of Treyway Multi Treatment Services program is to provide essential substance abuse services so clients can recover and live as independently as possible.

 

Treyway Multi Treatment Services’ program is designed for men and women over the age 18 with substance abuse or co-occurring MH/SA disorders. Group counseling shall be provided daily and based on level of care.

Treatment services include case management, linkage or integration of multiple services, assessment, and reassessment of the client’s need for services. Clinical staff informs the recipient about benefits of treatment, community resources, and services; and assists the client in accessing benefits and services. In addition, the Clinical Staff assists in coordinating services for each person served by:

  • Individual counseling and support;
  • Group counseling and support;
  • Toxicology (e.g. urine drug screens);
  • Strategies for relapse prevention to include community and social support systems in treatment
  • Life skills to reenter the community
  • Crisis contingency planning
  • Disease Management; and In addition, the Agency may sponsor

 

  • Peer support
  • Family education/support
  • Local advocacy groups

Outpatient treatment services will include case management, referrals to multiple services, assessment, and reassessment as need. Clinical staff will continue to inform consumers of the possible benefits of treatment, community resources, and services; and assists the consumer in accessing benefits and services. In addition, the Clinical Staff assists in coordinating services for each person served by:

· Assuming responsibility for ensuring the implementation of the person-centered plan.

· Ensuring that the person served is oriented to his or her services.

· Promoting the participation of the person served on an ongoing basis in discussions of his or her plans, goals, and status.

· Identifying and addressing gaps in service provision.

· Sharing information on how to access community resources relevant to his or her needs.

· Advocating for the person served, when applicable.

· Communicating information regarding progress of the person served to the appropriate persons.

· Facilitating the transition process, including arrangements for follow-up services.

· Involving the family or legal guardian, when applicable or permitted.

· Coordinating services provided outside of the organization.

 

Location of Services

Consumers may be residents with his or her own home, a substitute home, group care setting, or crisis housing; however, the treatment services must be provided in a licensed setting separate from the consumer’s residence. The program is provided over a period of several weeks or months

 

Service Delivery

Traditional outpatient treatment is offered 75 minutes a day and can range from 1 to 4 times per week; this is based on the individualized needs of the consumer. If a scheduled group session is missed day, a 75-minute make up group is attended on Friday. Consumer at OP level of care will meet with his or her primary counselor at a minimum once per month. However, consumers assessed to be inappropriate for group setting may meet with his or her primary counselor individually at least twice per week.
