Definitions
- the present invention relates to a crystalline implant intended for the correction of ametropia due to the loss of the crystalline diopter removed surgically.
- the extra capsular extraction of the cataract lens and the implantation of an artificial lens in the capsular bag have become in recent years the main method for treating cataracts.
- the most used is called extra-capsular extraction. It consists in removing the nucleus and the cortex after having made an opening in the anterior part of the capsule enclosing them.
- the extraction is often carried out manually by the surgeon or by means of a technique called phacoemulsification. This latter technique has the advantage over manual extraction of a small incision in the corneoscleral wall of the eye. We know that a small incision presents fewer risks of clinical or optical complications.
- the present invention intends to remedy the drawbacks of flexible implants while offering the possibility of accommodation, with simpler means than those hitherto known.
- the implant according to the invention has the means necessary for converting the action of the ciliary muscle into modification of the power of its optical part.
- the subject of the invention is therefore a lens implant made of flexible material comprising a central optical zone and peripheral means for its support and its centering in the capsular bag, in which the optical zone is a deformable lens whose posterior face is convex, the peripheral means being constituted by a peripheral edge whose external diameter is substantially equal to the equatorial diameter of the capsular bag at rest and by a thin dome connecting the lens to the peripheral edge, the latter being elastically deformable forming a dome tensioner .
- the primary role of the peripheral edge is to allow the deployment of the implant inside the capsular bag after its introduction in the folded state. It also ensures the proper positioning of the latter at the meridian of the bag since its peripheral length will be chosen very close to the length of this meridian so that any risk of dislocation or expulsion of the implant is eliminated.
- the stiffness of the peripheral edge can be imparted by its shape (for example a gutter with a small radius of curvature) or by its thickness to form a bead.
- the dome forms a convex veil whose surface is substantially identical to the surface of the corres ⁇ ponding annular part of the capsular bag, so that the implant perfectly matches this face by exerting a very slight pressure by which the posterior capsule is held without folds.
- the part of the dome close to the outer edge may be thicker than that close to the lens where it will be advantageous for it to be relatively thin or, in an equivalent manner, of a different elastic modulus (for example irradiating treatment of a synthetic material in order to create more or less crosslinked zones) in order to create a bond between veil and lens more deformable than a thicker veil would allow.
- FIG. 1 is a view in diametral section of a first embodiment of the invention, housed in a capsular bag in the rest state,
- FIG. 2 is a half view of the implant of FIG. 1 illustrating the amplitude of the deformations which it undergoes under the effect of the zonular traction
- FIG. 3 illustrates, in a perspective view with parts broken away, a second embodiment of the invention
- FIG. 4 is a sectional view of a third embodiment of the invention.
- the implant of the invention is made of a material with a high refractive index (for example greater than 1.50) such as an indication that the polymers and copolymers based on the polydiphenylsiloxanes which are. transparent and biocompatible flexible synthetic materials or hydrogels. This high refractive index allows the construction tion of a powerful implant with a small thickness.
- the implant 1 is in the general shape of a hollow ellipsoid of revolution around its minor axis 2.
- One of the hemispheres 3 has a complete wall while the wall of the other has a central opening 4 and is limited to an annular portion 5 in return above the complete wall 3.
- the complete wall 3 comprises an optical zone 6 forming a lens, for example biconvex, located in the center of the wall. Note that the front face of this lens (the one facing the opening 4) can be flat or even concave.
- This lens is extended by an annular veil 7 in the form of a dome up to the equatorial zone 8 of the implant where the wall forms a gutter and is a little thicker than the veil in order to constitute an elastically deformable tensioner.
- This implant is intended to be placed in the capsular bag represented by the line 9 in FIG. 1.
- the zonular fibers 10 are inserted into the capsular bag in the vicinity of the equator thereof and their action is substantially directed along a cone whose tip is turned forward and which is widely open at its top.
- Figure 1 there is shown the generatri ⁇ these 11 of this cone in the section plane.
- the edge 8 has a function of holding the implant in the bag 9 because it will have been chosen with an outside diameter substantially equal to the equatorial diameter of the bag. capsular 9. This stiffness born from the gutter and / or the extra thickness of the edge 8 and this dimension also make it possible to practice on the bag a certain tension which suppresses the formation of the folds of the capsule.
- the dome 7 and the optical part 6 are therefore properly supported on the rear wall of this capsular bag. We know that this contact is favorable because it constitutes an effective means of inhibiting cell growth. laire in the bag and therefore prevention of the development of fibrosis that should then be treated with laser for example.
- FIG. 2 is a diagram which illustrates the behavior of the implant requested by the zonule.
- the implant In solid lines, the implant is shown in its rest position where it is in contact with the capsular bag 9, the zonular fibers being released.
- the posterior radius of curvature IL, of the lens here is 18 millimeters and that of the anterior R 2 is 30 millimeters.
- the optical zone was also moved slightly towards the front of the capsular bag. This deformation is explained by the fact that the posterior wall of the capsular bag urged in circular traction by the zonule exerts on the implant at the level of the optical lens 6 and of the dome 7 a pressure towards the front while the edge 8 more rigid deforms very little (slight rotation). This thrust therefore results in the variations observed in the radii of curvature and the slight advance of the implant in the direction of the cornea.
- the shape at rest of this implant is not limited to that shown in Figures 1 and 2.
- Figure 3 for example, there is no return 5 beyond a bead 8 forming the tensioner of the dome 7.
- this dome 7 is of variable thickness, decreasing from the periphery 8 towards the lens 6.
- the optical lens has considerable freedom to be deformed by the pressure generated by the capsular wall requested by the zonule.
- the dome may include radial stiffening elements 12 which allow the design to adjust the stiffness or even the shape of the dome so that it is less deformable in the vicinity of the edge and more and more flexible in the direction of the lens.
- Openings 13 are made in the dome to facilitate the implementation of the manipulation.
- the implant may be different from the ellipsoid shown without departing from the scope of the invention.
- the implant shown has a second optical wall 14 at the front of the lens 6 which can be a blade with parallel faces or a lens and therefore participate in the power of the implant.
- This second wall which closes the ellipsoid on the front, is provided with orifices 15 for the liquid circulation inside the implant and its folding.
- the wall 14 does not undergo deformations due to the tension of the capsular bag because this bag has been opened by its anterior wall which therefore no longer exists.
