Description

Steps to Reproduce:

  1. Navigate till Visual Editor
  2. Navigate till Insert menu
  3. Navigate till "Template" and hit enter ,
  4. Add a template.
  5. click on the "Show Options" tab.
  6. Verify using the accessibility insights tool

Actual Results:
Missing labels in form elements

Expected Results:
All form elements must have labels.
